Endoscopic Outcomes and Inflammatory Marker Correlation in Adult Patients with Corrosive Substance Ingestion.

Background/Objectives: Corrosive substance intake remains a significant public health concern due to its potential for severe gastrointestinal (GI) injury and associated morbidity. Early risk stratification is crucial for appropriate management, yet there is a lack of reliable non-invasive predictors of injury severity. This study aimed to evaluate the clinical characteristics of adult patients with corrosive ingestion and to investigate the correlation between inflammatory markers and endoscopic injury severity. Methods: In this retrospective study, 83 adult patients who underwent esophagogas

Surgical reconstruction of caustic esophageal strictures: a retrospective comparative study of esophagogastroplasty and esophagocoloplasty.

BACKGROUND: Caustic soda ingestion can result in severe esophageal injuries, leading to chronic strictures that may require surgical reconstruction. Esophagogastroplasty and esophagocoloplasty are the most common surgical options; however, comparative outcome data remain scarce. METHODS: We conducted a retrospective analysis of medical records from patients treated for caustic soda ingestion at Hospital João XXIII (HJXXIII), Belo Horizonte, Brazil, between 2014 and 2020. Data were reviewed and finalized in 2024. Among 277 patients, 101 adults had acute esophageal injuries classified by esopha

Role of Endoscopic Ultrasound (EUS) in Predicting Stricture Formation Following Caustic Injury.

BACKGROUND: Caustic ingestion remains a significant public health concern, particularly in low- and middle-income countries, and often results in long-term complications such as esophageal strictures. Esophagogastroduodenoscopy (EGD) is widely used for grading mucosal injury (Zargar classification) but cannot assess wall depth. Endoscopic ultrasound (EUS) allows visualization of transmural involvement, yet its prognostic role in caustic injuries is underexplored. METHODS: This prospective cohort study included 63 adults with confirmed caustic ingestion who underwent both EGD and EUS within 24

Feasibility of Obtaining Pulse Oximetry Readings From the Oropharynx.

BACKGROUND: As access to monitoring sites on the hands and feet may be limited due to surgical procedures, burns, trauma, or injuries, alternative sites for the measurement of pulse oximetry may be needed. AIMS: The current study assesses the feasibility of obtaining pulse oximetry measurements from the oropharynx, using a pulse oximeter probe attached to a standard oral airway. METHODS: Two cohorts were studied, each including 50 patients, with the oximeter facing toward the palate (cohort 1) or the tongue (cohort 2). RESULTS: The oropharyngeal oximeter functioned effectively with an absol

Umbilical Cord Amniotic Membrane Graft as a Skin Substitute in Periocular Reconstruction: A Case Series.

Loss of periocular skin due to cancer, trauma, or surgery is a major reconstructive challenge; resultant tissue contracture can cause eyelid malposition with poor functional and aesthetic outcomes. The authors describe the successful use of cryopreserved umbilical cord amniotic membrane as a wound covering and scaffold for periorbital anterior lamellar defects. This is a retrospective case series of 4 patients (mean 21 years, range 9-30 years, 3 male) who underwent periocular reconstruction with umbilical cord amniotic membrane of 9 different sites. Follow-up time ranged from 10 to 22 months.

A Word of Caution With Esophageal Replacement for Corrosive Esophageal Injury: A Case Report.

Corrosive ingestion remains a common problem in developing countries such as India due to the lack of strict laws that regulate the sale of caustics. Corrosives can damage any segment of the gastrointestinal tract. The chronic phase of corrosive injuries may result in several complications, with the most common among them being stricture. These chronic sequelae are frequently managed by esophageal replacement. Due to the associated injury to the larynx and the trachea, the airway management is complicated in the late phase when fibrosis and strictures distort the anatomy. We present a case of

In vivo assessment of airway wall compliance during inhalation injury response using anatomical optical coherence elastography.

SIGNIFICANCE: Inhalation injury, a critical complication in patients with severe burns, contributes substantially to morbidity and mortality. Current diagnostic practices suffer from subjectivity and lack quantitative metrics. Enhanced diagnostic accuracy is imperative for improving treatment outcomes. AIM: Our objective was to develop normalized cross-sectional compliance (nCsC) of the airway wall, as measured by anatomical optical coherence tomography (aOCT), to reflect the severity of trachea inhalation injury. APPROACH: We employed a customized aOCT system that incorporates an intralumin

A Newborn Case of Esophagus and Vocal Cord Burn After CS (O-Chlorobenzylindene Malononitrile) Gas Inhalation.

Among the agents used for self-defense, O-chlorobenzylidene malononitrile (CS) is the most commonly used substance. It has been suggested that CS use is characterized by rapid onset of action, short duration of action, and minimal side effects. When the clinical situations and case series resulting from exposure to CS were examined, we saw that no cases of exposure during the neonatal period were identified. A male baby born at 40 weeks, weighing 3260 grams, was brought to the emergency room on the 24th postnatal day with bruising and respiratory distress due to a stranger spraying CS into his

Esophageal reconstruction using a hypopharyngeal anastomosis - a single center experience with review of the literature.

BACKGROUND: Lye ingestion or other esophageal trauma may require surgical reconstruction. The hypopharyngeal anastomosis during esophageal reconstruction is a technically demanding procedure with many nuances in approach. Patients often have a challenging post-operative course, and few regain the ability to tolerate a normal diet. CASE PRESENTATION: We describe a case series of three patients (2 colon interpositions and 1 gastric pull-up) who underwent esophageal reconstruction with a hypopharyngeal anastomosis at our institution from years 2017 to 2024, then review the literature. Button Battery Ingestion in a Neonate: Risk, Management, and Implications.

BACKGROUND Although the ingestion of button batteries (BBs) in neonates is exceedingly rare, it poses severe clinical challenges with potentially catastrophic outcomes. The increase in such cases, particularly among toddlers, is largely due to the widespread availability of portable electronic devices. Ingestion of button or disk batteries is notably more dangerous than other foreign bodies, often leading to acute complications such as burns and esophageal perforation. This report details the diagnosis and management of a 21-day-old neonate who presented with a button battery lodged in the eso

Successes and failures of using the intestine as a pedicled oesophageal substitute of corrosive burns.

Anatomy is the primary factor in planning and performing reconstructive surgery of the oesophagus. When it becomes necessary to replace the natural, pathologically obstructed oesophagus-for example, after corrosive intake-a tube can be created using the stomach or intestine. We performed intestine-based reconstructions of injured and obliterated oesophagi in more than 300 patients. Of these, 147 (60 women, 70 men, and 17 children: 7 girls and 10 boys) treated between 1990 and 2010 were analysed in this study. The decision to use the intestine, whether large or small, as a substitute was based

Pyloric Stricture in Children Following Ingestion of Corrosive Substance from Recycled Water and Beverage Bottles: Report of Two Cases and Review of Literature.

BACKGROUND: Pyloric stricture in children after ingestion of corrosive substances is a serious public health issue bordering on poor parental supervision, inappropriate storage of harmful corrosives in the home, and poor socioeconomic status. It is worthy of note that in the two reported cases where the corrosives were acids the pylorus was affected sparing the esophagus and stomach. OBJECTIVE: To highlight the danger of use of recycled water and beverage bottles for storage of corrosive substances in the home and to indicate that surgical bypass of the pylorus is often inevitable when strict

Gastric mucosal necrosis caused by ingestion of crystalline sodium chloride: A case report.

Acute sodium chloride (NaCl) poisoning is a life-threatening condition primarily known to cause systemic hypernatremia. However, the local gastrointestinal effects of ingesting a large mass of crystalline NaCl are not well-documented. This case illustrates that crystalline NaCl can cause a severe corrosive injury pattern akin to that of classic caustic agents. We present a 42-year-old woman with abdominal pain and hematemesis after ingesting 200 g of crystalline NaCl in a suicide attempt. She had hypernatremia (155 mmol/L) and hyperosmolality (serum osmolality 324 mOsm/kg). An upper endoscopy

Anatomical delineation of the tubarial gland amidst ongoing debate: Macroscopic study with microscopic validation and novel duct discovery.

Conflicting anatomical descriptions and functional interpretations have emerged regarding glandular structures located in the nasopharynx, particularly near the Eustachian tube. While some studies describe them as microscopic submucosal or tubal glands, others propose a distinct gland potentially salivary in nature. However, the majority of existing research has relied on imaging or histological analysis, with limited gross anatomical exploration. In view of these divergent perspectives and the paucity of macroscopic data, the present study aims to investigate this region through detailed cada

Machine learning identifies esophageal luminal temperature patterns associated with thermal injury in catheter ablation for atrial fibrillation.

INTRODUCTION: Luminal esophageal temperature (LET) monitoring during atrial fibrillation (AF) ablation is widely used to reduce the incidence of endoscopically detected esophageal lesion (EDEL). We sought to assess whether specific patterns of LET variation are associated with EDEL. METHODS: A high-fidelity multisensor probe was used to record LET in AF patients undergoing radiofrequency ablation (RFA) or cryoballoon ablation (CBA). Explainable machine learning and SHapley Additive exPlanations (SHAP) analysis were used to predict EDEL and assess feature importance. RESULTS: A total of 94 pa
