Numerical investigation of airflow, heat transfer and particle deposition for oral breathing in a realistic human upper airway model.

Inhalation injury from exposure to fire smoke is one of the causes of burn-related death. In this study, a realistic three-dimensional human upper airway model was built from magnetic resonance imaging (MRI) scanned images, including the nasal, oral, pharynx, larynx, trachea and part of the first generation of the tracheobronchial tree, as well as a tissue region from the pharynx to the upper bronchi. The Transition Shear Stress Transport (SST-transition) turbulence model, Pennes bioheat transfer equation, convective boundary conditions and a Lagrangian frame were applied and verified with exp

Nose burns: 4-dimensional analysis.

The nose is the central organ of the face. It has two essential roles, aesthetic and breathing. It is often seriously damaged in the context of facial burns, causing grotesque facial disfigurement. As this disfigurement is visible on frontal and profile views, the patient suffers both socially and psychologically. The nose is a three-dimensional organ. Reconstruction is therefore more difficult and needs to be more precise than in other parts of the face. Maintaining symmetry, contour and function are essential for successful nasal reconstruction. Multiple factors determine the optimal method

Neurotoxicity of Vanadium.

Vanadium (V) is a transition metal that presents in multiple oxidation states and numerous inorganic compounds and is also an ultra-trace element considered to be essential for most living organisms. Despite being one of the lightest metals, V offers high structural strength and good corrosion resistance and thus has been widely adopted for high-strength steel manufacturing. High doses of V exposure are toxic, and inhalation exposure to V adversely affects the respiratory system. The neurotoxicological properties of V are just beginning to be identified. Recent studies by our group and others

Accidental aspiration of denture cleanser tablets caused severe mucosal edema in upper airway.

A 78-year-old man accidentally swallowed denture cleanser tablets (Polident® ). Although he immediately spat them out, he felt difficulty in breathing. Observation using a fiber optic bronchoscope revealed a constricted glottis with edema and redness. The instillation of 1% xylocaine with epinephrine to the epiglottis and larynx reduced the edema and enabled the bronchoscope to pass the glottis. Red and edematous mucosa of the trachea and major bronchus were also observed. We should be aware that accidental aspiration of denture cleansers may cause laryngeal edema.

Autologous Cricoid Cartilage as a Graft for Airway Reconstruction in an Emergent Technique - A Case Report.

INTRODUCTION: Laryngotracheal stenosis can be caused after traumatic injuries to the neck from the subglottic larynx to the trachea. Patients with laryngotracheal stenosis often need a tracheotomy and occasionally may become tracheotomy dependent. Different procedures have been described for the management of these lesions. Management options include techniques of endoscopic dilation, laser resection, laryngo-fissure, and an innovative array of plastic reconstructions with or without the use of stents. CASE REPORT: This paper presents airway reconstruction in a young patient with severe subgl

Development of an in vivo model of laryngeal burn injury.

OBJECTIVES/HYPOTHESIS: Inhalation injury significantly increases morbidity and mortality in burn patients. Approximately one in five burn patients have acute injury to the larynx, trachea, and/or lungs-and as many as 70% have long-term laryngeal abnormalities. Although inhalation injury to the lung has been studied extensively, no models exist to study these insults to the larynx. As such, we developed an in vivo rabbit model to create precise and reproducible laryngeal burn with resultant tissue damage as a foundation for interventional studies. METHODS: Following tubeless tracheotomy, a cus

Systematic review of the anaesthetic management of non-iatrogenic acute adult airway trauma.

INTRODUCTION: Non-iatrogenic trauma to the airway is rare and presents a significant challenge to the anaesthetist. Although guidelines for the management of the unanticipated difficult airway have been published, these do not make provision for the 'anticipated' difficult airway. This systematic review aims to inform best practice and suggest management options for different injury patterns. METHODS: A literature search was conducted using Embase, Medline, and Google Scholar for papers after the year 2000 reporting on the acute airway management of adult patients who suffered airway trauma.

Computational simulation of temperature and velocity distribution in human upper respiratory airway during inhalation of hot air.

The present study provides an accurate simulation of velocity and temperature distributions of inhalation thermal injury in a human upper airway, including vestibule, nasal cavity, paranasal sinuses, nasopharynx, oropharynx, larynx, and upper part of main bronchus. To this end, a series of CT scan images, taken from an adult woman, was used to construct a three dimensional model. The airway walls temperature was adjusted according to existing in vivo temperature measurements. Also, in order to cover all breathing activities, five different breathing flow rates (10, 15, 20, 30, and 40 l/min) an

A fire death with a rare finding: anthracosis or soot embolism?

Charred human remains were found in the smoking ambers of a dying fire in an oil palm plantation in Selangor, Malaysia in the midnight of January 28, 2013. Investigations showed that palm fronds and rubber tires were used to light and sustain the blaze. At least four to five tires were estimated to be used based on the residual burnt metal wires at the site. The remains were brought to the Department of Forensic Medicine, Hospital Sungai Buloh, Selangor for post-mortem examination. Pre-autopsy imaging showed a fractured skull with presence of a bullet in the head. The body belonged to a male w

Toxic smoke inhalation in fire victim dogs.

Fifteen dogs were found dead in a house that was on fire. Several of these dogs were partially burned. Four dogs were submitted for postmortem examination, 2 of which were determined to have died prior to the fire. Of the 2 submitted fire fatalities, only 1 dog had burns on its body (dorsum and right side of body). Internally, both dogs had soot deposits mixed with mucus in the larynx, trachea, and primary bronchi. Microscopically, soot was identified within both airways and alveolar spaces. There were no macroscopic or microscopic indications of vital heat exposure. High levels of carboxyhemo

Bronchoscopic topical steroid instillation in prevention of tracheal stenosis.

Corrosive acid poisoning commonly results in chemical injuries to respiratory and upper gastrointestinal tract. Corrosive mucosal erosion of the larynx and trachea may occur if the patient aspirates acid. We successfully used local anti-inflammatory action of dexamethasone instilled through a fiber-optic bronchoscope for regression of mucosal edema and prevention of subsequent development of stricture in a young female.

Subarachnoid hemorrhage and carbon monoxide exposure: accidental association or fatal link?

This case study reports a fatal subarachnoid hemorrhage (SAH) with concomitant accidental carbon monoxide (CO) exposure in 32-year-old man. Autopsy results indicated an antemortem aspiration of smoke, and a massive SAH was identified as the cause of death. Intriguingly, the carboxyhemoglobin level was 30%, suggesting that CO could have played a specific role. Intracranial hemorrhages following CO exposure in brain areas and tissues such as the basal ganglia, globus pallidus, or white matter are rare, but well characterized, whereas SAH related to CO exposure has not been previously described.

Measuring surface temperature and grading pathological changes of airway tissue in a canine model of inhalational thermal injury.

BACKGROUND: Airway tissue shows unexpected invulnerability to heated air. The mechanisms of this phenomenon are open to debate. OBJECTIVE: This study was designed to measure the surface temperatures at different locations of the airway, and to explore the relationship between the tissue's surface temperature and injury severity. METHOD: Twenty dogs were randomly divided into four groups, including three experimental groups (six dogs in each) to inhale heated air at 70-80 °C (group I), 150-160 °C (group II) and 310-320 °C (group III) and a control group (two dogs, only for histological observ

Endotracheal tube and laryngeal mask airway cuff pressures can exceed critical values during ascent to higher altitude.

OBJECTIVES: Tracheal mucosal perfusion is compromised at an endotracheal tube (ETT) cuff pressure of 30 cm H(2)O, and blood flow is obstructed at a pressure of 50 cm H(2)O. METHODS: We measured the change in pressure of air-filled cuffs of 6.0 and 7.5 ETTs and a size 4 laryngeal mask airway (LMA) from sea level to 2400 m. The ETTs and LMA cuff measurements were done with the devices uncontained, and an additional 6.0 ETT was placed in a 10-mL syringe barrel to mimic placement in a trachea. This restricted cuff expansion simulating what would occur when it is placed within the trachea. The pre
