Laryngotracheal trauma.

Isolated trauma to the airway is infrequent yet potentially life-threatening. The larynx and cervical trachea are vulnerable to external forces, whether penetrating or blunt, as well as internal injuries from endotracheal or nasogastric intubation and to thermal burns from the inhalation of fumes or the ingestion of caustic substances.

New technique for laryngotracheal mucosa transplantation. 'Stamp' welding using indocyanine green dye and albumin interaction with diode laser.

OBJECTIVE: To investigate (1) the possibility of survival of free mucosa "stamp" grafts fixed in the airway with a new technique using indocyanine green-dyed albumin solder activated with a diode laser and (2) the degree of improvement of wound healing in the airway by applying modified microskin transplantation techniques from burn surgery to cover a relatively large wound with a few small pieces of mucosa anchored in place with the previously mentioned technique. DESIGN: Three (one control and two experimental) rectangular (10 x 8 mm) wounds in tracheal mucosa were produced in four experime

Anesthesia for microlaryngeal surgery: the case for subglottic jet ventilation.

Although the techniques for surgery on the endolarynx using suspension and the operating microscope have been fully developed, the safest, and least obtrusive anesthetic technique has yet to be manifested, as evidenced by more than 200 references to anesthesia for microlaryngoscopy in the world literature. This study reviews the physiology, physics, and problems of each anesthetic technique. In light of this review, animal and human studies are reported demonstrating the utility and safety of subglottic ventilation when provided with proper monitoring using an automatic ventilator. [Diagnosis of inhalation injury by larynx-tracheal respiratory sound].

The experiment in rabbits demonstrated that the more narrow the tracheas were, the higher the frequency of respiratory sound was. The frequency of tracheal respiratory sound was about 50-100 Hz in normal rabbits. In mildly narrow frequency of respiratory sound was 50-200 Hz, in mediately narrow trachea 200-300Hz, in severely narrow trachea 400-700 Hz. 74 patients were studied. The patients with more severe inhalation injury had higher tone of respiratory sound and simultaneously showed dyspnea because their larynx-tracheas became more narrow due to edema. When high tone appeared obviously trac

Upper airway compromise after inhalation injury. Complex strictures of the larynx and trachea and their management.

OBJECTIVE: Strictures of the upper airway caused by burns have features distinct from other benign stenoses. The authors reviewed their experience with burn-related stenoses to define the principles of treatment. SUMMARY BACKGROUND DATA: The combined effects of inhaled gases and heat in burn victims produce an intense, often transmural, inflammation of the airway, further complicated by intubation. The incidence of laryngotracheal strictures in survivors of inhalation injury is high, but the reported experience with their treatment is limited and often unduly separated into injuries of larynx

Caustic intoxication.

After a retrospective study of 17 cases of caustic intoxication in the intensive care unit, the epidemiologic, pathophysiologic, clinic and therapeutic aspects are reviewed. The following procedure is suggested: early endoscopies, abstention of any gastric lavage, corticosteroids and empiric antibiotic therapy and the most early possible introduction of a parenteral nutrition.

High incidence of CO2 laser beam contact with the tracheal tube during operations on the upper airway.

Polyvinyl chloride tracheal tubes from 50 consecutive CO2 laser operations of the larynx and trachea were collected after tracheal extubation. In all cases, the helium protocol for laser operations was used, which includes the following: helium in the anesthetic gas mixture at 60% or more during laser resection (FIO2 less than or equal to 0.4); tracheal intubation with plain, unmarked polyvinyl chloride tubes; laser power density less than or equal to 1,992 W/cm2; and laser bursts of less than or equal to 10-second duration. No tracheal tube fires or airway burns occurred. The polyvinyl chlori

The use of carbon dioxide laser in dealing with fibrous strictures of the larynx and trachea.

The carbon dioxide laser has proved valuable in treating patients with severe scarring of the pharynx and supraglottic larynx. There is minimal re-scarring, and an open operation may be avoided. In conjunction with an open procedure, the laser provides a good method of controlling granulation tissue. Two cases of complete pharyngeal stenosis, resulting from caustic ingestion, are described. In one case, repair was achieved by supraglottic laryngectomy and gastro-hypopharyngeal transposition which temporarily obscured the larynx completely. The laser was used to re-open the laryngeal airway, fo

Airway reconstruction following laryngotracheal thermal trauma.

Care of burn patients is a complex problem for the trauma team involving management of fluid and electrolyte imbalances, infections, and ultimately reconstruction. Patients with burns of the head, neck, and upper thorax present a series of unique problems because of a high incidence of associated upper and lower airway thermal trauma necessitating intubation and ventilatory support. A series of cases requiring laryngotracheal reconstruction following severe burns to the head and neck region is reported. Methods of treatment are discussed as well as modifications of standard burn therapy to dec

Near-fatal complication of tracheal T-tube use.

Stenosis of the larynx and trachea is an unfortunate sequel to many thermal injuries. Numerous surgical techniques have been developed for correction of such problems, many involving use of a tracheal T-tube. We report a serious complication attributed to the use of such a tube. Factors contributing to this complication are analyzed and methods for avoiding similar near-catastrophes discussed.

[Follow-up of patients with burn injuries following long-term ventilation].

Follow-ups were performed five years post burn on 13 severely burned patients after long-term ventilation in order to identify damage to the trachea and lung. The study comprised clinical examination, tracheal X-rays, pulmonary function tests with whole-body plethysmography for thoracic compliance, and spiroergometry. Tracheal stenosis was detected in five patients (38%), two patients had reduced total lung capacity and functional residual capacity was increased. Six patients had high values of pulmonary resistance. The comparison between actual and predicted physical work capacity in the spir

Voice restoration after total laryngopharyngectomy and cervical esophagectomy using the duckbill prosthesis.

We have presented a series of 10 patients who were successfully rehabilitated using the tracheoesophageal puncture technique for voice restoration. These patients underwent total laryngopharyngectomy or laryngopharyngoesophagectomy and reconstruction with visceral transposition (five patients), myocutaneous flaps (four patients), and skin graft and cervical flaps (one patient). All patients were able to produce adequate voice and carry on a conversation. Clinical evaluation of voice quality and acoustic analysis of voice samples indicate that the voice obtained in these patients was intelligib

Experience with partial cricoid resection and thyrotracheal anastomosis.

Since 1973, 28 patients have undergone tracheal and subglottic resection with reconstruction by primary laryngotracheal anastomosis. Nine patients had postintubation stricture, eight had blunt trauma with cricotracheal disruption, seven had a neoplasm, two had an inhalation burn, and two had idiopathic stenosis. In all cases, the anterior cricoid arch was completely resected and a submucosal segment of the posterior cricoid plate was partially resected. A posterior shell of cricoid plate was preserved in 21 patients with intact laryngeal nerve function. Postoperatively, a Montgomery T-tube was

Comparison of tracheal damage from laser-ignited endotracheal tube fires.

Studies were undertaken by the Departments of Otolaryngology-Head and Neck Surgery and Anesthesiology at Northwestern University Medical School and the Medical College of Wisconsin (Milwaukee) to compare the potential for tissue injury to the trachea and lungs of canines. Polyvinylchloride (PVC), Rusch red rubber, and silicone tubes were tested. The effects of an intraluminal tube fire on the larynx and trachea were documented with laryngeal and bronchoscopic photographs taken immediately postburn and at the time of sacrifice six hours later. The most severe burns were associated with the PVC

Laryngotracheal reconstruction in children. Five-year follow-up.

Congenital and acquired subglottic stenosis is a commonly encountered problem in the pediatric population. In acquired cases endotracheal intubation is responsible for its development in the great majority of cases, but high tracheotomy, laryngeal burns, external neck trauma, and tumors, both intrinsic and extrinsic, are occasionally seen. The management of mature subglottic stenosis in children remains a controversial issue. The prevailing attitude of otolaryngologists is to perform a tracheotomy and hope for decannulation after one or two years, due to the expected growth of the larynx. Unfo
