Retrospective Evaluation of the Association of Hypoalbuminemia With Acute Treatment Outcomes in Methicillin-Susceptible Staphylococcus Aureus Bacteremia.

BACKGROUND: Cefazolin and antistaphylococcal penicillins (ASP) are first-line agents for methicillin-susceptible Staphylococcus aureus (MSSA) bacteremia. These agents are substantially protein-bound; however, scant data exist evaluating the impact of hypoalbuminemia on microbiologic and patient outcomes for this indication. OBJECTIVE: The purpose of this study was to assess whether hypoalbuminemia is associated with delayed blood culture clearance in patients with MSSA bacteremia being treated with cefazolin or ASPs as definitive therapy. METHODS: This retrospective cohort study included pat

Lessons Learned: Approaches to Delabeling Penicillin Allergy-A Clinical Management Review of Population-Level Strategies.

Penicillin allergy labels are a major barrier to optimal antimicrobial stewardship, yet most reported reactions are inaccurate and can be safely removed after evaluation. This Clinical Management Review article summarizes lessons from population-level delabeling initiatives, highlighting the value of incorporating nonallergist perspectives and practical implementation tools. Effective strategies extend beyond specialist settings and rely on multidisciplinary engagement-including primary care, pharmacy, and nursing-to scale interventions. A practical toolkit offering standardized risk-stratific

Clinical progress note: De-labeling penicillin allergies.

About 15% of hospitalized adults report a penicillin allergy. Of these patients, approximately 90% tolerate penicillin when re-challenged, potentially due to misattribution of nonallergic rash as hypersensitivity or outgrowing the index reaction. Patients with penicillin allergy labels are more likely to receive less effective therapy, develop antibiotic resistance, incur higher healthcare costs, and have poorer outcomes. Although cephalosporins are recommended for most patients with a reported or confirmed penicillin allergy, de-labeling and removing penicillin allergies represent an impactfu

Hospitalist-driven penicillin allergy delabeling at a quaternary care academic medical center.

A penicillin allergy label is not benign and significantly impacts patient care. Penicillin allergy delabeling can be conducted in an efficient, cost-effective, and safe manner by proceeding directly to oral challenge. Through a quality improvement initiative, we developed and implemented a protocol for hospitalist clinicians to delabel penicillin allergies among low-risk hospitalized patients. Risk stratification was determined by PEN-FAST, a validated screening tool. The primary outcome was the total number of patients that underwent delabeling within 1 year. We exceeded our goal of delabeli

When and How to Tackle Penicillin Allergy During Pregnancy.

Pregnant women who report penicillin allergy have higher use of broader-spectrum antibiotics and associated complications, yet few have confirmed allergy when evaluated. Proactive evaluation of penicillin allergy in pregnant patients is encouraged by obstetrical societies, though historically allergy testing during pregnancy has not been widely performed. Numerous recent studies have reported positive outcomes for penicillin allergy testing in pregnancy through oral challenge with or without preceding skin testing for patients with low-risk history with few adverse reactions. Patients with hig

Knowledge, attitudes, and practices of livestock veterinarians on antibiotic use and resistance: a cross-sectional study.

Antimicrobial resistance (AMR) is a pressing global health threat that requires integrated efforts under the One Health approach. Together with human medicine and agriculture, veterinary medicine is key to mitigating AMR emergence and spread. Veterinarians work at the forefront as public health agents and their antibiotic prescribing decisions may be influenced by several factors. This cross-sectional study aimed to assess the knowledge, attitudes, and practices (KAP) of livestock veterinarians regarding AMR and antibiotic use. A questionnaire-based study was conducted in Portugal, from Septem

Genomic and Pathogenic Characterization of Francisella salimarina Associated with Mortality in Fourfinger Threadfin (Eleutheronema tetradactylum) in Taiwan.

In 2024, a mortality event affected farmed fourfinger threadfin (Eleutheronema tetradactylum) in Pingtung County, Taiwan, resulting in significant economic losses. Affected fish displayed anorexia and lethargy, with internal lesions characterized by multifocal white nodules. Histopathological examination confirmed systemic granulomatous disease. Two bacterial isolates, designated F5 and F69, were recovered from internal organs and identified as Francisella spp. by PCR. Experimental challenge demonstrated that strain F69 was less virulent than F5, exhibiting a higher LD50 (5.6 × 106 CFU/fish vs

Unravelling the triad of penicillin-binding proteins, β-lactamase activity, and mRNA dynamics in Pseudomonas aeruginosa AmpC induction.

OBJECTIVES: The main objective of the present work was to dissect the interplay between penicillin-binding protein (PBP) occupancy, ampC transcriptional dynamics, and β-lactamase activity in Pseudomonas aeruginosa. METHODS: Using wild-type PAO1 and isogenic LMW-PBP knockout mutants (PAOΔdacB, PAOΔdacBdacC, and PAOΔdacBdacCpbpG), we assessed ampC induction following exposure to 18 β-lactams and β-lactamase inhibitors. PBP binding (IC50) was quantified via Bocillin-FL labelling. AmpC mRNA levels were measured by qRT-PCR, and β-lactamase activity was determined in crude extracts, periplasmic and

Microbial mechanisms as tools for monitoring and treating emerging contaminants in urban pollution: an overview.

Emerging contaminants are molecules, either novel or previously recognized, that persist in the environment and may pose risks to ecosystems and human health. Their increasing occurrence, particularly in pharmaceuticals, personal care products, and industrial processes, has intensified research on their detection, monitoring, and ecological impact. Advances in analytical technologies now enable the identification of these compounds at trace concentrations, yet their long-term effects remain uncertain. This review compiles recent findings on microbial ecotoxicology, focusing on representative c

Study the role of interleukin-4 and interferon- in patients with penicillin allergy.

Penicillin, the most well-known β-lactam antibiotic, is thought to cause allergic responses in 0.7-10% of the human population. Atopic and other allergy illnesses are believed to be developed and regulated in part by excessive secretion of Interleukin-4 (IL-4) and interferon-γ (IFN-γ). In this study, the frequency of penicillin allergy was analyzed by IL-4 and IFN-γ using the enzyme linked immunosorbent assay in 45 patients with an allergy to penicillin and 45 apparently healthy subjects as a control group. Also, we determined the IL-4 receptor α gene (IL-4 Rα) by using tetra primer-amplificat
