Analysis of somatic cell count, bacteria distribution and antimicrobial susceptibility in lactating dairy cows from small holder dairy farms in Kenya.

Subclinical mastitis is a major cause of economic loss and antimicrobial use in dairy cattle globally. In Kenyan smallholder systems, limited evidence on somatic cell counts (SCC), mastitis pathogens, and antimicrobial resistance patterns constrains effective control strategies. We conducted a cross-sectional study of 320 lactating cows from 119 smallholder farms in a peri-urban Kenyan county. Quarter milk samples (n = 1261) were cultured for Escherichia coli, Klebsiella spp., Staphylococcus aureus, and Streptococcus spp. and confirmed by MALDI-TOF MS. SCC was measured in 863 samples. Antimicr

A Quality Improvement Project to Decrease Inaccurate Penicillin Allergies in Resource-Limited Ambulatory Care Clinics.

BACKGROUND: There is a lack of data on the impact of implementing an interprofessional approach in ambulatory clinics to de-label penicillin-allergic patients (PA) in diverse and lower-income populations. METHODS: A quality improvement (QI) interprofessional adult PA assessment/intervention protocol was implemented at two resource-limited ambulatory sites over 4 months, utilizing a post-group-only quasi-experimental study design. Pharmacists performed allergy assessments/Pen-FAST scores with nonallergic histories directly de-labeled. Nonpregnant patients with Pen-FAST scores less than 3 were

Design and verification of a hyperspectral instrument for atmospheric and water optical properties observations.

Long-term, continuous in-situ observations of atmospheric and water optical properties are crucial for understanding the impacts of climate change, revealing pathways of atmospheric material transport, and studying air-sea interactions. For ocean color satellite sensors, sustained in-situ measurements are crucial in supporting their on-orbit calibration, the validation of high-level data products, and the development of data processing algorithms. This study presents what we believe to be a novel autonomous instrument called the hyperspectral sun photometer (HSP) designed for long-term, contin

[AND IT WAS NAMED PENICILLIN-CRUSTOSIN VIEM].

The article, on the basis of analysis of declassified archival documents and published sources, reveals the circumstances of identification of the strain that served as producer of the first National preparation of penicillin, named Crustozin. It is established that, since the middle of 1940s, among scientists circulated opinion about its specific affinity that differed from the one officially accepted in the USSR. The existing situation forced Professor L. I. Kursanov to take measures to protect his scientific reputation. The mode of action he choose permitted him to prove his professional co

Piperacillin-tazobactam tolerability in patients with a labeled penicillin allergy (PIPPEN).

BACKGROUND: Piperacillin-tazobactam is a beta-lactam antibiotic with a distinct R1 side chain compared to other penicillins, therefore the risk of cross-reactivity should be theoretically low. Despite this difference, antimicrobial guidelines recommend against using piperacillin-tazobactam in patients with penicillin allergies. There is limited data available regarding piperacillin-tazobactam use in patients with a penicillin allergy, or rates of cross-reactivity. Therefore, the objective of this study was to assess the incidence of cross-reactivity to piperacillin-tazobactam in patients with

Penicillin Allergy, Really?-A Cross-Sectional Mixed-Methods Study in Baden-Württemberg, Germany, to Explore General Practitioner Perspectives on Delabeling Potential in Primary Care.

Background: Most penicillin allergy labels are documented in early childhood and result from events of low risk for allergy. In Germany, evidence-based strategies to evaluate the likelihood of a true penicillin allergy are still lacking. As general practitioner input is indispensable regarding required resources for the implementation of successful delabeling strategies in outpatient care, a mixed-methods study in Baden-Württemberg, Germany explored untapped delabeling potential and conditions for successful initiatives based on their experiences, to support preservation of penicillin as a tre

Improved hydrolysis of piperacillin by OXA-48-like R214G variants, a selective advantage under piperacillin-tazobactam exposure.

OXA-48-like carbapenemases have rapidly disseminated worldwide, becoming the most common carbapenemase in many countries, with more than 60 variants reported. Among them, OXA-244 (OXA-48-R214G) and OXA-484 (OXA-181-R214G) are increasingly reported, despite overall reduced hydrolytic activities for β-lactams, including temocillin and carbapenems. R214, located in the β5-β6 loop, through the interaction with D159, is crucial for carbapenem hydrolysis by structuring the active site. R214G variants of OXA-48-likes were analyzed by β-lactam susceptibility testing, steady-state kinetic analyses in t

Alternative enzymatic pathways to penicillin antibiotics.

The discovery of penicillin, more than a century ago, has been one of the most significant advances in medicine. Despite the growing threat of antimicrobial resistance, which has rendered many other antibiotics ineffective, penicillin derivatives remain among the most widely prescribed antibiotics. Penicillin is biosynthesised by a large nonribosomal peptide synthetase (NRPS) enzyme, which assembles a tripeptide precursor ACV. This intermediate is subsequently cyclised by isopenicillin N synthase (IPNS) to form penicillin. ACV is similar in structure to glutathione, a ubiquitous, tripeptide an

Infections Caused by Fusobacterium Species: Microbiology, Clinical Syndromes and Management: A Narrative Review.

Fusobacterium spp. are Gram-negative, obligate anaerobic bacteria associated with a broad clinical spectrum, including head and neck infections; soft tissue infections; gastrointestinal and genitourinary infections; and bacteremia without an identified source. Clinical manifestations vary by species and site of colonization. F. necrophorum is linked to head and neck infections that may culminate in Lemierre syndrome, a life-threatening septic thrombophlebitis of the internal jugular vein. Although Fusobacterium bacteremia is uncommon, it is associated with substantial mortality. F. necrophorum

COMPARATIVE SUSCEPTIBILITY PROFILES OF CLINICAL AND REFERENCE BACTERIAL STRAINS ACROSS MULTIPLE ANTIBIOTIC CLASSES.

Antimicrobial resistance (AMR) is a major global public health threat, undermining the efficacy of commonly used antibiotics. Resistance patterns differ across bacterial taxa, including Enterobacteriaceae, non-fermenting Gram-negative bacilli, and Gram-positive cocci. This study aimed to provide a comparative analysis of antimicrobial susceptibility among reference strains with defined susceptible and resistant phenotypes, alongside selected clinical isolates, to evaluate the preservation of phenotypic traits and the impact of antibiotic use. Reference susceptibility strains exhibited high sus

Adaptive laboratory evolution of Lactobacillus gasseri-01 toward a safe probiotic for enhanced sulforaphane transformation.

Sulforaphane is a potent isothiocyanate with significant physiological activity. Its bioavailability from dietary sources is relatively low and relies on biotransformation by the gut microbiota from glucoraphanin. Lactobacillus gasseri (L. gasseri) is a promising candidate for enhancing sulforaphane transformation because of its probiotic safety and inherent myrosinase-like activity. However, the natural transformation efficiency of the wild-type strain is limited, necessitating enhancement of its transformation capability. This research employed adaptive laboratory evolution (ALE) to enhance

Cefazolin versus Alternative Antibiotics for Surgical Prophylaxis in Children With a Reported Penicillin Allergy.

INTRODUCTION: Cefazolin is the antibiotic of choice for prevention of surgical site infections for many pediatric surgical procedures. In patients with a reported penicillin allergy, cefazolin is often replaced with vancomycin or clindamycin. Outcomes may be worse when alternatives are used. MATERIALS AND METHODS: We performed a retrospective review among children (aged 0-18 y) who underwent surgical procedures (orthopedic, cardiac, neurosurgical, or plastic surgery) where cefazolin was indicated for surgical prophylaxis to determine the risk of developing a new drug allergy when cefazolin wa

Resilience of recombinant antibiotic resistance gene-containing plasmids against common cell culture disposal methods.

Antibiotics have saved an untold number of people and animals since penicillin's miraculous discovery in 1928. In the following half-century, progressive discoveries involving antibiotic resistance genes (ARGs), the microorganisms responsible, and their transferrable genetic material have yielded the tools necessary for genetic engineering, birthing the biotechnologies that continue to revolutionize healthcare. After half a century of antibiotic use in the biological sciences, we are, however, faced with an inconvenient question: what happens to residual antibiotics and ARG-containing recombin
