During the years 1978 and 1979 there has been a marked change in the proportion of bacteria isolated from dental infections compared with the ten year period to 1978. There has been a reduction in the proportion of streptococci found and an increase in the proportion of the Gram negative species encountered. There has been a steady decline in the effectiveness of penicillin although this has to some extent been offset by the introduction and widespread use of amoxycillin. The greatest change in antibiotic sensitivity has been the reduction in sensitivity to the tetracyclines.

Thirteen definite and 3 probable cases of chlamydial eye infection were diagnosed in young adults attending the Bristol Eye Hospital between June 1978 and May 1980, and incidence of about 1 case per 44000 per year in the 15 to 44-year-old community served by this hospital, and 1 per 100000 in the total population of this community. We treated 289 men who had nongonococcal urethritis with minocycline, 100 mg once or twice daily for 7 to 21 days. After 21 +/- 7 days, urethritis persisted or recurred in 31 (27%) of 114 given 7-day therapy and only nine (8%) of 110 given 21-day therapy (p = 0.0005). However, by 49 +/- 14 days, the cumulative percent rate of failure was 31% for 7-day and 30% for 21-day therapy. Thus, 21-day therapy only delayed recurrence. A simple, rapid bioassay for the measurement of chloramphenicol in serum or cerebrospinal fluid was developed using a multiply antibiotic-resistant strain of Escherichia coli. The agar diffusion system involved the addition of patients' specimens and three standard concentrations of chloramphenicol to 7.5 mm diameter wells cut in agar seeded with the test organism. A review of the side effects of antibiotics in human patients is presented. At least three classes of untoward reactions may be distinguished: development of drug hypersensitivity, toxicological hazards, and microbial effects such as emergence of drug resistance. Current agar dilution methods for susceptibility testing of Mycobacterium marinum versus the tetracyclines have failed to show in vitro susceptibility despite good in vivo results. We found that the tetracyclines were unstable in agar and resulted in a fine haze of growth for several concentrations before complete inhibition of growth was seen. In contrast, the aminoglycosides resulted in sharp minimal inhibitory concentration endpoints, which were generally independent of the length of incubation. The problems with the tetracyclines can be lessened by shortening the incubation time to 5 days

A 26-year-old man had been treated for acne vulgaris with tetracycline and minocycline orally for 12 years. We report two patients with quinidine-induced esophagitis along with a review which reveals drug-induced esophagitis to be uncommon. Certain drugs implicated time and again include tetracycline, doxycycline KCl, quinidine, clindamycin, and a British drug, emepronium. Other drugs are mentioned in isolated case reports.
