[STAT1 gain-of-function mutation leading to disseminated Talaromyces marneffei infection combined with hemophagocytic syndrome: a case report].

This study reports a 28-year-old HIV-negative male with a STAT1 gain-of-function mutation who presented with a systemically disseminated Talaromyces marneffei (TM) infection, which was complicated by hemophagocytic lymphohistiocytosis (HLH). The patient presented with recurrent fever, weight loss, oral mucosal ulcers, as well as lymphopenia and markedly elevated inflammatory markers during the acute phase of the illness. Imaging revealed scattered ground-glass opacities and nodular shadows in both lungs, as well as localized bronchiectasis, and splenomegaly. Metagenomic next-generation sequenc

Adjunctive GM-CSF therapy enhances host defense against systemic Candida auris infection in immunosuppressed mice.

INTRODUCTION: Candida auris is an emerging, multidrug-resistant fungal pathogen associated with high mortality in immunocompromised individuals. Resistance to current antifungal drugs emphasizes the need for new therapeutic approaches. We investigated granulocyte-macrophage colony-stimulating factor (GM-CSF) as a standalone immunotherapy and in combination with a sub-therapeutic dose of micafungin in an immunosuppressed mouse model of systemic C. auris infection. METHODS: Immunosuppressed ICR CD-1 mice (4-6 weeks old) were infected with C. auris and treated daily via intraperitoneal injection

Fusarium keratoplasticum infection in diabetic foot ulcers following antimicrobial therapy.

Fusarium species are important opportunistic fungal pathogens. Diabetic foot infection (DFI) involves diverse causative pathogens; however, the potential for fungal etiologies is often overlooked. We describe a case of DFI following foot trauma. Despite prior broad-spectrum antimicrobial therapy, the wound failed to heal and presented persistent swelling and pain. Microbiological re-evaluation and molecular identification confirmed the causative agent as Fusarium keratoplasticum (F. keratoplasticum), a pathogenic species within Fusarium solani species complex (FSSC). The isolate exhibited high

Harnessing secondary metabolites of endophytic microbes: a next-generation biopesticide for crop disease management.

This review highlights the potential of endophytic microorganisms and their secondary metabolites as innovative biopesticides for sustainable disease management in agriculture. Agriculture faces substantial challenges from phytopathogens, resulting in significant economic losses worldwide, which are typically addressed with synthetic pesticides that pose environmental and health hazards. Endophytic microorganisms residing within plant tissues without inducing disease provide a natural defence alternative by synthesising a variety of beneficial secondary metabolites, including alkaloids, terpen

Exploring the role of serotonin as a plant stress modulator.

Serotonin (5-hydroxytryptamine), an indoleamine with a dual evolutionary legacy in animals and plants, has transcended its initial classification as a secondary metabolite to emerge as a central regulator of plant stress adaptation. This review moves beyond cataloging stress-associated effects to propose a unified framework for serotonin as a dynamic signaling and metabolic hub. I synthesize evidence that serotonin's role is defined not merely by its antioxidant capacity, but by its sophisticated integration into the core stress-signaling circuitry of plants. The key to this function is its in

Cholesterolated Rapamycin Prodrug Liposomes Induce Antigen-Specific Tolerance and Enable AAV Redosing.

Antidrug antibodies (ADAs) compromise the pharmacokinetics and efficacy of biologics and can trigger adverse reactions. We engineer a tolerogenic liposomal platform in which rapamycin is covalently conjugated to cholesterol (RAPA-chol) and formulated as nanoliposomes (RA-c@L) to induce antigen-specific immune tolerance to coadministered proteins. Covalent anchoring enables high drug encapsulation (>95%) and improves colloidal stability. In a three-dose weekly tolerization regimen with uricase, followed by a high-dose challenge, RA-c@L markedly suppresses ADA formation: by day 42, antiuricase I

Triglycerides induce endoplasmic reticulum lipid bilayer stress to activate PERK and enhance antifungal immunity.

Invasive fungal infections remain a major clinical challenge due to limited antifungal drugs, drug toxicity, and resistance. Fungal infections trigger endoplasmic reticulum (ER) stress. However, the mechanism through which this stress response affects antifungal immunity remains unclear. Here, we showed that de novo triglyceride synthesis promotes antifungal innate immune signaling and proinflammatory gene expression in macrophages. Upon fungal stimulation, triglycerides induce ER lipid bilayer stress and activate the protein kinase R (PKR)-like ER kinase (PERK) branch of the unfolded protein

Profound cell wall remodeling in Candida parapsilosis during systemic infection confers simultaneous tolerance to echinocandins and host immunity.

Antifungal tolerance can promote the emergence of resistance yet often incurs fitness costs for fungal pathogens. How tolerant populations compensate for these deficits and how they may be therapeutically targeted remain poorly understood. Here, we investigate four sequential Candida parapsilosis isolates recovered from a patient with persistent candidemia and failure of micafungin therapy. The infection was ultimately cleared with liposomal amphotericin B (LAMB). Whole-genome sequencing (WGS) confirmed clonal relatedness and the absence of known resistance mutations. Later isolates displayed

Caspofungin enhances antifungal immunity in human blood through transcriptional reprogramming.

Candida albicans is a leading cause of invasive candidiasis, a life-threatening infection with high mortality despite antifungal therapy. Echinocandins are first-line agents due to their fungicidal activity and low toxicity profile. Beyond direct killing, they may also modulate host immunity, e.g. through fungal cell wall remodeling. However, the systemic impact of echinocandins on human immune cells remains incompletely understood, particularly in the context of drug resistance. Using an ex vivo human whole-blood infection model, we assessed the effects of caspofungin during infection with ec

Precision dosing of systemic antifungals in adults: therapeutic drug monitoring, empiric dose optimization and barriers to implementation.

Therapeutic drug monitoring (TDM) is essential for optimizing systemic antifungal therapy, particularly for agents with narrow therapeutic windows, variable pharmacokinetics (PK) or established exposure-response relationships. By enabling individualized dosing, TDM improves efficacy, reduces toxicity and helps prevent resistance. This review synthesizes current evidence and recommendations for antifungal TDM across available and emerging agents, including triazoles, flucytosine, echinocandins, amphotericin B, and novel therapies such as ibrexafungerp, olorofim and fosmanogepix. "Should I stay or should I go"-a kinase delays escape of Candida glabrata from macrophages.

Candida glabrata is an opportunistic fungal pathogen that causes both superficial and systemic infections in humans, accounting for 15%-25% of invasive candidiasis cases. Macrophages play a crucial role in antifungal immunity by internalizing C. glabrata; however, the fungus has evolved strategies to survive and even proliferate within phagosomes. It has been suggested that C. glabrata may utilize its life within macrophages to evade immune detection and disseminate throughout the body. We observed that, compared to fungi like C. albicans, C. glabrata only slowly escapes from macrophages, with

Two-stage revision combined with anti-infective therapy for mixed fungal and bacterial knee periprosthetic joint infection: a case report and literature review.

BACKGROUND: Fungal periprosthetic joint infection (PJI) is rare and challenging to manage, particularly when complicated by concurrent bacterial infection. Evidence guiding optimal surgical strategies, the role of antifungal-loaded cement spacers, and the duration of systemic antimicrobial therapy remains limited. CASE PRESENTATION: We report a single case of mixed fungal–bacterial knee PJI in a 61-year-old woman following total knee arthroplasty. Multiple intraoperative cultures yielded Candida parapsilosis and Staphylococcus warneri. The patient was treated using a planned two-stage revision

Exploring the antifungal effects and underlying mechanism of Carica papaya seed essential oil against Candida tropicalis in vitro and in vivo.

ETHNOPHARMACOLOGICAL RELEVANCE: Carica papaya seeds can improve indigestion, relieve rheumatic pain and be applied as a paste for treating ringworm and psoriasis in traditional medicine, which indicates that they possess antibacterial, antifungal, and anti-inflammatory properties. As one of the most common non-albicans Candida species, Candida tropicalis has led to a notable rise in morbidity and mortality rates because of increased drug resistance. Papaya seed essential oil (PSEO) contained over 99% benzyl isothiocyanate and exhibited antifungal activity against C. tropicalis, particularly fl

Simple, stable, non-toxic nanoemulsion of fluconazole containing imidazolium-based ionic liquids with acetate and amino acid-based anions.

In this work, imidazolium-based ionic liquids (ILs) bearing a 12-carbon alkyl chain (C12MIM) and carboxylate anions (acetate and phenylalaninate) were assessed as surfactants in the formulation of fluconazole-loaded nanoemulsions. Emulsification methodologies using ultrasound, ultra-turrax, and a combination of both were employed. Results showed that ILs with carboxylate anions are effective surfactants for producing stable fluconazole-loaded emulsions. Ultrasound was the most efficient emulsification method, yielding nano-sized, spherical droplets with low polydispersity and high stability. A

Multifunctional theranostic magnetic PLGA nanoparticles encapsulating cyclosporine A: addressing challenges in pancreas transplantation for type 1 diabetes.

Type 1 Diabetes (T1D) is a high-incidence chronic autoimmune disease, with patients requiring lifelong insulin therapy. In the most severe cases, pancreas transplantation (PTA) arises as the first choice of treatment for these patients in the hope of achieving insulin independence. However, the long-term success of PTA is hindered by ischemia-reperfusion injury (IRI) and immune rejection, both of which limit graft survival. To address these challenges, we have developed multifunctional theranostic nanoparticles (t-PLGA NPs) co-encapsulating Fe3O4 and MnO NPs, along with cyclosporine A, an immu
