The increased understanding of the pathophysiology of acute lung injury that has been achieved over the last decade has led to several new pharmacologic approaches for the prevention and management of ARDS. Based on in vitro information and animal models, many of these strategies are quite compelling. Nevertheless, to date, no specific pharmacologic approach for the prevention or treatment of ARDS has been validated conclusively in clinical trials. Itraconazole, an oral antifungal agent, was evaluated for the clinical efficacy and safety in patients documented or suspected of systemic fungal infection (SFI) complicated with hematological malignancies. The data was also evaluated according to serological tests and fungal culture. Out of a total of 79 patients, the clinical efficacy of itraconazole was evaluated in 52 patients, comprising 4 patients with proven SFI, 33 with clinical SFI, and 15 with suspected SFI. The dosage of cyclosporine administered in the treatment of autoimmune diseases has generally been comparable to those required in cases of transplantation. Here we report on the successful treatment using an extremely low dose cyclosporine (1 mg/kg/day) on four patients, involving thrombocytopenia with systemic lupus erythematosus, and interstitial pneumonitis with Sjögren's syndrome, and discuss the optimal dose of cyclosporine for autoimmune-mediated manifestations.

Terbinafine is the first orally active allylamine. Though it is available since several years, experience in children is rather limited. We report about five cases of dermatophytoses in children successfully treated with oral terbinafine.
