The Gain-of-Function GLP1R R131Q Variant Preserves β-Cell Function and Enhances Response to GLP-1 Receptor Agonists.

OBJECTIVE: Genetic variation in the glucagon-like peptide 1 receptor (GLP1R) has been implicated in type 2 diabetes (T2D) risk and treatment response. We investigated whether GLP1R R131Q, which was previously reported as a T2D-protective variant in genome-wide association studies, alters receptor signaling, β-cell function, and response to glucagon-like peptide 1 receptor agonists (GLP-1RAs). RESEARCH DESIGN AND METHODS: Trajectory of β-cell function was evaluated in a 20-year community-based prospective cohort in Korea (n = 6,373), where participants underwent biennial 2-h 75-g oral glucose

Knowledge, attitude, and contraceptive use among married women attending a maternity hospital in Kabul, Afghanistan: a cross-sectional study.

BACKGROUND: Family planning is a foundational component of reproductive health that empowers individuals to make informed decisions about childbearing. In Afghanistan, where maternal mortality remains critically high and contraceptive prevalence is low, understanding family planning dynamics is essential for improving reproductive outcomes. This study assessed knowledge, attitudes, and contraceptive use among married women attending a major maternity hospital in Kabul. METHODS: A cross-sectional analytical study was conducted among 356 married women attending Rabia Balkhi Maternity Hospital i

Stress-induced maternal effects on offspring: linking multivariate egg investment to environmental quality.

Hormone-mediated maternal effects can shape offspring development in variable environmental conditions resulting in adaptive behavioral phenotypes. Numerous hormones can vary with environmental conditions and lead to different adaptive offspring responses. However, most of what we know about hormone-mediated maternal effects comes from studies on individual hormones even though the endocrine response to the environment is multifaceted and complex. Physiological stress responses in mothers may lead to greater offspring exposure to glucocorticoids or influence the allocation of other classes of

Oral Insulin Enhanced by Dual-Ligand-Modified Nanoparticles via Trans-Golgi Network-Mediated Transport.

Oral insulin delivery is challenged by a cascade of gastrointestinal barriers, including mucus entrapment, poor apical endocytosis, extensive lysosomal degradation, and insufficient basolateral exocytosis. To overcome these obstacles in a coordinated fashion, we engineered a dual-ligand PLGA nanoparticle (PLGA-Tf/BAC-NPs) decorated with transferrin (Tf) and the sorting peptide BAC (ADDIDLLK). Tf facilitates efficient apical endocytosis via transferrin receptor (TfR) recognition, whereas BAC not only participates in AP-2-mediated internalization but, more critically, reroutes intracellular traf

Intestinal Macrophage-Mediated Exosomal Repackaging Establishes Bioactive Reservoirs for Long-Acting Oral Peptide Therapy.

Oral peptide delivery systems are conventionally engineered to improve intestinal absorption, but they rarely address the progressive and systemic nature of chronic inflammatory disorders such as type-2 diabetes (T2D), which requires both durable glycemic control and immunometabolic management. Here, we report an oral delivery system based on porous calcium carbonate/sodium alginate (PCC/SA) microparticles loaded with exenatide-chitosan oligosaccharide (EXE-COS). The PCC/SA carrier facilitates the efficient intestinal absorption and transepithelial transport of EXE-COS. Notably, in the lamina

A randomized controlled double-blinded trial to assess the risk of gestational diabetes mellitus in non-anemic pregnant women who receive oral iron supplementation.

BACKGROUND: Pregnant women in India are routinely prescribed oral iron supplements as part of the national Anemia Mukt Bharat (AMB) program for anemia control. In women with iron-deficiency anemia, the benefits of iron supplementation during pregnancy, both for the mother and child, are very clear. In non-anemic pregnant women, however, the benefits of iron supplementation vis-à-vis associated risks that have been reported have not been studied adequately. Studies have suggested that iron may impair insulin secretion, thus increasing the risk of gestational diabetes mellitus (GDM). PRIMARY OB

Cardiovascular and thromboembolic risks of sex hormone therapy: from contraception to transgender care.

The risk of venous thromboembolism is increased with combined oral contraceptives, rises with the dose of ethinylestradiol, and is higher with desogestrel, gestodene, drospirenone or cyproterone acetate than with levonorgestrel; the determinant is not the generation of the pill but the residual androgen-receptor activity of the progestin, which modulates the hepatic effect of the estrogen. Age over 35 years, obesity, smoking and a family history of thrombosis amplify it. Pills based on a native estrogen have a more limited hepatic impact and rank below most ethinylestradiol combinations in con

Comparative efficacy and safety of oral pharmacological and complementary interventions for premenstrual syndrome: a protocol for a systematic review and a network meta-analysis.

BACKGROUND: Premenstrual syndrome (PMS) affects around 48% of women of reproductive age worldwide, interfering with psychological and physical well-being as well as functioning. A multitude of interventions exists for PMS, including oral pharmacological interventions, such as antidepressants, combined oral contraceptives, or alternative and complementary interventions. While the use of pharmacological interventions for PMS is linked to multiple side effects, evidence is limited for complementary interventions. Further, placebo effects are high in PMS intervention studies, affecting the estimat

Antihyperglycemic and pancreatic tissue-preserving effects of L. sativum oil in streptozotocin-induced diabetic rats: exploratory analysis of PDX-1 and GCK.

BACKGROUND: Diabetes mellitus constitutes a major global health burden. L. sativum has long been employed in traditional medicine for glycemic management, an effect attributed to its rich phytochemical constituents, including flavonoids, phenolic acids, and glucosinolates. This study investigated the ameliorative effects of L. sativum oil against streptozotocin-induced type 1 diabetes in rats, with particular emphasis on pancreatic and duodenal homeobox 1 (PDX-1) and glucokinase (GCK) gene expression. METHODS: A total of 48 young male Wistar rats were randomly divided into four experimental g

GLP-1 Receptor Agonists and Reproductive Health: Implications for Contraception, Fertility, and Pregnancy.

Glucagon-like peptide-1 receptor agonists (GLP-1 RAs) have rapidly expanded in use for the treatment of obesity and type 2 diabetes, including among women of reproductive age. Emerging evidence suggests that GLP-1 signaling may influence reproductive physiology through central effects on the hypothalamic-pituitary-ovarian axis and indirect metabolic pathways. In women with polyendocrine metabolic ovarian syndrome (PMOS), GLP-1 RAs improve insulin resistance and androgen profiles and may improve menstrual regularity. Most GLP-1 RAs do not significantly affect oral contraceptive pharmacokinetics

Genetic polymorphisms and early responses predict refractory hyperthyroidism during antithyroid drug therapy.

Antithyroid drugs (ATDs) are the first-line treatment for Graves' disease (GD), but variable outcomes require early risk stratification. We analyzed 578 adults, including 240 in a randomized trial of dose titration versus modified partial block-and-replace therapy and the remainder in a temporal observational cohort. Baseline characteristics, 3-month treatment indicators, and 13 GD-susceptibility single nucleotide polymorphisms (SNPs) were evaluated. An ATD-levothyroxine dose-correction method quantified thyroid function and autoantibody changes, informing clinical and SNP risk score-integrate

Revisiting the β-Cell: The Key to the Type 2 Diabetes Puzzle: The 2025 Banting Medal for Scientific Achievement Award Lecture.

Major discoveries in the past have laid the foundation on which modern diabetes research has been built, and future findings can be anticipated. From this groundwork, we have pursued studies evaluating the role of β-cell function and β-cell mass in the pathogenesis of type 2 diabetes. In considering β-cell function, studies of β-cell responses to intravenous and oral glucose have highlighted the critical nature of interpreting these responses in the face of the prevailing insulin sensitivity, and not as isolated responses. The critical role of β-cell dysfunction in the pathogenesis of prediabe

The Use of the Drospirenone 4 mg-Only Pill in Overweight and Obese Women with Polyendocrine Metabolic Ovarian Syndrome: A Retrospective Cohort Study.

Objective: Polycystic ovary syndrome is frequently associated with hyperandrogenemia-related hirsutism, which creates notable cosmetic concerns for many affected women. Although multiple antiandrogenic treatments exist, many patients remain dissatisfied, and combined oral contraceptives are often contraindicated due to metabolic and cardiovascular risk factors. This study evaluates whether a drospirenone-only pill improves hormonal profiles and hirsutism in PMOS compared to cyclic dydrogesterone. Methods: This retrospective cohort study assessed hyperandrogenemia and hirsutism using the modifi

Cross-sectional analysis of 152 hyperthyroid cats presented for radioactive iodine therapy in South Korea.

INTRODUCTION: Feline hyperthyroidism is one of the most common endocrinopathies in cats; however, its epidemiological characteristics remain poorly understood in South Korea. Although radioactive iodine therapy (RAIT) is a definitive treatment, studies focusing specifically on cats with hyperthyroidism undergoing RAIT are limited. METHODS: Medical records of 152 cats with hyperthyroidism presented for RAIT were retrospectively reviewed to describe their epidemiological and clinical features. RESULTS: The median age at diagnosis was 12 years, and 30.3% of the cats were diagnosed incidentally.

Positioning gestrinone among the progestins: a narrative review of pharmacology, efficacy, safety, and tolerability.

INTRODUCTION: Gestrinone is an androgenic progestin derived from 19-nortestosterone, in use for four decades in endometriosis and other estrogen-dependent conditions. It has largely disappeared from contemporary progestin taxonomies and is sometimes described as an anabolic-androgenic steroid. This review examines its pharmacology and the evidence for its use. AREAS COVERED: Narrative review without a prespecified protocol, based on searches of PubMed, trial registries and regulatory sources, without date or language limits. It integrates chemistry and multireceptor pharmacology, pharmacokine

Small Molecule Insulin Sensitisers: New Leads and Targets for Next-Generation Insulin Sensitising Strategies.

Type 2 diabetes (T2D) is a major global health problem driven largely by insulin resistance, the impaired cellular response to insulin. Few current therapies directly address this underlying cause. Thiazolidinediones, potent peroxisome proliferator-activated receptor gamma (PPARγ) agonists, remain the only true small-molecule insulin sensitisers, but their clinical use is limited by adverse effects associated with non-selective activation of this target. Consequently, research has shifted toward alternative pathways that enhance insulin signalling without relying on PPARγ agonism. This review

Metreleptin improves glucose metabolism and hepatic steatosis in a patient with Werner syndrome and partial lipodystrophy: a case report with review of literature.

Werner syndrome (WS) is a rare autosomal recessive progeroid disorder caused by mutations in the WRN gene and is characterized by distinctive physical features and metabolic complications, including insulin resistance, diabetes, hypertriglyceridemia, and hepatic steatosis. Although lipodystrophy is a recognized component of WS, clinical reports of metreleptin use in this setting remain very limited. We report the case of a 38-year-old woman with WS complicated by partial lipodystrophy, diabetes with suboptimal glycemic levels, hypertriglyceridemia and severe hepatic steatosis. She presented wi
