Drug cross-reactivity between methimazole and propylthiouracil causing recurrent pancreatitis in thyroid storm.

SUMMARY: Drug-induced pancreatitis represents 1.1% of acute pancreatitis cases. To date, eight cases of antithyroid drug (ATD)-induced pancreatitis have been reported, including seven associated with methimazole (MMI) and one with carbimazole; however, no cases related to propylthiouracil (PTU) have been reported in the literature. We report a patient with thyroid storm who developed acute pancreatitis during MMI therapy, with recurrence following PTU substitution, suggesting cross-reactivity between antithyroid agents. The patient demonstrated elevated serum immunoglobulin G4 (IgG4) concentra

Management and presentation of pediatric thyroid eye disease: A systematic review and meta-analysis.

BackgroundPediatric Thyroid Eye Disease (TED) is a rare autoimmune condition primarily associated with Graves' disease. Although usually milder than adult TED, it can still cause functional and psychological morbidity. Pediatric management remains unclear due to the absence of specific guidelines and concerns about treatment-related risks such as growth suppression.MethodsWe conducted a systematic review and meta-analysis following PRISMA guidelines, searching MEDLINE, Embase, and Emcare from inception to March 2024. Studies included interventional and observational reports involving patients

Empirical Thyrotropin-Free Thyroxine Relationship Across the Thyrotropin Spectrum and Implications for Reference Intervals.

BACKGROUND: The relationship between thyrotropin (TSH) and free thyroxine (FT4) is commonly assumed to be log-linear. However, this assumption may oversimplify hypothalamic-pituitary-thyroid axis regulation. Characterizing the empirical structure of this relationship could inform the interpretation of thyroid function tests and the validity of current TSH reference intervals. Segmented regression was used to examine the TSH-FT4 relationship to determine whether statistically derived TSH breakpoints corresponded to the current reference limits. METHODS: This study retrospectively analyzed 782

Nanozymes with controllable inhibition modes assist primary antithyroid drug screening.

The enzyme inhibition effect is vital in drug development. However, the high cost of enzymes becomes the main limitation. Hence, we report low-cost PtNP/NC nanozymes for screening antithyroid drugs via inhibition modes. As a concept application, a dual-channel sensor array accomplishes drug efficacy screening, laying a foundation for nanozyme-based inhibitor evaluation.

Predictors of Treatment Outcomes in Pediatric Graves' Disease.

BACKGROUND: Graves' disease (GD), the most common cause of hyperthyroidism in children, is primarily treated with antithyroid drugs (ATDs). The aim of this study was to evaluate factors that could predict remission, relapse, and the need for persistent high doses of methimazole (MMI) in pediatric GD. METHODS: This was a retrospective study of medical records that included GD diagnosed before 19 years of age from January 2004 to December 2023. Remission was defined as maintaining the euthyroid state for more than 6 months after stopping ATDs. The high-dose group was defined as those receiving

Impact of antithyroid drug duration on radioactive iodine therapy efficacy in Graves' disease in Japan: a propensity score-matched analysis using thyroid weight-based outcomes.

INTRODUCTION: Antithyroid drugs (ATDs) are commonly administered prior to radioactive iodine therapy (RAIT) for Graves' disease (GD), but their impact on RAIT efficacy remains unclear, largely due to insufficient adjustment for confounding factors in previous studies. METHODS: This retrospective study evaluated the impact of ATD treatment duration on RAIT outcomes using propensity score matching (PSM) to control for key covariates. Eighty patients with GD who underwent RAIT between 2015 and 2023 were classified into short-term (S group, <3 months) and long-term (L group, ≥3 months) ATD treatm

Financial Burden, Insurance Type, and Medication Adherence Among Community-Managed Patients with Hyperthyroidism: A Cross-Sectional Study from Nanning, China.

BACKGROUND: Hyperthyroidism necessitates prolonged pharmacotherapy; however, adherence is frequently inadequate in community settings. Although knowledge gaps are frequently cited, the impact of socioeconomic factors and health system fragmentation - particularly in middle-income countries undergoing health reforms - remains less understood. The present study aimed to investigate the prevalence of non-adherence and its association with socioeconomic status, out-of-pocket expenditure, and health insurance schemes among community-dwelling patients with hyperthyroidism in urban China, with a spec

TSHR Gly413Ser in familial nonautoimmune hyperthyroidism: a case report.

OBJECTIVES: Familial nonautoimmune hyperthyroidism (FNAH) is a rare disorder caused by activating germline variants in the thyroid-stimulating hormone receptor gene (TSHR) and can mimic antibody-negative Graves' disease. We report a family with hyperthyroidism and the TSHR variant Gly413Ser and provide clinical, segregation, and functional evidence that this p10-region variant is a novel cause of FNAH. CASE PRESENTATION: The disorder was recognized after a 15-year-old boy presented with abdominal pain and diarrhea and was found to have suppressed thyrotropin, high-normal free thyroxine, and s

Traditional Chinese Medicine as an Adjunct in Hyperthyroidism Management: A Narrative Review of Pathophysiology, Proposed Mechanisms, and Clinical Evidence.

Hyperthyroidism, characterized by elevated serum thyroxine levels, can be classified into overt and subclinical forms. Untreated hyperthyroidism poses substantial health risks. Antithyroid drugs, radioactive iodine, and surgery are established conventional therapeutic options; however, their clinical application may be limited by relapse, adverse effects, contraindications, or patient preference. This narrative review examines the epidemiology of hyperthyroidism, particularly its association with iodine status, strong genetic predisposition, clinical manifestations predominantly seen in women

Maternal TSH-receptor antibodies predict fetal/neonatal hyperthyroidism in pregnancies with a history of Graves disease.

CONTEXT: Transplacental transfer of thyrotropin receptor antibodies (TRAbs) during pregnancy may cause fetal/neonatal thyroid dysfunction. Current guidelines recommend a universal TRAb titer threshold, irrespective of gestational age or assay platform. However, evidence supporting trimester-specific and assay-specific thresholds is lacking. OBJECTIVE: This work aimed to determine whether trimester-specific TRAb levels predict fetal/neonatal thyroid dysfunction and to establish assay-specific cutoff values for clinical risk stratification. METHODS: In this prospective, multicenter, observatio
