Variable hyperthyroidism outcomes related to different treatment regimens: an analysis of UK Biobank data.

BACKGROUND: UK guidance on the assessment and management of thyroid disease was set out in NICE guideline NG145 in 2019 and is expected to result in an increase in radioactive iodine (RAI) being offered as a first-line definitive treatment for hyperthyroidism. METHODOLOGY: In this work we analyse longitudinal UK Biobank data to assess all-cause mortality and comorbidity risks associated with the main treatment modalities for 793 participants with hyperthyroidism, specifically antithyroid drugs (ATDs), RAI and thyroidectomy. RESULTS: Participants treated with RAI showed reduced all-cause mort

Estimation of kidney function in Graves' disease using creatinine and cystatin C.

INTRODUCTION: There is no consensus regarding methods to estimate kidney function in hyperthyroidism. The aim was therefore to assess changes in filtration markers in patients with Graves' disease undergoing treatment with antithyroid drugs. METHODS: Thirty patients with de novo Graves' disease were included. Blood sampling, including thyroid-stimulating hormone, fT3, fT4 and creatinine, was performed at baseline, 6 weeks, 3, 6, 12 and 24 months. Cystatin C was measured from frozen samples. To calculate creatinine- and cystatin C-based eGFR, the Lund-Malmö equation (LMR) and the CAPA formula

Thyroid peroxidase antibodies and their role in predicting outcomes in Graves' disease treatment.

INTRODUCTION: Graves´ disease (GD) is the predominant cause of hyperthyroidism. Treatment options include antithyroid drugs (ATD), surgery, and radioactive iodine ablation (RI). Although thyroid peroxidase antibodies (anti-TPO) are prevalent in patients with GD, their role in driving relapse or hypothyroidism after treatment in patients with GD remains unclear. This study aimed to determine if patients with anti-TPO at GD diagnosis are more likely to relapse after ATD or RI treatment, and if patients with anti-TPO are at increased risk of developing hypothyroidism post-ATD treatment. METHODS:

Radioiodine Exhalation Following Oral I-131 Administration in a Mouse Model.

Background: The exhalation of radioiodine following radioiodine therapy (RIT) presents a challenge in radiation protection, though the mechanisms remain incompletely understood. Previous studies have indicated that radioiodine is predominantly exhaled in an organically bound form in humans. Methods: This study investigates the chemical composition and exhaled amounts of radioiodine, as well as the impact of thyroid-targeted pharmacological interventions, using a controlled mouse model. Female Balb/c mice (25 g) were administered oral doses of radioiodine (0.1, 1, 2, 10, and 23 MBq per animal)

Analysis of Cost and Treatment Effects in the Care Given for Graves' Disease: A Swedish Cost-Utility Analysis.

BACKGROUND: Guidelines in healthcare should be evidence-based, satisfy patient needs and improve patient outcome. METHODS: We performed a cost-utility analysis in Graves' disease (GD) and estimated incremental costs after the introduction of a national guideline adding the Graves' Recurrent Events After Therapy (GREAT) score with genetic determinants (GREAT+) to predict recurrence, a thyroid nurse, preoperative calcium/vitamin D treatment and thyroid-stimulating immunoglobulins. FINDINGS: Antithyroid drugs (ATDs) were less costly, achieved 0.88 quality-adjusted life years (QALYs) over 8 year

Comparing antithyroid drugs vs. radioactive iodine in paediatric Graves' disease: literature review.

INTRODUCTION: Paediatric Graves' disease (PGD) is an autoimmune condition, which if left untreated, can result in cardiac complications. National Institute for Health and Care Excellence (NICE) Guidance (NG145) advocates the use of antithyroid drugs (ATD) as first-line therapy for PGD, with a consultation to consider a move to definitive therapy in the form of radioactive iodine (RAI) or thyroidectomy if the initial 2-year course failed to achieve normal thyroid function. We aim to evaluate the effectiveness, adverse events, and potential predictors of remission for ATD and RAI in treating PGD

Carbimazole-induced acute myeloid leukemia: A very rare case report and review of the literature.

Neutropenia is a common side effect in hyperthyroid patients with long-term use of antithyroid drugs. Antithyroid drugs have been used for the management of hyperthyroidism and rarely may cause life-threatening side effects such as pancytopenia and very rarely may cause acute leukemia. The current case report is of a 34-year-old man diagnosed with hyperthyroidism who presented with pancytopenia after treatment with carbimazole for past 7 months; he initially improved with supportive treatment. The patient restarted treatment with carbimazole but with a reduced dose. He was readmitted for fever

Unilateral Graves' disease: a case report with concomitant thyroid cancer and systematic review of literature.

INTRODUCTION: Unilateral uptake (i.e., increased radiotracer in one lobe) on a thyroid scan in a patient with Graves' disease (GD) is the distinctive feature of unilateral GD (UGD), representing a rare entity and variant of GD with few documented cases to date. Considering the diagnostic and therapeutical implications of the knowledge of this form of GD, this study was designed to bring more light on the UGD entity within the bilobar thyroid gland. EVIDENCE ACQUISITION: The Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines were followed in reporting this s

Owners' Experience and Satisfaction with Radioiodine Treatment in Hyperthyroid Cats-A Prospective Questionnaire Study.

Radioiodine (RAI) treatment (RAIT) is considered the gold standard for treatment of feline hyperthyroidism. This study aimed to assess owners' motivation, concerns and satisfaction with RAIT and health-related quality of life (HRQoL) changes in RAI-treated cats. Two surveys (before and six months post-RAIT) were sent to owners of cats scheduled for RAIT between April 2023 and March 2024. The owners of 78 and 68 cats completed the first and the second surveys, respectively. The main reasons for choosing RAIT were that RAIT was considered the gold standard treatment (n = 27/78; 35%) and difficul

Precocious puberty in male with hypertension and hypokalemia; a definite diagnostic clue for 11β hydroxylase deficiency CAH.

SUMMARY: 11β-hydroxylase deficiency (11βOHD) (5-7%) is an uncommon cause of congenital adrenal hyperplasia compared to 21-hydroxylase deficiency (21OHD) (90%). We report a case of a 5-year-old boy who presented with gonadotropin-independent precocious puberty along with hypertension, generalized hyperpigmentation and hypokalemia. 17-hydroxyprogesterone came raised along with low stimulated cortisol. With the unavailability of urinary steroid profiling and serum 11-β deoxycortisol levels, we went ahead with molecular analysis of the patient. It confirmed exon deletion (1-6) of CYP11B1 and exon

Atomic Ga Site Enables Photonanozymes with Specific Inhibition Modes for Primary Drug Screening.

Enzyme inhibition plays a crucial role in drug discovery by governing interactions between molecules and distinct enzymatic sites, facilitating the identification of early drug candidates. However, most nanozymes have been limited to single active site inhibition models, leaving gaps in understanding inhibitor interactions and their dynamic modulation by external stimuli. Hence, an in-depth understanding of nanozyme inhibition across different functional domains, particularly under external stimuli like light irradiation, remains challenging. Herein, we report a carbon nitride photonanozyme wi

Thyroid Storm: A Case Report.

Thyroid pathology is quite common worldwide. Thyroid storm is a severe form of hyperthyroidism, particularly in Graves' disease, which is rare. It is in association with various triggering factors (therapy discontinuation, infections, surgical interventions) and can lead to multiorgan dysfunction, particularly liver and cardiovascular injury. Prompt diagnosis and early therapy are fundamental to solving the thyroid storm and its complications. Multiple therapies are needed for adjustment according to the particular case, including the use of synthetic antithyroid drugs, beta-blockers, corticos

Comparison of the Changes in Quality-of-Life of Cats with Hyperthyroidism Treated with Radioiodine or Antithyroid Drugs-A Preliminary Study.

A previous cross-sectional study found impaired health-related quality of life (HRQoL) in hyperthyroid cats, but the longitudinal impact of treatment-especially differences between radioiodine (RAIT) and antithyroid drug (ATD) approaches-remains unclear. This study aimed to evaluate changes in HRQoL in recently diagnosed (<6 months) hyperthyroid cats that underwent RAIT or received ATD. HRQoL was assessed using the validated HyperthyroidismQoL-cat questionnaire at baseline and after 1, 3, and 6 months, alongside thyroid status (eu-/hypo-/hyperthyroid) evaluation. Mixed-effects model analysed t

2025 Korean Thyroid Association Management Guidelines for Radioactive Iodine Therapy in Patients with Hyperthyroidism.

Hyperthyroidism is a condition marked by excessive thyroid hormone production, most commonly due to Graves' disease. Treatment options include antithyroid drugs (ATD), radioactive iodine (RAI) therapy, and thyroidectomy. To develop standardized clinical recommendations for RAI therapy with a focus on safety, efficacy, and monitoring, the Korean Thyroid Association formed a task force to create evidence-based guidelines. Six key clinical questions were identified through expert consensus, and a systematic literature review from 2013 to 2022 was conducted. Clinical indications for RAI therapy we

The relationship between levothyroxine dosage and free thyroxine levels in hypothyroid patients: a large retrospective study.

OBJECTIVE: This study examined the relationship between levothyroxine dosage and free thyroxine levels in hypothyroid patients. The aim was to ascertain whether elevated free thyroxine in treated patients suggests overmedication or is essential for maintaining appropriate free triiodothyronine levels, guiding improved monitoring practices during therapy. METHODS: A retrospective analysis was conducted on 3,020 free thyroxine measurements from 1,409 patients between July 2021 and March 2024. Patients with thyrotropin receptor antibodies or treated with antithyroid drugs such as thiamazole, pro

Comparison of mortality and cardiovascular morbidity following treatment for hyperthyroidism: A systematic review and bayesian network meta-analysis.

INTRODUCTION: Therapeutic approaches for hyperthyroidism, namely antithyroid drugs, radioiodine, and thyroidectomy, aim to restore normal thyroid function while minimizing harm. However, the difference in outcomes related to mortality and cardiovascular diseases between them remains controversial. METHODS: PubMed, Embase, Cochrane Central, and Web of Science were searched for randomized controlled trials and cohort studies that compared any of the three supracited therapeutic approaches between themselves. A Bayesian model estimated hazard ratios (HR) with 95% credible intervals (CrI) using a
