Population-based trends in antidiabetic medication utilization during the COVID-19 pandemic: results from the PANDUTI-TR study.

AIM: We aimed to examine changes in antidiabetic drug utilization associated with implementation and subsequent easing of pandemic-related restrictions. BACKGROUND: The COVID-19 pandemic and associated restrictions disrupted healthcare access and glycemic regulation, affecting diabetes management. METHODS: We collected nationwide outpatient antidiabetic drug sales and projected prescribing data from IQVIA Turkey between 01.03.2018 and 31.12.2022. We assessed average monthly consumption, expenditure, and quarterly prescribing trends across 'before restrictions' (BfR, 01.03.2018-31.03.2020), '

Converging TCF7L2 and CDKAL1 pathways in the pathogenesis of type 2 diabetes mellitus.

Type 2 diabetes mellitus develops when insulin resistance raises insulin demand and pancreatic beta cells fail to sustain insulin supply. Genome-wide association studies support a polygenic architecture and enrich for loci that affect beta cell stimulus secretion coupling and secretory stress responses. Polygenic risk scores summarize liability across many variants and guide genetic stratification in cohort studies. This review uses TCF7L2 and CDKAL1 as two mechanistically interpretable loci to connect inherited variation to measurable physiology. TCF7L2 risk alleles reduce incretin potentiati

Factors associated with poor glycaemic control among adults with type 2 diabetes mellitus in Albania: a cross-sectional study.

BACKGROUND: Poor glycaemic control is common among adults with type 2 diabetes mellitus (T2DM) in low- and middle-income settings in South-Eastern Europe, yet patient-level data on its correlates from Albania are scarce. We examined the demographic, clinical and treatment-related factors associated with poor glycaemic control (HbA1c ≥ 7%) in Albanian adults with T2DM. METHODS: Cross-sectional study of 176 consecutive adults with T2DM attending a hospital-based outpatient diabetes service in northern Albania. Poor glycaemic control was defined a priori as HbA1c ≥ 7% (53 mmol/mol). Sociodemogra

Equine Metabolic Syndrome and Insulin Dysregulation.

Equine metabolic syndrome (EMS) is defined as a collection of metabolic and clinical features that contribute to the development of hyperinsulinemia-associated laminitis (HAL). Insulin dysregulation (ID) is the hallmark feature of EMS and a risk factor for development of HAL. There are several diagnostic tests that can be used to diagnose various forms of ID in horses and ponies. Treatment of EMS/ID includes providing an appropriate diet, exercise program, and pharmacologic intervention in some severe cases.

Glucose-oxidase-integrated zinc-based metal-organic framework (GOx@ZIF-8) nanoparticles as a dual pH/glucose-responsive insulin delivery system for type 2 diabetes management: Synthesis, characterization, and in vitro/in vivo evaluation.

Type 2 diabetes mellitus (T2DM) affects over 537 million adults worldwide; conventional insulin therapy suffers from poor oral bioavailability, enzymatic degradation, and hypoglycemia risk. We developed a dual pH- and glucose-responsive PEG-GOx@ZIF-8-Ins nanoplatform for oral insulin delivery via biomimetic co-precipitation. The optimized nanoplatform (178 ± 7.0 nm; zeta potential -26.5 ± 1.8 mV; surface area 891 m2/g) achieved 79.8 ± 2.7% encapsulation efficiency and 87.4% GOx retention. Release was minimal under gastric conditions (<10%) but reached 97.2% at 25 mM glucose, with high glucose

Safety Assessment and Hypoglycemic Effect of Moringa oleifera Lam. Seed Lectin (WSMoL) in Alloxan-Induced Diabetic Mice.

Moringa oleifera Lam. seeds contain WSMoL, a lectin previously studied for toxicity and pharmacological effects. In this study, we investigated WSMoL (10, 25, or 50 mg/kg i.p.) for acute and 14-day repeated-dose toxicity, as well as genotoxicity, in Swiss mice. In addition, the hypoglycemic activity of WSMoL (25 and 50 mg/kg i.p.) was evaluated in alloxan-treated mice. Animals were monitored for body weight, water and food consumption, behavioral changes, hematological and biochemical parameters, and histological patterns of the liver, spleen, and kidneys. Acute toxicity assay showed that WSMo

Updates in Cystic Fibrosis-Related Diabetes (CFRD).

Cystic fibrosis-related diabetes is a unique form of diabetes caused by a combination of exocrine and endocrine pancreatic dysfunction. These changes lead to early problems with insulin secretion, disrupted communication within the islets, and an abnormal glucagon response. As a result, people develop a pattern of glucose abnormalities that is different from both type 1 and type 2 diabetes, often showing postmeal hyperglycemia and unpredictable fluctuations. Diagnosis still relies mainly on the oral glucose tolerance test, but continuous glucose monitoring is becoming increasingly helpful, esp

Cordyceps militaris polysaccharides alleviate hyperglycemia in type 2 diabetes mellitus mice by modulating gut microbiota.

Type 2 diabetes mellitus (T2DM) involves pancreatic dysfunction and hyperglycemia, leading to hepatic impairment and gut dysbiosis. Cordyceps militaris (C. militaris) possesses polysaccharides (CMP) with hypoglycemic and hypolipidemic effects. However, the potential of C. militaris to reconfigure gut microecology in T2DM remains inadequately explored. This study assessed the therapeutic effects of CMP on glucose homeostasis and microbial composition using a high-sugar and high-fat diet combined with streptozotocin-induced T2DM in a mouse model. Structural characterization identified CMP as a h

Nutrition support-focused strategies for glycemic management in hospitalized adults with diabetes: A narrative review.

Diabetes is commonly encountered in hospitalized adult patients. Blood glucose (BG) management in acutely ill hospitalized patients receiving nutrition support therapy with enteral nutrition (EN) or parenteral nutrition (PN) often varies from strategies utilized in stable patients with diabetes in the outpatient setting. Some degree of BG elevation is expected in hospitalized patients due to the acute stress response. However, hyperglycemia should be prevented, if possible, and treated if it occurs, as it has been shown to increase infections, length of stay, and mortality. While a variety of

Type A Insulin Resistance Syndrome Due to a Pathogenic Variant in the INSR Gene.

PRESENTATION: A woman in her 40s initially diagnosed with type 1 diabetes in 1994 exhibited an atypical 30-year metabolic course, characterised by fluctuating insulin sensitivity, prolonged insulin independence and later insulin resistance despite a non-obese phenotype. DIAGNOSIS: Owing to the unusual clinical trajectory and a strong family history of young-onset diabetes, genetic testing identified a heterozygous pathogenic INSR mutation (p.M1180K), confirming Type A Insulin Resistance Syndrome (TAIRS). TREATMENT: Management evolved from insulin therapy to oral hypoglycaemic agents, includi

The effect of ertugliflozin on serum insulin and glucose in healthy horses receiving dexamethasone.

BACKGROUND: Sodium-glucose cotransporter 2 inhibitors (SGLT-2i) can be used to manage insulin dysregulation (ID). Their effect on corticosteroid-induced ID is unknown. HYPOTHESIS/OBJECTIVES: Evaluate effects of ertugliflozin on serum insulin and glucose concentrations in horses receiving dexamethasone (DEX). We hypothesized that DEX would induce ID and ertugliflozin coadministration would mitigate that effect. ANIMALS: Seven healthy horses. METHODS: Randomized crossover design; horses received DEX (0.06 mg/kg IV q48h; DEX)or ertugliflozin (0.05 mg/kg PO q24h) plus DEX (SGLT-2i group) for se

Safety and efficacy of human gingiva-derived mesenchymal stem cells in patients with type 1 diabetes: a double-blind, randomized, placebo-controlled, phase 1 trial.

Current therapies for type 1 diabetes mellitus (T1DM) rarely halt progressive β-cell function loss due to autoimmune attack. Human gingiva-derived mesenchymal stem cells (GMSCs) have been shown to have therapeutic effects on a variety of autoimmune diseases through their potent immunomodulatory capacity, but clinical evidence for these effects is lacking in patients with T1DM. Here, we performed a double-blind, randomized, placebo-controlled, phase 1 trial to evaluate the safety and efficacy of GMSC therapy in T1DM patients (ChiCTR1800018339). Patients received 4 intravenous injections of GMSC
