Type 2 diabetes mellitus (T2DM) is the most prevalent metabolic disorder. Gardenia jasminoides Ellis is a traditional herbal medicine used in metabolic disease treatment; however, the effects of Gardenia jasminoides Ellis polysaccharides (GPS) on T2DM remain unknown. This study aimed to investigate the antidiabetic effect of GPS in a T2DM mouse model induced by high-fat diet (HFD) and streptozotocin administration, and to elucidate the potential molecular mechanism. Treatment with low and high doses (200 and 400 mg/kg) of GPS significantly reduced the fasting blood glucose level of T2DM mice b

INTRODUCTION: Following the identification of the incretin effect, the role of the enterohormones in regulating food intake and energy balance was recognized, ushering in a new era in the pharmacotherapy of metabolic diseases. A constellation of hormones is released from the gastrointestinal tract, some of them with biological functions still not completely defined. The insulin-like peptide 5 (INSL5) is a recently discovered enterohormone with basic studies proposing a role in the gut-brain axis and hunger regulation, although evidence in humans remains limited. It is primarily produced in the

Type 2 diabetes mellitus (T2DM) is often accompanied by a glucose metabolism disorder and intestinal microbial dysbiosis. In this study, underlying hypoglycemic mechanisms of surfactin-producing strain Bacillus amyloliquefaciens Fmb50 (Fmb50) and its spores were investigated. Fmb50 and its spore alleviated hyperglycemia in T2DM mice by reducing fasting blood glucose and improving oral glucose tolerance. Fmb50 and its spore decreased serum (glucose, GSP) and liver indicators (TG, TC), inhibited hepatic lipid accumulation, and reversed liver, pancreas, epididymal adipose tissue, and intestinal b

BACKGROUND: With an increasingly aging population, the prevalence of acromegaly in the elderly is rising. The treatment goal is defined in the guidelines as "IGF-I normalized for age;" however, in the super-elderly, where physiological interindividual variability is particularly pronounced, this goal remains uniform and may require further consideration. We present a case of well controlled acromegaly in an elderly patient, experiencing a good quality of life even with mildly elevated serum growth hormone concentration.
CASE PRESENTATION: A 97-year-old Japanese woman was diagnosed with acrome

Glucocorticoids (GCs) are widely utilised for the treatment of inflammatory and autoimmune conditions but often precipitate significant hyperglycaemia. People with type 1 diabetes (PWT1D) may be particularly affected due to the challenges of adjusting insulin dosing, for which recommendations remain unclear. Our aim is to synthesise the evidence on the glycaemic effects of GCs in PWT1D across various formulations, doses and administration routes, and to outline management strategies. In August 2025, a systematic search of MEDLINE, Embase and CENTRAL was conducted in accordance with Preferred R

BACKGROUND: Hypoglycaemia stimulates growth hormone (GH) secretion, whereas hyperglycaemia suppresses it. However, the underlying mechanisms are not fully understood, particularly the potential role of gut-derived hormones released in response to oral glucose.
AIM: To investigate whether GH suppression is modulated by the route of glucose administration.
METHODS: A two-day intervention study in healthy volunteers. GH, insulin, glucagon-like peptide-1 (GLP-1), and glucose-dependent insulinotropic polypeptide (GIP) responses during a 2-h oral glucose tolerance test (OGTT) were compared with th

20(S/R)-ginsenoside Rh1 is a less-studied ginsenoside. This study investigated its role in restoring glycaemic homeostasis via the gut microbiota-bile acid axis in type 2 diabetes mellitus (T2DM). The antidiabetic mechanism was evaluated in a high-fat diet/streptozotocin (HFD/STZ)-induced T2DM mouse model treated with 20(S/R)-ginsenoside Rh1 (60 mg/kg/day) for 4 weeks. Fasting blood glucose, oral glucose tolerance test (OGTT), serum insulin and glucagon-like peptide-1 (GLP-1) levels, and lipid profiles were assessed. Gut microbiota composition was analyzed by 16S rRNA sequencing, and targeted

BACKGROUND: Injectable peptide-based glucagon-like peptide-1 receptor (GLP1R) agonists (GLP1RAs) are effective treatments for obesity and diabetes but are limited by patient compliance and scalability challenges. In contrast, non-peptide small-molecule GLP1RAs offer the advantage of oral delivery but are relatively inactive at the rodent GLP1R, limiting their utility in preclinical pharmacodynamic studies. This study aimed to bridge the translational gap by engineering and thoroughly validating a humanised GLP-1 receptor (hGLP1R) mouse model that more accurately recapitulates human receptor ph

Oral delivery of insulin faces significant challenges due to various barriers present in the gastrointestinal tract, leading to minimal therapeutic outcomes. To overcome these obstacles, chitosan-based nanoparticles modified by L-valine were designed and subsequently coated with fucoidan (FU) to improve the permeation efficiency through the mucus layer and intestinal epithelium. The insulin-loaded nanoparticles were characterized by dynamic light scattering (DLS), transmission electron microscopy (TEM), encapsulation efficiency (EE), and loading capacity (LC). The prepared nanoparticles VCS-IN
