Liver cancer risk in the context of today's hormonal contraceptives.

Combined oestrogen-progestin oral contraceptives are classified as hepatic carcinogens based largely on older studies of outdated formulations, creating uncertainty about risks of contemporary hormonal contraceptives. We conducted a Danish nationwide registry-based cohort study including 1,957,490 women aged 15 to 49 years followed from 1995 to 2021 to examine current hormonal contraceptive use and liver cancer risk. Use was assessed as a time-dependent exposure using prescription data, and cancer diagnoses were identified through Danish Cancer Registry. Adjusted incidence rate ratios (IRRs) w

Age- and sex-specific reference values for phosphate homeostasis parameters-fibroblast growth factor 23 and soluble Klotho.

BACKGROUND: Currently, uniform reference values for parameters of phosphate (Pi) homeostasis-the phosphaturic hormone fibroblast growth factor 23 (FGF23) and its coreceptor, soluble Klotho (sKlotho)-are used for women and men regardless of age. This may not be adequate, especially in older subjects. We established Lambda-Mu-Sigma (LMS)-based continuous age- and sex-specific reference values for key parameters of Pi homeostasis, FGF23 and sKlotho. METHODS: The HAnnover Reference values for Adults study included 504 participants (51% women) aged 18-70 years. Main outcome measures were serum Pi,

Prospective evaluation of the effect of stopping estrogen-containing contraceptives on serum CBG and cortisol levels.

CONTEXT: Oral estrogens raise corticosteroid-binding globulin (CBG) levels, leading to increased serum total cortisol concentrations. Guidelines advise discontinuation at least six weeks before performing endocrine function tests measuring total serum cortisol concentrations for evaluation of hypothalamus-pituitary-adrenal function in women using combined oral contraceptives (COC). The exact rate of normalization of CBG is unknown. OBJECTIVE: This study aims to identify the duration for CBG and total cortisol levels to normalize after stopping COC. DESIGN, SETTING, PARTICIPANTS: This prospec

Bridging the Rural-Urban Divide: Independent Pharmacies and Women's Contraceptive Access.

Independent community pharmacies serve as critical healthcare access points in rural areas, yet they consistently underperform chain pharmacies on contraceptive access measures. This narrative commentary draws on mystery caller studies, implementation research, and policy analyses to examine pharmacy-based contraceptive access in the United States (US). Using emergency contraception (EC) as a case study, we show that independent pharmacies stock EC at dramatically lower rates than chains (e.g., 14.6% vs. 76.3% in West Virginia), provide less accurate information about purchase requirements and

Ovarian cancer tumor immune profiles associated with intrauterine device and oral contraceptive use.

BACKGROUND: Non-hormonal intrauterine devices (IUDs) create an inflammatory uterine environment while oral contraceptives (OC) suppress ovulation and have different associations with ovarian cancer risk. We have evaluated the associations of these two contraceptive exposures with ovarian tumor immune infiltration. METHODS: This study assessed associations of IUD and OC use with tumor immune features via multiplex immunofluorescence in 24 ovarian tumor tissue microarrays from four case-control and two cohort studies. Multivariable-adjusted beta-binomial models estimated the odds of tumor T cel

Region-specific regulation of glucocorticoid and mineralocorticoid receptor signaling in a mouse model of oral contraceptive exposure.

Combined oral contraceptives (OCs), containing synthetic estrogen and a progestin such as levonorgestrel (LVNG), are widely used, and up to 10% of users experience adverse mood states and increased depression risk. It is well-established that OCs modulate the hypothalamic-pituitary-adrenal (HPA) axis and blunt the cortisol responses to acute stress. This interaction with stress regulatory pathways is one mechanism by which OCs might impact mood. Here, we used a mouse model of OC exposure (ethinyl estradiol (EE) + LVNG) to investigate how OCs affect regulation of the diurnal CORT cycle and stre

Pharmacological treatment of polyendocrine metabolic ovarian syndrome in adults.

Polyendocrine metabolic ovarian syndrome (PMOS), previously named polycystic ovary syndrome, is a common endocrine disorder in women of reproductive age. It is associated with excessive levels of androgens and ovulatory dysfunction and has reproductive, metabolic and psychological impacts across the lifespan. Pharmacotherapy in PMOS should be directed at the concerns of the individual patient and may include the combined contraceptive pill, antiandrogen agents, metformin, antiobesity agents and ovulation induction agents. A narrative review of non-hormonal therapies was conducted. Metformin is

Oral Contraceptive Use Associated With Lower Rates of Knee Instability After Anterior Cruciate Ligament (ACL) Reconstruction.

Background Sex hormones influence connective tissue biology and pain processing, but the relationship between oral contraceptive pill (OCP) use and postoperative outcomes after anterior cruciate ligament reconstruction (ACLR) remains unclear. The purpose of this study was to evaluate whether perioperative OCP use is associated with postoperative knee instability and other clinical outcomes after ACLR. Methods Using a multicenter database, TriNetX, females aged 14-40 years undergoing arthroscopic ACLR were grouped as OCP users (prescriptions within six months before and within two years after s

Uncovering a hidden modulator for women in sports: An exploratory study of menstrual pain and brain connectivity.

BackgroundThe menstrual cycle (MC), the use of combined oral contraceptives (COCs), and the presence of menstrual pain have long been overlooked factors in female athletes' health. The influence of hormonal fluctuations and pain may not only alter perceived performance and trainability but could also induce changes in brain network connectivity.ObjectivesThis study aimed to investigate whether resting-state functional connectivity (rsFC) differs between phases across the MC and COC cycle, as well as between naturally cycling (NC) subjects and COC users. A secondary goal was to explore associat

Contraceptive Progestogens and Incident Meningioma.

IMPORTANCE: Meningioma is a known adverse reaction of high-dose progestogens, but evidence regarding the risk associated with progestogens used as contraception is limited. OBJECTIVE: To examine whether different progestogens used as hormonal contraception are associated with increased risk of meningiomas. DESIGN, SETTING, AND PARTICIPANTS: This nested case-control study conducted over a 25-year study period from January 1, 2000, to December 31, 2024, is a Danish nationwide population-based register study, which included 3 million females aged 15 to 59 years with residence in Denmark. Oral contraceptive usage among healthcare workers and its impact on COVID-19 booster vaccination immunogenicity.

Oral contraceptives (OCs) can substantially modulate immune responses; however, their impact on vaccine immunogenicity remains poorly understood. This study investigated OC use patterns and their determinants among healthcare workers (HCWs), and the effects of OCs on humoral and T-cell-mediated immune responses following COVID-19 booster vaccination. From 29 September 2021 to 31 December 2023, 1061 female HCWs aged 18-50 years were enrolled in the CoVacSer study. OC users were compared to non-users. Blood samples combined with a questionnaire were collected before and after third and fourth CO

Association between combined oral contraceptive pills and depression in women with polyendocrine metabolic ovarian syndrome (formerly polycystic ovary syndrome): a comparative cross-sectional study.

BACKGROUND: The prevalence of depression among women with polyendocrine metabolic ovarian syndrome (PMOS), formerly polycystic ovary syndrome (PCOS), is high. Combined oral contraceptives (COCs) are widely recommended for the treatment of women with PMOS, potentially reducing stress related to managing PMOS symptoms and improving body image. However, concerns exist regarding a potential association between COC use and depression. Given these conflicting effects, the present study aimed to evaluate the relationship of COCs on depression and its associated factors in Thai women with PMOS, compar

Prevalence, patterns and motives for hormonal contraceptive use among German servicewomen: results from a nationwide survey.

Hormonal contraceptive (HC) use is common among servicewomen and has been discussed in relation to health, performance, and operational readiness. No systematic data exist on HC prevalence among German servicewomen. This study analyzes contraceptive practices and motivations for contraceptive choice in German servicewomen. A nationwide, cross-sectional online survey was conducted among female members of the German Armed Forces. Recruitment occurred via military institutions and a digital flyer. The anonymous questionnaire (40 items, 10-15 min) assessed demographic characteristics and contracep

Identifying High-Risk Patients With Cerebral Venous Thrombosis for Early Intervention.

BACKGROUND: Cerebral venous thrombosis presents with diverse clinical features, making early recognition of patients at risk for deterioration challenging. The cerebral venous thrombosis score was developed by our group and later renamed the CLOTS-EVT score as a mnemonic to facilitate recall and improve ease of clinical use. This 10-point tool, including decreased consciousness (C), low platelets and sodium (L), absence of oral contraceptive use (O), thrombosis burden >3 sinuses (T), seizure (S), and papilledema (E), was developed to predict early deterioration and guide timely venous thrombec

Oral contraceptive-associated hypertension as an under-recognized contributor to secondary and apparent resistant hypertension: agent-specific blood pressure liability, RAAS testing pitfalls, and practical assessment.

Oral contraceptive-associated hypertension is a long-recognized yet still under-recognized and potentially reversible contributor to elevated blood pressure in women, particularly adolescents and young adults. In Japan, prescribing of low-dose estrogen-progestin preparations has increased; however, available prescription data neither demonstrate an increase in oral contraceptive-associated hypertension nor establish a pill-specific rise in blood pressure among young women. Hormonal exposure may be overlooked because patients may not identify hormonal pills as blood pressure-relevant medicines

Tolerability of hormonal treatments in acute hepatic porphyria patients.

BACKGROUND: Acute hepatic porphyria (AHP) flares have been associated with female hormones and the use of hormonal treatments. Women with AHP are traditionally advised to avoid exogenous oestrogen and progesterone, limiting options for contraception and management of menorrhagia, dysmenorrhoea and menopausal symptoms. Evidence on specific hormonal therapies is limited; a 2003 study found that 25% of women with acute intermittent porphyria (AIP) experienced attacks with contraceptives containing progesterone, oestrogen or both. AIM: To update the current understanding of the tolerability of ho

Determinants of contraceptive discontinuation among sexually active women in Nigeria: a systematic review.

INTRODUCTION: Contraceptive use is critical for improving reproductive health and reducing unintended pregnancies, yet many women discontinue its use despite the need for it. In Nigeria, the total fertility rate remains high at 4.8 children per woman, while modern contraceptive prevalence among women of reproductive age is only 17-20%, highlighting persistent gaps in family planning. This systematic review aimed to synthesize evidence on the prevalence, patterns, and determinants of contraceptive discontinuation among Nigerian women. METHODS: A systematic review was conducted focusing on stud

Effect of Hormonal Contraceptives on Circulating Biomarkers of Inflammation, Chemotaxis, Angiogenesis, and Vascular Stress.

Hormonal contraceptive (HC) use is known to be associated with elevated CRP levels. However, evidence on the effect of HC on several circulating inflammatory biomarkers in healthy humans is limited. The aim of this study was to examine the association between hormonal contraceptives and circulating biomarker levels and differential cell counts. A cross-sectional observational study design with biomarker measurements from 4,160 healthy females was used, coupled with registered HC prescriptions. We compared 752 high and low dose combined oral contraceptive (COC) users, 77 progestin-only pill use
