In recent years, the field of glomerulonephritis has become more accessible not only to physicians who need to understand and recognize these conditions, but also to patients who now have new treatment options available to them. For glomerulonephritis with a membranoproliferative phenotype, a better understanding of the pathophysiology has led to a new classification, enabling more targeted diagnosis; in addition, two new active ingredients have been approved for specific treatment.

Rheumatoid arthritis is a chronic immune-mediated inflammatory disease, and therapeutic strategies predominantly rely on continuous immunomodulation. Despite major advances with biological and targeted synthetic disease-modifying antirheumatic drugs, some patients develop persistent, objectively active inflammatory disease that remains refractory to multiple therapeutic mechanisms. Durable drug-free remission is rare, highlighting a major unmet need. Emerging deep B-cell depleting therapies, including bispecific T-cell engagers and chimeric antigen receptor (CAR) T-cell-based therapies, challe

BACKGROUND: Ordinal outcomes are frequently used in clinical and observational studies. However, because ordinal variables do not have clearly defined intervals between categories, causal effect estimation for ordinal outcomes is more challenging than that for binary or continuous outcomes. Therefore, accurate causal inference for ordinal outcomes is important, as inadequate covariate adjustment may lead to biased estimates due to confounding. In this study, we defined the average treatment effect (ATE) for ordinal outcomes and compared the performance of various causal inference methods for e

BACKGROUND: Methotrexate is a first-line therapy for rheumatoid arthritis, psoriatic arthritis, and psoriasis, but its long-term use has been associated with hepatic fibrosis, highlighting the need for effective screening strategies. In this context, and considering the limitations of liver biopsy as well as the high cost and limited availability of elastography, the FIB-4 score has emerged as the most widely used noninvasive screening tool. This multicenter cross-sectional study evaluated patients followed at specialized Rheumatology and Dermatology centers to assess the accuracy of FIB-4 in

Conventional anti-inflammatory therapies, including nonsteroidal anti-inflammatory drugs (NSAIDs) and corticosteroids, remain the mainstay of treatment for inflammatory disorders. However, their long-term use is often limited by adverse effects, insufficient target specificity, and an inability to achieve sustained resolution of chronic inflammation. Advances in the understanding of inflammatory signalling pathways have led to the identification of several promising protein targets for the treatment of chronic inflammatory and autoimmune diseases. This review provides a comprehensive overview

BACKGROUND: Immune-mediated inflammatory diseases (IMIDs) are associated with increased cardiovascular risk, but surgical outcomes in the modern era of immunosuppressive therapy remain unclear. We evaluated early and late outcomes after cardiac and thoracic aortic surgery in patients with IMIDs, focusing on the impact of glucocorticoids and disease-modifying antirheumatic drugs (DMARDs).
METHODS: A total of 140 patients with IMIDs who underwent cardiac or thoracic aortic surgery between 2008 and 2025 were retrospectively analyzed. The primary endpoints were in-hospital mortality and long-term

INTRODUCTION: Human cytomegalovirus (CMV), Epstein-Barr virus (EBV), and human herpesvirus 6 (HHV-6) remain important causes of morbidity after allogeneic hematopoietic cell transplantation (HCT), yet biomarkers predicting viral reactivation are lacking. Extracellular vesicles (EVs) have emerged as promising liquid-biopsy tools in inflammatory and infectious diseases.
METHODS: In this exploratory biomarker discovery study, 32 consecutive recipients of haploidentical HCT with post-transplant cyclophosphamide were prospectively enrolled. EV surface marker profiles were assessed by flow cytometr

Rheumatoid arthritis (RA) is a persistent autoimmune condition that predominantly affects the joints resulting in inflammation, pain and progressive disability. Immunological imbalance, especially the stimulation of pathogenic immune cells, including T and B cells, macrophages, amongst others, is the cause of the disease, as it is critical to the development of the disease. The limitation of traditional anti-RA medications, such as non-steroidal anti-inflammatory drugs (NSAIDs) and disease-modifying anti-rheumatic drugs (DMARDs), in the treatment of the disease led to the development of more s

UNLABELLED: Diffuse large B-cell lymphoma (DLBCL) is the most prevalent non-Hodgkin lymphoma subtype. Although R-CHOP remains the standard first-line therapy, up to 30% of patients experience relapse or refractory disease. Currently, therapeutic monitoring relies on imaging, which is less sensitive than molecular techniques for detecting residual disease.
AIM: To evaluate BCL2 and BCL6 oncogene expression in Circulating Tumor Cells (CTCs) as potential molecular biomarkers for clinical outcomes in DLBCL patients after R-CHOP therapy.
METHODS: We analyzed BCL2 and BCL6 expression in CTCs from

Current disease-modifying antirheumatic drugs (DMARDs) for rheumatoid arthritis (RA) predominantly target systemic immune-inflammatory pathways, yet fail to effectively reverse progressive articular structural damage driven by fibroblast-like synoviocytes (FLS), representing a core unmet clinical need in the field. Non-coding RNAs (ncRNAs) are key epigenetic regulators of pathological FLS activation; however, the field is constrained by three core limitations: fragmented evidence, mechanistic homogenization, and a persistent basic-to-clinical translational gap. Adopting a structured evidence-g

FLLL32 is a curcumin analogue with reported anti-tumor activity, mainly through inhibition of signal transducer and activator of transcription 3 (STAT3) signaling and induction of apoptosis. However, whether FLLL32 regulates ferroptosis in colorectal cancer (CRC) and the underlying mechanism remain unclear. Here, using a patient-derived CRC organoid screening platform, we identified FLLL32 as a potent anti-CRC compound. FLLL32 suppressed CRC organoid growth, inhibited CRC cell proliferation and migration in vitro, and inhibited xenograft tumor growth in vivo. In parallel, FLLL32 induced ferrop

OBJECTIVE: To compare the retention and disease activity trajectories of subsequent biologic and targeted synthetic disease-modifying antirheumatic drugs (b/tsDMARDs) after Janus kinase inhibitor (JAKi) discontinuation in patients with rheumatoid arthritis (RA).
METHODS: We analysed 500 treatment courses (TCs) from the ANSWER cohort in which subsequent b/tsDMARDs were initiated after JAKi discontinuation. Subsequent treatments were classified as TNF inhibitors (TNFi), anti-IL-6 receptor antibodies (aIL-6R), CTLA4-Ig and JAKi. Drug retention was stratified by the reason for prior JAKi disconti

BACKGROUND: Early-life exposure to allergens is a critical window that shapes immune development. Epidemiological and experimental evidence suggests that neonatal cytokine responses, particularly interleukin (IL)-4 and IL-5, drive type 2 immunity and increase the risk of allergic sensitization and asthma in adulthood. However, whether blocking these pathways in neonates can alter long-term susceptibility remains unclear.
METHODS: Neonatal BALB/c mice were exposed to 1% aerosolized ovalbumin (OVA) for 20 min, whereas controls received PBS. A subset of OVA-exposed neonates received 0.5 mg anti-

BACKGROUND: Calcineurin inhibitor (CNI) nephrotoxicity drives long-term kidney-function decline after simultaneous pancreas-kidney (SPK) and liver-kidney (LK) transplantation, but belatacept conversion, as an alternative strategy, is poorly studied in these recipients, whose non-kidney allograft rejection risk differs.
METHODS: We retrospectively studied six SPK and six LK recipients converted from CNI- to belatacept-based immunosuppression and analyzed the cohorts separately alongside a narrative review of published SPK/LK conversion experiences. Outcomes were estimated glomerular filtration

BACKGROUND: Hepatocellular carcinoma (HCC) is the sixth most common cancer and the third leading cause of cancer-related mortality worldwide. Despite the clinical success of immune checkpoint inhibitors, a substantial proportion of patients exhibit primary resistance, largely attributable to the immunosuppressive "cold" tumor microenvironment (TME). Saikosaponin D (SSd), a triterpenoid saponin derived from Bupleurum falcatum L., has emerged as a multifaceted immunomodulator with potential to remodel the TME, yet its role in converting "cold" HCC to an immunologically responsive "hot" phenotype

OBJECTIVES: Emerging evidence supports the presence of autoantibodies targeting interleukin-1 receptor antagonist (IL-1Ra), a regulator of IL-1 signaling, in adult-onset Still's disease (AOSD). However, existing data are limited by small cohorts and potential confounding due to prior exposure to IL-1 inhibitors. This study aimed to develop a simple and reproducible indirect ELISA for detecting anti-IL-1Ra antibodies and to assess their frequency and immunological features in IL-1 inhibitor-naïve AOSD patients.
METHODS: Serum samples from IL-1 inhibitor-naïve AOSD patients (n=46) and healthy c

Effective perioperative analgesia remains essential in laparoscopic cholecystectomy (LC), where pneumoperitoneum produces the most intense nociceptive stimulus despite the minimally invasive nature of the procedure. Objective monitoring of nociception using the nociception level (NOL) index may optimize intraoperative opioid titration; however, its integration into multimodal analgesia (MMA) strategies incorporating regional techniques has not been adequately studied. The study was conducted using a double-blind design in which patients and the postoperative outcome assessor were blinded to gr

INTRODUCTION: Birdshot uveitis (BU) is a chronic, debilitating posterior uveitis strongly associated with HLA-A*29, supporting a suspected autoimmune etiology. We used high-dimensional mass cytometry and clinical stratification to characterize the peripheral immune landscape in BU and identify disease activity-dependent signatures.
METHODS: In this cross-sectional study, we included 36 treated BU patients and 31 age- and sex-matched healthy controls (HCs). All participants underwent comprehensive ophthalmologic evaluation with multimodal imaging. Patients were stratified by an expert ophthalm

Patients with immune-mediated inflammatory rheumatic diseases (IMIRDs) may be more vulnerable to severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection because of underlying immune dysregulation. However, it remains unclear which factors determine coronavirus disease 2019 (COVID-19) severity in this population, especially in Eastern European settings. We conducted a retrospective, multicenter observational study including 211 patients with IMIRDs diagnosed with COVID-19 between 01.03.2020 and 31.12.2024. We collected clinical, demographic, and laboratory data within the first 7
