Short- and long-term sustained remission and the role of joint accumulation in early and very early rheumatoid arthritis: a real-world perspective.

OBJECTIVE: We aimed to identify predictors of short- and long-term sustained remission in early rheumatoid arthritis (eRA) and very early rheumatoid arthritis (veRA), and to assess their relevance within the joint accumulation model paradigm. METHODS: This retrospective real-world cohort study included adult Colombian RA patients. Based on symptom onset, patients were classified as eRA (≤12 months) or veRA (≤3 months). Sustained remission was defined using Clinical Disease Activity Index (CDAI) thresholds maintained for ≥6 months (short-term) and ≥24 months (long-term). Predictors were identi

Altered expressions of CGRP, SULT1A1, HMGB1, and HIF-1α in the trigeminal ganglion in medication overuse headache in female rats.

BACKGROUND/AIM: Medication-overuse headache (MOH) is a female-predominant secondary headache generally associated with nonsteroidal anti-inflammatory drug overuse and chronification of migraine. MOH is associated with elevated serum levels of inflammatory and nociceptive molecules and intestinal leak in migraine patients. We aimed to characterize the changes in expression patterns of high mobility group box 1 (HMGB1) and hypoxia-inducible factor 1 alpha (HIF-1α) in the trigeminal ganglion cells, thereby offering new insights into the molecular mechanisms contributing to MOH. METHODS: MOH was

Adenosine A2A receptor blockade promotes oligodendrocyte differentiation and myelination: Highlighting the potency of an edaravone-conjugated A2A receptor antagonist.

Many neurodegenerative diseases, including demyelinating pathologies, are characterized by complex pathological processes mediated by multiple damage mechanisms. While current drugs predominantly target oxidative stress pathways, understanding multi-target modulation could clarify mechanisms underlying demyelination. This study aimed to explore the pharmacological effects of different selective adenosine A2A receptor antagonists, alone or conjugated with antioxidant edaravone, on oligodendrogliogenesis and myelination processes in purified rat cultured oligodendrocyte precursor cells (OPCs) or

Duhuo Jisheng Decoction mitigates postmenopausal rheumatoid arthritis by suppressing eEF2K-mediated cellular senescence.

Rheumatoid arthritis (RA) disproportionately impacts middle-aged and elderly women, with postmenopausal RA (PM-RA) showing more severe inflammation and bone destruction than nonmenopausal RA. Despite its clinical significance, the molecular mechanisms underlying this disease subtype remain inadequately characterised, which has hindered the development of targeted therapies. In this study, to explore potential molecular targets, we analysed clinical samples and found that eukaryotic elongation factor-2 kinase (eEF2K) expression in peripheral blood mononuclear cells (PBMCs) was significantly hig

Comparison of Opioid Utilization in Older Adults with Rheumatoid Arthritis before and after Initiating Biological or Targeted Synthetic Disease-Modifying Antirheumatic Drugs.

INTRODUCTION: Opioid use is common in rheumatoid arthritis (RA) for pain management; however, evidence of opioid-associated adverse events is increasing. While biological (b) or targeted synthetic (ts) disease-modifying antirheumatic drugs (DMARDs) improve functional outcomes such as pain, little is known about their impact on opioid utilization patterns. This study investigated opioid utilization before and after b/tsDMARD initiation and assessed effect modification by sex. METHODS: Using 5% Medicare claims data from 2012 to 2020, this cohort study included older adults (≥ 65 years) with RA

Sulfasalazine-quercetin co-amorphous systems: Sustained release behaviors and enhanced pharmacokinetics.

Sulfasalazine (SULF) is a well-established therapeutic agent for rheumatoid arthritis (RA) and inflammatory bowel disease (IBD), but its oral absorption is limited by efflux transporters, particularly MRP2 and BCRP. Consequently, high daily doses (1-3 g) and frequent administration (three to four times daily) are required, increasing the risk of adverse effects during long-term therapy. To overcome these limitations, co-amorphous systems (CASs) of SULF with quercetin (QUE) were developed in this study. The CASs were systematically characterized using powder X-ray diffraction (PXRD), polarized

5-Aminosalicylic Acid-induced Colitis in a Patient with Rheumatoid Arthritis without Inflammatory Bowel Disease.

This report presents a rare case of 5-aminosalicylic acid (5-ASA)-induced colitis in a 76-year-old man with no history of inflammatory bowel disease (IBD). The patient, who was undergoing treatment for rheumatoid arthritis, developed persistent diarrhea and fever following sulfasalazine (SASP) administration and later experienced a high fever shortly after taking mesalazine. The drug-induced lymphocyte stimulation test result was positive for mesalazine and weakly positive for SASP, thereby supporting the diagnosis of 5-ASA intolerance. This case emphasizes that 5-ASA-induced colitis may occur

Early aggressive treatment with DMARDs vs. step-up therapy in rheumatoid arthritis: a traditional review of joint damage and functional outcomes.

Rheumatoid arthritis (RA) is a chronic autoimmune disease characterized by progressive joint inflammation and structural damage. Two primary treatment strategies - step-up therapy and early aggressive DMARD therapy have been widely debated in clinical practice. The step-up approach begins with NSAIDs or corticosteroids, progressing to DMARDs based on disease severity, whereas early aggressive therapy prioritizes immediate DMARD initiation to prevent irreversible joint damage. This comprehensive review evaluates the effectiveness of these strategies by analyzing randomized controlled trials (RC

Artesunate ameliorates leflunomide low-response against bone destruction in rheumatoid arthritis via cGMP-PKG signalling.

BACKGROUND: Leflunomide (LEF) is a commonly prescribed first-line disease-modifying antirheumatic drug (DMARD) for the treatment of rheumatoid arthritis (RA). However, its therapeutic efficacy is frequently limited, particularly in patients with severe bone destruction who exhibit suboptimal responses. Artesunate (ARS), a derivative of artemisinin, has shown potential in inhibiting osteoclast differentiation and ameliorating bone destruction in RA. This study aimed to evaluate whether ARS could enhance the efficacy of LEF in mitigating bone destruction in RA and to elucidate the underlying mec

Geroscience insights into difficult-to-treat rheumatoid arthritis: the role of unhealthy aging, comorbidity, and therapeutic complexity.

Difficult-to-treat rheumatoid arthritis (D2T RA) is an emerging challenge in aging populations, where disease persistence and therapeutic failure often reflect not only autoimmune dysregulation but also the cumulative effects of age-related biological changes across multiple organ systems. This review reframes D2T RA through the lens of geroscience, highlighting how immunosenescence, inflammaging, and organ system vulnerability converge to create a treatment-resistant disease phenotype. Age-associated alterations in adaptive and innate immunity-such as diminished T cell diversity, impaired reg
