Background: We aimed to investigate the risk for a serious infection in rheumatoid arthritis (RA) patients after tapering the dose of biological/targeted synthetic disease-modifying antirheumatic drugs (b/tsDMARDs). Methods: This nested case-control study investigated the risk for a serious infection in RA patients who underwent mandatory b/tsDMARDs dose reduction 2.5 years after starting therapy with a single b/tsDMARD in the National Health Insurance Research Database (NHIRD). Cases were those patients who developed a serious infection afterwards. Matched controls were selected from those pa

A 55-year-old man presented to the emergency department (ED) with diffuse abdominal pain, vomiting, and anuria following a night of heavy alcohol intake. The patient's medical history included hypertension. Two weeks prior to presentation, he was prescribed etoricoxib for back pain. On presentation, he was found to have severe acute kidney injury, and dialysis was initiated. Renal biopsy showed acute interstitial nephritis with numerous eosinophilic infiltrates. Despite stopping etoricoxib and initiating high-dose steroids, kidney function did not improve. The administration of benralizumab-an

Rheumatoid arthritis (RA) is a systemic autoimmune condition that leads to chronic arthritis, disability, and reduced lifespan. Current therapies show limited effectiveness and often cause severe side effects, with up to 50% of patients discontinuing disease-modifying antirheumatic drugs (DMARDs) due to unsatisfactory outcomes. Natural bioactive compounds (NBCs), such as glycosides, alkaloids, terpenoids, flavonoids, polyphenols, and coumarins, have gained attention for their immunomodulatory and anti-inflammatory properties. However, challenges like poor solubility, high dosage requirements,

Spondyloarthritis (SpA) is a group of diseases characterized by chronic inflammation and extra-articular involvement of tissues and organs. Acute anterior uveitis (AAU), the most common extra-articular manifestation of SpA, typically presents as a unilateral acute disruption of the blood-aqueous barrier, characterized by photophobia, redness of the eye, pain, and blurred vision. The underlying pathogenesis is not fully understood but is thought to involve genetic factors, gut microbiota, and dysregulation of the immune system. Corticosteroids remain the cornerstone of AAU treatment. However, a

This study aims to explore the synergistic effect of triptolide(TP) combined with methotrexate(MTX) on lipopolysaccharide(LPS)-induced RAW264.7 macrophages and its potential mechanism based on the cyclic guanosine monophosphate-adenosine monophosphate synthase(cGAS)-stimulator of interferon genes(STING) signaling pathway, in order to infer the therapeutic effect of the combination of the two drugs on rheumatoid arthritis. RAW264.7 cells were cultured in vitro, and cell counting kit-8(CCK-8) assay was used to detect cell survival rates under the intervention of different concentrations of drugs

Background/Objectives: Serum calprotectin is a promising biomarker of inflammation in rheumatoid arthritis (RA), yet real-world longitudinal comparisons across different targeted therapies remain limited. We aimed to evaluate the dynamics and remission-predictive ability of serum calprotectin and C-reactive protein (CRP) in RA patients treated with adalimumab, upadacitinib, or tocilizumab. Methods: In this retrospective cohort study, patients with RA initiating one of the above therapies were included. Serum calprotectin and CRP were measured at baseline, month 3, and month 6. Pharmacological treatment is commonly used for patients with chronic low back pain (CLBP). However, very low-quality evidence exists for paracetamol as a treatment option for CLBP; therefore, recommendations are contradictory among the guidelines. This randomized controlled study aimed to compare the analgesic effects of paracetamol and celecoxib in a non-inferiority trial. This non-inferiority trial tested the hypothesis that paracetamol is as effective as celecoxib for the treatment of CLBP within a pre-specified non-inferiority margin. One hundred fifty-six patients with CLBP were considere

Background Prolonged glucocorticoid use remains common in rheumatoid arthritis (RA), particularly in resource-limited settings. However, long-term exposure may reduce the effectiveness of biologic disease-modifying antirheumatic drugs (bDMARDs) and increase the risk of metabolic and skeletal complications. This prospective observational study evaluated the association between cumulative glucocorticoid dose and clinical response to biologic therapies in patients with RA. Methodology Adults with RA fulfilling the 2010 ACR/EULAR classification criteria and treated with biologic therapy were prosp

Retinal ischemia is a key factor in the progression of vision-threatening ocular diseases, including central retinal artery/vein occlusion, exudative age-related macular degeneration (eAMD), and proliferative diabetic retinopathy. This study investigates the effects of paeoniflorin along with its related neuroprotective molecular pathways in the treatment of retinal ischemia. Free radical or ischemic-like damage was induced by incubating retinal pigment epithelium (RPE) cells for 24 h with 1 mM hydrogen peroxide (H2O2) or by subjecting retinal neuronal cells to 8 h of oxygen-glucose deprivatio

Small doses of NSAIDs can cause acute renal failure in healthy children. We present the case of a 15-year-old girl who developed acute tubulointerstitial nephritis after three days of treatment with NSAID. The patient presented with lower back pain, sore renal beds, vomiting and loss of appetite. Tests revealed affected kidney function with elevated creatinine levels and proteinuria. Final diagnosis happened through a kidney biopsy, which revealed acute tubulointerstitial nephritis. The patient was treated with prednisolone, and the paraclinical parameters were normalized.

The antimetabolite methotrexate (MTX) was originally used in chemotherapeutic treatment for cancer. Since the early 1980s, MTX has become indispensable in the treatment of rheumatoid arthritis and is established worldwide as the standard of care for the treatment of many inflammatory rheumatic diseases. The mechanism of action of MTX is not completely understood; the adenosine signaling pathway likely plays a role. There are several undesired side-effects associated with MTX (e.g., nausea, elevated transaminase levels, and cytopenia). Based on numerous case reports, it is also postulated that

Peripheral nerve injuries (PNIs) are a significant health concern, affecting millions of individuals and result in debilitating sensory and motor deficits, as well as severe neuropathic pain. Treatment of PNIs depend on severity and gap length, with small gaps repaired by sutures and larger ones requiring autologous nerve grafting, the gold standard for bridging defects. However, autologous grafting also has significant limitations, including low recovery rates and complications such as neuroma formation. Tissue engineering and regenerative medicine offer promising alternatives but lack effect

Chronic kidney disease-associated pruritus (CKD-aP) seriously affects the quality of life and currently has no satisfactory treatment. Mas-related G protein-coupled receptor X1 (MRGPRX1) in humans, corresponding to MrgprC11 in rodents, is an itch receptor. Berbamine is an alkaloid that has the effect of relieving pruritus. This study aimed to investigate the role of MRGPRX1/MrgprC11 in CKD-aP and the potential therapeutic effects of berbamine. MrgprC11 knockout (KO-MrgprC11) mice and MRGPRX1 knockout (KO-MRGPRX1) human dorsal root ganglion (DRG) cells were constructed using the CRISPR/Cas9 tec

INTRODUCTION: The advent of biological therapies has greatly improved the treatment and management of rheumatoid arthritis (RA) and spondyloarthritis (SpA). However, evaluating the efficacy and long-term safety of these therapies is a necessity. So far in Tunisia, no large prospective multicentric trial reflecting national data has been published. Thus, the objective of the study was to collect data on sociodemographic characteristics of Tunisian patients with RA and SpA receiving biologics and to evaluate the clinical efficacy and safety of this therapy.

BACKGROUND: Rituximab, a chimeric monoclonal antibody targeting CD20+ B lymphocytes, has demonstrated efficacy in various immune-mediated kidney diseases beyond its original indications for non-Hodgkin lymphoma, rheumatoid arthritis, and granulomatosis with polyangiitis. Because traditional therapies for kidney diseases often involve substantial toxicity and limited efficacy, rituximab offers a more targeted approach. However, comprehensive safety evaluations in kidney diseases remain limited, necessitating a detailed investigation of adverse reactions associated with long-term rituximab use.

Knee osteoarthritis (KOA) commonly causes pain and functional limitations, especially in hilly areas where mobility demands increase stress on the knee joint. High-Intensity Laser Therapy (HILT) and Ibuprofen Gel Phonophoresis (IGP) are both effective treatments, though comparative data is limited. Hence, we aimed to determine the effect of high-intensity laser therapy (HILT) (class-IV) and ibuprofen gel phonophoresis (IGP) for treating knee osteoarthritis (KOA) among people living in hilly terrain. A randomized controlled trial was conducted with 116 patients, aged 45 to 70, diagnosed with kn
