Urinary excretion of thromboxane B2 in patients with venous thromboembolic disease.

Platelet activation occurs in the initial phase of venous thrombus formation. To determine if thromboxanes (Tx) are released during this process and if Tx measurements are useful in the diagnosis, urinary immunoreactive TxB2 was measured by a rapid, inexpensive assay in 100 consecutive patients with suspected thromboembolic disease. Urinary iTxB2 was not increased in patients who took aspirin, nor in patients studied several weeks after onset of symptoms. Of the remaining patients, iTxB2 was increased in 11 of 15 with confirmed deep vein thrombosis and in seven of ten with confirmed pulmonary

The covalent binding of acetaminophen to protein. Evidence for cysteine residues as major sites of arylation in vitro.

Covalent binding of the reactive metabolite of acetaminophen has been investigated in hepatic microsomal preparations from phenobarbital-pretreated mice. Low molecular weight thiols (cysteine and glutathione) were found to inhibit this binding, whereas several other amino acids which were tested did not. Bovine serum albumin (BSA), which contains a single free sulfhydryl group per molecule and which thus represents a macromolecular thiol compound, inhibited covalent binding of the reactive acetaminophen metabolite to microsomal protein in a concentration-dependent manner. The acetaminophen met

A convenient thin-layer chromatographic screening method for acetaminophen in serum.

A thin-layer chromatographic screening procedure for acetaminophen in serum was developed that meets the clinical need to identify candidates for antidotal therapy within the critical first 24 hr after ingestion. The key feature of this approach is the use of a detection reagent containing Fe(III) and 2,4,6-tris-(2'-pyridyl)-s-triazine (TPTZ). Acetaminophen on the plate reduces the Fe(III) to Fe(II), which then forms an intensely blue-colored complex with the TPTZ. Concentrations as low as 3 to 4 mg/L can be detected and the procedure is free of interference from 38 other common drugs and meta

[Bronchial asthma, nasal polyposis and analgesic intolerance (the ASA triad). A successful computer based analysis of free texts].

In the period 1977 to 1980 the case histories were collected of 1731 asthmatics and 3590 non-asthmatics and stored in free text form in a computer data bank. Analyzing the data, 1643 primary asthmatics were selected comprising 537 seasonal and 417 perennial asthmatics. The incidence of nasal polyposis and chronic sinusitis on the one hand, and intolerance to analgesics on the other, was, at 9.5% and 4.5% respectively, higher in asthmatics than in non-asthmatics. In perennial asthmatics the particularly high incidences of 16% and 7.2% were observed for the two symptoms. As expected, the ASA tri

Stimulus-response coupling in human platelets. Changes evoked by platelet-activating factor in cytoplasmic free calcium monitored with the fluorescent calcium indicator quin2.

The role of changes in cytoplasmic free calcium, [Ca2+]i, in the responses to platelet-activating factor (PAF) was studied in human platelets loaded with the fluorescent calcium indicator, quin2. In the presence of 1 mM external calcium, PAF raised [Ca2+]i 8-10-fold in a few seconds to peak near 1 microM. [Ca2+]i then declined over several minutes towards the basal level. In the absence of external calcium there was a much smaller increase in [Ca2+]i of similar pattern. These findings suggest that PAF increases [Ca2+]i partly by discharge of internal Ca2+, but mainly by stimulated influx. Bloc

Possible role of central serotoninergic neurons in the development of dental pain and aspirin-induced analgesia in the monkey.

The effects of aspirin or 5-hydroxytryptamine (5-HT)-related drugs on the dental pain induced by electrical stimulation of tooth pulp afferent fibers were assessed in conscious monkeys. The electrical current required for producing jaw opening is referred to as the pain threshold. Both systemic (25 to 75 mg/kg, i.p.) or central (0.5 to 1.5 mg, third cerebral ventricle) administration of aspirin produced analgesia in monkeys. In addition, activation of central 5-HT receptors with central injection of either 5-HT or its precursor, 5-hydroxytryptophan, also produced analgesia. On the other hand,

Tobacco smoking and drugs: a clinically important interaction?

Tobacco smoking may interact with the metabolism of a number of drugs. This has been demonstrated clearly in pharmacokinetic studies in animals and man. HOwever, tobacco smoking as a variant has been studied with relatively few clinically essential drugs and these studies do not always demonstrate a consistent effect of smoking. This review therefore not only records what data have emerged from pharmacokinetic or other studies, but it also attempts to determine whether such interactions have clinical significance. The main finding of this review is that, in the majority of examples, there is l

The effects of acetylsalicylic acid on swelling, pain and other events after surgery.

In a double-blind crossover study acetylsalicylic acid (ASA) in low (2 g daily) or high (4 g daily) dosage was tested against placebo in two groups of 20 patients who each underwent identical oral surgical procedures on two separate occasions. Medication started 3 h post-operatively and continued for 3 days. ASA in low dosage tended to increase the postoperative swelling, which on day 3 and 6 averaged 109 and 133% respectively of that with placebo (P greater than 0.05). In contrast, ASA in high dosage tended to reduce the swelling, which on day 3 and 6 averaged 85 and 90% respectively of that

The significance of carotid stenosis or ulceration.

Data from the medical treatment group of the Aspirin in TIA study were reviewed, and prospective analysis of patients with asymptomatic bruits was performed to see whether carotid stenosis (0 to 49% or 50 to 99%) or ulceration produced an increased risk of ipsilateral TIA or infarct. In symptomatic arteries, greater than 50% stenosis without ulceration implied a higher risk of subsequent symptoms. Ulceration was associated with an increased risk only in nonstenotic vessels. Lesion anatomy was not related to outcome in asymptomatic arteries, and the incidence of cerebral infarct was low. Factor

[The analgesic and antipyretic effects of a non-steroidal anti-inflammatory drug, oxaprozin, in experimental animals].

The analgesic and antipyretic effects of oxaprozin were investigated in comparison with those of indomethacin, ibuprofen, phenylbutazone and aspirin. On the various writhing tests in mice, the analgesic effect of oxaprozin was about 2 to 9 times more potent than those of ibuprofen, phenylbutazone and aspirin. On the other hand, the analgesic and antipyretic effects of oxaprozin in rats were roughly equivalent to those of aspirin, but less effective than those of the other drugs tested. On the urate synovitis test in dogs, only oxaprozin showed a prophylactic effect. Therefore, The effect of ox

The relative roles of hydrolases of the erythrocyte and other tissues in controlling aspirin survival in vivo.

The rate of aspirin (acetylsalicylic acid, ASA) deacetylation to salicylate in vivo determines the availability of the intact molecule for therapeutically important transacetylation reactions. Experiments were carried out to assess the relative contribution of a previously isolated human erythrocyte ASA esterase to the overall hydrolysis rate in vivo, as opposed to similar enzymes in other tissues. In vitro, the rates of ASA hydrolysis were relatively slow in plasma from humans and dogs. The hydrolysis rates were significantly greater in whole blood in dogs, rabbits, and humans. In human and c

Alcohol metabolism and toxicity: role of cytochrome P-450.

A new isozyme of cytochrome P-450, designated from 3a on the basis of its relative electrophoretic mobility, has been purified to homogeneity from liver microsomes of rabbits treated chronically with ethanol. This cytochrome has the highest activity of the known rabbit P-450 isozymes in the oxidation of ethanol to acetaldehyde. In view of the reports of others that the hepatotoxicity of acetaminophen is increased in ethanol-treated animals and the human alcoholic, we have determined the activity of the six available P-450 isozymes in the activation of the drug to give an intermediate which for

Inhibition of acetaminophen sulfation by 2,6-dichloro-4-nitrophenol in the perfused rat liver preparation. Lack of a compensatory increase of glucuronidation.

The effectiveness of 2,6-dichloro-4-nitrophenol ( DCNP ) as an inhibitor of sulfation of acetaminophen, and the effect of DCNP on other conjugation reactions such as glucuronidation and glutathione conjugation were investigated in the once-through perfused rat liver preparation. The formation of glucuronide and glutathione conjugates of acetaminophen was insignificant under conditions when sulfation was suppressed either by the absence of inorganic sulfate or the presence of DCNP , suggesting that acetaminophen was a poor substrate for glucuronidation (high Km) and for the formation of the rea

Gastroduodenal damage due to drugs, alcohol and smoking.

In man, convincing data have been obtained in short-term observations that some drugs can cause acute gastroduodenal damage including gastritis and erosions. Useful clinical and epidemiological studies on the relationship between these acute lesions and peptic ulceration, and between the chronic ingestion of these drugs and peptic ulceration or massive upper gastrointestinal haemorrhage are, however, rare. Even for the most widely used and studied greatest offender--acetylsalicylic acid (ASA)--an association with major bleeding or gastric ulceration could only be established for frequent and h

Prostaglandin synthesis in rat embryo tissue: the effect of non-steroidal anti-inflammatory drugs in vivo and ex vivo.

Aspirin and salicylate are well-known but poorly understood teratogens in laboratory animals. Because aspirin inhibits PG synthesis, we systematically examined PG synthesis in rat embryo homogenates, the inhibition of PG synthesis in vivo and ex vivo by various non-steroidal anti-inflammatory drugs, and tested the hypothesis that the inhibition of PG synthesis is responsible for aspirin-induced limb defects in rats. We report that embryonic rat homogenates synthesize 6-keto-PGF1alpha, PGE, and PGF in large amounts from endogenous substrate, that aspirin and other non-steroidal anti-inflammator

Acetaminophen overdose in young children. Treatment and effects of alcohol and other additional ingestants in 417 cases.

Four hundred seventeen young children who had ingested a potentially serious amount of acetaminophen were examined and treated in a national multicenter open study. Fifty-five had plasma levels in the potentially toxic range, and 43 had a full course of acetylcysteine treatment. Three of 417 patients had SGOT values consistent with a hepatotoxic reaction. In the children with toxic levels of acetaminophen those who had also ingested alcohol had significantly lower SGOT levels than those who had not ingested alcohol. Patients consuming acetaminophen who had also consumed miscellaneous other ing

Transient ischemic attacks. Strategies for minimizing stroke risk.

In patients with transient ischemic attack (TIA), the risk of stroke increases greatly, especially in the months immediately following the initial attack. Diagnosis of TIA is based primarily on the patient's cerebrovascular history, since results of neurovascular examination are usually normal. TIA is often related to atherosclerotic arterial disease but can have numerous causes. Migraine, focal seizures, and other neurologic conditions can closely mimic TIA. Surgical and medical therapies help minimize the risk of stroke. The choice of therapy depends on the vascular territory of ischemia, th

Drug interaction in middle molecule analysis, with special reference to acetylsalicylic acid.

Concerning the middle molecules in uremia and other diseases, the potential artifact that can impede an accurate quantitation of middle molecules and that is related to the absorption of a very commonly used drug, namely aspirin, is discussed. Peak 7c and peak b 4-2 are two different middle molecules separated by gel permeation chromatography followed by anion-exchange chromatography. Oral ingestion of acetylsalicylic acid modifies the chromatographic pattern of the middle molecule fraction in normal subjects and uremic patients. Peak 7c is increased in the urine of healthy subjects, whereas t

Prostaglandin biosynthesis does not participate in isoproterenol-induced renin release.

Rat renal slices were incubated in two different media. One was a normal K, physiological saline solution and the other a high K medium. Renin release was measured every 15 min in the presence and absence of 10(-6) M isoproterenol and also in the presence and absence of aspirin, 0.8 or 1.6 X 10(-5) M. In all experiments renin release was linear during the 75 min of incubation. Isoproterenol increased renin release by approximately 100%. This was the case even in the presence of aspirin which significantly inhibited prostaglandin release (PGE2, PGF2 alpha and 6-keto-PGF1 alpha). Nor was there a
