Illicit opioid adulterant trends in patients presenting with acute opioid overdose.

Clandestine fentanyl manufacturing oftentimes introduces adulterants and contaminants. This article aims to evaluate trends in adulterants from a cohort of patients presenting to the emergency department (ED) with illicit opioid overdose across the United States. The Fentalog Study group is a multicenter toxicology study group which evaluated ED patients with suspected opioid overdose at 10 medical centers across the United States between 21 September 2020 through 5 February 2024. Comprehensive qualitative toxicology testing was performed on residual serum specimens. Study sites were divided i

Muscle Retracting Sign in Colorectal Macronodular Lesions: Prevalence, Endoscopic Submucosal Dissection Outcomes, and Submucosal Cancer Predictors.

Endoscopic submucosal dissection (ESD) is the preferred treatment for large colorectal lesions and those at risk of submucosal invasive cancer (SMIC).1 Macronodules ≥1 cm-typically seen in protruding lesions or granular mixed laterally spreading tumors (LST-GM)-are strongly associated with SMIC.2,3 The muscle retracting sign (MRS), first described by Toyonaga et al in 2015,4 appears as a triangular traction of the muscularis propria into the submucosal space. It is linked to fibrosis, a central feeding vessel, or deep invasion, but data distinguishing fibrosis-related MRS from cancer-related M

Proteomic Insights into Heroin Use: Links to Neurodegeneration.

Due to a lack of information related to molecular changes in heroin use, we aimed to examine heroin-dependent alterations in different regions of the post-mortem human brain. Tissues were obtained from males (n = 24 heroin users, n = 24 controls) through the Turkish Forensic Medicine Institute after structured verbal interviews with the relatives of the deceased to gather history of substance use. Following toxicological confirmation of heroin use, the hippocampus, putamen, and caudate nucleus were dissected from the left hemispheres. Proteomic analyses were performed using a high-resolution l

Somebody's going to emergency, somebody's going to jail: pre-booking diversion and officer discretion.

BACKGROUND: The opioid use epidemic has resulted in dramatic increases in drug overdoses and deaths. Traditional criminal justice approaches to drug use focus on making arrests to deter individuals from engaging in criminal behavior. To reduce the negative impacts generated by criminal justice system involvement for individuals with substance use disorders, police departments have begun turning to pre-booking diversion programs. Unlike traditional diversion programs, where individuals are diverted from the criminal justice system after an arrest, these programs authorize police officers to exe

Methadone maintenance therapy alters the expression of PLK1 and IFIH1 in patients with heroin addiction: a case-control study.

BACKGROUND: Heroin addiction poses a significant threat to public health. Methadone maintenance therapy (MMT) is one of the effective treatments for heroin addiction, yet its molecular mechanisms remain largely unclear. This study aims to explore the potential key genes and underlying mechanisms involved in the therapeutic process of MMT for heroin addiction. METHODS: Samples were collected from 84 patients with heroin addiction and 84 patients undergoing MMT at the Fourth People's Hospital of Urumqi. High-throughput transcriptome sequencing was used to identify differentially expressed genes

Leucine Aminopeptidase from Xanthomonas oryzae pv. oryzae with Esterase Activity Toward Heroin: Biochemical and Catalytic Insights.

Heroin is a highly addictive drug that exerts its primary effects through activation of μ-opioid receptors. Its principal active metabolite, 6-monoacetylmorphine (6-MAM), significantly contributes to heroin's neurological effects and acute toxicity. Current pharmacotherapies for heroin use disorder, employing opioid receptor agonist or antagonist, are often limited by risks of dependence, tolerance, and/or adverse side effects. In this context, enzyme-based therapy emerges as a promising alternative by rapidly converting drugs into inactive or less harmful metabolites in the blood. As a macrom

Isolation, Characterization, and Stability Assessment of Pure Enantiomers of Cathinone Derivatives via Semi-Preparative HPLC-UV Using a Phenomenex Lux® 5 Column.

In addition to well-known traditional synthetic illicit drugs like cocaine, amphetamines, and heroin, an increasing number of new psychoactive substances (NPS) are appearing on the global drug market. Among them, cathinones represent a prominent class. These amphetamine-like compounds contain a stereogenic center, resulting in the possible presence of two enantiomers. Pure enantiomers of cathinone derivatives are not commonly available, and their production is cost-intensive. Thus, there is very little knowledge about the possible distinct effects of single enantiomers of cathinones. The objec

Glucocorticoid receptor blockade reverses heroin and alcohol withdrawal-induced hyperalgesia in rats.

RATIONALE: Alcohol use disorder (AUD) and opioid use disorder (OUD) are associated with dysregulation of the hypothalamic-pituitary-adrenal axis and extrahypothalamic stress systems, including glucocorticoid receptor (GR) signaling. Hyperalgesia is defined as an increased sensitivity to nociceptive stimuli and is a prominent symptom of alcohol and opioid withdrawal, which is hypothesized to contribute to the development and persistence of AUD and OUD and heighten relapse vulnerability. OBJECTIVE: We tested the hypothesis that GR antagonism reverses hyperalgesia during both alcohol and opioid

Analysis of Illicit Street Heroin Seized in Pakistan by Gas Chromatography-Mass Spectrometry (GC-MS) for Identification of Adulterants and Impurities.

Heroin abuse poses a significant public health and law enforcement challenge globally, with concerns in Pakistan due to the prevalence of illicit street heroin and limited data on its composition. This study analyzed 706 heroin samples seized between 2017 and 2022 using gas chromatography-mass spectrometry (GC-MS) and Fourier transform infrared spectroscopy (FTIR) to determine their chemical composition. The results revealed that street heroin in Pakistan is extensively adulterated with a variety of pharmaceutical agents, including acetaminophen, caffeine, diazepam, and dextromethorphan. Paret

Comparison of anticipated and detected drug contents in samples submitted to a statewide drug checking program in Maryland, USA.

BACKGROUND: The unregulated drug supply in North America is volatile, with several adulterants emerging following the COVID-19 pandemic. These changes pose significant health risks, including wounds and overdose, to people who use drugs. Understanding discrepancies between perceptions and tested results of drug contents is vital in reducing overdose and other harms associated with the unregulated drug supply. METHODS: We analyzed data from Maryland's statewide drug checking program, Rapid Analysis of Drugs (RAD), from October 2021 to May 2024 (N = 792). Clients of participating RAD syringe se

Neurophysiological signatures of rumination's impact on opioid withdrawal severity.

Opioid withdrawal symptoms (OWS) represent a major clinical challenge during abstinence, imposing physiological distress and increasing psychological vulnerability to relapse. Psychological factors contributing to individual differences in OWS severity remain understood, and reliable biomarkers for OWS severity are still lacking, highlighting the need for early identification and timely intervention among high-risk heroin abstainers. Rumination is defined as uncontrollable negative thinking, potentially impairing the ability to suppress addiction-related negative thoughts. This study aims to i

Exploratory Analysis of the Characteristics Associated With the Use of Fentanyl Test Strips Among People Who Use Drugs in Rhode Island, 2021-2023.

OBJECTIVES: Fentanyl overdose deaths are a critical public health issue in the United States. Rhode Island, an early epicenter of the fentanyl crisis, has seen fentanyl overdose deaths increase 30-fold since 2009. In response, harm reduction tools such as fentanyl test strips (FTSs) have been introduced to help people who use drugs (PWUD) detect fentanyl in their substances. We analyzed FTS use among PWUD in Rhode Island from 2021 through 2023 and identified characteristics associated with their use. METHODS: We conducted a pooled cross-sectional analysis using aggregate data from the Rhode I

Exendin-4 enhances GLP-1 signaling and reduces anxiety-like behaviors in male heroin withdrawal mice.

Anxiety and depression significantly contribute to heroin relapse, and addressing these issues could lower relapse rates. The basolateral amygdala (BLA) and nucleus tractus solitarius (NTS) are involved in regulating these emotions, but the molecular mechanisms during heroin withdrawal are not yet understood. Subcutaneous injection of heroin into C57BL/6J mice to simulate chronic dependence, withdrawal, and Exendin-4 treatment. Assess anxiety and depression-like behaviors using open field test (OFT), elevated plus maze (EPM), forced swimming test (FST), and tail suspension test (TST). Analyze

Attitudes to Drug Policy in Australia, 2001-2022/23: What Does This Mean for Drug Policy Reform in Australia?

INTRODUCTION: We are at a critical juncture for drug policy reform. Toxic drug supply is driving unprecedented deaths in North America, with countries calling for decriminalisation. Policy reform in Australia has occurred in some states (e.g., decriminalisation in the Australian Capital Territory), stalling in others (e.g., the second supervised injecting facility [SIF] in Victoria). The objectives of this paper are to present trends in attitudes to drug policy in Australia, 2001-2022/23 and media reporting of related policies, to explore how public opinion and media attention have changed ove

Prevalence differences in lifetime methamphetamine and heroin use among American Indian reservation youth from 2015 to 2024.

BACKGROUND AND AIMS: Early use of methamphetamine and heroin are associated with severe consequences. American Indian (AI) youth are often portrayed as the highest risk ethnic group in the United States for early use of methamphetamine and heroin, but little empirical evidence supports this. We aimed to examine odds of lifetime methamphetamine and heroin use among the largest representative sample of reservation-area Indigenous youth in the United States, comparing males and females as well as mono-ethnic AI (AI-mono) youth with mono-ethnic non-AI (NAI-mono), multiethnic AI (AI-multi) and mult

Not a Disease of the Past: A Case Report of Heroin-Induced Toxic Leukoencephalopathy Causing Progressive Neurologic Decline.

Toxic leukoencephalopathy is a rare neurological complication associated with various toxins, including inhaled heroin ("chasing the dragon"). We report a case of a 36-year-old female with a history of heroin dependence, in remission for seven years until a recent relapse, who presented with progressive deterioration in speech clarity, swallowing difficulty, and imbalance. Neuroimaging revealed diffuse leukoencephalopathy involving the cerebellar white matter and internal capsules, suggestive of heroin induced leukoencephalopathy. Laboratory evaluation was unremarkable, and infectious, autoimm
