Xylazine in Ohio: Insights from individuals with criminal legal system involvement and a history of illicit stimulant use.

BACKGROUND: Xylazine, a non-opioid sedative not approved for human use, poses serious health risks. It is primarily found in illicitly manufactured fentanyl, but xylazine-fentanyl combinations also appear as illicit stimulant adulterants. Although strategies exist to reduce the risk of xylazine-related harms, some high-risk populations may be unaware of the drug, including people with recent criminal legal system involvement who use illicit stimulants. METHODS: A cross-sectional electronic survey was conducted in Ohio in October 2024 to assess xylazine-related knowledge, attitudes, and behavi

Trends in Associations Between Substance Use and Suicidality and Patterns of Current Substance Use in U.S. Adolescents: A Repeated Cross-Sectional Study.

INTRODUCTION: Substance use and suicidal thoughts and behaviors among U.S. adolescents are both major public health concerns. Despite extensive research establishing a link between substance use and suicidal thoughts and behaviors, little is known about how this relationship has evolved over time. Understanding the changes in these associations may illuminate factors driving the rise in adolescent suicidal thoughts and behaviors and guide public health interventions. METHODS: The 1991-2021 national survey data from the Youth Risk Behavior Surveillance System were analyzed. Race and ethnicity-

OxyContin reformulation and drug-related-arrest rates, property-related crimes, child maltreatment and food pantry participation.

BACKGROUND: Many policies at the federal and state level have been implemented to curb the opioid crisis. A signature policy, the OxyContin reformulation, was introduced in 2010. However, an increasing body of research has documented negative impacts of the OxyContin reformulation, such as a rise in heroin use and overdose deaths. OBJECTIVE: This study evaluates other unintended impacts of the OxyContin reformulation on the arrest rates for drug abuse violations, the arrest rates for drug offenses, the rates for property related crimes, child maltreatment and food pantry participation rate.

Severe Early-Onset Withdrawal Following Intentional Use of Mitragynine Pseudoindoxyl: A Case Report and Emerging Clinical Considerations.

Kratom is increasingly used in the United States for pain, mood, anxiety, and opioid substitution. Traditional kratom produces mild, delayed withdrawal, but semisynthetic derivatives such as mitragynine pseudoindoxyl (MP) are highly potent μ-opioid receptor agonists with δ-opioid receptor antagonism, leading to rapid-onset, full-spectrum opioid-like withdrawal. We describe a 34-year-old man with long-term remission from heroin use disorder who presented with suicidal ideation and severe opioid-like withdrawal despite no recent opioid use. He progressed from powdered kratom to 7-hydroxymitragyn

Occupational exposures to illicit drugs in forensic science divisions.

Forensic scientists from local, county, state, and federal law enforcement agencies analyze evidence daily in which unknown substances suspected of containing illicit drugs are handled, weighed, and chemically processed. To address concerns about potential occupational exposures to opioids like fentanyl and other illicit substances, the National Institute for Occupational Safety and Health (NIOSH) conducted a multi-site exposure assessment among forensic laboratory personnel. NIOSH evaluated personal and area air samples, handwipe samples, area surface samples, and personal biomonitoring urine

Seizure-to-market ratios and the resilience of the drug markets in France (2010-2023).

BACKGROUND: Drug seizures are widely used as indicators of law enforcement performance, yet their ability to disrupt drug markets remains unclear. Recent evidence highlights the lack of rigorous empirical research assessing whether seizures reduce drug availability, crime, or harms. This study compares estimated quantities consumed in France with official seizure volumes to evaluate the relative scale and policy relevance of drug seizures. METHODS: We used a demand-side, bottom-up approach to estimate consumption volumes for cannabis, cocaine, heroin, MDMA/ecstasy, and amphetamines for 2010,

Impact of Sex on Heroin Intravenous Self-Administration by Heterogeneous Stock Rats.

BACKGROUND: Intravenous self-administration (IVSA) of opioids by rats has been shown frequently to exhibit no sex differences, in many cases a higher intake of females, and only rarely higher rates in males. A diversity of methodological parameters (opioid identity, training doses, rat strain, session duration) makes it difficult to identify consistent contributions to these outcomes. OBJECTIVE: To determine if Heterogeneous Stock (HS) rats derived from 8 founder strains differ by sex in the IVSA of opioids. METHODS: Male and female Heterogeneous Stock (N=7-8 per sex) rats were permitted to

Mixed Presentation of Heroin-Associated Spongiform Leukoencephalopathy and CHANTER Syndrome After Fentanyl Use: A Case Report.

BACKGROUND: Toxic leukoencephalopathy is a heterogenous syndrome characterized by destruction of brain white matter. Inhalational heroin has been linked to a specific set of neuroimaging findings termed heroin-associated spongiform leukoencephalopathy (HSLE). Clinical findings in HSLE generally develop and progress through an initial, intermediate, and terminal stage. Fentanyl has been linked to a separate set of neuroimaging findings and neurological symptoms labeled Cerebellar Hippocampal And Basal Ganglia Transient Edema with Restriction (CHANTER) syndrome, in which patients often present w

Shared and reinforcer-specific alterations in the CRH and noradrenergic systems following short- and long-term withdrawal from cocaine, heroin, and sucrose self-administration.

Stress is known to play a critical role in relapse to drug use as well as in food craving. Craving itself is a key determinant of relapse, and cue-induced drug craving has been shown to increase, or 'incubate', over time for certain drugs such as cocaine and nicotine, though this effect is less consistent for others such as opiates. However, the modulations of stress-related biochemical systems after early or protracted withdrawal that could contribute to this incubation phenomenon have not yet been systematically examined in animal models, nor has the specificity of these mechanisms been test

The efficacy and safety of short-acting, full agonist opioids for symptom management of opioid withdrawal.

INTRODUCTION: Patients with opioid use disorder often have undertreated withdrawal while in hospital, leading to high rates of premature discharges. Short-acting, full agonist opioids (SAFAOs) have been proposed to alleviate withdrawal symptoms when provided as adjuncts to opioid agonist therapy (OAT). We aimed to synthesize evidence on the safety and efficacy of SAFAOs. METHODS: We searched the scientific databases MEDLINE, Embase, CINAHL, PsycInfo, Web of Science Core Collection, and the Cochrane Library for studies with adults whose opioid withdrawal symptoms were treated with SAFAOs. Syringe service program utilization, behavioral, and experiential factors associated with greater naloxone protection in a longitudinal cohort of people who use illicit opioids in New York city.

BACKGROUND: Despite efforts to blanket high-mortality communities with no-cost naloxone, drug checking technologies, and alternatives to using opioids alone, there remains a considerable lack of knowledge about how exposure to these low-threshold interventions may impact rates of naloxone protection. METHODS: People who used illicit opioids in New York City were surveyed between April 2019 and April 2022. Naloxone protection was defined as the proportion of opioid use events in the past 30 days when opioid use occurred with naloxone present and someone to administer it. Mixed-effect Poisson mo

Molecular recognition properties of water-soluble Prism[5]arene towards drugs of abuse.

We report our study of the binding of a water soluble prism[5]arene (WPr[5]) toward a panel of 11 drugs of abuse by a combination of 1H NMR spectroscopy, isothermal titration calorimetry (ITC), and molecular modelling. 1H NMR spectroscopy showed that the narrower drugs (fentanyl, methamphetamine, MDMA, mephedrone) exhibited dramatic upfield shifts (Δ up to -4.8 ppm) upon complexation due to the 5 convergent magnetically shielding naphthalene walls of WPr[5]. Analysis of the complexation induced changes in chemical shift, backed up by molecular modelling, showed that WPr[5] encircles the ammoni

Differential acquisition of cocaine and heroin self-administration in a rat model of internalizing versus externalizing temperament.

Substance Use Disorders (SUDs) constitute a major and rising public health concern. In addition, there is a growing appreciation that different classes of addictive substances are likely to lead to qualitatively different types of SUDs requiring differing treatment and relapse prevention strategies to be most effectively managed. Biological temperament, particularly on the internalizing - externalizing axis, is well established to influence addiction susceptibility. Externalizing behavior has long been understood to predispose individuals to addiction through novelty-seeking, sensation-seeking

Failing those who most need support to quit smoking: Australia must do better.

Tobacco remains the leading cause of preventable death in Australia, and although overall smoking has declined, rates remain much higher among disadvantaged populations. Thus, those most likely to be nicotine dependent are least able to afford cessation treatment. With strict limitations on duration of treatment and the range of products currently subsidised through the Australian Pharmaceutical Benefits Scheme, Australian Federal Government spending on tobacco addiction treatment falls well short of levels recommended by the World Health Organization, and best-practice interventions are not a

Literature review on forensic implications of illicit drug metabolism: A medicinal chemistry perspective.

The metabolic processes of illicit substances are crucial within the realm of forensic toxicology, affecting the detection, analysis, and interpretation of drugs in legal and investigative contexts. Substances like opioids, cannabinoids, hallucinogens, benzodiazepines, and novel psychoactive substances (NPS) experience metabolic alterations primarily through Phase I (oxidation, reduction, hydrolysis) and Phase II (glucuronidation, sulfation) transformations. Fundamentally governed by cytochrome P450 (CYP) enzymes, they significantly influence the detectability and potential toxicity of the com
