Impact of a National Stepped-Care Model of Opioid Use Disorder Treatment on Access to Medication Therapies Among Homeless-Experienced Veterans.

OBJECTIVE: To determine whether the Veterans Health Administration's (VHA) Stepped Care for Opioid Use Disorder Train-the-Trainer (SCOUTT) initiative increased medication treatment for opioid use disorder (MOUD) among persons experiencing homelessness (PEH). STUDY SETTING AND DESIGN: We conducted a national retrospective cohort study of MOUD, including buprenorphine, naltrexone, or methadone, in federal fiscal years (FY) 2016-2020. In late FY2018, VHA regional directors nominated facilities to receive SCOUTT implementation support to expand prescribing of MOUD (primarily buprenorphine) in sel

Comparison of peribulbar anesthesia with neosaxitoxin or bupivacaine in dogs undergoing reconstructive keratoplasty.

BACKGROUND: This study compared the analgesic efficacy of peribulbar anesthesia using neosaxitoxin versus bupivacaine in dogs undergoing reconstructive keratoplasty. METHODS: Of 28 dogs, four were excluded for not meeting the inclusion criteria. The anesthetic protocol included dexmedetomidine and methadone, propofol and isoflurane. Immediately after intubation, 24 dogs were randomly assigned (12 per group) to receive peribulbar block with either neosaxitoxin (3.5µg; NeoSTX group) or 0.5% bupivacaine (0.2mL/kg, Bupi group). Intraoperatively, cardiopulmonary variables and end-tidal isoflurane

Health Information Technology Infrastructure and Availability of Medications for Opioid Use Disorder and Naloxone in U.S. Substance Use Disorder Treatment Facilities.

Medications for opioid use disorder (MOUD) reduce overdose mortality, yet most individuals with opioid use disorder receive no pharmacotherapy. Facility-level health information technology may reflect broader organizational capacity, but it has rarely been examined as a marker associated with readiness to offer evidence-based opioid care. This cross-sectional secondary analysis of the 2024 National Substance Use and Mental Health Services Survey (N-SUMHSS) Public Use File included a base sample of 20,813 substance use disorder treatment facilities and used complete-case adjusted logistic regre

Two decades of prescription opioid-related cardiovascular safety signals: a FAERS-based pharmacovigilance study using machine learning.

INTRODUCTION: Prescription opioids are essential for pain management but have been increasingly linked to cardiovascular adverse events. Understanding these risks is critical for informed prescribing opioid drugs. METHODS: Approximately 18 million adverse event reports from 2004 Quarter 1 to 2024 Quarter 3 in the U.S. FDA Adverse Event Reporting System (FAERS) were downloaded and processed. RxNorm was used to normalize prescription opioid drug names. Empirical Bayes Geometric Mean analysis was then performed to detect signals of disproportionate reporting. Cardiovascular-related Preferred Ter

A Biopsychosocial Model of Predictors of Methadone Maintenance Treatment Retention Among Iranian Men With Opioid Use Disorder.

BACKGROUND: Opioid use disorder remains a significant public health burden in Iran. While methadone maintenance treatment (MMT) is effective, its success is often limited by high attrition rates. Most protocols emphasize pharmacological factors, neglecting the multifaceted biopsychosocial nature of treatment retention. METHODS: This prospective longitudinal cohort study involved 342 men with opioid use disorder from 10 public and private treatment centers in Iran. Path analysis with diagonally weighted least squares (DWLS) was used to evaluate biological, social, and psychological predictors

Chronic pain treatment experiences and barriers among opioid agonist treatment participants who inject drugs: A qualitative study.

BACKGROUND AND AIM: Co-occurring chronic pain affects nearly half of people receiving opioid use disorder (OUD) treatment. Although this pain is often severe, it is usually inadequately managed. Stigma and clinical complexity associated with OUD are thought to further restrict access to pain care. This study examined barriers to pain treatment from the perspectives of people with both chronic pain and OUD. METHODS: A qualitative study was conducted using semi-structured interviews with 20 adults with lived experience of chronic pain and opioid agonist treatment (OAT) (i.e. prescribed methadon

Outpatient opioid dispensing during the 2024 floods in Southern Brazil: a population-based ecological time-series analysis.

BACKGROUND: Climate-related disasters can disrupt health-care delivery and treatment continuity. Evidence on their effects on opioid dispensing in middle-income countries remains scarce. We assessed whether the May 2024 floods were associated with changes in outpatient opioid dispensing in Rio Grande do Sul, Brazil. METHODS: We conducted a retrospective ecological time-series study using official Brazilian Ministry of Health data on codeine, morphine, and methadone dispensations from 2015 to 2025. Monthly rates per 10,000 inhabitants were modelled with quadratic secular and seasonal terms, mo

Prospective Preference Assessment of Potential Participants in the Perioperative Rectal Methadone in Spine Surgery (PROMISE) Trial.

BACKGROUND/AIMS: Perioperative pain management in spine surgery is challenging, with many patients suffering from chronic pain. The favorable pharmacokinetic-dynamic profile of methadone may offer a novel approach to perioperative analgesia; however, stigma around methadone and rectal administration may bias patient enrollment. Before initiating a trial comparing rectal methadone to placebo, we conducted a prospective preference assessment (PPA) to identify barriers and motivators for participation and characterize differences between willing and unwilling participants to refine the trial prot

Efficacy and safety of adjunct methadone with opioid therapy for cancer pain: a systematic review.

CONTEXT: The use of opioid analgesia for cancer-related pain is challenging. Adequate pain relief is not always achieved where prolonged prescription of opioids carry risks including adverse effects, tolerance and hyperalgesia. Recent studies suggest that methadone as an adjunct to ongoing opioid therapy is effective in reducing pain severity without increases in adverse effects. OBJECTIVES: To systematically evaluate current evidence of the efficacy, safety and tolerability of adjunct methadone in combination with ongoing opioid regimens for cancer-related pain. METHODS: A systematic search

Population-Stratified Distributions of Post-Mortem Blood Drug Concentrations: An Epidemiological Framework for Contextual Interpretation.

Interpretation of post-mortem drug concentrations remains a major challenge in forensic toxicology because factors, including post-mortem redistribution, drug tolerance, polypharmacy and inter-individual variability, limit the utility of fixed therapeutic, toxic or purportedly fatal concentration thresholds. Using data from the National Programme on Substance Use Mortality (NPSUM), we examined post-mortem blood drug concentrations within a population-based epidemiological framework. Quantitative concentration data were available for 93 psychoactive substances reported to NPSUM up to 1 November
