Effects of pregabalin added to a multimodal anesthetic protocol in cats undergoing ovariohysterectomy.

Advancement in anesthetic protocols is necessary to address the evolving challenges of perioperative cats' management in veterinary practice. In this study, 54 cats were enrolled and were randomly assigned to either Group P (Pregabalin, PGB) or Group C (Control). Group P received 5 mg/kg of PGB orally 90 min before hospital admission, while Group C received identical empty capsules. Sedation was assessed using a Feline Multiparametric Sedation Scale and a Simple Qualitative Scale. All cats were premedicated with methadone (0.2 mg/kg IM) and alfaxalone (1 mg/kg IM). Anesthesia was induced with

Decreasing fentanyl concentrations in urinalysis results for methadone treatment admissions: Evidence of changing fentanyl exposure in street drugs.

BACKGROUND: Recent declines in opioid-related overdose fatalities (OROF) in Chicago and nationally in the U.S. have prompted questions about whether changes in fentanyl exposure may be occurring in the illicit opioid supply. OBJECTIVE: This study examined temporal trends in fentanyl exposure among individuals entering opioid treatment, using admission urinalysis as a biological indicator of recent opioid exposure. METHODS: We conducted a secondary analysis of clinical admission data collected between 2023 and 2025 from a community-based opioid treatment program (OTP) on Chicago's West Side (

Methadone: Historical success, modern challenges and lessons for opioid drug discovery.

Methadone is a long-standing synthetic opioid that remains clinically important for pain management and opioid use disorder, yet it exemplifies the dual nature of opioid drug discovery. Despite its therapeutic success, methadone exhibits complex pharmacokinetics, polypharmacology and safety liabilities that have limited its broader clinical application. This review examines methadone as a case study to illustrate how chemical structure, stereochemistry, receptor pharmacology and pharmacokinetic properties collectively shape therapeutic outcomes and risk. We summarize key advances in methadone

Jail-based opioid treatment implementation, outcomes, and costs: lessons learned from the Massachusetts Justice Community Opioid Innovation Network (MassJCOIN).

Limited access to medications for opioid use disorder (MOUD) is a major driver of the opioid overdose crisis. Jails have been slow to implement MOUD treatment, especially agonist medications (methadone, buprenorphine). Massachusetts mandated certain county jails pilot-test delivery of all FDA-approved MOUD treatments, stipulating jails continue MOUD, or initiate it prior to release, and facilitate MOUD continuation in the community post-release. The Massachusetts Justice Community Opioid Innovation Network (MassJCOIN) partnered with seven pilot jails, the Massachusetts Department of Public Hea

Opioid Prescribing Changes in Advanced Cancer Before and After Withdrawal of Hydromorphone Slow-Release Tablets.

AIM: Ongoing disruptions to opioid supply in Australia have raised ongoing concerns about their impact on cancer pain management. The withdrawal of hydromorphone slow-release (SR) tablets from the Australian market has presented challenges to advanced cancer pain management. This study aimed to determine how this affected the utilisation of alternative slow-release opioids in patients with advanced cancer. METHODS: A retrospective cohort study was conducted at a quaternary cancer centre, comparing opioid prescribing patterns across two 12-month periods: before hydromorphone SR tablet disconti

Opioid exposure in neonates and infants following cardiac surgery: a comparative analysis.

INTRODUCTION: Both untreated pain and opioid exposure impact neurodevelopment in critically ill infants. Mitigating both remains a challenge in the management of infants undergoing cardiac surgery. Our aim was to describe and compare opioid exposure in neonates (≤ 30 days of age), and infants' (>30 days of age) post-cardiac surgery. METHODS: We conducted a secondary analysis of a single center retrospective cohort of all CICU surgical patients < 1 year of age to quantify opioid exposure within 5 days post-surgery. Patient demographics, surgical data, pain medications and pain/sedation scores

Postoperative pain outcomes following minor oral surgery in opioid tolerant patients: a retrospective cohort study.

BACKGROUND: Evidence guiding postoperative pain management in patients taking prescription or illicit opioids undergoing minor oral surgery is limited. It remains unclear whether standard analgesic protocols for opioid naïve patients provide adequate pain relief in opioid tolerant individuals. This study evaluated whether standard postoperative analgesia achieves adequate pain control in opioid tolerant patients undergoing minor oral surgery with expected mild to moderate pain. METHODS: A single centre retrospective study was conducted of patients undergoing minor oral surgical procedures at

Management of Patients on Medications for Opioid Use Disorder in the Perioperative or Peripartum Setting.

There is an increasing proportion of surgical and obstetric patients on medications for the treatment of opioid use disorder (medications for opioid use disorder [MOUD]), formerly known as medication-assisted treatment, in the United States. Due to lack of institutional protocols and limited evidence-based guidelines, perioperative management of patients on MOUD has largely been varied and often provider-dependent. This article explores the current evidence and recommendations for perioperative and peripartum management of patients on buprenorphine, methadone, and naltrexone; as well as key pr

A Retrospective Cross-Sectional Study on Methadone Poisoning: A Decade of Insights from Kerman, Iran.

INTRODUCTION: Methadone poisoning, involving both syrup and tablet formulations, represents a significant public health concern, particularly in regions where methadone is widely utilized for opioid substitution therapy. Methadone accounted for approximately 16% of pediatric acute poisonings and was the leading cause of drug-related deaths in 2023. This study provides a decade-long retrospective analysis of methadone poisoning cases in Kerman, Iran, with a focus on demographic patterns, clinical outcomes, and risk factors to guide prevention and treatment strategies. METHODS: This retrospecti

Managing the Operating Room Course for the Complex Pain Patient.

Patients with chronic pain require special attention when formulating the perioperative anesthetic plan. Understanding of the patient's preoperative medication regimen, physical and psychological limitations, type and location of pain, and presence of implanted analgesic devices is necessary to optimize the recovery process and minimize iatrogenic injuries. In general, it is recommended to continue the preoperative analgesic plan if not contraindicated and consider regional anesthetic techniques, multimodal analgesic adjuncts, and protocolized postoperative care plans (such as Enhanced Recover

Deviations from the standard of care in the treatment of pregnant people with opioid use disorder in jails; a mixed methods study.

BACKGROUND: Medications for opioid use disorder (MOUD) with methadone or buprenorphine are the standard of care for opioid use disorder (OUD) during pregnancy and postpartum. MOUD is not routinely available in US jails. Little is known about OUD management practices and patient experiences when MOUD is unavailable in jails. This study aims to assess deviations from standard pregnancy OUD care in jail and demonstrate patients' experiences of such deviations. METHODS: Mixed-methods, secondary analysis from two prior studies: (1) a survey of US jails (n = 836) regarding availability MOUD in preg

Comparative efficacy of naltrexone-accelerated versus sublingual buprenorphine in opioid withdrawal management and initiating injectable naltrexone treatment: study protocol for a randomized controlled trial.

BACKGROUND: Extended-release injectable naltrexone (XR-NTX), a long-acting opioid antagonist, is an effective relapse prevention strategy in the treatment of opioid use disorder (OUD). However, standard XR-NTX initiation requires completion of opioid withdrawal management and a 1-2 week opioid-free period. This interval is associated with heightened vulnerability to lapse and relapse, and many patients fail to initiate XR-NTX treatment successfully. Therefore, alternative strategies that enable rapid XR-NTX initiation are needed. This study aimed to compare the effectiveness and safety of nalt

Takotsubo Cardiomyopathy Associated With Severe Medetomidine Withdrawal: A Case Report.

BACKGROUND: Takotsubo cardiomyopathy (TCM) is a transient syndrome of cardiac dysfunction with no occlusive coronary arterial disease, which may mimic an ischemic cardiac event. The pathogenesis of TCM is thought to be primarily driven by the effects of a temporary catecholamine surge on cardiac and vascular tissue. Medetomidine (an a2 agonist commonly found in the Philadelphia fentanyl supply) withdrawal is characterized by agitation and sympathetic hyperactivity that can precipitate a catecholamine surge. Though withdrawal from other Central Nervous System (CNS)-depressing substances such as
