Case Report: Anaesthetic management in a canine patient with severe atrioventricular septal defect and pulmonary hypertension undergoing non-cardiac surgery.

INTRODUCTION: A 9-year-old, 6.8-kg intact male Petit Brabançon with a congenital partial atrioventricular septal defect (AVSD) presented for dental extractions. Pre-anaesthetic echocardiographic assessment revealed a large ostium primum atrial septal defect with bidirectional interatrial shunting, severe right atrial and ventricular enlargement, abnormal atrioventricular valves with severe tricuspid regurgitation, mild relative pulmonic stenosis, and moderate pulmonary hypertension (PH). Anaesthetic goals were to minimize increases in pulmonary vascular resistance (PVR), preserve right ventric

Opioid agonist treatment dispensing trends between 2018 and 2022 across six Canadian provinces.

Opioid agonist treatment (OAT) is the recommended first-line therapy for individuals with opioid use disorder. The availability of different products and prescribing guidance for OAT has rapidly evolved in recent years. Therefore, we conducted a repeated cross-sectional study using pharmacy dispensing data from six Canadian provinces to examine trends in methadone and buprenorphine use between January 2018 and December 2022. Monthly population-adjusted rates of OAT recipients were calculated by province, and annual cohorts were stratified by demographic characteristics and OAT type. Prevalence

The relationship between methadone dosage and plasma level changes with aging in methadone maintenance treatment (MMT) patients.

BACKGROUND: As aging is associated with physiological changes, patients in methadone maintenance treatment (MMT) may require an adequate methadone dose adjustment. We aimed to study the relationship between aging and methadone dose and plasma level. METHODS: Patients with two methadone plasma level results, above one year apart, were included. Methadone dose and plasma levels, as well as their changes over time, were studied by age group. RESULTS: 318 patients, 36.5% aged <50 years, 53.5% 50–64, and 10.1% aged ≥65 years, were compared. Methadone dose did not differ between groups (120.8±38.6 m

A mixed methods assessment of an electronic health record-embedded intervention with supportive education and outreach to increase in-hospital opioid use disorder treatment initiation.

BACKGROUND: In-hospital opioid use disorder medication (MOUD) initiation remains low despite its effectiveness to reduce mortality. OBJECTIVE: Assess whether a non-interruptive electronic health record (EHR)-embedded intervention with supportive education and outreach increased in-hospital MOUD initiation without disrupting clinical workflow. METHODS: In this mixed-methods study across 12 hospitals, we conducted an interrupted time series analysis of hospitalizations involving OUD and/or opioid poisoning. We condcuted a directed content analysis of interviews from 60 hospital-based clinician

Neuroleptic Malignant Syndrome Induced by Multiple Antipsychotics in a Patient Receiving Methadone.

Neuroleptic malignant syndrome (NMS) is a rare but potentially life-threatening adverse reaction to dopamine antagonists. In palliative care settings, recognition of NMS may be difficult because delirium, polypharmacy, and metabolic disturbances are common, and atypical presentations have been reported. A 73-year-old woman with advanced gastric cancer was admitted for refractory cancer-related pain. Methadone was initiated, and multiple antipsychotics were subsequently prescribed for delirium. On hospital day 20, she developed altered mental status and generalized lead-pipe rigidity without fe

Analgesic efficacy of peri-operative methadone in orthopaedic surgery: protocol for a systematic review of randomised controlled trials.

INTRODUCTION: Optimising post-operative pain management is crucial for recovery in orthopaedic surgery. Methadone has attracted interest due to its long half-life, N-methyl-D-aspartate -receptor antagonism and potential to reduce post-operative opioid consumption. Existing reviews combine multiple surgical populations, limiting applicability to orthopaedic settings. This protocol outlines a systematic review assessing the analgesic efficacy and safety of peri-operative methadone in adult and adolescent orthopaedic patients. METHODS AND ANALYSIS: This review will include randomised controlled

Characteristics of United States Food and Drug Administration Drug Recalls Involving Opioid Medications, 2002-2025.

PURPOSE: To characterize the frequency, causes, and severity of opioid medication recalls using United States Food and Drug Administration (FDA) Enforcement Reports. METHODS: We conducted a 2002-2025 descriptive analysis of recalls involving seven opioids. FDA free-text recall reasons were categorized into five standardized domains (wrong dose/potency, contamination, mispackaging/mislabeling, defective delivery system, and quality assurance deviations) by two independent reviewers, with agreement assessed using Cohen's κ (95% CI). Recall severity (Class I-III) was compared across drugs using

Geographic Disparities in the Opioid Overdose Crisis: Access, Treatment, and Mortality.

The opioid overdose crisis in the United States (U.S.) exhibits profound and persistent geographic inequalities, where place of residence is a key determinant of risk, access to care, and mortality. This narrative review synthesizes evidence on the spatial evolution of the epidemic-from its prescription opioid origins in Appalachia to the nationalized spread of illicit fentanyl and polysubstance use-revealing that mortality "hotspots" are direct manifestations of underlying social vulnerability, economic distress, and healthcare infrastructural deficits. We document a severe, multi-layered cha

Exploring Preferences in Emergency Department Coordination of Opioid Use Disorder Treatment by Race and Ethnicity.

OBJECTIVE: Significant racial and ethnic disparities exist in treatment referral engagement for opioid use disorder (OUD) after an emergency department (ED) visit. Little is known about how contextual factors influence preferences for ED OUD treatment coordination and how these preferences vary by race and ethnicity. This study seeks to evaluate and compare structural and support-related factors by eliciting ED OUD treatment coordination preferences among Black, Hispanic, and White individuals with OUD. METHODS: Two trained qualitative investigators conducted semi-structured telephone individ

Evaluation of Therapeutic Opioids in Skin-Derived Matrices (Sweat and Sebum) of Neonatal and Pediatric Patients and Their Role in Opioid Incorporation Into Hair.

Hair analysis is a valuable tool in forensic toxicology to assess opioid exposure in both adult and pediatric populations. However, interpretation of analysis results is challenged by the scarcity of reliable reference data on opioids in hair and by the lack of knowledge on involved hair incorporation pathways, e.g., via skin-derived matrices such as sweat and sebum. In order to address these limitations, we conducted a clinical study in cooperation with the University Children's Hospital Zurich, where we obtained and analyzed hair and skin swab samples of 150 pediatric intensive-care patients

Centrifugal pipette-tip extraction using pyrolyzed cotton fibers combined with mass spectrometry: High-throughput analysis of methadone and its major metabolite in human urine.

A microextraction procedure is presented based on centrifugal-pipette tip microsolid phase extraction using pyrolyzed cotton fibers as sorbent to determine methadone and its major metabolite in urine samples by substrate spray mass spectrometry. The variables affecting the extraction and ionization of the analytes were studied, including the amount of sorptive phase (4 mg), sample volume (1 mL), washing volume (0.5 mL), centrifugation speed (4000 rpm), and eluent (acetonitrile/isopropanol, 1/1). The proposed analytical method was validated yielding limits of detection lower than 1.5 µg/L and d

Opioid Agonist Therapy for Fentanyl-Related Opioid Use Disorder: A Systematic Review.

OBJECTIVES: While treatment guidelines for opioid use disorder (OUD) are well-established, specific guidance for people who use fentanyl remains limited. This systematic review is the first to examine effectiveness and safety outcomes associated with opioid agonist therapy (OAT), specifically buprenorphine, methadone, and slow-release oral morphine, in this patient population. METHODS: Following PRISMA guidelines, we systematically searched EMBASE, Medline, PsycINFO, CENTRAL (all via Ovid), and Scopus from inception to April 2025 for studies reporting OAT for fentanyl-related OUD. Buprenorphine Oral Lyophilisate for Treatment of Opioid Use Disorder: Pharmacology and Clinical Efficacy.

Background/Objectives: Opioid use disorder (OUD) is a chronic relapsing condition associated with elevated mortality and substantial psychiatric and somatic comorbidity. Oral methadone and sublingual and depot buprenorphine are the undisputed gold standard in opioid agonist treatment (OAT). More recently, another oral buprenorphine formulation, buprenorphine lyophilisate (BUP-Lyo), has been introduced into clinical practice, offering potentially faster bioavailability and simplified administration. This review aims to summarize the available clinical and pharmacological data on BUP-Lyo and ass

Medications for Opioid Use Disorder, Opioid Withdrawal, and Opioid Overdose: A Review.

IMPORTANCE: Opioid use disorder (OUD) involves compulsive opioid use that causes substantial distress or impairment at work, school, or home. OUD, which is the third most prevalent substance use disorder worldwide, affected an estimated 3.7% of US adults (9 367 000) in 2022 and caused 53 774 deaths in the US in 2024. Medications for opioid use disorder (MOUD) reduce morbidity and mortality. Individuals with OUD also benefit from medications to treat opioid withdrawal and reverse acute opioid overdose. OBSERVATIONS: Methadone, buprenorphine, and naltrexone are medications approved by the US Fo

The use of cellphone based automated pupillometry to quantify opioid withdrawal in patients receiving treatment for opioid use disorder.

INTRODUCTION: Overdose deaths attributed to opioid misuse resulted in over 50,000 deaths in 2024. Opioid agonist therapies such as methadone and buprenorphine are effective for the treatment of opioid use disorder (OUD). Yet, patient retention is an issue due to the challenges of stabilizing patients on an appropriate dose during the initial phase of treatment (induction). Accurate and objective quantification of opioid withdrawal can better optimize dose titration during induction. The study goal was to validate the use of cellphone-based pupillometry for assessing withdrawal in OUD. METHODS

When profit meets public health: private equity in methadone treatment.

Opioid overdose deaths in the U.S. exceed 80,000 annually and methadone, an effective treatment for opioid use disorder (OUD), remains difficult to access. Understanding how ownership structures affect access, quality, cost, and regulation in methadone treatment for opioid use disorder in the U.S. is thus a public health imperative. Private equity (PE) has rapidly consolidated opioid treatment programs (OTPs), now owning nearly one-third of all facilities that are the primary means for receiving methadone treatment. This growth is rooted in a complex federal regulatory regime and public-financ
