Buprenorphine utilization following removal of prior authorization requirements in the Kentucky Medicaid population.

PURPOSE: To evaluate changes in buprenorphine dispensing for treatment of opioid use disorder (OUD) among Kentucky Medicaid enrollees following the removal of prior authorization (PA) requirements for the transmucosal buprenorphine combination product. METHODS: We analyzed Kentucky Medicaid claims from 1/1/2016 through 6/30/2023 to identify enrollees with OUD during each quarter (Q1/2017-Q2/2023), using a 12-month lookback window. Segmented regression analysis was used for statistical modeling of an interrupted time series (ITS) design to assess changes in the quarterly proportion of enrollee

The past is prologue: Reconceptualizing methadone and buprenorphine for post-operative pain.

Opioids remain a central element of perioperative pain management despite increased utilization of multimodal adjuvant strategies intended to reduce their use. Contemporary use of methadone and buprenorphine, both conceived as opioid analgesics, is dominated by their essential roles in the treatment of opioid use disorder. Compared to usual care opioids, however, they provide improved post-operative pain control while reducing analgesic requirements. These findings argue for their expanded role within perioperative multimodal analgesia strategies.

Darters (Etheostoma spp.) as indicators of antidepressant and drug of abuse exposure in an urban watershed.

Pharmaceuticals and drugs of abuse (DoA) are continuously released into aquatic environments, but their biological uptake and effects in wild fish remain poorly characterized. Neuroactive substances, including antidepressants, opioids, and stimulants, are particularly concerning due to their potential to alter fish behavior and physiology. The present study aimed to investigate the bioaccumulation of 26 neuroactive substances in wild fish associated with an outfall from a municipal wastewater treatment plant (WWTP). First, an extraction method was developed for small-bodied fish that combines

Impact of enhanced practices on opioid overdose deaths: A community-based modeling approach.

BACKGROUND AND AIMS: The opioid crisis is still a public health emergency in the United States, despite recent declines in opioid overdose deaths (OODs) and increased availability of evidence-based practices (EBPs) for opioid use disorder (OUD). The geographic variability in OODs drives the need for localized decision-making, where interventions are tailored to the unique needs of communities. This study aimed to develop and calibrate a simulation model that evaluates the impact of enhanced implementation of EBP on OODs at the community-level. DESIGN: We developed OPSiM (Opioid Policy Simulat

Intraoperative Methadone for Post-craniotomy Pain Control: A Matched Cohort Exploratory Framework Study.

Background Postoperative pain following craniotomy is commonly treated with multimodal analgesic regimens requiring frequent dosing, which contributes to an increased opioid burden. Methadone, a μ-opioid receptor agonist and N-methyl-D-aspartate (NMDA) receptor antagonist, may provide extended analgesia in craniotomy patients from a single intraoperative dose. Methods We performed a single-center feasibility retrospective matched cohort study of adult patients undergoing craniotomy for tumor resection between October 2024 and March 2025. Primary outcomes included postoperative pain scores and

Racial and ethnic differences in medications for opioid use disorder and overdose in 11 state Medicaid programs from 2016 to 2020.

INTRODUCTION: Access to medications for opioid use disorder (MOUD) varies across racial and ethnic groups. As the largest payer for MOUD, Medicaid has potential to ensure access for all enrollees. We aimed to quantify recent trends and state variation in MOUD use by race and ethnicity in Medicaid. METHODS: Using a distributed research network, we conducted a cross-sectional study of 11 states' Medicaid data from 2016 to 2020. Among enrollees diagnosed with opioid use disorder (OUD), we measured the percent receiving MOUD (overall; and methadone, buprenorphine, naltrexone, separately), continu

Opioid-specific brain connectivity dynamics distinguish analgesia from secondary effects: Studies in male mice.

The µ-opioid receptor (MOP) is a critical pharmaceutical target that mediates both the therapeutic benefits and adverse effects of opioid drugs. However, the large-scale neural circuit dynamics underlying key opioid effects, such as analgesia and respiratory depression, remain poorly understood, hindering the development of safer analgesics. Here, we present a multimodal experimental framework that integrates functional ultrasound imaging through the intact skull with behavioral and molecular analyses to investigate opioid-induced large-scale functional responses and their physiological releva

Perception of Malaysian primary care practitioners towards patients with substance use disorders in Selangor and its associated factors: A cross-sectional study.

INTRODUCTION: Substance use disorders (SUDs) represent a significant public health concern, with an estimated 36.3 million people affected worldwide and approximately 100,000 active drug users in Malaysia. As individuals with SUDs frequently disengage from treatment, the cultivation of non-judgemental attitudes among healthcare professionals is critical. This study aimed to assess the perceptions of primary care practitioners (PCPs) in Selangor towards patients with SUDs and identify the associated factors. METHODS: A cross-sectional study was conducted from June 2021 to November 2023 among 2

"Talk About It!" Shared Decision-Making for Methadone and Buprenorphine Initiation: Development and Preliminary Evaluation of a Conversation Aid via Community-Based Participatory Research.

OBJECTIVES: Although robust evidence exists supporting opioid agonist treatment (OAT) as the best currently available treatment for opioid use disorder (OUD), many barriers prevent widespread access to the two most efficacious forms of OAT, buprenorphine and methadone. We sought to address interpersonal and knowledge-related barriers to OAT implementation by facilitating shared decision-making in the context of ED care. The objective of this study was to refine and evaluate an OUD treatment conversation aid in partnership with people with lived experience (PWLE). METHODS: We combined communit

Investigation of the impact of D, L-methadone on ALA-PDT in multiple glioblastoma cell lines.

BACKGROUND: Glioblastoma multiforme (GBM) is the most common and aggressive malignant brain tumor, characterized by infiltrative growth, resistance to chemotherapy and radiotherapy, and an immune-suppressive microenvironment. Interstitial Photodynamic Therapy (iPDT), which involves oral ALA administration and localized light activation, offers a promising approach by killing tumor cells, disrupting tumor vasculature, and stimulating the immune response. However, iPDT faces limitations due to the blood-brain barrier (BBB), insufficient irradiation of satellite and margin tumors, hypoxia within
