The impact of medications for opioid use disorder (MOUD) on retention and recovery capital in recovery housing: An exploratory causal inference analysis.

BACKGROUND: Medications for Opioid Use Disorder (MOUD), including methadone (MET), sublingual buprenorphine (SL-BPN), extended-release buprenorphine (ER-BPN), and naltrexone (NTX), are increasingly used in recovery housing settings. However, their differential impact on retention and recovery capital remains underexplored. METHODS: We applied marginal structural models to explore the causal effects of MOUD on retention and recovery capital among individuals with substance use disorders in Virginia Recovery Residences (January 2020-May 2024). Model 1 compared MOUD-exposed (n = 509) and non-exp

High-Dose Buprenorphine Initiation to Rapidly Transition From High-Dose Methadone in the Hospital.

Transitions from high-dose methadone to buprenorphine remain challenging, and the evidence base for high-dose buprenorphine initiation (HDBI) strategies in this context is limited. We reviewed the published literature on HDBI with a particular focus on their application for patients maintained on methadone and identified no prior reports of successful transition from methadone doses as high as 180 mg. We then describe a hospitalized patient who underwent a safe and effective transition from 180 mg of methadone to buprenorphine through a high-dose initiation protocol with support of adjunctive

Intravenous paracetamol does not have significant opioid-sparing effects when used as part of a multimodal analgesic protocol in dogs undergoing elective orthopaedic surgery.

BACKGROUND: Data evaluating paracetamol combined with NSAIDs in dogs are scarce. Results of clinical studies in dogs investigating intravenous paracetamol vary. METHODS: Dogs were randomised to either receive 10 mg/kg paracetamol intravenously after induction of anaesthesia and every 8 hours during hospitalisation (test) or not (control). In both groups, meloxicam or robenacoxib was administered at the licensed dose, interval and route. Intraoperative nociception or postoperative pain, defined by a Glasgow composite measure pain scale-short form score greater than 4 of 20, was treated with 0.

Intraoperative Methadone Versus Epidural Analgesia for Perioperative Pain Management in Major Abdominal and Thoracic Surgery: A Retrospective Single-Center Study.

Background: Adequate analgesia is essential for enhanced recovery following major abdominal and thoracic surgery. Intravenous methadone has emerged as an alternative analgesic modality to traditional epidural analgesia. This study compares intravenous methadone with epidural analgesia in postoperative pain. Methods: We retrospectively analyzed adult patients who underwent laparotomy or non-cardiac thoracotomy between January 2019 and December 2022 and who had either general anesthesia with epidural analgesia or intravenous methadone. Co-primary outcomes were mean numeric rating scale (NRS) pai

Perspectives from opioid treatment program staff on community pharmacy-based methadone dispensing in North Carolina.

INTRODUCTION: Methadone is an effective medication for the treatment of opioid use disorder, yet barriers to treatment access exist. Community pharmacies are more accessible than opioid treatment programs (OTPs) and are utilized in countries other than the United States to observe methadone dosing and dispense take-home doses. The purpose of this study was to assess North Carolina OTP staff perceptions towards community pharmacy-based methadone dispensing. METHODS: A cross-sectional online survey was used to assess the perceived acceptability, appropriateness, and feasibility of community pha

Commonly Used Opioids in the Orthopedic Operating Room: A Narrative Review.

Orthopedic surgical procedures are consistently associated with significant postoperative pain due to extensive tissue manipulation, osseous injury, and inflammatory responses. Effective perioperative analgesia is therefore fundamental to successful orthopedic outcomes, influencing early mobilization, complication rates, length of hospital stay, and patient satisfaction. Although opioid-sparing and enhanced recovery after surgery (ERAS) pathways have gained popularity, opioids remain central to analgesic management in orthopedic operating rooms. This narrative review examines the opioids most

Assessing the stability of drugs of abuse and pharmaceuticals in postmortem blood samples.

BACKGROUND: Reliable toxicological analysis is crucial for accurate forensic and clinical interpretation; however, pre-analytical factors such as handling and storage can significantly alter drug concentrations in postmortem (PM) samples, potentially leading to misinterpretation. Postmortem degradation, influenced by enzymatic and microbial activity, can change drug levels, making it essential to understand drug stability in biological matrices. OBJECTIVES: This preliminary study investigates the long-term stability of drugs of abuse and psychoactive substances in PM blood samples from drug-r

Treating Opioid Use Disorder With Methadone in Pharmacies.

IMPORTANCE: Methadone, a medication for opioid use disorder, is underused in the US response to the opioid overdose crisis. Under federal law, methadone can be offered in pharmacies via medication units, although a change to federal statutes or regulations could allow more options for pharmacists to dispense methadone. OBJECTIVE: To analyze innovative return on investment (ROI) for 2 pharmacy-based models: (1) pharmacy-based medication unit and (2) pharmacist-dispensed methadone. DESIGN, SETTING, AND PARTICIPANTS: This cross-sectional economic evaluation, conducted from March 13, 2024, to Au

[Deprescribing methadone for chronic non-cancer pain: management of withdrawal symptoms].

In this case report, a 60-year-old patient was referred to a multidisciplinary pain clinic to discontinue methadone. Before referral, a self-managed dose reduction from 40 to 15 mg had led to loss of function due to withdrawal symptoms. After referral, methadone was successfully deprescribed over three months. Withdrawal symptoms were managed through an individually tailored plan, supportive consultations with a nurse, and the use of clonidine. The patient regained function after deprescribing methadone. Deprescribing opioids can be managed through a patient-centered approach and close monitor

Comparison of naloxone, naltrexone, nalmefene, and methocinnamox for preventing and reversing the discriminative stimulus effects of buprenorphine and carfentanil in rats discriminating fentanyl from saline.

Naloxone and nalmefene are μ opioid receptor (MOR) antagonist medications for reversing opioid overdose and naltrexone is a MOR antagonist medication for treating opioid use disorder (preventing relapse). MOR agonist medications for treating opioid use disorder include methadone and buprenorphine. The magnitude of effect of buprenorphine can be less than the effect of higher efficacy MOR agonists with some effects of buprenorphine (eg, ventilatory depression) resistant to reversal by naloxone, compared with reversal of the effects of other MOR agonists. Drug discrimination was used to compare

Chronic tramadol abuse as a cause of serotonin syndrome.

Serotonin syndrome is characterized by autonomic hyperactivity, altered mental status, and neuromuscular abnormalities, ranging from mild to life-threatening manifestations. Tramadol has been associated with serotonin syndrome, predominantly in combination with other serotonergic agents or following acute overdose. Reports describing serotonin syndrome caused by chronic high-dose tramadol use as a single serotonergic agent are scarce. A case of serotonin syndrome in a young adult with chronic high-dose tramadol dependence in the absence of co-ingested serotonergic drugs is presented. A 22-year

The Association Between Receiving Medications for Opioid Use Disorder and Human Immunodeficiency Virus Testing: Findings From the Rural Opioid Initiative.

OBJECTIVES: Injection drug use is associated with increased risk of blood-borne infectious disease transmission. People with opioid use disorder (OUD) may be less likely to receive health care services, including human immunodeficiency virus (HIV screening), especially in rural areas. Addiction treatment settings may provide additional opportunities to screen for HIV in this population. METHODS: The rural opioid initiative (ROI) conducted a survey of people who use drugs (PWUD) on substance use, health care access, and utilization. From January 2018 to March 2020, PWUD were enrolled across 65

A Case of 7-Hydroxymitragynine Use Disorder Treated With Buprenorphine.

BACKGROUND: Kratom is a plant widely consumed for the stimulant and opioid properties of its primary psychoactive component, mitragynine. A related compound, 7-hydroxymitragynine (7-HMG), also known colloquially as "7-OH," "7-Hydroxy," "7-OHMG," or "7," is present in kratom in smaller amounts but is much more potent at the µ-opioid receptor. Increasingly, concentrated 7-HMG is being marketed at gas stations, smoke shops, and online, exposing people to this higher potency opioid in larger quantities. Little has been published about the management of withdrawal from and addiction to 7-HMG. CASE

Changes in Blood DNA CpG Methylation Levels in Response to Methadone Maintenance Treatment: Epigenome-Wide Longitudinal Study.

BACKGROUND/OBJECTIVES: Methadone maintenance treatment (MMT) is one of the major pharmacotherapies for opioid use disorder. The underlying mechanisms of addiction and the treatment response are only partially understood. The study's main goal was to identify differential DNA CpG methylation that occurred in response to MMT. METHODS: Toward this goal, we have conducted a longitudinal epigenome-wide study of blood samples from 64 patients at the beginning and after 1-3 years of MMT, using a linear mixed model. RESULTS: A total of 1881 differentially methylated probes (DMPs) were identified (FD
