RATIONALE: Maladaptive drug choice is a defining component of substance use disorders; whereby drug misuse persists despite adverse consequences. A goal of behavioral interventions, such as contingency management programs, is to promote reallocation of behavior towards adaptive pursuits. The neurobiological mechanisms that govern behavioral reallocation in the context of ongoing cocaine use remain poorly understood. OBJECTIVE: Here, we demonstrate a cocaine versus sucrose choice paradigm in male and female rats using a novel format that dissuades exclusive sucrose or cocaine choice by offering

Using the Dimensional Model of Psychopathology as our theoretical framework, we investigated whether developmental patterns of maternal harshness and quality of the home environment predicted autonomic regulation in a high-risk sample of adolescents. Maternal harshness was assessed at 5-points (infancy-preschool), quality of the caregiving environment was assessed at 3-points (early school age) and autonomic regulation during a social stressor was assessed during early adolescence in 203 mother-child dyads. Quality of the home environment increased over time whereas maternal harshness increase

Substance use disorder (SUD) is a major worldwide health problem with a historic global high of 316 million people using drugs, representing a 15% rise in prevalence over the previous decade. A comprehensive understanding of the pathophysiology of SUD will enable the development of novel therapeutic strategies to improve patient outcomes. Preclinical research on the neurobiology of SUD primarily focuses on the immediate monoaminergic systems response, changes in gene expression, and long-term maladaptive synaptic and circuit-level alterations as the key pathophysiological mechanisms. A few rec

BACKGROUND: A current model categorizes drug takers into high versus low takers (HT and LT) based on their drug intake levels, with the assumption that these groups represent different phenotypes. When several drug doses are considered, the inverted u-shaped dose-response curves (IUDR) of HT are shifted upwards and rightward, relative to that of LT. However, these IUDR 'shifts' are not quantitative metrics and may be subjective. Also, differences in intake levels do not necessarily imply distinctions in other variables (such as demand elasticity) that are important for drug user phenotypology.

Cocaine use disorder (CUD) remains a major public health challenge with limited effective therapeutic options. Cannabidiol (CBD) has been proposed as a potential treatment due to its neuroprotective and anti-craving properties. This scoping review aims to synthesize and map the existing evidence from clinical trials evaluating CBD's effects on craving reduction, relapse prevention, cognitive function, and emotional regulation in individuals with CUD. Although CBD was well tolerated and associated with fewer adverse events than conventional pharmacological treatments, clinical studies included

OBJECTIVE: To analyze the burden of disease from drug use disorders (DUD) in Mexico by state from 1990 to 2021 and to assess the association of this burden of disease with the Healthcare Access and Quality Index (HAQI).
METHODS: Secondary data analysis from the GBD-2021 study. The paper analyzes mortality, premature mortality, disability and disability-adjusted life years (DALYs). A log-linear segmented regression model to analyze trends in DUD DALYs over time was used. The information was disaggregated by age-groups and sub-causes, at the national and state levels.
RESULTS: DUD mortality in

AIMS: Cocaine use disorder prevalence is around 2.2% for individuals age 12 and older, with higher rates reaching 6.2% in young adults ages 18-25. In 2021, around 23% of all US overdose deaths were related to cocaine. Cannabis use disorder is more prevalent than cocaine use disorder with prevalence reaching 14.7%. This study aims to evaluate the impact of cocaine and cannabis on clinical cardiovascular outcomes. Specifically, it investigates the incidence of new-onset cardiac arrhythmia between the cocaine and cannabis users, as well as the incidence of cardiac arrest events, major adverse car

Abstinence from cocaine use has been shown to elicit a progressive intensification or incubation of cocaine craving/seeking that is posited to increase the likelihood of relapse. While the mechanisms underlying incubation of cocaine seeking remain elusive, considerable evidence suggests that abstinence from cocaine promotes mesolimbic dopamine adaptations that contribute to exaggerated cocaine seeking. Consequently, preventing these dopamine adaptations may reduce incubation of cocaine seeking and thereby decrease the likelihood of relapse. In the present studies, we first examined the relatio

Background: In January 2023, British Columbia (BC) implemented a three-year pilot decriminalizing possession of up to 2.5 grams of certain illegal drugs, including opioids, cocaine, methamphetamine, and MDMA, the first policy of its kind in Canada. This initiative has faced scrutiny, culminating in a May 2024 amendment banning possession and use in public spaces. Objectives: To examine public opinion on BC's decriminalization policy by assessing perceptions of the 2024 policy amendment, potential changes in support between 2024 and 2025, and demographic factors associated with support for repea

INTRODUCTION: Suicide is a major global cause of premature mortality and a critical public health concern. Chronic alcohol misuse and illicit drug use are well-established risk factors, but conventional post-mortem toxicology frequently fails to detect long-term substance exposure. Hair analysis represents a valuable forensic tool, enabling the reconstruction of pre-mortem behavioral patterns and offering insight into chronic substance use and its association with suicidal behavior.
MATERIALS AND METHODS: This retrospective study examined 74 suicide cases (59 males, 15 females) from 2023 to 2

Substance use is frequently implicated in psychosis. However, the prognostic assumptions guiding differential treatment decisions between substance-induced and primary/affective psychoses have received inconsistent support. Substance-induced psychosis research has often been limited by aggregating substances or using diagnosis as both a grouping variable and an outcome (i.e., diagnostic conversion). The current study examined medical records for 1379 patients for 5 years following their first presentation with psychosis after January 2013 to a regional medical center. Clinically relevant outco

Progressive increases in cocaine craving or seeking during abstinence from cocaine are thought to contribute to relapse. Recent observations suggest that sleep disruptions during abstinence from cocaine may have a significant influence on cocaine seeking. While the neural mechanisms underlying the association between sleep disruptions and increases in cocaine seeking during abstinence continue to be investigated, alterations in mesolimbic dopamine transmission may be a common factor. In these studies, we assessed whether sleep disruptions during abstinence are associated with incubation of coc

Excessive and compulsive drug use despite adverse consequences is a hallmark of substance use disorder, yet individuals differ markedly in their vulnerability to develop these behaviors. Drugs of abuse are long known to alter endogenous dopamine (DA) signaling, but shared principles for how DA dynamics impact compulsive use among individuals and across drug classes are lacking. Here, we monitored DA release in the medial shell of nucleus accumbens (NAc) during cocaine and fentanyl self-administration, with or without coincident punishment, in large cohorts of mice. Contingent cocaine and fenta

Cocaine Use Disorder (CUD), like all other stimulant use disorders, remains a challenging condition to treat, particularly in the absence of FDA-approved pharmacotherapies. While contingency management (CM) is considered first-line treatment and has demonstrated strong evidence for efficacy, it remains underutilized due to financial, philosophical, and logistical barriers. Off-label use of psychostimulants has emerged as a potential therapeutic strategy, especially in patients with comorbid attention-deficit/hyperactivity disorder (ADHD). We present a case of a middle-aged male with a long-sta

Cocaine use disorder (CUD) is a neuropsychiatric disorder marked by compulsive drug-seeking and loss of control over cocaine intake, with women experiencing a faster addictive development and greater adverse outcomes despite lower incidence compared to men. Currently, there are no effective pharmacological treatments for CUD. Cannabidiol (CBD), a multitarget compound acting mainly on mediators of the expanded endocannabinoid system, has emerged as a potential therapeutic agent for substance use disorders. Though its effects have been researched in preclinical studies with males, its role in fe

RATIONALE: Human self-report data and rodent models indicate cocaine can induce negative affect, which may impact future cocaine use severity. Despite this, understanding of the neurobiology driving cocaine avoidance is limited. Within the striatum, the opioid peptides enkephalin and dynorphin are associated with cocaine reward and aversion, respectively. Additionally, striatal dynorphin signaling resulting from stress or cocaine withdrawal acts as a negative reinforcer to increase cocaine seeking.
OBJECTIVES: We validated the use of the cocaine self-administration runway in mice to measure t

Synaptic zinc (Zn2+) modulates dopamine and glutamate neurotransmission by binding to the dopamine transporter and glutamate receptors. Among other neurotransmitters, dopamine and glutamate critically regulate physiological processes and behaviors relevant to substance use disorders (SUDs) and addiction. In addition, Zn2+ interacts with inhibitory neurotransmitter systems, including GABA and glycine receptors, further influencing the excitatory-inhibitory balance within circuits relevant to addiction. Nevertheless, the specific involvement of synaptic Zn2+ in such processes is unknown. We prop

RATIONALE: One of the leading prognosticators of whether an adolescent or young adult will use drugs is whether the person's peers use drugs. Preclinical studies report that cocaine intake increases in the presence of a social partner that is also self-administering cocaine, but it is not known whether this effect extends across reinforcers.
OBJECTIVES: The purpose of this study was to examine the role of the reinforcing stimulus in the effects of social contact on cocaine self-administration.

INTRODUCTION: The south-central region of Somalia has faced recurrent armed conflicts, unrest and climatic shocks resulting in forced displacement, marginalization and social exclusion of the people affected by these enduring humanitarian crises. While psychological trauma and economic hardships are recognized contributors to mental health conditions and substance use, evidence on the prevalence and patterns of substance use in Somalia remains scarce. This study investigates the prevalence and patterns of substance use among populations living in conflict-affected districts of South-Central So
