Esophageal Perforation and Fistulization After Ingestion of Multiple Foreign Bodies: A Case Report.

Esophageal perforation is a life-threatening condition associated with high morbidity and mortality, particularly when diagnosis or treatment is delayed. Foreign body ingestion is a known cause of esophageal injury and may result in severe complications requiring surgical intervention. We report the clinical course and therapeutic management of a young adult who developed cervical esophageal perforation after deliberate ingestion of multiple foreign bodies associated with acute cocaine intoxication and self-induced emesis. The patient presented with cervical pain, extensive subcutaneous emphys

Formulation and Delivery Systems of Cocaine and Methamphetamine Scent Mimics.

Narcotic scent mimics, which are chemicals that emit a perceivable scent similar to that of the illicit drug but do not possess the health and addiction risks of the drug, have applications ranging from canine training in law enforcement agencies to antinarcotic public education. However, low-cost and easily accessible formulation and delivery systems for narcotic scent mimics have thus far been lacking. In this work, we assess the potential of synthetic (metal-organic framework, MOF) and natural (cotton) materials as carrier and delivery agents for pseudoscent chemicals mimicking cocaine and

The neuropeptide in ischemic brain injury: insights, challenges, and horizon of targeted interventions.

BACKGROUND: Cerebral ischemic stroke (CIS) is a highly lethal and disabling global disease with complex pathological mechanisms that pose significant challenges for effective treatment. In recent years, endogenous neuropeptides have emerged as critical signaling molecules, attracting increasing attention for their regulatory roles in the pathophysiology of CIS. METHODS: This systematic review examines the dynamic functions of various neuropeptides—including opioid peptides, oxytocin, orexin, pituitary adenylate cyclase-activating polypeptide (PACAP), calcitonin gene-related peptide (CGRP), coc

Lead optimization of AMS-17 identifies AMS-4 as a modulator of inflammasome-driven cocaine-associated neuroinflammation.

Cocaine use disorder (CUD) affects approximately 22.5 million people worldwide and is a major public health concern due to its widespread abuse and severe neuropsychiatric effects. Cocaine exposure induces neuroinflammation primarily through NLRP3 inflammasome activation in microglial cells, which releases proinflammatory cytokines such as IL-1β that contribute to neuronal dysfunction, making NLRP3 a key mediator of cocaine-induced neuropathology. Due to its critical role in neuroinflammatory signaling, the NLRP3 inflammasome is a promising therapeutic target for CUD. However, the development

MeCP2 in the nucleus accumbens regulates operant cocaine self-administration and effort-based responding.

Methyl-CpG binding protein 2 (MeCP2) is a transcriptional regulator implicated in diverse brain functions, including addiction. However, its specific role in drug-seeking motivation remains unclear. To investigate this, we targeted MeCP2 knockdown within the nucleus accumbens (NAc), a key region involved in reward processing, and assessed its impact on cocaine intake. Mice with NAc-specific MeCP2 knockdown (shMeCP2) underwent a cocaine self-administration paradigm. During the initial 7-day food training period, both control and shMeCP2 groups showed comparable improvements in lever-reward lear

Chemogenetic regulation of direct and indirect pathway neurons in the dorsomedial striatum reshapes cocaine-induced conditioned place preference behaviors in mice.

The rewarding properties of cocaine generate potent and long-lasting associations with environmental cues through altering neuronal properties, underlying reinforced drug-taking and seeking behaviors. Disrupting these associations could reduce cravings and forestall relapse. The dorsomedial striatum (DMS) is implicated in reward processing and goal-directed behaviors, yet its specific role in modulating cocaine-associated contextual memory remains poorly defined. To elucidate DMS's role in cue-induced memory retrieval, multifaceted approaches were employed in male mice, including the condition

Sociodemographic Characteristics and Drug Use Practices Associated with Fentanyl Use Frequency Among People Who Use Drugs in Rhode Island.

BACKGROUND: Since 2013, fentanyl has dominated unregulated drug supplies in the northeast and has become a key driver of overdose deaths. In this analysis, we sought to identify the characteristics associated with fentanyl use frequency among people who use drugs in Rhode Island. METHODS: We examined baseline data from the Rhode Island Prescription and Illicit Drug Study, which enrolled participants from August 2020 to February 2023. We used an ordinal logistic regression to identify sociodemographic and drug use characteristics that were associated with prior month fentanyl use frequency, wh

Granulocyte colony-stimulating factor acts through calcium-permeable AMPA receptors to potentiate cocaine reward.

Neuroimmune interactions have emerged as critical modulators of substance use disorders and may represent promising translational therapeutic targets. In prior work, we demonstrated that the cytokine granulocyte colony stimulating factor (G-CSF) is elevated in mice following cocaine exposure, with circulating levels correlating with cocaine intake and locomotor sensitization. Additionally, exogenous G-CSF enhances cocaine reward and increases low-dose cocaine self-administration. We have further shown that repeated G-CSF administration alters expression of glutamatergic synapse-associated prot

Synaptogyrin-3 plays a critical role in addiction-related dopamine dysfunction and behavioral maladaptations.

Cocaine use disorder is marked by deficits in dopamine signaling; however, the molecular mechanisms driving these deficits remain unclear. The synaptic vesicle protein Synaptogyrin-3 (Syngr3) has recently garnered attention for its association with disorders involving dopamine dysfunction and impaired cognition. Here, we found low Syngr3 expression in the ventral tegmental area (VTA) of men who died of cocaine overdose and in male rats that had chronically self-administered cocaine. Syngr3 was confirmed to be in dopamine neurons, and its expression was correlated with dopamine markers in both

Concurrent Serial Feature Negative and Simple Discrimination Training and Testing in the Mouse.

Prior research shows that rats can solve serial feature negative (serFN) and simple discrimination (SD) problems when both are trained concurrently. Furthermore, previous studies indicate that the solution to the serFN (but not the SD) problem depends on the functional integrity of the hippocampus. The present research aimed to determine if mice solve the serFN problem and if this is solved in a manner mediated by inhibitory modulation similar to that seen in rats. Establishing if mice, like rats, can solve the serFN problem using a potentially hippocampal-dependent strategy would provide rese

The role of polysubstance use in the development, maintenance, and treatment of stimulant use disorders.

Stimulant use disorders represent a significant public health challenge, with no U.S. Food and Drug Administration (FDA) pharmacotherapies currently available. A growing concern is that stimulant use rarely occurs in isolation. Instead, it often involves sequential or simultaneous use of multiple substances. This review explores the mechanistic, epidemiological, clinical, and preclinical dimensions of polysubstance use involving stimulants, particularly cocaine and methamphetamine. Key gaps in the existing literature are identified to underscore the critical need for polysubstance use research

Peer-led drug checking in Chile: a case study of the country's first harm reduction service at electronic music events.

BACKGROUND: The health risks for people who use drugs (PWUD) in Chile are exacerbated by evolving drug markets, unpredictable supplies, and the limited availability of harm reduction services. Community-based drug-checking programs have shown effectiveness in reducing some of these risks; however, regional evidence remains limited. CASE PRESENTATION: In 2024, Reduciendo Daño (RD), a Chilean non-profit organization, conducted a peer-led drug-checking program, attending 22 electronic music events in Santiago. Event attendees interested in testing their substances placed small samples on the des

Spontaneous ultra-early resorption of an intracerebral hematoma in a cirrhotic patient: case report and analysis of this unusual phenomenon.

Spontaneous intracranial hemorrhage (ICH) is the second most common form of stroke and represents a life-threatening neurological emergency. The clinical course of an intracerebral hematoma varies widely, ranging from stable clot containment to progressive hematoma expansion with resulting neurological deterioration and increased mortality. Conversely, rapid resorption of acute hematomas is an uncommon phenomenon that typically unfolds over several days. The association between cirrhosis and spontaneous ICH remains controversial, and the factors influencing hematoma evolution in cirrhotic pati

Glucagon-like peptide 1 receptor agonists in substance use disorders: A systematic review of ClinicalTrials.Gov.

BACKGROUND: Substance use disorders (SUDs) are widely prevalent and associated with high morbidity and mortality. Current treatments have limited efficacy and there are no United States Food and Drug Administration (FDA)-approved treatments for several SUDs (such as methamphetamine, cocaine, and cannabis use disorders). Emerging evidence suggests glucagon-like peptide 1 receptor agonists (GLP-1RAs) may improve outcomes related to SUD. Therefore, a systematic survey of ongoing clinical trials that are evaluating the effects of GLP-1RAs for SUDs is warranted. METHODS: We searched ClinicalTrials

Evaluating methylphenidate as a potential treatment for cocaine use disorder: a propensity score-matched study.

Background: Cocaine's psychoactive effects arise primarily from dopamine transporter (DAT) inhibition. Prior trials suggest methylphenidate, a DAT inhibitor with pharmacologic overlap with cocaine, may reduce cocaine use, but real-world data are limited. Objectives: To evaluate whether methylphenidate exposure is associated with reduced likelihood of subsequent positive cocaine screens among veterans treated within the VA healthcare system. Methods: This retrospective cohort study used the Veterans Affairs Informatics and Computing Infrastructure (VINCI) to identify 109,608 patients with a posit

Assessing working musicians' perspectives of need and barriers regarding lay harm reduction efforts for substance-related harm in South Carolina.

BACKGROUND: Harm Reduction (HR) approaches applied to substance use aim to decrease the toxic consequences of overdose. The addition of xylazine and benzodiazepines to fentanyl, the primary opioid in the US drug supply, increases the likelihood of overdose. Likewise, adding fentanyl to other popular substances, such as cocaine and methamphetamine, has further driven the exponential increase in opioid and stimulant overdoses. Engaging lay community members in HR efforts may help to combat the overdose crisis. Substance use is common in live music settings, and working musicians may be uniquely

Naloxone Training Among Sorority-Affiliated College Students: An Observational Study.

BACKGROUND: As a population, college students are known to be at risk for adverse substance use-related events, such as opioid overdose, as they seek relief from academic and social stressors. Sorority affiliation in this population is reputed to be another risk factor for increased substance use and misuse. Many college students are not aware that illicitly made fentanyl is often mixed with other commonly used substances, such as marijuana and cocaine (among others), potentially leading to unintentional overdose and even death. The timely administration of naloxone, an opioid antagonist, can

Observation of aldicarb hydrolysis by a cocaine hydrolase.

Aldicarb is a carbamate pesticide used for pest control in agriculture. As a fast-acting acetylcholinesterase inhibitor, aldicarb interferes with the nervous system by preventing the breakdown of acetylcholine. Aldicarb could be used as a chemical-warfare agent to cause mass casualty incidents. There is no specific FDA-approved medication for aldicarb detoxification. Our previous study revealed that an Fc-fused butyrylcholinesterase (BChE) mutant, known as CocH3-Fc(M3), can be inhibited rapidly by aldicarb and that the aldicarb-inhibited enzyme CocH3-Fc(M3) can be reactivated spontaneously, su
