Transcranial magnetic stimulation in cocaine use disorder and the risk of seizure: a review of the evidence.

Transcranial magnetic stimulation (TMS) has emerged as a promising intervention for cocaine use disorder (CUD). However, a key concern when employing TMS in CUD is the potential risk of seizures. Our goal was to assess seizure risk in individuals with CUD undergoing TMS and to propose parameters that could mitigate it. Our review of the literature indicated that seizures are primarily associated with high-dose cocaine use necessitating urgent medical care - and that the risk is likely low outside of this setting. Thus, to mitigate potential seizure risks during TMS sessions, we suggest an asse

Perinatal exposure to delta-9-tetrahydrocannabinol (THC) alters goal-directed behavior and dopamine functioning in wistar rats.

RATIONALE: Cannabis use during 2021pregnancy is common as many pregnant women consider cannabis as a safe way to alleviate symptoms associated with pregnancy because it is "natural". However, clinical evidence links perinatal cannabis exposure to externalizing behavior in offspring. In preclinical research, most studies focus on exposure to the psychoactive constituent of cannabis, delta-9-tetrahydrocannabinol (THC). THC is lipophilic, allowing it to cross the placental barrier and be secreted in maternal milk, thereby exposing the fetus/neonate. OBJECTIVE: We used operant procedures to measu

Differential effects of adolescent fatty acid amide hydrolase and monoacylglycerol lipase inhibition on adolescent and adult behaviors in male rats.

RATIONALE: Endocannabinoid signaling during adolescence plays a critical role in brain development and shapes both healthy and maladaptive behaviors, influencing the risk for neuropsychiatric disorders in adulthood. OBJECTIVES: The present study examines the effects of disrupted catabolism of the endocannabinoids 2-arachidonylglycerol (2-AG) and N-arachidonoylethanolamine (AEA) during early adolescence on adolescent and adult behaviors in male rats. METHODS: Male Sprague-Dawley rats received daily injections with the monoacylglycerol lipase inhibitor JZL184 (JZL; 10 mg/kg, ip), the fatty aci

Drugs in paradise: caffeine, cocaine, and painkillers detected in sharks from The Bahamas.

Pharmaceuticals and illicit drugs are increasingly recognized as contaminants of emerging concern (CECs) in marine environments, particularly in areas undergoing rapid urbanization and tourism-driven development. Potential exposure to such contaminants, however, remains largely unexplored in The Bahamas. This study provides the first investigation into the occurrence of selected CECs (acetaminophen, benzoylecgonine, caffeine, carbamazepine, ciprofloxacin, citalopram, clindamycin, cocaine, diclofenac, fipronil, fluoxetine, nimesulide, piroxicam, sertraline, sulfamethoxazole, triclosan, trimetho

Comorbid HIV and Cocaine Use Exacerbate Accelerated Brain Aging.

BACKGROUND: HIV and cocaine use (CU) each relate to cognitive deficits and brain abnormalities, yet their combined impact on brain aging remains unclear. This study examined how comorbid HIV and CU relate to brain aging and cognitive impairment. METHODS: We trained a morphometry-based brain-age model using harmonized Human Connectome Project-Aging data (HCP-A; n=725) with Gaussian Process Regression. The model was applied to an independent cohort with varying HIV/CU burden (HIV-/CU-, n=34; one disorder [HIV+/CU- or HIV-/CU+], n=72; HIV+/CU+, n=80). Brain age gap (BAG; predicted minus chronolo

Development and validation of a simple and robust LC-ESI single quadrupole MS-based method for cocaine detection in human urine.

PURPOSE: Advanced mass spectrometric platforms such as LC-MS/MS or quadrupole time-of-flight (QToF) are considered reliable for high-sensitivity detection of cocaine in human urine. However, these instruments are often not accessible to routine forensic laboratories due to financial and logistical constraints. The present study aims to develop and validate a cost-effective, rapid and environmentally suitable method using a single quadrupole-based LC-MS technique for the detection of cocaine in human urine. METHOD: The method employs an Acclaim C18 Column (150 × 3 mm, 5 μm) with a retention tim

Metabolism, Not Mendacity: Rethinking Prolonged Cocaine Positivity.

BACKGROUND: Urine toxicology is a cornerstone of monitoring abstinence in substance use disorder treatment, yet commonly cited detection windows are based on studies in healthy volunteers and do not account for metabolic variability. Prolonged metabolite positivity is typically interpreted as continued use, which can jeopardize treatment engagement and erode the therapeutic alliance. CASE SUMMARY: We describe a 42-year-old man with severe stimulant use disorder whose urine toxicology remained positive for cocaine metabolites for 18 days, including 12 days after verified abstinence in a reside

PKMζ in the prefrontal cortex alters cocaine cue-induced reinstatement in a sex-specific manner.

RATIONALE: Cocaine alters glutamate in the prefrontal cortex (PFC), which is a region critical for mediating relapse behaviors. Long-lasting changes in glutamate transmission indicate a role for cocaine-induced synaptic plasticity in the reward circuit. Protein kinase M-ζ (PKMζ), a constitutively active isoform of protein kinase C (PKC), plays a critical role in AMPA receptor (AMPAR) trafficking, including that which occurs after drug exposure. There is evidence for a role of PKMζ in drug reward as constitutive deletion of PKMζ potentiates cocaine self-administration. OBJECTIVES: As the PFC pl

Rapid Assignment of Chemical Shifts From Crystal Structures in Solid-State NMR.

Chemical shift assignment in solid-state nuclear magnetic resonance (NMR) is a challenging process that usually relies on a set of 1D and 2D experiments to determine the assignment by establishing connectivities along the covalent backbone. A Bayesian probabilistic assignment method was recently introduced based on a fragment analysis using a database of chemical shifts. Here, we propose a fast 3D structure validation method that utilizes predictions from a crystal structure as a starting point for Bayesian probabilistic chemical shift assignment. We demonstrate the approach with improved conf

Direct violence as drug-related harm: Basuco and harm reduction in Bogotá, Colombia.

In Bogotá, Colombia, the primary drug of social concern is basuco, a form of smokeable cocaine. Drug markets in Colombia are characterised by high levels of direct violence, a source of drug-related harm rarely addressed by harm reduction. Using data from eleven months of ethnographic fieldwork among people who use basuco and are experiencing homelessness in Santafé, Bogotá, this paper explores this direct violence, its association with basuco use, and how it could be incorporated into a harm reduction approach. It finds that people who use basuco face violence from: criminal actors through so

MicroRNA-132/212 negatively modulates opioid reward by targeting dopamine transporter in the ventral tegmental area.

Addictive drugs, notably opioid drugs, have significant societal implications, yet the cellular and molecular mechanisms underpinning rewarding effects remain largely elusive. Noncoding transcripts, including the microRNAs (miRNAs), are pivotal regulators of diverse biological functions. Notably, the microRNAs miR-132 and miR-212, arising from a shared noncoding transcript, have links to several psychiatric conditions, including cocaine addiction. However, their contribution to opiate rewarding remains speculative. In this study, we discovered that repeated morphine administration decreases th

Immunosuppressant treatment reduces cocaine-induced behavioral sensitization in mice.

BACKGROUND: Neuroinflammation plays fundamental, though still not fully understood, roles in the pathophysiology of substance use disorders, including cocaine addiction. Chronic cocaine exposure promotes neuroinflammatory signaling and synaptic alterations in brain regions involved in reward and memory, such as the striatum and hippocampus. Among the intracellular pathways regulating these processes, calcineurin, a calcium calmodulin-dependent phosphatase, has been implicated in synaptic plasticity, neuroinflammation, and psychiatric disorders. FK506 (tacrolimus), a calcineurin inhibitor and i

Social context in renewal of cocaine and fentanyl seeking: role of peer presence versus peer absence.

RATIONALE: Drug relapse among individuals with substance use disorders can be triggered by social cues that were present in the original drug-taking environment. Previous preclinical work demonstrated this process using the ABA renewal design in which rats were first trained to lever press for contingent cocaine with a peer (context A), then the response was extinguished without the peer (context B), and finally responding was shown to return or renew with re-presentation of the peer (context A). OBJECTIVE: This study sought to determine if the presence and/or absence of a peer would differen

Mechanisms of overdose from unintentional fentanyl use when intending to use stimulants: a qualitative study of people who use stimulants and avoid opioids.

BACKGROUND: As many as two-fifths of nonfatal opioid overdoses in San Francisco, CA are from unintentional fentanyl use, with most people intending to use stimulants. However, the mechanisms of fentanyl exposure are unclear. OBJECTIVE: To characterize mechanisms of unintentional fentanyl use resulting in non-fatal overdose among people who intend to use stimulants and not opioids. METHODS: We interviewed 17 people using stimulants and avoiding opioids who experienced an overdose attributed to unintentional fentanyl use between August 2023 and July 2024. Interviews were semi-structured and in

Distinct interneuronal dynamics selectively gate target-specific cortical projections in drug seeking.

Drug craving persists after prolonged abstinence, posing a major challenge in treating substance use disorders. The ventral medial prefrontal cortex (vmPFC) plays a critical role in impulsivity and decision-making, making it a promising target for mitigating drug craving by orchestrating downstream brain-wide activity. However, the dynamics of vmPFC sub-circuits during the progression of drug addiction remain unclear. Here, we uncover a circuit-level mechanism by which distinct vmPFC sub-circuits, defined by cell-type-specific interneurons and projection-specific cortical outputs, differential

Cracking the code - an integrated wastewater-based approach for a more comprehensive assessment of current trends in cocaine and crack use in Switzerland.

In Switzerland, as in the rest of Europe and beyond, increasing cocaine and crack use, along with related social and health harms, is a serious concern to authorities. The aim of this study was to provide objective data on this complex situation by integrating wastewater analysis, targeting benzoylecgonine (BE) and anhydroecgonine methyl ester (AEME) in 13 Swiss cities from 2021 to 2024, with street-level cocaine purity data obtained from near-infrared spectroscopy analyses, and survey data on user populations and administration routes used to compute a weighted BE excretion rate. Results high

Hypocretin receptor 1 blockade early in abstinence reduces future demand for cocaine.

Relapse to cocaine use after periods of abstinence remains a significant challenge for treating cocaine use disorder. While the neurobiological mechanisms underlying relapse are still under investigation, adaptations in mesolimbic dopamine systems may contribute to cocaine craving and propensity for relapse. Therefore, reversing or preventing these dopamine adaptations may reduce motivation for cocaine and decrease the likelihood of relapse. The hypocretin/orexin system has been shown repeatedly to regulate cocaine-associated behavior and dopamine transmission. For example, our previous studie
