Comparative tolerability profiles of the inhaled anaesthetics.

Improved understanding of the structure/activity relationship of inhaled anaesthetics has resulted in the synthesis of fluorinated compounds which are more potent and less toxic than their unfluorinated antecedents. The toxic effects of inhaled anaesthetics on the liver and kidney are complex but, in general, are related to the extent to which individual inhaled agents are metabolised. Halothane hepatotoxicity is a rare, idiosyncratic reaction which typically occurs in obese women having more than one exposure to the drug within a short time interval. All currently available volatile anaesthet

Effects of alfentanil on the equine electroencephalogram during anaesthesia with halothane in oxygen.

Opioids have variable effects on the minimum alveolar concentration of inhaled anaesthetics in the horse. During halothane anaesthesia at an end-tidal halothane concentration between 0.75 and 0.85 percent, the electroencephalogram (EEG) frequency power spectrum and the auditory evoked potential were recorded continuously in eight ponies during an infusion of approximately 40 micrograms kg-1 alfentanil over five minutes, and for a further 55 minutes. The spectral edge and median frequency of the EEG and the mid-latency of the auditory evoked potential at the time of maximum change of these vari

Sites of alcohol and volatile anaesthetic action on GABA(A) and glycine receptors.

Volatile anaesthetics have historically been considered to act in a nonspecific manner on the central nervous system. More recent studies, however, have revealed that the receptors for inhibitory neurotransmitters such as gamma-aminobutyric acid (GABA) and glycine are sensitive to clinically relevant concentrations of inhaled anaesthetics. The function of GABA(A) and glycine receptors is enhanced by a number of anaesthetics and alcohols, whereas activity of the related GABA rho1 receptor is reduced. We have used this difference in pharmacology to investigate the molecular basis for modulation

[Serum fluoride concentrations and exocrine kidney function with sevoflurane and enflurane. An open, randomized, comparative phase III study of patients with healthy kidneys].

UNLABELLED: Sevoflurane is a "new" volatile inhaled anaesthetic. Owing to its lower blood-gas solubility coefficient, emergence from anaesthesia is faster with sevoflurane than with isoflurane, enflurane, or halothane. Sevoflurane undergoes metabolic biodegradation, releasing inorganic fluoride ions that could produce nephrotoxicity. In this study, we compared serum inorganic fluoride concentrations (IFCs) in patients receiving either sevoflurane or enflurane. Furthermore, indices of renal function were evaluated until the 3rd postoperative day. METHODS: Thirty patients with no history of ren

Effect of age on MAC in humans: a meta-analysis.

It is well known that MAC, the minimum alveolar concentration required to prevent movement in response to surgical incision in 50% of patients, decreases with age. Regression analysis showed that data for humans derived from a comprehensive literature survey were consistent, for age > 1 yr, with log10 MAC decreasing with increasing age at the same rate for all inhaled anaesthetics; approximately equivalent to 6% change per decade of age. With some slight reservation on differences between data from different institutions, the present data for humans are consistent (for age > 1 yr), with the eq

[Duchenne muscular dystrophy and anesthesia. A retrospective study].

This retrospective study comprising 37 patients with Duchenne's muscular dystrophy undergoing 56 general anaesthesias shows that 1) no atypical reactions occurred when suxamethonium was omitted, that 2) wherever atypical reactions were observed they were always preceded by the use of halothane and suxamethonium. Haemodynamic instability and intubation difficulties were the major complications encountered. Preoperative optimization and cautious anaesthetic practice avoiding suxamethonium and preferably also avoiding potent inhaled anaesthetics is recommended.

Arterial pressure control with isoflurane using fuzzy logic.

Arterial pressure is still one of the most important measures in estimating the required dose of inhaled anaesthetics. It is measured easily and reacts rapidly which makes it suitable as a variable for feedback control of depth of anaesthesia. Fuzzy logic, a novel approach to feedback control, was used to control arterial pressure in 10 patients during intraabdominal surgery by automatic adjustment of the concentration of isoflurane in fresh gas. During anaesthesia, fuzzy control periods of 45-min duration were alternated randomly with human control periods of equal duration. During the skin i

Implications of chemical and physical properties of desflurane for longer surgery.

Increased duration of anaesthetic administration has implications for recovery from anaesthesia, has cardiovascular effects, and potential for toxicity through metabolism and breakdown of the anaesthetics. Recovery of function after desflurane or sevoflurane anaesthesia, because of the low blood gas and tissue solubilities of these agents, is more rapid than after halothane, isoflurane or enflurane, with recovery being most rapid after desflurane. Increased duration of anaesthesia amplifies the differences in rate of recovery because of the additional anaesthetic (greater with more soluble age

[Emergence times, hemodynamics and adverse effects of sevoflurane and isoflurane: an open, randomized, comparative phase iii study].

Sevoflurane is a "new" volatile inhaled anaesthetic that is currently undergoing phase III clinical trial in Europe and the United States. Owing to the low blood solubility, rapid induction of anaesthesia and emergence from anaesthesia would be expected. In this study, we compared emergence times and haemodynamics in patients receiving either sevoflurane or isoflurane. Furthermore, all adverse effects were recorded and the relationship to the drug administered was rated. METHODS. Fifty ASA physical status I and II patients were studied in an open, prospective, randomised clinical trial. Anaest

A physiological model for the distribution of injected agents, with special reference to pethidine.

The model is based on Mapleson's Model P for inhaled anaesthetics, but has compartments for lungs, peripheral shunt, kidneys, portal bed, liver, other viscera, muscle, other lean, fat, brain, i.m. injection site and for various blood "pools". Intracellular and extracellular fluids are represented separately in each compartment and in blood. In each fluid, four forms of the agent are distinguished: unionized dissolved in water ("standard" form), ionized dissolved in water, unionized dissolved in lipid, and bound to protein. Equations define the equilibrium between these four forms in any one fl

Molecular interactions of anaesthetics with biological membranes.

1. There is not yet a consensus as to which of the neuronal membranes, which molecular component of any particular membrane or what specific function of the membrane is critical for general anaesthesia. 2. However, when considering anaesthetic effects on different synapses, with neurotransmitter receptors, ion channels etc., the unifying central concept is action at a membrane level. 3. This paper will review the general evidence for this unifying hypothesis, and consider the apparent exceptions and limitations. 4. The membrane hypothesis is usually stated in the form of the Meyer and Overton

Interactions between nimodipine and general anaesthesia--clinical investigations in 124 patients during neurosurgical operations.

Haemodynamic, respiratory, metabolic and endocrine investigations were performed in a total number of 124 patients, divided into four different groups, during opiate anaesthesia for neurosurgical operations in order to characterize general effects of nimodipine, a calcium channel blocking agent with a preferential cerebrovascular action. These studies led to the following conclusions: Nimodipine is a vasodilator drug with a hypotensive action, which is especially obvious in hypertensive patients and in combination with similarly acting agents, such as sodium nitroprusside or nitroglycerin. Cardiovascular and blood gas responses to inhaled anaesthetics in normoxic and hypoxic dogs.

Changes in haemodynamics and blood gases were investigated before and after administration of 0.5, 1 and 1.5 MAC of halothane, enflurane and isoflurane in respectively 7, 7 and 9 dogs ventilated alternatively with a fraction of inspired O2 in N2 (FiO2) of 0.4 and with brief periods (10 min) of FiO2 of 0.1. Anaesthesia was induced with pentobarbital and the animals were paralysed with pancuronium. Acute hypoxic challenges with FiO2 of 0.1 consistently decreased arterial PO2 to 3.5-4.5 kPa and increased pulmonary vascular resistances by 60-100%. At identical inspired concentrations, as expressed

[Modeling and BASIC program for the kinetics of inhalation anesthetic agents].

The pharmacokinetics of inhaled anaesthetics has been well described by Eger's model using a monoalveolar lung with continuous alveolar ventilation and four extrapulmonary compartments defined by their blood supply and anaesthetic solubility. The model is presented with its equations. A Basic microcomputer programme is proposed for this model: expected concentrations of anaesthetic in various conditions of administration, ventilation or cardiac output are shown, such as the uptake and distribution of nitrous oxide during anaesthesia. Different inhaled anaesthetics were compared for induction a

Inhaled anaesthetics have no effect on fertility in Drosophila melanogaster.

The potential effects of enflurane, isoflurane, halothane, fluroxene and nitrous oxide on mortality and fertility in Drosophila melanogaster were examined. The LD50 value 1 day after exposure for 1 h was the same for males and females and was about 8% for enflurane, isoflurane and halothane, about 20% for fluroxene, and greater than 100% for nitrous oxide. Male and female fertility, however, were not affected by any of the anaesthetics, even at concentrations well above those used clinically.

Metabolism of the inhaled anaesthetics: implications of enzyme induction.

Treatment with drugs and exposure to many environmental chemicals results in enzyme induction. However, the clinical significance of increased (or altered) metabolism of the inhaled anaesthetics appears to be trivial. Enzyme induction does not affect the conduct of inhalation anaesthesia. Thus, only delayed organ toxicity is at issue. Since methoxyflurane has fallen into disuse, nephrotoxicity secondary to its administration is no longer a problem. Nephrotoxicity as a result of enhanced defluorination of enflurane or isoflurane is also unlikely: enflurane biotransformation, in most circumstanc

The pharmacology of isoflurane.

The physical and pharmacological properties of the structural isomers isoflurane and enflurane differ from each other and from those of other potent inhaled anaesthetics. The minimum alveolar concentration (MAC) for isoflurane (1.15%) is one-and-one-half times that for halothane (0.75%) and two-thirds that for enflurane (1.7%). The blood/gas partition coefficient (1.4) for isoflurane is lower than the coefficients for all other potent inhaled agents. Despite this lower blood solubility, induction of anaesthesia is slightly faster with halothane because of isoflurane's mild pungency. Enflurane

Glutathione and glutathione S-transferases in rat liver after inhalation of halothane and enflurane.

Male Sprague-Dawley rats were exposed in inhalation chambers to halothane and enflurane in concentrations from 50 ppm-1000 ppm (0.0025-0.05 minimum alveolar concentration; MAC) 6 h a day for 3-9 days. Repeated subanaesthetic concentrations were used to avoid effects of general anaesthesia and to increase the metabolized fraction of the inhaled anaesthetics. Exposure to 0.05 MAC of halothane (500 ppm) and enflurane (1000 ppm) for 9 days reduced the activity of glutathione S-transferases. A decrease in liver concentration of reduced glutathione (GSH) was observed after inhalation of enflurane, p

Structure and quantification of a physiological model of the distribution of injected agents and inhaled anaesthetics.

Mapleson's (1973) physiological, circulation-time model of the distribution of inhaled anaesthetics has been elaborated to be suitable for modelling agents in which hepatic metabolism and renal excretion are important factors; as well as the obvious improvement of providing separate compartments for liver and kidney, the arterial and portal supplies to the liver are separately represented, as is the portal blood pool. The separate portal pool also leads to a more realistic total circulation time for the majority of the cardiac output. The quantification for a "standard man" is fully documented
