[Intravenous anesthesia--current aspects].

Intravenous anaesthesia as a defined technique has become popular in recent years, since new developed drugs and more sophisticated application systems were introduced into clinical practice. However, only a minority of anaesthesiologists really practise this new technique. The scientific basis of continuous intravenous anaesthesia has not been spread widely in the anaesthetic community yet. The following review therefore focusses on recent findings and developments in the field of pharmacology, pharmacokinetics, pharmacodynamics and modern application systems of intravenous anaesthetic drugs.

Interactions of volatile anaesthetics with rocuronium bromide in perspective.

A review of several publications shows that there is general agreement that inhalational anaesthetic agents potentiate the neuromuscular effects of rocuronium in the same order as other similar agents, namely; enflurane and isoflurane > halothane > intravenous anaesthetics. However, such potentiation is not evident during induction and only becomes significant as anaesthesia becomes more prolonged. It is manifest as a prolongation in the duration of action of maintenance doses and a decrease in the recovery rate. During long-lasting procedures in which enflurane or isoflurane are used, smaller

Risk factors for postoperative nausea and vomiting.

Although the aetiology of postoperative nausea and vomiting is not completely clear, a number of key contributing factors increase the risk for an individual patient. The inhalational agents are variably associated with postoperative nausea and vomiting, and nitrous oxide is particularly emetogenic. Older inhalational anaesthetics, such as cyclopropane, are associated with a high incidence, while the currently used agents, isoflurane, enflurane and halothane, cause less, but still significant postoperative nausea and vomiting. Intravenous anaesthetics are also associated with differing degrees

[Interactions of intravenous anesthetics with human CNS ion channels. Electrophysiologic studies with a new type of voltage clamp technique].

UNLABELLED: Despite a widespread approach to explain the molecular mechanisms of anaesthetic agents, the complex state of "general anaesthesia" is still not completely understood. Voltage-activated sodium channels from human brain cortex served as a model membrane protein to investigate anaesthetic drug-protein interactions using a novel electrophysiological voltage-clamp technique. Sodium channels are already well-characterized important integral membrane proteins responsible for the generation of the fast-propagated action potential and thus are vital components for neuronal signal integrati

Effects of intravenous anaesthetics on function and metabolism in the isolated rat heart-lung preparation.

The influence of several intravenous anaesthetics on the heart was assessed using the isolated rat heart-lung preparation. Each group received 10(-3)mol litre-1 and 10(-4)mol liter-1 of ketamine, 6 micrograms ml-1 and 60 micrograms ml-1 of midazolam, 6 micrograms ml-1 and 60 micrograms ml-1 of diazepam or 0.6 micrograms ml-1 and 6 micrograms ml-1 of flunitrazepam. Systolic blood pressure in rats receiving high doses of midazolam, diazepam and ketamine were higher than that in the control group. Heart rate in rats receiving high doses of ketamine and diazepam were lower than that in the control

The influence of intravenous anaesthetics on polymorphonuclear leukocyte function.

Polymorphonuclear leukocytes (PMNL) play a vital role in the defence against invading bacteria. It is known that some anaesthetics inhibit PMNL function and, thus, possibly enhance perioperative infection. We investigated the effect of methohexitone, flunitrazepam, and droperidol on three bactericidal PMNL functions, i.e., superoxide anion production, hydrogen peroxide generation, and activity of released myeloperoxidase, in vitro. Approved photometrical assays were used. Superoxide anion was measured by the reduction of cytochrome C, hydrogen peroxide by the horse radish peroxidase catalysed

Severe respiratory depression after epidural morphine in a patient with myotonic dystrophy.

We describe a patient with myotonic dystrophy who underwent cholecystectomy, and developed severe respiratory depression following epidural administration of morphine to provide postoperative analgesia. At preoperative assessment, he demonstrated near normal vital capacity and maximal voluntary ventilation, but the presence of chronic ventilatory failure with a resting value of PaCO2 51 mmHg. Anaesthesia was produced by a combination of epidural and light general anaesthesia without intravenous anaesthetics, narcotics or neuromuscular relaxants. Five hours after epidural administration of 2 mg

Patient evaluation of four different combinations of intravenous anaesthetics for short outpatient procedures.

We studied 200 female patients (ASA group 1) scheduled for termination of pregnancy under general anaesthesia. The patients were randomly allocated to receive one of four anaesthetic combinations; (1) propofol in combination with ketamine 20 mg, (2) propofol in combination with fentanyl 0.1 mg, (3) thiopentone in combination with fentanyl 0.1 mg, (4) methohexitone in combination with fentanyl 0.1 mg. All patients were breathing oxygen in nitrous oxide 1:2. Patients' self assessments of per- and postoperative course and time to discharge were compared. No patient's response suggested light anae

[Effects of intravenous anesthetics on neurons of the central nervous system: mechanisms of cellular and molecular action].

The mechanisms of action of intravenous anaesthetics are not yet completely elucidated. Until recently, most of the studies had focused on the interactions between anaesthetics and lipid bilayers. It has been proposed that loss of consciousness is produced by disorganization of the lipid phase of nerve membranes, which impairs the action potential propagation. However, new data obtained with sophisticated neuropharmacological tools such as the patch clamp technique have recently contributed to challenge this hypothesis. Indeed, several lines of evidence suggest that intravenous anaesthetics ar

Practical treatment recommendations for the safe use of anaesthetics.

General anaesthesia is the reversible depression of central nervous system function. There is still no agreement over what constitutes depth of anaesthesia, and the clinical anaesthetist must thus titrate drug input according to clinical signs (heart rate, blood pressure, somatic movement, autonomic responses). The potency of inhalational agents may be expressed in terms of the MAC (minimum alveolar concentration); comparable end-points (including blood concentrations) have been proposed for the intravenous agents. Kinetic infusion regimens can be constructed for the intravenous agents to achi

[The molecular action profile of intravenous anesthetics].

A close look at the molecular interactions of intravenous anaesthetics reveals a complex and non-uniform picture. A multitude of membrane structures are affected by the individual anaesthetic compounds, although with specific differential profiles of molecular action for the different groups of anaesthetic substances. This is consistent with the hypothesis of a multi-mechanistic mode of action, where anaesthesia results from the superposition and integration of several anaesthetic actions on the molecular level. This overall concept is illustrated by using the well characterised voltage-activa

[Systematics and concept of a future integrated anesthesia device].

On the grounds of the more than one hundred years lasting development of the anaesthesia machine the systematics of the components of a future integrated anaesthesia machine is described. The new generation of today's anaesthesia machine incorporates four main functions: dosing, ventilation, monitoring and data management. Given the achievements of the last 15 years in the fields of data and signal analysis, of the clinical pharmacology of intravenous anaesthetics, of measurement and evaluation of anaesthetic drug action, the four main functions are only partially realized in today's anaesthes

Kinetics of nicotinic acetylcholine ion channels in the presence of intravenous anaesthetics and induction agents.

1. Single channel currents activated by 250 nM acetylcholine were recorded from cell-attached patches of BC3H1 mouse tumour cells grown in culture. Channels were recorded in the absence and presence of alphaxalone, diazepam, etomidate, fentanyl, ketamine, meperidine, or propofol. 2. All of the anaesthetics tested shortened channel open time but did not alter single channel current amplitude. Drug concentrations calculated to reduce the time constant of open-time distributions by 50% were 99 microM alphaxalone, 66 microM diazepam, 57 microM etomidate, 26 microM fentanyl, 15 microM ketamine, 16

Electroencephalogram effect measures and relationships between pharmacokinetics and pharmacodynamics of centrally acting drugs.

Electroencephalogram (EEG) effect parameters may be useful in pharmacokinetic-pharmacodynamic modelling studies of drug effects on the central nervous system (CNS). Effect parameters derived from a quantitative analysis of the EEG appear to be perfectly suited to characterise the relationships between pharmacokinetics and pharmacodynamics of benzodiazepines and intravenous anaesthetics. EEG parameters represent many of the characteristics of ideal pharmacodynamic measures, being continuous, objective, sensitive and reproducible. These features provide the opportunity to derive concentration-ef

Convulsive thresholds in mice during the recovery phase from anaesthesia induced by propofol, thiopentone, methohexitone and etomidate.

1. Convulsive thresholds were measured with intravenous pentylenetetrazol in mice during the recovery phase after intravenous anaesthetic doses of propofol (10 and 20 mg kg-1), thiopentone (30 mg kg-1), methohexitone (10 mg kg-1), and etomidate 3 mg kg-1). 2. The convulsive threshold rose after each agent, indicating an anticonvulsant action for all the drugs tested; this declined to control values with initial half times of: 1.56 min (propofol 10 mg kg-1); 1.03 min (propofol 20 mg kg-1): 1.02 min (methohexitone); 3.35 min (etomidate); 13.7 min (thiopentone). 3. Multiple medication: problems of the elderly patient.

Elderly persons are apt to receive multiple drugs for many diseases. Prescribers should take extra care in persons aged over 65-70 years, and especially the very old. The main problem of the elderly is the unpredictability of their response to drugs. Self-medication with both prescribed and over-the-counter drugs worsens this problem. Drug actions may be altered by impairment of the liver, kidneys and brain, or acute illness, or both. Unwanted (adverse) drug reactions often produce rapid and unduly severe illness in the old. Some antibacterial antibiotics like the sulphonamides are best avoide

[Infusion or repetitive bolus injection? A clinical study of midazolam/fentanyl and diazepam/fentanyl combination anesthesia in neurosurgical operations].

The objectives of this study were to compare two narcotic/benzodiazepine combinations given either as continuous infusion or intermittent bolus injections in neurosurgical patients. 24 patients scheduled for elective craniotomy were investigated. They were assigned randomly to four different groups for treatment. Groups 1 and 2 received a combination of midazolam and fentanyl, groups 3 and 4 a combination of diazepam and fentanyl. Anaesthesia was maintained either by continuous infusion (groups 1 and 3) or intermittent bolus injections (groups 2 and 4). At six key intervals cardiovascular vari

Propofol. A review of its pharmacodynamic and pharmacokinetic properties and use as an intravenous anaesthetic.

Propofol is an intravenous anaesthetic which is chemically unrelated to other anaesthetics. Induction of anaesthesia with propofol is rapid, and maintenance can be achieved by either continuous infusion or intermittent bolus injections, with either nitrous oxide or opioids used to provide analgesia. Comparative studies have shown propofol to be at least as effective as thiopentone, methohexitone or etomidate for anaesthesia during general surgery. The incidence of excitatory effects is lower with propofol than with methohexitone, but apnoea on induction occurs more frequently with propofol tha
