Clinical efficacy of intra-articular lidocaine with or without adrenaline during shoulder arthroscopy in dogs.

INTRODUCTION: Intra-articular (IA) administration of local anaesthetic drugs has been widely reported in shoulder, elbow, ankle and knee arthroscopy in human medicine. The use of IA adrenaline to control bleeding in arthroscopy is relatively common, but its combined effect with local anaesthetics on perioperative pain has not been studied in dogs. This study aims at evaluating the efficacy of IA shoulder administration of lidocaine or lidocaine plus adrenaline in dogs (Canis lupus familiaris) as pre-emptive analgesia during arthroscopic surgery under general anaesthesia. METHODS: Twenty-four

Comparative evaluation of segmental spinal anaesthesia with conventional spinal anaesthesia in endoscopic transurethral surgery.

BACKGROUND AND AIMS: Lumbar spinal anaesthesia (LSA) for endoscopic urological procedures often causes extensive sympathetic blockade, leading to hypotension, bradycardia, and delayed ambulation. Thoracic segmental spinal anaesthesia (TSSA) offers targeted sensory blockades with limited motor involvement and improved haemodynamic stability. The primary objective of the study was to compare motor block onset between TSSA and LSA. Secondary objectives included sensory block characteristics, two-segment regression time, haemodynamics, perioperative complications, and patient and surgeon satisfact

Specific neurotoxicity of local anaesthetics revisited: An evidence-based narrative review.

Local anaesthetics are essential to regional anaesthesia but may exert neurotoxic effects on peripheral nerves under specific conditions. Although perioperative nerve injury is increasingly recognised, the contribution of local-anaesthetic neurotoxicity, the relative toxicity of different agents and concentrations, and the influence of patient-related risk factors remain incompletely defined. This evidence-based narrative review synthesises experimental and clinical data on peripheral nerve neurotoxicity associated with local anaesthetics and discusses implications for clinical practice and fu

Safe Use of Adrenaline in Cryosurgery for Eyelid Basal Cell Carcinoma Evaluated With Infrared Thermography.

The efficacy of cryosurgery for skin cancers is believed to depend on thaw time, which may be influenced by the presence of adrenaline in local anaesthetics due to its vasoconstrictive effects. This is the first study to perform detailed, non-invasive temperature measurements using infrared (IR) thermography during cryosurgery for basal cell carcinomas (BCCs), evaluating the impact of adrenaline on thaw time. A total of 15 patients with eyelid BCCs underwent cryosurgery with local anaesthetics both with and without adrenaline. Thawing was continuously monitored using a high-resolution IR camer

Comparison of 0.1% Ropivacaine and 0.0625% Bupivacaine With Fentanyl as an Adjuvant for Labour Analgesia.

Background and aims Effective pain relief during labour is an important component of obstetric care, with epidural analgesia being the most reliable method. Using low concentrations of local anaesthetics combined with opioids can provide adequate analgesia while minimizing adverse effects. In this study, we compared the analgesic effectiveness of 0.1% ropivacaine and 0.0625% bupivacaine, each administered with fentanyl (2 mcg/mL), for epidural labour analgesia. Methods A double-blinded randomized controlled study was conducted with 40 parturients belonging to ASA physical status I or II. They

Periodontal Mechanoreceptor Contribution to Minimal Holding Force Between Incisor Teeth.

BACKGROUND: The functional importance of periodontal mechanoreceptors (PMRs) is highlighted by impaired force control when periodontal input is reduced or absent. Previous studies used handheld transducers to measure the minimum holding force between the incisors. In the present study, a high-resolution force transducer was held passively between the incisors against gravity. OBJECTIVE: To quantify the contribution of anterior PMRs to minimum holding force and examine the effects of body position and periodontal health. METHODS: Twenty healthy adults (12 females, 8 males; 23.2 ± 1.9 years) p

Impact of single vs. dual local anaesthetic regimen on the efficacy and safety of percutaneous stellate ganglion block for electrical storm: new insights from the STAR study.

AIMS: Percutaneous stellate ganglion block (PSGB) is a promising treatment for refractory electrical storm (ES). However, there is a lack of solid indications regarding the optimal pharmacological regimens and combination of local anaesthetics (LAs). This study, from the multicentre STAR registry, aimed to evaluate the impact of single vs. dual LA regimen on the efficacy and safety of PSGB. METHODS AND RESULTS: We analysed 422 PSGB procedures from 298 patients (30% single LA, 70% dual LAs). Both regimens significantly reduced the number of treated ventricular arrhythmias (VAs) in the 12 h pos

Crystal precipitates of local anaesthetic and adjuvant mixtures can be visualised and graded using ultrahigh-frequency ultrasound imaging.

BACKGROUND: The use of mixtures of local anaesthetics and/or adjuvants is a common practice in regional anaesthesia, aimed at optimising onset time and prolonging block duration. However, several previous in vitro studies have demonstrated that mixing local anaesthetics may lead to crystallisation. A major barrier to understanding their potential physiological effects has been the lack of imaging techniques capable of detecting crystal formations beyond conventional microscopy. METHODS: In this in vitro study, ropivacaine 0.75%, lidocaine 2%, chloroprocaine 2% and mepivacaine 2%, as well as s

Preoperative Neutrophil-to-Lymphocyte Ratio is Associated with Early Postoperative Pain and Opioid Consumption after Total Knee Arthroplasty: A Retrospective Study.

BACKGROUND: Improving quality and effectiveness of health care is one of the priorities of health policies. Hospital or physician volume represents a measurable variable with a relevant impact on effectiveness of health care. A recent Italian law, the "spending review", calls for the definition of "qualitative, structural, technological and quantitative standards of hospital care". There is a need for an accurate evaluation of the available scientific evidence in order to identify these standards, including the volume of care above or below which the public and private hospitals may be accredi
