The ED95 of lidocaine and prilocaine for ultrasound-guided brachial plexus blocks for surgical anaesthesia: a randomised controlled clinical trial.

BACKGROUND: Our trial addresses the gaps in the current literature by directly estimating the ED95 of short-acting local anaesthetics for ultrasound-guided axillary and supraclavicular brachial plexus blocks for surgical anaesthesia. METHODS: Four double-blind prospective studies were organized in two separate arms. Patients were randomised between studies A (lidocaine 1% with adrenaline) and B (lidocaine 2% with adrenaline) for axillary blocks and between studies C (prilocaine 1%) and D (lidocaine 1% with adrenaline) for supraclavicular blocks. All statistical modelling and analysis were per

Analysis of Adverse Events Associated With Dental Local Anaesthetics Using Food and Drug Administration Adverse Event Reporting System Data.

OBJECTIVE: Local anaesthetics (LAs), almost indispensable aspect of dental practice for pain-free procedures, occasionally leads to worrisome adverse events (AEs). We aimed to compare serious and nonserious AEs of LAs for dental reasons in the US Food and Drug Administration AE Reporting System. METHODS: We retrospectively analysed dental AE reports associated with LAs in the Food and Drug Administration AE Reporting System database from its inception to April 2024. We described AEs by patients' demographic and clinical characteristics and compared ester and amide LA-associated AEs by their s

Less is better: role of adjuvants.

To update published data about adjuvants and examine their evolution in use over the last 2 years. This scoping review highlights current and updated published evidence on using adjuvants and future research directions.Regional techniques (including peripheral nerve blocks) have become relevant for pain relief and postoperative quality recovery. Examples of adjuvant drugs for regional techniques include dexamethasone, alpha-2 agonists, buprenorphine, ketamine, magnesium, and so on. Adding adjuvants we can modify the characteristics of the blocks. We have made significant progress in understand

Intra- and Post-Operative Bacteriological Surveys of Surgical Site in Horses: A Single-Centre Study.

Bacterial contamination of the surgical site in horses is a major risk factor for the development of surgical site infections (SSIs), which increase morbidity, mortality, the hospitalisation period, antibiotic use, and management costs. While contamination is a prerequisite for infection, its progression to clinical infection depends on additional factors that compromise host defences. The present study, conducted at the Veterinary Teaching Hospital of the University of Perugia over an 11-month period, investigated bacterial contamination in 70 surgeries (53 clean and 17 clean-contaminated) at

Local anaesthetics in interventional radiology: a primer for radiologists on applications and management of complications.

Local anaesthetics (LAs) allow a range of procedures to be performed in interventional radiology (IR) through improving patient comfort and reducing pain. This review serves as a primer for interventional radiologists, providing an overview of commonly used LAs and practical tips for their implementation. With its quick onset time and moderate duration of action, the amide lidocaine is the most used and applicable to a variety of procedures such as biopsies and embolization. In contrast, bupivacaine and ropivacaine (both amides) have longer durations of action, and are therefore suitable for l

The state of local anaesthetic systemic toxicity in 2025: the emergence of lidocaine as our next challenge.

Local anaesthetics are an integral part of surgical and medical practice, but their systemic uptake can result in local anaesthetic systemic toxicity (LAST). A recent study by Fettiplace and colleagues examined the US Food and Drug Administration's Adverse Events Reporting System between 1968 and 2023. Although the reporting of death from long-acting local anaesthetics decreased after the publication of relevant practice advisories, Fettiplace and colleagues also found that deaths from lidocaine remained essentially unchanged. The emergence of lidocaine as the leading reported cause of deaths

Feasibility and Safety of Intercostal Cryoneurolysis During Pleuroscopy: A Novel Analgesic Approach Using a 1.7 mm Cryoprobe.

INTRODUCTION: Pleuroscopy is a minimally invasive and highly effective procedure used for diagnosing pleural diseases. Despite its utility, pain during and after the procedure can be significant. Traditional analgesic approaches, including systemic opioids and local anaesthetics albeit potent, may provide incomplete pain relief and can be associated with side effects. Cryoneurolysis has emerged as a feasible analgesic technique in general surgery. However, an equivalent treatment modality has yet to be established for pleuroscopy. OBJECTIVES: To investigate the safety and feasibility of cryon

Assessment of anaesthesiologist and public hospital preparedness for the prevention and treatment of local anaesthetic systemic toxicity (LAST): a national survey.

BACKGROUND: Local anaesthetic systemic toxicity (LAST) is a significant risk associated with the use of local anaesthetics. Despite existing prevention and management guidelines, the increasing use of local anaesthetics raises concerns about LAST. This study aims to assess the preparedness of anaesthesiologists and hospitals to manage LAST, following the Third American Society of Regional Anesthesia and Pain Medicine Practice Advisory. METHODS: The research questionnaire is developed and validated. The questionnaire focuses on a three-dimensional framework for anaesthesiologist preparedness:

Impact of dexamethasone-enhanced anaesthetics on postoperative pain, oedema, and trismus following third molar extraction: a systematic review and meta-analysis.

The extraction of third molars (3Ms) is frequently associated with postoperative inflammation and discomfort. This systematic review aimed to compare the efficacy of combining local anaesthetics with dexamethasone (treatment group) versus local anaesthetics alone (control group) in 3M surgeries. A comprehensive search of PubMed, Cochrane CENTRAL, Virtual Health Library, and Embase was conducted on June 9, 2025, to identify randomized controlled trials (RCTs) assessing outcomes such as pain, oedema, and trismus. Of 300 studies initially identified, nine met inclusion criteria for qualitative sy

Contemporary local anaesthetic-associated adverse events and mortality: a pharmacovigilance analysis of a US reporting system.

BACKGROUND: One hundred years after the American Medical Association's report on death from local anaesthetics, their contemporary lethality is not well characterised. METHODS: We investigated local anaesthetic-associated deaths reported to the US Food and Drug Administration Adverse Event Reporting System from 1968 to 2023, comparing adverse events from local anaesthetics with adverse events from other drugs using the reporting odds ratio (ROR). We assessed the impact of professional society practice advisories about systemic toxicity on reporting of mortality from long-acting local anaesthe

Perineural fresh frozen plasma with local anaesthetics in coagulopathic patients undergoing peripheral nerve blocks: A clinical case series.

Peripheral nerve blocks (PNBs) in coagulopathic patients are typically avoided due to the risks of bleeding and potential nerve compression. This case series explores the use of perineural fresh frozen plasma (FFP) to enhance local haemostasis in such settings. Five patients with an international normalised ratio of > 1.8, requiring urgent surgery, received PNBs with perineural FFP after haematology consultation. Seven ultrasound-guided blocks were performed, with FFP injected perineurally and along the needle tract, followed by systemic FFP 20 minutes later. All procedures were completed unde

A national survey on percutaneous tracheostomy practice in Malaysian adult general intensive care units.

INTRODUCTION: Percutaneous tracheostomy (PT) has gained increasing acceptance over surgical tracheostomy (ST) in the last few decades due to lower rates of postoperative infections, less bleeding, and cost-effectiveness. However, there has been little information regarding the PT practice in Malaysian adult general intensive care units (ICU). The objective of the study was to assess the current practice of PT in Malaysia. MATERIALS AND METHODS: This observational cross-sectional study used a validated questionnaire with 15 items. A total of 61 ICUs consisting of adult general ICUs under Minis
