Neurotransmitters and Drug Repurposing in Cancer Therapy: Targeting the Nervous System for Tumor Suppression.

Emerging research indicates that the nervous system plays a complex role in cancer, with neurotransmitters regulating tumour growth, immune evasion, and resistance to therapy. It has been reported that several important oncogenic pathways are modulated by neurotransmitters, including dopamine, serotonin, norepinephrine, and glutamate, thereby governing cell proliferation, angiogenesis, and immune regulation in the tumor microenvironment. This has created new opportunities for therapeutic intervention, particularly drug-repurposing strategies that target neurotransmitter signalling. Previous re

Toxicological profiles in adolescent suicide before and after COVID-19: A forensic autopsy study of the involvement of over-the-counter medications and sex differences in urban Japan.

Adolescent suicide remains a major public health concern, with recent increases observed, particularly among females in Japan. However, the toxicological characteristics in forensic settings have not yet been sufficiently described. This study examined the toxicological patterns in adolescent suicide cases before and after the onset of the coronavirus disease 2019 (COVID-19) pandemic, using forensic autopsy data from three institutions in metropolitan Japan (2015-2024). A total of 41 cases (10 males, 31 females) were included in the study. Toxicological findings were compared between the pre-2

Underuse of Palliative Medications and Services in Children with Life-Limiting Conditions.

CONTEXT: Children with life-limiting conditions experience substantial pain, depression, and anxiety, yet the real-world use of symptom-directed medications and palliative services in this population is largely uncharacterized. OBJECTIVES: To examine the use, and potential underuse, of antidepressant and other symptom-directed psychotropic medications, psychological services, and dedicated palliative care services (palliative consultation, hospice referral, and pain consultation) among children with life-limiting conditions. METHODS: Retrospective population-based cohort study using the elec

Alcohol-involved suicide mortality prevalence estimates and identification of co-occurring substance use from the National Violent Death Reporting System, 2022-2023.

Using National Violent Death Reporting System data (2022-2023), 14 909 suicide decedents (34%) tested positive for alcohol. The most common substances co-occurring with alcohol were marijuana, antidepressants, opioids, and benzodiazepines; polysubstance use was high, with 52% testing positive for alcohol and ≥1 other substance. Findings suggest an overall alcohol-involved suicide mortality rate of 24%, appearing higher than prior estimates.

Recurrent Pancytopenia Associated with Olanzapine Rechallenge: A Rare Case Report with Literature Review.

Olanzapine is a frequently used second-generation antipsychotic, often considered to have a favorable hematological safety profile than clozapine. We report a rare case of recurrent pancytopenia associated with olanzapine rechallenge in a 46-year-old woman with acute psychotic episode. She had previously responded well to olanzapine but developed moderate hematological decline, which resolved on discontinuation. During current psychotic episode, olanzapine was re-initiated, and serial blood counts suggested progressive pancytopenia, which was confirmed on peripheral smear. Alternative causes i

Case Report: Rapid resolution of psychogenic cough in an adolescent following bromazepam treatment.

BACKGROUND: Somatic cough syndrome, formerly termed psychogenic cough or habit cough, is an under-recognized cause of chronic cough in pediatric populations, characterized by repetitive, dry, and honking cough that is absent during sleep. Behavioral interventions are first-line treatment with high success rates of 80%-90%. However, when access to behavioral therapy is unavailable, alternative pharmacological interventions, particularly benzodiazepines, have received minimal investigation in this population. CASE PRESENTATION: I report a 13-year-old previously healthy male patient with abrupt-

Trends in Dispensing of Opioids and Benzodiazepines Among Older Adults Admitted for an Acute Hip Fracture Between 2008 and 2024 in Ontario, Canada.

INTRODUCTION: Opioids and benzodiazepines (BZDs) are commonly prescribed for managing pain, anxiety, and sleep-related problems following a hip fracture. Both opioids and BZDs carry risks of persistent dependence, falls, and mortality with long-term use. Despite these risks, Canada lacks standardized prescribing guidelines for older adults with hip fracture. We examined population-level trends in opioids and BZDs dispensing before hospitalization and up to 1-year post-discharge among older adults admitted for hip fractures in Ontario, Canada. METHODS: We conducted a retrospective cohort study

A 12-week study on the effects of betahistine (an H₃ antagonist/H₁ agonist) on cognitive functions in schizophrenia patients treated with olanzapine.

Evidence on betahistine for cognitive impairment in schizophrenia is limited. This study evaluated its feasibility, safety, and preliminary efficacy to lay the groundwork for future randomized controlled trials. Thirty-one inpatients with schizophrenia, aged 18-60 years, undergoing treatment with olanzapine received betahistine (16 mg, three times daily) for 12 weeks. Cognitive function, clinical symptoms, and side effects were assessed at baseline and again at 12 weeks postintervention. After 12 weeks of betahistine treatment, patients with schizophrenia showed a significant increase in the t

Are Depot Antipsychotics Ever Self-Tapering? Exploring the Discontinuation of Long-Acting Injectable Psychosis Drugs and Those With Delayed Onset Through In Silico Modelling.

BACKGROUND AND HYPOTHESIS: Reducing and stopping antipsychotic medications can have longer-term positive outcomes for some patients. Despite common wisdom, abrupt discontinuation of several long-acting injectable (LAI) antipsychotic drugs results in rapid D2 occupancy reductions. We explored if abrupt discontinuation of LAI antipsychotics with slow clearance remained within specified constraints on rate of D2 occupancy change (RODOC), which might reduce risk of emerging symptoms of psychosis related to withdrawal or relapse. STUDY DESIGN: Pharmacokinetic data were collated for 6 LAI antipsych

Patient experiences of benzodiazepine tapering: a qualitative study of treatment in addiction care.

BACKGROUND: Tapering by gradual dose reduction is a key component of treatment for individuals with addiction to benzodiazepines. Although many patients recover, treatment dropouts and relapses are common. Few studies have explored how benzodiazepine tapering is experienced from the patient perspective. METHODS: This qualitative study involved semi-structured interviews with 19 adults with addiction to benzodiazepines who were undergoing tapering treatment at a publicly funded outpatient addiction clinic. Reflexive thematic analysis was used to analyze the data. RESULTS: Participants describ

Effects of remimazolam versus propofol on cerebral oxygenation in elderly patients undergoing prone spine surgery: study protocol for a randomised controlled trial.

INTRODUCTION: Prone spine surgery may increase the risk of cerebral hypoperfusion and postoperative neurological adverse events, particularly in elderly patients. Remimazolam is a novel ultra-short-acting benzodiazepine anaesthetic that may provide greater haemodynamic stability than propofol. However, its effects on cerebral oxygenation during prone spine surgery remain unclear, indicating clinical equipoise between remimazolam and propofol. This study compares the effects of remimazolam and propofol on cerebral oxygenation in elderly patients undergoing thoracic or lumbar spine surgery in th

Olanzapine for the Prevention and Treatment of ICU Delirium: Protocol for a Systematic Review With Meta-Analysis.

BACKGROUND: Intensive care unit (ICU) delirium is a multifactorial acute brain dysfunction associated with prolonged hospitalization, increased morbidity and mortality, and higher healthcare costs. Despite these substantial negative impacts of delirium, the optimal prevention and treatment strategies are unclear. Olanzapine, an atypical antipsychotic drug, is commonly used for delirium management. We will perform an updated systematic review and meta-analysis to assess the benefits and harms of olanzapine for the prevention and treatment of ICU delirium. METHODS: This protocol is developed in

Daily sedation interruption versus no daily sedation interruption for critically ill adults and children requiring invasive mechanical ventilation.

RATIONALE: Daily sedation interruption (DSI) aims to limit drug accumulation and promote a more awake state, thereby reducing the duration of mechanical ventilation and its associated complications. This is an update of a Cochrane review first published in 2014. OBJECTIVES: To assess the effects of DSI versus sedation management without DSI on the duration of invasive mechanical ventilation, mortality, intensive care unit (ICU) length of stay, adverse events related to under- or oversedation (including accidental removal of endotracheal tube and tracheostomy), total doses of sedative and anal

Hospitalist perspectives and practices in treating hospitalized patients with alcohol withdrawal syndrome in the era of phenobarbital re-emergence.

Medication treatment for moderate to severe alcohol withdrawal syndrome (AWS) is evolving, particularly with renewed phenobarbital use. However, little research has examined how hospitalists manage AWS. We conducted a multiple-methods study involving hospitalists from a national US Hospital Medicine research network. Thirty-three hospitalists from 27 tertiary hospitals completed surveys and participated in seven focus groups on August 8, 2025. Benzodiazepines remained the most common treatment, but phenobarbital use was reported in 67% of emergency departments and 44% of inpatient medical unit
