Pain That Challenges Survival: A Novel SCN9A Variant (p.Leu1623Gln) Causing Carbamazepine-Refractory Paroxysmal Extreme Pain Disorder in a Chinese Family - Case Report.

Background and Clinical Significance: Paroxysmal extreme pain disorder (PEPD) is an extremely rare autosomal dominant sodium channelopathy caused by SCN9A gain-of-function variants. It is characterized by infantile-onset excruciating paroxysmal pain, typically in rectal, ocular, or mandibular regions, triggered by innocuous stimuli and accompanied by autonomic flares. Carbamazepine is dramatically effective in most reported cases. To date, only two genetically confirmed cases have been documented in Chinese patients, and fewer than 20 disease-causing variants are reported worldwide. We report

Epidemiological Assessment of Benzodiazepine Dependence via Pharmacist-Led EMR Review in Pain and Palliative Care Institution.

BACKGROUND/OBJECTIVES: Benzodiazepines (BZDs) are used routinely in cases requiring sedation for anxiety, insomnia, and procedures that require pain management, and daily use of these agents may extend over several months; therefore, monitoring patients is essential to reduce the risk of developing dependence. However, the high patient volume in pain and palliative-care settings often limits physicians' ability to both conduct consultations and perform comprehensive evaluations. In this context, the pharmacist plays a key role in supporting patient care by contributing professional activities

Bipolar major depression: A comprehensive review of pharmacotherapy.

Bipolar major depression involves recurrent depressive episodes requiring balanced treatment. Effective options include quetiapine, lurasidone, and olanzapine-fluoxetine. Lamotrigine prevents episodes, lithium reduces suicide risk, and emerging treatments show promise. Antidepressant monotherapy is discouraged. A patient-centered approach optimizing efficacy, safety, and stability is essential.

[Alcohol withdrawal : benzodiazepines, neither automatic nor systematic].

The CIWA-Ar (Clinical Institute Withdrawal Assessment for Alcohol-revised) is the gold standard for assessing the severity of alcohol withdrawal. It allows the dosage of benzodiazepines to be adjusted according to symptom intensity rather than prescribing them at a fixed dose, with the aim of limiting prolonged use and the associated risk of dependence. Randomized trials have shown that this approach reduces both the total amount of benzodiazepines administered and the duration of treatment without compromising clinical efficacy or increasing alcohol withdrawal-related complications. However,

Prescription patterns of benzodiazepines and Z-drugs in Berlin prisons.

BACKGROUND: Benzodiazepines and Z-drugs are commonly prescribed medications in psychiatric treatment. Prisoners are a unique patient population due to their elevated prevalence of substance abuse and psychiatric morbidity. Given the limited data about the medical treatment for prisoners, this study aims to provide an initial exploratory overview of the prescription patterns of benzodiazepines and Z-drugs in Berlin prisons. MATERIALS AND METHODS: A data set was compiled by extracting information from the documentation systems of all seven Berlin prisons. All inmates with a prescription for ben

The Use of Benzodiazepines and Risk of Incident Type 2 Diabetes in the General Population with Insomnia.

INTRODUCTION: Insomnia is a recognised risk factor for type 2 diabetes (T2DM). Although benzodiazepines (BDZs) are commonly prescribed for the treatment of insomnia, evidence regarding their use and the onset of T2DM among individuals with insomnia remains limited. This study aimed to assess the association between BDZ use and incident T2DM in a population with insomnia. METHODS: Within the UK Biobank prospective cohort of participants reporting insomnia, we examined the association between BDZ use and the risk of incident T2DM using multivariable adjusted Cox regression models. Robustness wa

From warfarin resistance to warfarin overdose: an unusual case report.

OBJECTIVES: Warfarin resistance represents a rare yet clinically significant challenge, especially among patients with mechanical heart valves. It may result from either hereditary or acquired factors. CASE PRESENTATION: A 27-year-old male patient with a history of aortic and mitral valve replacement was followed with subtherapeutic International Normalized Ratio (INR) of 1.8-2.0 despite receiving 20 mg/day of warfarin. He was hospitalized for evaluation, and potential acquired causes of warfarin resistance were excluded. The patient was discharged with a plan for close INR monitoring. Follow

Continuous ketamine infusion for surgical patients in the intensive care unit: a systematic review and meta-analysis of randomized controlled trials with GRADE assessment.

BACKGROUND: Optimal pain and sedation management in intensive care unit (ICU) remains challenging. While opioids and benzodiazepines are widely used, their adverse effects highlight the need for alternative or adjunctive strategies. Ketamine, with its analgesic and opioid-sparing has been proposed as an adjuvant. However, current evidence regarding its efficacy in ICU patients, particularly within the surgical population, remains inconclusive. We evaluated whether continuous low-dose ketamine infusion in postoperative surgical ICU patients reduces opioid consumption and improves pain intensity

Challenges of benzodiazepine deprescribing in elderly patients attending primary healthcare.

Benzodiazepine deprescribing in older adults is a complex process influenced by systemic, structural, cultural, and individual factors. This qualitative study examined the challenges faced by primary care professionals during the implementation of a supervised deprescribing protocol in a municipality in Minas Gerais, Brazil. Through thematic content analysis, based on Bardin's methodology and relevant theoretical frameworks for primary care, interviews with community health workers and nurses were analyzed to identify barriers related to structure, process, and outcomes. The main structural ba

Risk of miscarriage after benzodiazepine use during pregnancy: updated systematic review and meta-analysis.

BACKGROUND: Benzodiazepine use during pregnancy remains common despite guidelines discouraging it, except for short-term treatment of severe anxiety or agitation. Previous studies have suggested an increased risk of miscarriage after in utero exposure, raising concern at the European level. However, the most recent meta-analysis, published in 2020, included neither several large-scale observational studies nor an assessment by specific benzodiazepine agents. This study aimed to provide an updated synthesis of evidence on the association between early pregnancy exposure to benzodiazepines and m

Comparison of remimazolam versus propofol under deep sedation for elderly patients during prostate biopsy: a protocol for a randomised controlled trial.

INTRODUCTION: With the rapid increase in the ageing population, the use of procedural sedation and analgesia (PSA) for diagnostic procedures such as prostate biopsy in older adults is increasing. However, elderly patients are particularly susceptible to respiratory depression during PSA testing and have a significantly higher risk of hypoxaemia during procedures requiring deep sedation. Although propofol combined with fentanyl is commonly used, it frequently causes hypoxaemia and apnoea. Remimazolam, a novel ultrashort-acting benzodiazepine, may be a safer alternative with less respiratory com
