The effect of sugammadex on postoperative urinary retention and recovery characteristics: a systematic review and meta-analysis.

BACKGROUND: Postoperative urinary retention (POUR) is a frequent complication following general anesthesia. There is growing interest in whether sugammadex, a neuromuscular blockade (NMB) reversal agent that does not require anticholinergic coadministration, might reduce its incidence. This systematic review and meta-analysis aimed to evaluate the effects of sugammadex on POUR and other postoperative recovery outcomes, such as 30-day readmission rates and postoperative pain scores (POPS). METHODS: PubMed, Cochrane, Embase, and Web of Science were comprehensively searched for studies published

Comparative effects of glycopyrrolate vs. atropine combined with neostigmine on heart rate dynamics and safety outcomes in pediatric patients: a prospective randomized controlled trial.

BACKGROUND: Neostigmine is a widely used muscle relaxant antagonist that requires combination with anticholinergic agents like atropine to mitigate its muscarinic effects. Glycopyrrolate exerts less influence on cardiac conduction than atropine, making it a preferable option. However, clinical research on its pediatric use is limited. This study aims to compare the effects of glycopyrrolate and atropine, in combination with neostigmine, on heart rate and adverse events in pediatric patients. METHODS: Children undergoing penile flap surgery under general anesthesia combined with a caudal block

Preclinical efficacy of the muscarinic agonist ML-007 in psychosis models depends on both M1 and M4 receptors.

Muscarinic agonists represent a new class of treatments for psychosis with a mechanism distinct from typical and atypical antipsychotics. The muscarinic subtype M4 has been proposed as the primary mediator of efficacy but results from recent clinical trials with M4-selective compounds have drawn this hypothesis into question. Instead, activation of both M1 and M4 receptor subtypes may be required for robust treatment effects. Here, we characterize the clinical-stage muscarinic agonist ML-007 in preclinical models and explore its therapeutic potential for treating psychosis in schizophrenia and

Sugammadex for reversing neuromuscular blockade in infants and children.

This is a protocol for a Cochrane Review (intervention). The objectives are as follows: The primary objective of this review is to evaluate the benefits and harms of sugammadex compared to other neuromuscular blockade reversal agents (neostigmine and pyridostigmine) in infants and children undergoing surgical procedures. A secondary objective is to assess whether these outcomes differ by age group, comorbidities, co-medications, or type of neuromuscular blocking agent.

Hybrid molecules with dual inhibition of acetylcholinesterase and tau hyperphosphorylation: design, inhibitory activity evaluation, apoptosis assessment, and mechanistic exploration.

A comprehensive therapeutic strategy for Alzheimer's disease (AD) requires simultaneous inhibition of acetylcholinesterase (AChE) and targeting of hyperphosphorylated Tau (P-Tau)-mediated pathogenesis. To address this need, the present study designed a series of hybrid molecules by integrating three pharmacophoric scaffolds with established P-Tau-modulating activity (phenothiazine, dibenzazepine and benzothiazepinones) into AChE-inhibiting frameworks: indanone (derived from the clinical AChE inhibitor Donepezil) or 9-chloro-1,2,3,4-tetrahydroacridine (derived from Tacrine, another clinically a

Dynamic associations of cholinesterase inhibitors and memantine with cognitive trajectories in individuals with Alzheimer's or mixed dementia: a real-world analysis using the quality registry SveDem.

BACKGROUND: Alzheimer’s disease (AD) and mixed dementia (MxD) represent major public health concerns, yet there is limited real-world evidence on the long-term associations of commonly prescribed pharmacological treatments, particularly cholinesterase inhibitors (ChEIs) and their combination with memantine. This study aims to evaluate the long-term, time-varying associations of ChEIs and memantine on cognitive decline in a large, nationwide cohort of individuals diagnosed with AD or MxD. METHODS: This observational study utilized data from the Swedish registry for cognitive/dementia disorders

Efficacy of xanomeline and xanomeline trospium in the treatment of cognitive impairment: A systematic review of preclinical and clinical trials.

The high prevalence, persistence and clinical burden of cognitive impairment on health outcomes in persons with schizophrenia and bipolar disorder invites the need for innovative, mechanistically informed pharmacotherapeutic interventions. Evidence implicates dysregulation of muscarinic signalling as a mediator of cognitive dysfunction, identifying it as a potential therapeutic target. In accordance with the PRISMA guidelines, a systematic search was performed using the following electronic databases: PubMed, Medline, Cochrane Library, PsycInfo, Embase, Scopus, and Web of Science. [Availability of Standard Reference Materials for Lycorine and Galanthamine, Toxic Alkaloids of Narcissus, After Long-term Storage].

Lycorine and galanthamine are toxic alkaloids found in Narcissus. Using HPLC analysis and quantitative NMR analysis, the degrees of purity of these standard reference materials purchased in 2013, 2019, 2021, and 2025 were calculated and compared. The results showed almost no degradation of any component. However, other findings suggest that the lycorine hydrochloride weight changed because of moisture absorption (hydration). This weight change might be avoided by stocking lycorine hydrochloride monohydrate, which is originally hydrated. Maintaining a stock of lycorine and galanthamine referenc

Divergent Total Syntheses of (-)-Galanthamine, (-)-Narwedine, (-)-Lycoraminone and (-)-Lycoramine, and Formal Syntheses of (-)-Morphine and (-)-Codeine.

Polycyclic alkaloids have attracted great attention from the synthetic community because of their interesting structures and biological activity. Herein, we report the divergent syntheses of several galanthamine- and morphine-type alkaloids that share a fused 6/5/6 cis-hydrodibenzofuran core. The key transformation in these syntheses features a Pd-catalyzed Heck/aminocarbonylation reaction, which allowed for the rapid assembly of the fused 6/5/6 cis-hydrodibenzofuran core of these alkaloids with excellent diastereoselectivity. This approach provides a new strategy for the synthesis of other po

Neostigmine Versus Colonoscopic Decompression for Acute Colonic Pseudo-Obstruction Not Resolving With Conservative Measures: A Meta-Analysis.

INTRODUCTION: Acute colonic pseudo-obstruction is an uncommon but not infrequent encounter among hospitalized patients. The American Society for Gastrointestinal Endoscopy recommended neostigmine first before colonoscopy; however, the Association of Coloproctology of Great Britain and Ireland consensus guidelines in emergency colorectal surgery acknowledge the lack of evidence in the superiority of one approach over the other. This is the first meta-analysis aimed to fill this gap of evidence by including studies comparing neostigmine and colonoscopy in resolving the pseudo-obstruction. MATER

Effects of hydromethylthionine mesylate and rivastigmine in a pharmacological mouse model of Alzheimer's disease.

The results from clinical trials have indicated that the tau aggregation inhibitor hydromethylthionine mesylate (HMTM) produces disease-modifying effects in Alzheimer's disease (AD) patients when administered alone, but less of an effect when administered in conjunction with cholinesterase inhibitors (ChEIs). The use of ChEIs for AD has been supported by their ability to reverse scopolamine-induced cognitive impairments in rodents reminiscent of those seen in AD patients. We have previously shown that another tau aggregation inhibitor, methylthionine chloride (MTC), is able to reverse scopolam

Sugammadex or Neostigmine for prevention of post-operative pulmonary complications after major abdominal or thoracic surgery: study protocol for the SINFONIA (Sugammadex for preventioN oF pOst-operative pulmonary complIcAtions) randomised controlled superiority trial.

BACKGROUND: Post-operative pulmonary complications (PPCs) are an important source of morbidity and mortality after major abdominal and thoracic surgery. The use of neuromuscular blocking drugs in general anaesthesia is an important risk factor for PPCs. The incomplete reversal of this neuromuscular blockade at the end of surgery leads to residual weakness of respiratory muscles and predisposes to aspiration of pharyngeal contents, hypoventilation, and thus to PPCs such as atelectasis and pneumonia. Two reversal drugs for neuromuscular blocking agents are available: neostigmine and sugammadex.

Neostigmine for postoperative surgical urine retention: A systematic review and meta-analysis.

Postoperative urinary retention (POUR) is a common surgical complication that can result in bladder overdistension, urinary tract infection and an extended hospital stay. Although neostigmine is an effective therapy for POUR, its usage remains controversial. The purpose of this study was to investigate the effectiveness of neostigmine in improving POUR after surgery. PubMed, Embase, Web of Science, and the Cochrane Library databases were reviewed. A methodical search approach was used for data extraction, while meta-analysis and bias analysis employed Review Manager 5.2 and MedCalc. Fourteen s

Sugammadex and urinary retention after hysterectomy: A propensity-matched cohort study.

Postoperative urinary retention (POUR) is a well-known complication after gynecologic surgery. Our objective was to investigate whether the choice of pharmacologic agent for reversing neuromuscular blockade at the end of a hysterectomy is a risk factor for POUR. Among adult patients undergoing hysterectomy with general anesthesia from 2012 to 2017, those who received aminosteroid nondepolarizing neuromuscular agents followed by pharmacologic reversal were identified, and electronic health records were reviewed. The cohort was dichotomized into two groups by reversal agent: 1) sugammadex and 2)

Sugammadex Versus Neostigmine for Reversal of Neuromuscular Blockade in Patients With Severe Renal Impairment: A Randomized, Double-Blinded Study.

BACKGROUND: Sugammadex is not advised for patients with severe renal impairment, but has been shown in a variety of other populations to be superior to neostigmine for reversal of neuromuscular blockade. The objective of this study was to determine if reversal of rocuronium-induced neuromuscular blockade with sugammadex versus reversal of cisatracurium-induced neuromuscular blockade with neostigmine results in a faster return to a train-of-four ratio (TOFR) ≥90% in patients with severe renal impairment. Novel Pitolisant-Derived Sulfonyl Compounds for Alzheimer Disease.

Alzheimer's disease (AD) is a complex and multifactorial neurodegenerative disorder characterized by cognitive decline, memory loss, behavioral changes, and other neurological symptoms. Considering the urgent need for new AD therapeutics, in the present study we designed, synthesized, and evaluated multitarget compounds structurally inspired by sulfonylureas and pitolisant with the aim of obtaining multitarget ligands for AD treatment. Due to the diversity of chemical scaffolds, a novel strategy has been adopted by merging into one structure moieties displaying H3R antagonism and acetylcholine
