Successful treatment of hairy cell leukemia with TP53 abnormality using cladribine combined with low-dose rituximab: a case report and literature review.

Hairy cell leukemia (HCL) is a rare mature B-cell malignancy characterized by the BRAF V600E mutation. TP53 abnormalities in HCL are associated with poor response to standard therapies and a higher risk of relapse. We report an elderly male patient diagnosed with classical HCL (BRAF V600E-positive) who also presented with a TP53 deletion detected by fluorescence in situ hybridization (FISH) and bone marrow fibrosis. The patient was treated with one cycle of cladribine combined with low-dose rituximab (100 mg). The treatment was well tolerated. At the 7-month follow up, the patient achieved com

Potentiating In Vivo Precision Cancer Therapy With Modularized Aptamer-Chemotherapeutics Conjugates Based on DNA Tiling-Mediated Structural Nanotechnology.

While DNA origami nanotubes have been often used in the biomedical field, the technical challenges in the assembly at large scale and the susceptibility to degradation limit their exploration for clinical application. In the current contribution, we propose a structural DNA nanotechnology (TMM) for the construction of a degradation-resistant DNA nanotube (DNT) via periodically tiling two structural modules (M) into a modularized (M) tubular DNA nano-architecture. The tube circumference is 118.6 nm, the tube length is 478 nm and the assembly efficiency is almost up to 90%. Upon installation of

[Efficacy-toxicity duality of Tripterygium wilfordii in treatment of rheumatic and immune diseases: mechanisms, strategies for toxicity attenuation and efficacy enhancement, and future perspectives].

Rheumatic and immune diseases constitute a major global disease burden. Tripterygium wilfordii demonstrates remarkable therapeutic efficacy in treating rheumatoid arthritis, systemic lupus erythematosus and related conditions. However, its prominent hepatic, renal, and reproductive toxicities engender a characteristic "efficacy-toxicity duality", thereby limiting its broader clinical adoption and safe application. This study systematically synthesized domestic and international research over the past two decades to construct a regulatory network governing the "efficacy-toxicity

Epidemiology, risk factors and clinical outcome of community-acquired pneumonia in severely immunosuppressed patients: a comprehensive analysis from the multinational, prospective CAPNETZ Cohort.

PURPOSE: Despite growing evidence that immunosuppression (IS) influences aetiology and prognosis of community-acquired pneumonia (CAP), there remains a lack of data from representative multicentre studies. METHODS: The multinational prospective CAPNETZ study includes patients with CAP and HIV since 2017, and with other causes of IS since 2020. We present the first comprehensive analysis of patients with CAP and IS (HIV with low CD4 cells, systemic steroids, immunosuppressive drugs, antineoplastic treatment, neutropenia, primary immunodeficiency, transplantation, haematological neoplasia) incl

Cascade-responsive HA-decorated liposome/mesoporous silica nanoplatform co-delivering BMF-219 and 5-FU enhances gastric cancer chemotherapy through Menin- and p53-associated apoptotic signaling.

BACKGROUND: Our preliminary study found that Men1 is an oncogene and a potential therapeutic target in gastric cancer (GC). Chemotherapy remains a common therapeutic regimen for GC, but is frequently limited by multidrug resistance and poor intratumoral coordination of combination drugs. Targeting Menin may enhance chemotherapy sensitivity, but efficient co-delivery of Menin inhibitors and chemotherapeutics is hindered by the profound spatiotemporal pharmacokinetic decoupling of the drugs in vivo. METHODS: We first evaluated the synergistic antitumor effect and Menin/p53-associated mechanism

Recent advances in tumor-activated prodrug nanoparticles for cancer immunotherapy.

Numerous therapeutic agents have been demonstrated to initiate antitumor immune responses, thereby driving significant progress in cancer immunotherapy. However, clinical translation is severely limited by persistent challenges, including systemic toxicity, the immunosuppressive tumor microenvironment (TME), and low nanoparticle delivery efficiency. Tumor-activated prodrug nanoparticles (PDNPs) integrate prodrug chemistry with nanoscale delivery, enabling localized drug activation in response to tumor microenvironmental cues or exogenous stimuli. Therapeutic specificity is improved and off-tar

A novel pH-responsive nanoreactor for pyroptosis induction: multifaceted ROS amplification drives potent immunotherapy against triple-negative breast cancer.

Triple-negative breast cancer (TNBC) is associated with rapid progression and poor sensitivity to monotherapies, necessitating combination strategies to enhance tumor immunogenicity, reprogram the tumor microenvironment (TME), and convert immunologically "cold" tumors into "hot" ones. In this study, we develop a pH-responsive, pyroptosis-inducing nanoreactor, ZnS/ZIF-90:Fe/Vk3 (ZZFV), designed to elicit potent antitumor immunity. Under acidic conditions, ZZFV rapidly degrades, releasing Zn2+, Fe2+, H2S, and Vk3. H2S promotes glucose uptake and consumption while downregulating cytochrome c oxid

Anti-CD20 monoclonal antibodies in membranous nephropathy.

Novel anti-CD20 monoclonal antibodies have been developed to optimize B-cell depletion. A key mechanism of action of these therapeutic monoclonal antibodies is the activation of immune cells via Fc receptors (FcγR). B-cell depletion with anti-CD20 monoclonal antibodies was initially developed for the treatment of B-cell lymphoma. Subsequently, its use was extended to autoimmune diseases. Rituximab has long been the main monoclonal antibody used to treat immune-mediated glomerulopathies. In glomerular diseases, rituximab was initially used to treat membranous nephropathy, demonstrating a better

Non-Proliferative Immune Restoration by Euglena gracilis EK Powder in Cyclophosphamide-Induced Immunosuppressed Mice.

Cytotoxic chemotherapy frequently induces immunosuppression, impairing the innate cellular and humoral immune responses. Therefore, immunomodulatory agents that restore immune function without indiscriminate leukocyte expansion are required. This study examined whether Euglena gracilis EK powder (EKP), a paramylon-rich β-1,3-glucan preparation, supports immune recovery under cyclophosphamide (CPA)-induced immunosuppression. Male BALB/c mice received EKP (80, 200, or 500 mg/kg/day for 21 days) under a dual-temporal CPA design: acute phase (days 7 and 9) and established phase (days 14 and 17). T

Dual blockade of PD-1 and NKG2A prevents NK cell senescence and reprograms the immunosuppressive microenvironment in pancreatic cancer.

Immune checkpoint blockade (ICB) shows limited efficacy in pancreatic ductal adenocarcinoma (PDAC). Here, we demonstrate that ICB-induced IFN-γ signaling upregulates H2-T23 on pancreatic tumor cells, which interacts with NKG2A on NK cells to induce NK cell senescence through activation of p38 MAPK and STAT1/3 pathways. This impairs NK cell cytotoxicity and restricts antitumor immunity. Dual blockade of PD-1 and NKG2A effectively prevents NK cell senescence, restores NK cell function, and enhances antitumor immunity. Mechanistically, the combination therapy promotes NK cell-derived CCL5 and fac

Pulmonary embolism associated with CRBN-targeting immunomodulatory drugs: a FAERS-based pharmacovigilance and mechanistic analysis.

CRBN-targeting immunomodulatory drugs (IMiDs), including lenalidomide and pomalidomide, are widely used in hematologic malignancies but are associated with an elevated risk of venous thromboembolism (VTE). However, real-world risk patterns and the potential biological mechanisms underlying pulmonary embolism (PE) remain incompletely characterized. We conducted a comprehensive pharmacovigilance analysis using the FDA Adverse Event Reporting System (FAERS) from 2004 to Q3 2025. Disproportionality analyses (ROR, PRR, BCPNN, MGPS) and Weibull distribution modeling were employed to assess signal st

Micro-modulation of the intra-structural environment in electrospun drug delivery systems: effects on drug release and hydrolytic degradation.

Poor aqueous solubility can limit drug release from biodegradable implants. We developed a multifunctional dual-jet electrospun drug-delivery system (DDS) designed to accelerate the release of poorly water-soluble bioactive substances. Its principle is based on introducing the drug-carrying matrix and release-rate modifier as separate, interlaced fractions. Poly(D,L-lactide-co-glycolide) (PDLGA) nonwovens or poly(ε-caprolactone-co-trimethylene carbonate) (PCLTMC) membranes served as sirolimus carriers, whereas poly(vinyl alcohol) nanofibres loaded with lactic acid (PVA(LA)) were designed to in

Untargeted identification and targeted quantification of the marker compound in Gmelina asiatica L. using UHPLC-Q Exactive Orbitrap MS and validated UHPLC-TQ-MS/MS methods.

Gmelina asiatica L. plays a significant role in traditional medicine, with diverse applications in the Ayurvedic and Siddha systems and multiple pharmacological activities. However, no validated analytical method has been reported for its standardization and quality control through the quantification of its major phytoconstituent, despite its importance for pharmaceutical applications. Therefore, a sensitive and validated method for its standardization is required. In this context, liquid chromatography-mass spectrometry is one of the most suitable analytical techniques for this purpose. In th
