Anticoagulation in Mechanical Circulatory Support: Contemporary Challenges and Future Directions in Cardiovascular Pharmacotherapy.

Mechanical circulatory support (MCS) is increasingly used in patients with cardiogenic shock, advanced heart failure, and severe respiratory failure; however, anticoagulation management remains challenging due to the competing risks of thrombosis, bleeding, hemolysis, and device dysfunction. In this review, we synthesize contemporary evidence on anticoagulant selection, monitoring strategies, and device-specific management across major MCS platforms, including extracorporeal membrane oxygenation, temporary ventricular assist devices, Impella devices, and durable left ventricular assist devices

FXI and FXII in experimental stroke and FXI in clinical stroke trials.

Targeting the contact activation system through Factor XI (FXI) or Factor XII (FXII) inhibition represents a major paradigm shift in antithrombotic therapy for ischemic stroke. Unlike traditional anticoagulants, contact pathway factors drive pathological thrombus propagation and thromboinflammation; crucially, while FXII plays a negligible role in physiological hemostasis, FXI contributes only secondarily, thereby substantially uncoupling antithrombotic efficacy from major bleeding risk. However, a notable disconnect exists between experimental models and clinical development. Preclinical stro

Delayed deterioration after surgical treatment of a spinal dural arteriovenous fistula detected by three spinal angiographies: Case report.

Spinal dural arteriovenous fistulas (DAVFs) are the most common subtype of spinal arteriovenous malformations, and they often cause progressive myelopathy. Early detection and intervention are critical to preventing irreversible neurological deficits. Selective spinal angiography remains the gold standard for diagnosing this condition. However, for some patients with typical clinical features and magnetic resonance imaging (MRI) findings consistent with DAVF, the initial angiography may fail to detect the lesion. In this article, we report a case of T4 low-flow spinal DAVF that was only identi

Painless Spontaneous Spinal Epidural Hematoma Mimicking Acute Ischemic Stroke: A Case Report.

AIM: Spontaneous spinal epidural hematoma is a rare neurological emergency. Its painless variant is particularly uncommon and can easily be misdiagnosed as acute ischemic stroke, especially in patients receiving anticoagulant therapy. This study aims to enrich the clinical recognition of painless spontaneous spinal epidural hematoma and emphasize its differential diagnosis from acute stroke in high-risk medicated patients. CASE PRESENTATION: We report a rare case of painless spontaneous spinal epidural hematoma mimicking acute ischemic stroke in a woman in her early 60s receiving anticoagulat

Is a Repeat Head CT Scan Necessary After an Initial Negative Scan in Head Trauma Patients on DOAC Therapy in the Emergency Department?

Background: Head trauma in patients receiving direct oral anticoagulants (DOACs) presents a clinical dilemma because of concern for delayed intracranial hemorrhage (ICH). Although some emergency departments (EDs) routinely perform repeat head CT after an initially negative scan, the clinical value of this practice remains uncertain. Methods: We conducted a retrospective observational cohort study at Hadassah Mount Scopus University Hospital Emergency Department (ED), a level 3 trauma center with approximately 70,000 visits per year, spanning January 2019 to December 2022. Adult patients receiv

Acute ischemic stroke despite NOAC therapy in atrial fibrillation: Clinical factors associated with AIS in a 2-center observational case-control study.

Acute ischemic stroke (AIS) can occur in patients with non-valvular atrial fibrillation (AF) despite treatment with non-vitamin K antagonist oral anticoagulants (NOACs). This study evaluated clinical, treatment-related, inflammatory, and echocardiographic factors associated with AIS in NOAC-treated patients with AF. This retrospective, 2-center case-control study included 322 patients with non-valvular AF who had received NOAC therapy for at least 3 months. Of these, 112 were hospitalized with AIS and 210 were outpatient controls without AIS at the study assessment. Group differences and facto

Compression therapy in the prevention of thromboembolic disease following total knee arthroplasty with a fast recovery protocol: A systematic review.

Thromboembolic disease (TED), including deep vein thrombosis (DVT) and pulmonary embolism (PE), remains a serious postoperative risk after total knee arthroplasty (TKA). Enhanced recovery after surgery (ERAS) protocols emphasize early mobilization, warranting reassessment of mechanical prophylaxis, such as compression therapy. This systematic review evaluates the benefits of compression therapy in preventing TED in TKA patients under ERAS protocols combined with pharmacological prophylaxis. A systematic review was conducted, analyzing seven studies comparing combined mechanical and pharmacolog

Anticoagulation strategies during peripheral ultrafiltration in diuretic-resistant congestive heart failure.

Introduction and objectives Peripheral ultrafiltration (UF) is an effective strategy for the management of diuretic-resistant congestion in patients with decompensated heart failure. However, extracorporeal circuit clotting remains one of the main technical limitations of the procedure, and the optimal anticoagulation strategy during UF has not been clearly established. The primary aim of this study was to evaluate the efficacy and safety of different anticoagulation strategies during UF. Methods This prospective, nonrandomized, single-center study included 87 consecutive patients who underwen

Inside the 'Black Box' of Pharmaceuticalisation: NICE, the Permissive Principle and the Case of Anticoagulants.

Sociological interest in pharmaceuticalisation continues to grow, especially regarding the expansion of pharmaceutical use in society. This article presents the first major empirical study focusing on the directionality of pharmaceuticalisation by taking cost-effectiveness assessment and regulation of anticoagulants in England by the National Institute of Health and Care Excellence (NICE) as a case study. During this century, anticoagulant therapy for the prevention of stroke and systemic embolism has changed direction profoundly away from warfarin towards newer and markedly more expensive dir

Factor XI and XIa Inhibitors: A State-of-the-Art Review Across Clinical Indications.

Balancing thrombotic and bleeding risk remains the central challenge of anticoagulation, and clinically significant bleeding continues to limit even direct oral anticoagulants, especially in high-bleeding-risk patients. Factor XI (FXI) and activated FXI (FXIa) inhibitors are a mechanistically novel class that seeks to uncouple thrombosis from hemostasis; FXI amplifies clot formation through the intrinsic pathway but contributes little to physiologic hemostasis. This state-of-the-art review examines three classes of FXI/FXIa inhibitors (antisense oligonucleotides, monoclonal antibodies, and sma

Adding artificial intelligence to the pulmonary embolism response team improves time to diagnosis and treatment of pulmonary embolism.

OBJECTIVE: There is limited data demonstrating the benefit of artificial intelligence (AI) technology for the diagnosis and triage of pulmonary embolism (PE). This study aimed to demonstrate improved time to diagnosis of PE and subsequent anticoagulation and intervention, with the goal of reducing in-hospital mortality. We hypothesized that implementation of AI-assisted computed tomography pulmonary angiogram (CTPA) detection would reduce time to diagnosis, anticoagulation, and intervention compared with the standard radiology-based workflow. METHODS: A single institution retrospective review

Personalized anticoagulant therapy in venous thromboembolism: current role of direct oral anticoagulants, unmet clinical needs, and future perspectives on Factor XI inhibition.

Venous thromboembolism (VTE), encompassing deep vein thrombosis and pulmonary embolism, remains a major cause of morbidity and mortality worldwide. The introduction of direct oral anticoagulants (DOACs) has transformed the management of VTE by providing effective and convenient alternatives to vitamin K antagonists. However, despite their favorable efficacy-safety profile, clinically relevant bleeding, treatment individualization, and the management of patients with competing thrombotic and hemorrhagic risks remain important unmet clinical needs. Contemporary VTE management increasingly requir

[Livedoid vasculopathy: A case report].

Livedoid vasculopathy (LV) is a painful and thrombotic obliterative vascular disorder characterized by spontaneous thrombus formation within skin capillaries or small veins, ultimately leading to localized hypoxia and the development of skin ulcers. LV is a rare condition predominantly affecting young women, with an incidence rate of approximately 1/100 000. Clinically, it manifests as recurrent reticular green spots and painful ulcers on the distal lower extremities, which subsequently heal, leaving behind white and atrophic scars, termed "white atrophy". The skin manifestations of the diseas

Differences in treatment and outcome of patients with ST- elevation myocardial infarction (STEMI) and non-STEMI in Germany.

BACKGROUND: Although ST-segment elevation myocardial infarction (STEMI) and non-STEMI (NSTEMI) are very similar regarding pathophysiology and clinical treatments, especially NSTEMI comprises a much more heterogenic group of patients and underlying diseases. We therefore aimed to assess the treatments and outcomes of both entities in a large contemporary cohort. METHODS: Patients with STEMI and NSTEMI between 01/01/2010-31/12/2018 were identified from the largest German health insurance (AOK, ≈ 26 million members). Patient demographics, their hospital course, adherence to guideline-directed dr

Correlation of anticoagulation assays with bleeding during extracorporeal membrane oxygenation at a tertiary pediatric hospital.

IntroductionDue to anticoagulation during Extracorporeal Membrane Oxygenation (ECMO), patients face a higher risk of bleeding. This study aimed to assess the correlation between anticoagulation assays and hemorrhage in ECMO patients.MethodsWe reviewed records from patients in our critical care units, comparing anticoagulation parameters with clinical outcomes. Statistical analyses included the Wilcoxon rank sum test for continuous variables and chi-square or Fisher's exact tests for categorical variables. We used Kaplan-Meier curves to depict hemorrhage incidence and logistic regression to ide

Protocols for anticoagulation management in pediatric extracorporeal membrane oxygenation: A comparative retrospective study.

IntroductionIn children undergoing Extracorporeal Membrane Oxygenation (ECMO), anticoagulation is given to counterbalance the risk of thrombosis. Several laboratory tests are available to monitor heparin, but the ideal method still needs to be determined.MethodsThis retrospective cohort study included all patients under 18 years on ECMO support between 2010 and 2021. At our institution, the test used to monitor unfractionated heparin changed over time, dividing patients into three periods, using either activated clotting time (ACT) (2010-2014), activated partial thromboplastin time (aPTT) (201
