Association of Thromboelastography With Hematoma Expansion and Disability in Spontaneous Intracerebral Hemorrhage: A Multicenter Study.

BACKGROUND AND OBJECTIVES: Hematoma expansion (HE), an increase in bleeding into the brain, is a modifiable cause of disability and death after intracerebral hemorrhage (ICH). Some specialized hemostatic biomarkers are associated with HE. Widely available hemostatic biomarkers could improve our understanding of HE and lead to targeted treatments. METHODS: We conducted a multicenter, prospective, cohort study of patients with spontaneous ICH between 2019 and 2023 at 6 medical centers across the United States. Patients with an initial CT scan within 12 hours of symptom onset were enrolled. All

Mechanical Thrombectomy Versus Anticoagulation Alone in Intermediate-High Risk PE: Improved Functional 90-Day Outcomes in STORM-PE RCT.

BACKGROUND: Up to 50% of pulmonary embolism (PE) survivors experience long-term functional limitations and reduced quality of life (QoL). Reperfusion strategies can rapidly relieve right-ventricular (RV) dysfunction and prevent clinical deterioration or hemodynamic collapse; however, these approaches have never been evaluated in a randomized trial against anticoagulation alone using patient-centered outcomes. OBJECTIVES: This study sought to evaluate whether mechanical thrombectomy with anticoagulation improves functional, QoL, and safety outcomes compared with anticoagulation alone in patien

Challenges in PT/INR harmonization across the total testing process.

The International Normalized Ratio (INR), introduced by the World Health Organization in 1983, is central to vitamin K antagonist (VKA) therapy monitoring. However, thromboplastin standardization alone does not ensure harmonized prothrombin time (PT)/INR results. Residual variability arises throughout the testing continuum. In this narrative review, we examine the principal sources of residual PT/INR non-equivalence across the "vein-to-report" pathway. Particular attention is devoted to clinical settings in which the assumptions underlying INR calibration are only partially satisfied, includin

[Caprini Risk Score validity and prevention of venous thromboembolism in emergency surgery].

OBJECTIVE: To evaluate validity of the Caprini Risk Score (CRS) for predicting postoperative venous thromboembolism (VTE) in emergency surgery, as well as to investigate the impact of compliance with prophylactic protocols on the risk of postoperative thrombosis. MATERIAL AND METHODS: A single-center prospective study included 267 patients after urgent or emergency major surgery. CRS scores were calculated before the intervention and at discharge. Routine mechanical and pharmacological prophylaxis for VTE was used after surgery. The primary endpoint was combination of asymptomatic and symptom

Percutaneous coronary intervention using sirolimus-eluting balloons in patients at high bleeding risk: the SELUTION DeNovo HBR substudy.

BACKGROUND: Drug-eluting stents (DES) are the current mainstay of percutaneous coronary intervention (PCI) in high bleeding risk (HBR) patients. However, drug-coated balloons may be an alternative that reduces reliance on prolonged courses of antiplatelet therapy. AIMS: The SELUTION DeNovo trial randomised 3,323 PCI candidates 1:1 to treatment with either sirolimus-eluting balloons (SEBs) and provisional stenting or systematic DES implantation. We report on a prespecified subgroup of HBR patients. METHODS: A total of 522 enrolled patients met the Academic Research Consortium-HBR criteria. At

Subclinical pulmonary embolism in first-line/induction therapy of lung cancer - a remaining clinically relevant adverse event.

INTRODUCTION: Pulmonary embolism (PE) at initial diagnosis and during first-line systemic treatment is clinically relevant in patients with lung cancer. However, real-world incidence of subclinical PE is not well established. METHODS: Retrospective single-center analysis of patients at least 18 years old with non-small-cell lung cancer (NSCLC) or small-cell lung cancer (SCLC) who received first-line/induction systemic therapy. All patients underwent contrast-enhanced computed tomography (CT) of the thorax with bolus tracking for PE detection. Treatment response was assessed by restaging at we

The Left Atrial Appendage: Anatomy, Electrophysiology, and Clinical Implications for Atrial Fibrillation and Stroke.

PURPOSE OF REVIEW: The left atrial appendage (LAA) is nowadays recognized as both an arrhythmogenic substrate and the dominant source of thromboembolism in atrial fibrillation (AF). In this review we tried to summarize recent data on the anatomy, physiology, and imaging of the LAA, and examine how these findings help guide rhythm-control and stroke-prevention strategies. RECENT FINDINGS: High-resolution mapping confirms the LAA as a non-pulmonary-vein trigger source in persistent and post-ablation AF, supporting LAA electrical isolation in selected patients, though isolation may induce mechan

Deep learning-guided high-throughput screening and molecular dynamics simulations facilitate the discovery of potential FXIIa inhibitors.

CONTEXT: Pathological thrombosis is a major cause of cardiovascular and cerebrovascular diseases. Current anticoagulants increase bleeding risk by impairing physiological hemostasis. Because coagulation factor XIIa (FXIIa) drives pathological thrombosis but is dispensable for normal hemostasis, inhibiting FXIIa may offer antithrombotic efficacy with reduced bleeding risk. However, the marked flexibility of its active site hampers the discovery of small-molecule inhibitors. Here, we developed an integrated computational workflow to address this challenge and screened a library of approximately

Postoperative Management of Direct Oral Anticoagulants After Elective Spine Surgery: When Can These Drugs be Safely Resumed?

STUDY DESIGN: Retrospective monocentric study. OBJECTIVES: To determine a safe interval for the resumption of DOACs based on the risk of postoperative surgical reoperation in the month following spine surgery. SUMMARY OF BACKGROUND DATA: More patients scheduled for spine surgeries are being treated with direct oral anticoagulants (DOACs). Scientific society guidelines recommend the resumption of DOACs 2-3 days postoperatively. However, spine surgeries could be associated with an increased risk of complications warranting urgent surgical revision. Early resumption of DOACs imposes a 3-5-day d

Short and Long-Term Outcomes of Intermediate-High Risk Pulmonary Embolism Treated with Catheter-Directed Reperfusion Therapy Versus Anticoagulation.

BACKGROUND: Treatment of intermediate-high risk pulmonary embolism (PE) with catheter directed therapy remains a topic of controversy. It is unknown if long-term post-hospitalization outcomes of these patients are improved compared to anticoagulation. RESEARCH QUESTION: What is the comparative effectiveness of catheter-directed reperfusion therapy versus conventional anticoagulation on both short- and long-term outcomes among patients with intermediate-high risk PE? STUDY DESIGN AND METHODS: This was a retrospective cohort study of hospitalized patients in the United States with intermediate

Presumed transdermal superwarfarin toxicity in a pest control worker.

A man in his early 60s presented with dyspnoea superimposed on a 2-month to 3-month history of spontaneous mucocutaneous bleeding. Laboratory testing revealed unmeasurably elevated international normalised ratio (INR) and activated partial thromboplastin time. CT of the neck demonstrated supraglottic soft tissue swelling with severe airway narrowing, requiring intubation for airway protection. Further history revealed occupational exposure to rodenticides during pest control work without consistent use of personal protective equipment. Markedly reduced activity of vitamin K-dependent clotting

Anticoagulation Outcomes in Nursing Home Residents With Atrial Fibrillation and ADL Disability.

IMPORTANCE: Long-term nursing home residents with atrial fibrillation (AF) and activities of daily living (ADL) disability are at high risk of bleeding and death, making the net benefit of anticoagulation uncertain. OBJECTIVE: To compare the association of oral anticoagulant (OAC) use vs nonuse with ischemic stroke and major bleeding in long-term nursing home residents with AF and ADL disability. DESIGN, SETTING, AND PARTICIPANTS: This cohort study used US Medicare claims linked with the Minimum Data Set (January 1, 2013, to December 31, 2022). Nursing home residents with nonvalvular AF and

Anticancer Drugs and Cancer-Associated Thrombosis.

The rapid development of novel cancer therapies has considerably improved the care of cancer patients and cancer-related survival; however, these therapies are associated with a broad range of adverse events that have become a major burden in this population. Alongside other cancer-related factors, vascular toxicity from anticancer therapies contributes to an increased risk of venous thromboembolism. Among anticancer drugs, chemotherapeutic agents (eg, anthracyclines, platinum-based drugs, antimetabolites), hormonal therapies (eg, selective estrogen receptor modulators), agents used for multip

Safety of antithrombotic therapy in hereditary hemorrhagic telangiectasia: results from a prospective cohort study.

BACKGROUND: Hereditary hemorrhagic telangiectasia (HHT) is a vascular disorder characterized by a predominant bleeding phenotype. However, patients are also at risk of thrombotic events and may require antithrombotic therapy (AT), for which safety data remain limited. OBJECTIVES: To prospectively evaluate the safety of AT in patients with HHT using standardized longitudinal assessment of bleeding outcomes. METHODS: We conducted a prospective observational study at a tertiary HHT referral center. Patients with a definite diagnosis of HHT and a new indication for antiplatelet or anticoagulant

Thromboembolism and Bleeding Risks in Atrial Fibrillation: Outcomes of Oral Anticoagulation After Catheter Ablation.

BACKGROUND: Despite the growing use of catheter ablation for atrial fibrillation, large-scale studies assessing the efficacy and safety of oral anticoagulation (OAC) after ablation, particularly in patients with high stroke risk, remain limited. METHODS: This prospective study enrolled consecutive patients from the China-AF (China Atrial Fibrillation) registry between August 2011 and June 2022. Stroke risk was stratified by CHA2DS2-VA score as low (0-1), intermediate (2), or high (≥3). The study end points were thromboembolism (ischemic stroke or systemic embolism), and bleeding (Internationa

Early change and new onset of vascular pathology in cervical artery dissection: implications for surveillance.

INTRODUCTION: Antithrombotic therapy is central to stroke prevention after spontaneous cervical artery dissection (sCAD), but its efficacy may depend on the underlying vascular morphology. We examined the temporal course of vascular changes after sCAD. PATIENTS AND METHODS: Consecutive patients with first-time sCAD underwent prospective serial duplex sonography for up to 10 years. In this retrospective cohort analysis, pathology change was defined as recanalisation of an occluded artery or progression from stenosis to occlusion. New pathology resulting from dissection recurrence was considere
