Liver sinusoidal endothelial cell (LSEC) defenestration in chronic heart failure is primarily dependent on cardiac diastolic dysfunction and is reversed by sodium-glucose cotransporter 2 inhibitor but not by angiotensin-converting enzyme inhibitor.

AIMS: Liver sinusoidal endothelial cells (LSECs) dysfunction was demonstrated to represent an early and persistent event in chronic heart failure (CHF), preceding congestive hepatopathy and contributing to the pathophysiology of the disease. However, it remains unknown whether classical CHF pharmacotherapy would reverse LSEC defenestration. Herein, we characterised the therapeutic effects of angiotensin-converting enzyme (ACE) inhibitor - perindopril, and sodium-glucose cotransporter 2 (SGLT2) inhibitor - empagliflozin - on the cardiohepatic axis and, in particular, on LSEC defenestration in t

Acute effects of volume-matched isometric exercise intensity on vascular function and autonomic regulation in men with elevated blood pressure.

Isometric exercise (IE) has been introduced as an effective non-pharmacological strategy for blood pressure (BP) management; however, the influence of exercise intensity remains unclear during lower-limb IE. This study investigated the acute effects of volume-matched high-intensity (HIIE) and low-intensity IE (LIIE) on cardiovascular function in men with elevated BP. Seventeen men with prehypertension or stage I hypertension completed three randomized crossover conditions, including HIIE (60% of maximal voluntary contraction (MVC)), LIIE (30% of MVC), and a non-exercise control condition (CON)

Comparative Efficacy of Treatments for Resistant or Uncontrolled Hypertension: A Network Meta-Analysis of Randomized Trials.

BACKGROUND: Many patients with hypertension fail to achieve recommended blood pressure targets despite treatment with 2 or more antihypertensive drugs. OBJECTIVES: We aimed to compare the efficacy of add-on treatments for the management of resistant or uncontrolled hypertension. METHODS: We searched Embase and MEDLINE through November 2025. We included randomized trials comparing add-on treatments for resistant or uncontrolled hypertension. The certainty of evidence was assessed using the Confidence In Network Meta-Analysis (CINeMA) framework. The primary outcome was the change in office sys

The p.Arg52Leu variant in GNB2 causes a gain-of-function due to an increase of IK, ACh in stimulated hiPSC-CMs.

BACKGROUND: Initially discovered and published in 2017, the heterozygous mutation (p.Arg52Leu) in the GNB2 gene is associated with sinus node dysfunction (SND) and atrioventricular block (AVB). Electrophysiological characterization using heterologous expression systems has demonstrated that this R52L GNB2 variant results in a larger G-protein-activated K+ (GIRK/IK, ACh) current. So far, this has not been functionally validated in patient-derived disease modelling using human induced pluripotent stem cell-derived cardiomyocytes (CM). METHODS: Here, we differentiated CMs derived from human indu

Refractory amiodarone-induced thyrotoxicosis causing malignant arrhythmias, treated with therapeutic plasma exchange-bridged thyroidectomy.

Amiodarone-induced thyrotoxicosis (AIT) is a potentially life-threatening complication of amiodarone therapy, particularly hazardous in patients with underlying structural heart disease. We report the case of a man in his mid-60s with severe cardiomyopathy and recurrent ventricular tachycardia who developed refractory thyrotoxicosis following amiodarone exposure. Overlapping features of type 1 and 2 AIT suggested mixed-type disease, posing a significant diagnostic challenge. Despite aggressive conventional therapy with high-dose thionamides and glucocorticoids, supplemented by adjunctive agent

Recurrent myocarditis masquerading as acute myocardial infarction: unmasking Behçet's disease.

Myocarditis can closely mimic acute myocardial infarction (AMI), particularly in young patients without atherosclerotic risk factors. We report a 27-year-old man with recurrent severe chest pai7n, transient ST-segment elevations, and markedly elevated high-sensitivity troponin-T, in whom coronary angiography was repeatedly normal. Serial cardiac magnetic resonance and 18F-fluorodeoxyglucose positron-emission tomography revealed a migratory pattern of myocardial inflammation, with new lesions emerging as earlier ones resolved, fulfilling criteria for recurrent myocarditis. Recurrent oral ulcers

Losartan, uric acid, and cardiometabolic disease: beyond blood pressure control.

Losartan has a unique uricosuric property among angiotensin II receptor blockers and may therefore be advantageous in patients with hypertension and hyperuricemia or gout. The study by Xu et al. combined a meta-analysis of placebo-controlled randomized trials with a target trial emulation using UK Biobank data and demonstrated that losartan reduced serum uric acid by approximately 0.3 mg/dL. Although this effect is modest and substantially smaller than that of established uric acid-lowering therapies, it may be clinically relevant when selecting antihypertensive treatment for patients with bor

Electrophysiological and computational analyses of anethole-induced inhibition of L-type calcium channels in rat tracheal smooth muscle.

Anethole, a naturally occurring phenylpropanoid, is a major constituent of several essential oils and exhibits a broad spectrum of pharmacological activities, including reduction of neuronal excitability, antispasmodic effects, and inhibition of Ca2+ influx. To elucidate the mechanism underlying calcium current inhibition, we investigated the effects of anethole on voltage-dependent calcium channels (VDCCs) in rat tracheal smooth muscle myocytes. Whole-cell patch-clamp recordings in dissociated tracheal myocytes were combined with molecular docking analyses. Ba2+ was used as the charge carrier

Vascular smooth muscle cell FTO deletion aggravates atherosclerotic plaque instability by promoting VSMC migration through the N6-methyladenosine modification of the COL3A1 mRNA.

Vascular smooth muscle cell (VSMC) migration is the key event in the pathogenesis of atherosclerosis (AS) and plaque instability. Trimethylamine-N-oxide (TMAO) induces VSMC migration to promote the progression of AS. However, the effects of the N6-methyladenosine (m6A) modification on this process remain unclear. Here, TMAO increased total m6A levels in human aortic smooth muscle cells (HASMCs) and decreased fat mass and obesity-associated protein (FTO) and YTH domain-containing family protein 2 (YTHDF2) protein expression. FTO or YTHDF2 overexpression significantly inhibited HASMC migration i

METTL7B Mitigates Hypertension and Vascular Remodeling.

BACKGROUND: Hypertension is a major risk factor for cardiovascular diseases. RNA N6-methyladenosine (m6A) modification is closely linked to hypertension pathogenesis, but key m6A-related factors that regulate blood pressure remain unclear. This study aims to identify these regulators and evaluate their therapeutic potential. METHODS: We assessed RNA m6A changes and METTL (methyltransferase-like) expression in hypertensive aortas via methylated RNA immunoprecipitation sequencing and RNA sequencing (RNA-seq). Global Mettl7b knockout mice and vascular smooth muscle cell (VSMC)-specific Mettl7b k

Mechanism of endoplasmic reticulum stress: Insights for potential therapeutic benefits in burns.

Burns are injuries to the skin and deep tissues caused by thermal effects, which can trigger inflammation, necrosis, and systemic pathological reactions, often leading to life-threatening complications such as shock, infection, and multi-organ failure. Endoplasmic reticulum stress (ERS) is a key driving factor for tissue damage after burns. Burns are proposed to induce ERS through multiple mechanisms, including protein denaturation, inflammatory cytokine storms, and metabolic disturbances, Under stress conditions, cells can activate three key sensors, IRE1, PERK, and ATF6, to fully activate th

Vericiguat for heart failure with reduced ejection fraction on quadruple therapy without recent worsening heart failure: an exploratory real-world cohort study.

BACKGROUND: Heart failure with reduced ejection fraction (HFrEF) patients remain at substantial risk despite quadruple guideline-directed medical therapy (GDMT [ARNI + SGLT2 inhibitor + β-blocker + MRA]). Data on vericiguat in stable HFrEF patients without recent worsening heart failure receiving contemporary quadruple GDMT are limited. This observational study aimed to explore the efficacy and safety of vericiguat in this population. METHODS: A total of 450 adult patients with LVEF < 45% were enrolled (200 received quadruple GDMT plus vericiguat, 250 received quadruple GDMT alone). After 1:1

Feasibility of 12-lead ECG reconstruction at rest and during adenosine stress perfusion inside a 3-Tesla MR scanner.

Background Electrocardiographic (ECG) devices in magnetic resonance (MR) scanners have a narrow bandwidth and suffer from signal distortion, preventing measurement of QRS/ QT durations, or detection of ST/ T-wave changes during ischaemia. We integrated an external device with novel post-processing software into a 3 Tesla (T) scanner to assess feasibility and accuracy for reconstructing 12-lead ECGs prior to and during adenosine infusion. Methods Reference 12-lead ECGs were first recorded outside the MR-environment. Using three MR-safe electrode patches, in-bore ECGs were then recorded prior to

Resistant Hypertension: State of the Art Review.

Resistant hypertension, defined as blood pressure above target despite treatment with at least three antihypertensive agents (including a diuretic) or controlled blood pressure requiring four or more medications, affects over 140 million individuals globally and confers increased risks of heart failure, stroke, and chronic kidney disease progression. A considerable proportion of cases represent apparent resistant hypertension, in which pseudo-resistance due to inaccurate blood pressure measurement, medication nonadherence, or white-coat effect has not yet been excluded. 2025 AHA/ACC/AANP/AAPA/ABC/ACCP/ACPM/AGS/AMA/ASPC/NMA/PCNA/SGIM guideline for the prevention, detection, evaluation and management of high blood pressure in adults: a commentary from the European Renal Best Practice (ERBP).

American Scientific Societies (AHA/ACC and others) updated in 2025 the guideline on High Blood Pressure following 2024 ESC and 2023 ESH updates from Europe. Guidelines mostly agree on who should receive pharmacotherapy and on a common target of systolic blood pressure (SBP; <130 mmHg) and diastolic blood pressure (DBP) <80 mmHg for most, as well as on individualizing BP targets according to patient characteristics. However, they differ in nomenclature, pharmacotherapy for people at lower cardiovascular risk and aiming at SBP <120 mmHg. Overall, 2025 AHA/ACC supports pharmacotherapy earlier and
