DES-Based Green Synthesis of 2D Fluorescent Molybdenum Disulfide for Highly Sensitive and Selective Detection of Nifedipine.

Highly fluorescent 2D MoS2-x nanosheets exhibiting intense blue emission were synthesized using a green chemistry approach, utilizing DES as both the reaction medium and a surface passivating agent. The material was synthesized via a hydrothermal method, followed by ultrasonication-assisted exfoliation to yield ultrathin nanosheets with enhanced photoluminescence properties. The 2D MoS2-x nanosheets demonstrated strong fluorescence at λem = 420 nm when excited at λex = 330 nm. These nanosheets were employed as a fluorescence probe for the detection of the calcium channel blocker drug nifedipin

1,2-seco-Bicyclic Polyprenylated Acylphloroglucinols as Isoform Selective Inhibitors of T-Type Calcium Channels.

Hyreptaones A-E (1-5, respectively), five undescribed 1,2-seco-bicyclic polyprenylated acylphloroglucinols (BPAPs), were discovered in Hypericum reptans. 1-3 constitute the first 1,2-seco-BPAPs originating from rare 3,3-diisoprenylated BPAPs. 4 and 5 possess a spirocyclic 6/5/5 scaffold arising from further cyclization of 1,2-seco-BPAP. Their structures were determined by spectroscopic analysis and X-ray crystallography. 1-5 preferentially inhibited Cav3.1 over other T-type calcium channels, whereas 4 and 5 exhibited state-dependent gating modulation of Cav3.1 and represent a novel class of Ca

Comparative Outcomes of Beta-Blockers vs Non-Dihydropyridine Calcium Channel Blockers in Heart Failure With Preserved Ejection Fraction: A Systematic Review and Meta-Analysis.

Determining the most effective medication regimen to manage blood pressure and heart rate in patients with heart failure with preserved ejection fraction (HFpEF) continues to lack a clear clinical consensus. Although beta-blockers (BBs) are widely prescribed, their negative chronotropic effects may worsen exercise intolerance. Alternative agents like verapamil and diltiazem, which belong to the non-dihydropyridine calcium channel blocker (non-DHP CCB) class, offer a physiological alternative, yet comparative data are scarce. A systematic review and meta-analysis were conducted, comparing BBs v

Amlodipine and Nifedipine Associations with B-Type Natriuretic Peptide in Women with Hypertension and Heart Failure with Preserved Ejection Fraction.

BACKGROUND: Calcium channel blockers (CCBs) are frequently prescribed to manage hypertension in patients with heart failure with preserved ejection fraction (HFpEF). However, whether individual dihydropyridine agents exert uniform class effects on myocardial wall stress remains unclear. METHODS: This retrospective, single-center observational study analyzed 465 consecutive patients with hypertension and HFpEF (354 women and 111 men) evaluated at Kumamoto Kinoh Hospital between May 2012 and July 2018. Multivariable linear regression models assessed associations between individual dihydropyridi

Small-Molecule Antivirals against Severe Fever with Thrombocytopenia Syndrome Virus: From Viral Life Cycle Targets to Host-Directed Strategies.

Severe fever with thrombocytopenia syndrome virus (SFTSV), an emerging tick-borne bunyavirus with a case fatality rate of up to 30%, poses a growing threat to global public health. No approved specific antivirals are currently available. This perspective critically evaluates recent advances in the development of small-molecule anti-SFTSV agents, organized through a dual framework encompassing both viral life cycle targets and host-dependent pathways. We highlight direct-acting antivirals targeting key viral components, including viral entry glycoproteins (Gc/Gn proteins), cap-snatching endonuc

Research Progress on Intraocular Pressure-lowering Drugs, Commonly Used Clinical Adjuvants, and Emerging Neuroprotective Agents for Glaucomatous Optic Neuropathy.

Glaucomatous optic neuropathy (GON) is the leading cause of irreversible blindness worldwide, characterized by progressive retinal ganglion cell (RGC) loss. Current clinical treatment relies primarily on IOP-lowering drugs, but a subset of patients continues to experience progressive optic nerve damage. Adjuvant therapies such as antioxidants and calcium channel blockers have limited efficacy. Emerging neuroprotective strategies including NMDA receptor antagonists, neurotrophic factors, and gene therapy remain in preclinical or early-stage trials. This review systematically summarizes the mech

Chemically Modified ω-Agatoxin IVA Enables Exploration of P/Q-Type Voltage-Dependent Calcium Channels.

P/Q-type voltage-dependent calcium channels (VDCCs) are involved in the regulation of neurotransmitter release and in the integration of dendritic inputs in the central nervous system (CNS). However, their precise localization and protein networks in the CNS remain elusive due to the lack of molecular tools applicable to the delicate and complex three-dimensional environment of the living brain. In this study, we synthesized peptide-based probes using ω-agatoxin IVA (AgTX), a specific channel blocker for P/Q-type VDCCs derived from the funnel-web spider Agelenopsis aperta. The far-red fluoroph

Effects of azelnidipine, a calcium channel blocker, in a rat femur fracture model.

BACKGROUND: Fractures are an important public health problem as they reduce quality of life due to the occurrence of pain and complications. Osteoblasts originate from mesenchymal stem cells and play a vital role in bone formation. They also contain voltage-gated calcium channels. Azelnidipine is a dihydropyridine-derivative blocker of L- and T-type calcium channels. The signalling process involving the L-type calcium channel is important for the function of osteoblasts. Nevertheless, the impact of dihydropyridine chemicals on bone metabolism remains largely unexplored. To the best of our know

Association of Antihypertensive Medications with Orthostatic Hypotension among OIder Adults.

BACKGROUND: Orthostatic hypotension (OH) is common among older adults actively prescribed hypertension pharmacotherapy. Associations between antihypertensives and OH are inconsistent, due to differences in drug classes and OH assessment protocols. METHODS: Three supine and six standing blood pressures (BPs) were measured over 6 minutes, timed to initiate immediately (0 minutes) and 1, 2, 3, 4, and 5 minutes after standing. OH was defined as a drop of systolic/diastolic BP ≥ 20/10 mmHg upon standing from the supine position. OH phenotypes were initial (immediately after standing), early (withi

Causal effects of antihypertensive drugs on thyroid diseases: A trans-ancestry drug-target Mendelian randomization study.

Thyroid disorders frequently coexist with cardiovascular disease, including hypertension, but whether long-term modulation of antihypertensive drug targets influences thyroid disease risk remains uncertain. We aimed to evaluate the associations of 7 antihypertensive drug-target classes with thyroid diseases in European and East Asian populations using a trans-ancestry drug-target Mendelian randomization (MR) framework. We performed a two-sample, trans-ancestry drug-target MR analysis of 7 antihypertensive target classes. Target-proximal single-nucleotide polymorphisms associated with systolic

Additive Value of Acetylcholine Testing in ANOCA: A US-Based Multicenter FlowLab Study.

BACKGROUND: Angina with non-obstructive coronary arteries (ANOCA) is an underrecognized cause of angina with non-universal diagnostic techniques and patient management strategies. Among ANOCA testing, vasospasm provocation is not always performed mainly due to differences in institutional availability. AIMS: Using the FlowLab Study, we quantified vasospasm prevalence among patients with ANOCA at US centers and compared physician decision-making, endotype distribution, and management between sites that did and did not include vasospasm testing in their workup. METHODS: The FlowLab Study was a

Angiotensin II Receptor Blockers and the Risk of Alzheimer's Disease Progression: A Population-Based Cohort Study.

We investigated the effects of angiotensin II receptor blockers (ARBs) on the progression of Alzheimer's disease (AD). This retrospective cohort study used data from the Korean nationwide claims data to identify patients with AD aged 60-79 years in 2010. ARB users and nonusers were 1:1 propensity score matched. Cox proportional hazards regression evaluated AD progression, defined as memantine initiation and mortality. Active-comparator and dose-response analyses were conducted. ARB use was associated with lower risks of memantine initiation (hazard ratio [HR] = 0.79; 95% confidence interval [C

The Evolving Landscape of Hypertrophic Cardiomyopathy Treatment: Are Guidelines Keeping Pace?

BACKGROUND: Hypertrophic cardiomyopathy (HCM) is a common inherited cardiovascular disorder associated with morbidity. Rapid advances in therapeutics-particularly the emergence of cardiac myosin inhibitors (CMIs)-suggest that current management guidelines are lagging best practices. AREAS OF UNCERTAINTY: Cardiac myosin inhibitors have revolutionized the management of patients with obstructive HCM. Patients with nonobstructive HCM have had limited therapeutic options historically; however, the recently published ACACIA-HCM trial demonstrated significant benefit in these patients and may repres

Reversible atrioventricular block associated with transdermal fentanyl use: a paediatric case with non-Hodgkin lymphoma.

Pain is a common and significant clinical problem in childhood cancers, arising both from the disease itself and from administered treatments. Although pain control is primarily achieved with routine analgesics, this may be insufficient in chronic or progressive stages of the disease. In such cases, treatment with potent analgesics, such as opioid derivatives, comes into play. Transdermal fentanyl is widely preferred in paediatric oncology practice due to its ease of administration and effective analgesic properties. The most common adverse effects of opioids include tolerance, nausea, vomitin

Integrated Use of Extracorporeal Membrane Oxygenation and Molecular Adsorbent Recirculating System for Refractory Cardiotoxic Ingestion: A Single-Center Case Series.

BACKGROUND: Cardiotoxic ingestions of calcium-channel blockers and beta-adrenergic antagonists are highly lethal, often causing refractory vasoplegic shock and noncardiogenic pulmonary edema. While venoarterial extracorporeal membrane oxygenation (ECMO) is a recognized rescue therapy for cardiogenic shock, the roles of venovenous ECMO and albumin dialysis-specifically the molecular adsorbent recirculating system (MARS)-as well as the effectiveness of linking patients to therapy through a coordinated referral process remain underexplored. CASE SUMMARY: We describe five consecutive patients wit

Bilateral Lower Extremity Edema During Donepezil Treatment: A Case Report.

Introduction/ Background: Acute bilateral lower-extremity edema is uncommon and often results from medication side effects, acute heart failure, nephrotic syndrome, or bilateral deep vein thrombosis. Drugs such as dihydropyridine calcium channel blockers, corticosteroids, thiazolidinediones, and nonsteroidal anti-inflammatory agents are known to cause edema. Donepezil, a widely used acetylcholinesterase inhibitor for Alzheimer's disease, has not been previously linked to leg edema. Recognizing rare adverse reactions is essential for patient safety, as edema may indicate serious conditions. Cas

Personalized haemodynamic modelling for non-invasive quantification of arterial stiffness in the human vasculature.

Arterial stiffness is a pivotal predictor of cardiovascular events, yet its in vivo characterization remains elusive because arterial wall mechanics cannot be directly measured. Conventional inverse approaches require high-resolution imaging to resolve wall motion, restricting their clinical applicability. Here, we present a non-invasive computational framework that infers regional arterial stiffness solely from routinely acquired pressure and blood flow velocity waveforms by leveraging personalized one-dimensional haemodynamic models of the human arterial network. Patient-specific models were
