In vivo visualization of cardiac extracellular adenosine dynamics and its pharmacological modulation in zebrafish heart failure models.

Heart failure (HF) remains a major clinical challenge with poor prognosis despite extensive research and established treatments. Extracellular ATP and adenosine (Ado), as damage-associated molecular patterns (DAMPs), contribute to HF pathogenesis through pro-inflammatory effects. However, in vivo evidence of ATP/Ado dynamics in failing hearts remains elusive. In this study, we investigated the spatiotemporal dynamics of extracellular Ado in a zebrafish HF model using a genetically encoded heart-specific extracellular Ado sensor (GRABAdo). Terfenadine-induced HF zebrafish showed increased extra

RNA editing in cardiovascular health and disease.

Post-transcriptional RNA modifications can alter RNA structure, stability, localization, and function. Adenosine-to-inosine (A-to-I) RNA editing is a post-transcriptional modification that converts adenosine nucleotides in RNA to inosine nucleotides, catalyzed by adenosine-deaminase-acting-on-RNA (ADAR) enzymes. Recent studies have shown that A-to-I RNA editing is required for cardiovascular development and homeostasis whilst aberrant RNA editing plays a role in cardiovascular diseases. This article provides an overview of A-to-I RNA editing events that have been implicated in cardiovascular b

Extracorporeal Cardiopulmonary Resuscitation due to Left Atrial Myxoma With Severe Pulmonary Hypertension: A Challenging Combination.

BACKGROUND: Myxomas are the most common primary cardiac tumors. Presentation as sudden cardiac death is uncommon. Management of patients on circulatory support with severe pulmonary hypertension (PH) is challenging. CASE SUMMARY: A 55-year-old woman was diagnosed with a giant left atrial myxoma. She experienced cardiac arrest requiring extracorporeal cardiopulmonary resuscitation (ECPR). Management of combined pre- and postcapillary PH was the key to the weaning of circulatory support. DISCUSSION: To our knowledge, this is the first reported case of ECPR owing to an atrial myxoma. It suggest

Comparative analysis of melatonin and sildenafil in a rat model of pulmonary arterial hypertension: Insights into oxidative stress, inflammation, and mitochondrial biogenesis.

Pulmonary artery hypertension (PAH) is characterized by increased pulmonary vascular resistance, leading to the augmented afterload of the right ventricle (RV), hypertrophy, and heart failure. Oxidative stress and inflammation in the RV may be involved in the physiopathology of PAH. Because of their antioxidant properties, melatonin and sildenafil could be possible therapeutic agents for the treatment of PAH. Therefore, the present study evaluated the protective effects of melatonin against oxidative stress, inflammation, and mitochondrial biogenesis in the RV of rats with PAH. Wistar rats wer

Systematic Review of the Application of Pulmonary Hypertension Treatments in Ventricular Septal Defect, Pulmonary Atresia, and Major Aortopulmonary Collateral Arteries.

Background: Pulmonary atresia (PA) with ventricular septal defect (VSD) and major aortopulmonary collateral arteries (MAPCAs), a life-threatening congenital heart defect (CHD), is frequently associated with abnormal pulmonary blood flow and vascular remodeling, causing hypoxia and heart failure. Segmental pulmonary hypertension (PH), a distinct PH type, may exist in some patients. Pulmonary vasodilators have been considered for treatment; however, evidence of their efficacy and safety remains lacking. Methods: A systematic review was conducted using PubMed, MEDLINE, The Cochrane Library, and I

Influence of Chronic Kidney Disease on Platelet Reactivity Response to Clopidogrel and Ticagrelor.

High platelet reactivity (HPR) in patients with coronary artery disease receiving P2Y12 inhibitors increases ischemic risk. Chronic kidney disease (CKD) is an established contributor to HPR during clopidogrel therapy. The objective of the study was to assess whether CKD influences platelet reactivity (PR) in patients treated with clopidogrel or ticagrelor. This double-blind, double-dummy study enrolled 106 stable patients more than one year after an acute coronary syndrome, with or without CKD. Participants were matched by age and sex and randomized to clopidogrel or ticagrelor. PR was measure

Quercetin Dilates Retinal Arterioles via Nitric Oxide-Dependent Mechanisms in Rats.

The purpose of this study was to investigate whether quercetin, a flavonoid abundantly found in onion leaves and other plant foods, induces the dilation of retinal blood vessels in rats. The time-course changes in retinal arteriolar diameter were measured using a retinal circulation evaluation system based on a high-resolution digital fundus camera developed in our laboratory. The intravenous administration of quercetin (10-100 µg/kg/min) increased the retinal arteriolar diameter in a dose-dependent manner. This vasodilatory effect of quercetin was almost completely suppressed through an intra

TNF-α-mediated down-regulation of mitochondrial transcription factors: Rescue by Angiotensin-(1-7) peptide.

Manifesting initially as a physiological adaptation to pressure/volume overload, cardiac hypertrophy helps to maintain normal cardiac output via optimal mitochondrial function. However, chronic hypertension leads to pathological cardiac hypertrophy that is characterized by compromised cardiac output and mitochondrial dysfunction. Against this backdrop, we sought to delineate the molecular mechanisms underlying the regulation of mitochondrial transcription factors (mTFs; viz. Tfam, Tfb1m and Tfb2m which play a pivotal role in maintaining mitochondrial homeostasis) in cardiomyocytes. We show tha

Phase 1 Study Evaluating the Effects of Single-Dose Frespaciguat, an Inhaled Soluble Guanylate Cyclase Stimulator, Co-administered with Multiple-Dose Sildenafil on Systemic Hemodynamics.

Co-administration of oral phosphodiesterase 5 inhibitors with oral soluble guanylate cyclase (sGC) stimulators is contraindicated due to the risk of systemic side effects. Frespaciguat, an inhaled sGC stimulator, has been studied in pulmonary hypertension (PH)-related conditions such as pulmonary arterial hypertension (PAH) and PH associated with chronic obstructive pulmonary disease (PH-COPD), and may allow combined use with oral PDE5i due to low systemic exposure. To evaluate a potential interaction between inhaled frespaciguat and oral sildenafil, 19 healthy participants were randomized to

The effect of atorvastatin combined with azithromycin on lung function and right heart function in COPD patients with concomitant pulmonary hypertension.

BACKGROUND: Chronic obstructive pulmonary disease (COPD) often coexists with pulmonary hypertension (PH), leading to exacerbated morbidity and mortality. Conventional treatments focus on bronchodilation, anti-inflammatory agents, and vasodilators, emphasizing the need for novel interventions. Atorvastatin and azithromycin have pleiotropic properties that may benefit COPD with concomitant PH. However, the combined impact of these medications on lung and right heart function in this patient population remains underexplored. METHODS: A retrospective cohort study involving 229 stable COPD patient

Hypoxia-activated dual donors of H2S and NO alleviate myocardial injury in coronary heart disease via synergistic mechanisms.

Coronary heart disease (CHD) and related hypoxic cardiac disorders involve a detrimental interplay between hypoxia and inflammation, which often fail to address effectively by traditional therapies. Here, we report the design and synthesis of a hypoxia-activated dual donor of hydrogen sulfide (H2S) and nitric oxide (NO), termed prodrug NS-1, which selectively releases therapeutic levels of two gases under hypoxic conditions. NS-1, which is stable under normoxia, exhibits high hypoxia responsiveness and target delivery to hypoxic tissues, minimizing systemic off-target effects. Both in vitro an

Selective ROCK2 inhibition reduces microvascular obstruction but does not reduce myocardial infarction after ischaemia and reperfusion.

INTRODUCTION: Myocardial ischaemia/reperfusion (I/R) injury causes infarction, microvascular obstruction (MVO), and haemorrhage. MVO, often driven by vasospasm, lacks effective therapy. The non-selective ROCK inhibitor fasudil, used for cerebral vasospasm, limits infarct size after myocardial I/R, but the roles of individual ROCK isoforms in limiting infarction and MVO remain unclear. AIMS: To determine the contribution of ROCK2 to myocardial and microvascular obstruction and to assess the vasodilatory potential of ROCK2 inhibition. METHODS: ROCK1/2 expression was analysed in rat hearts by R

Multiple Intimal Injuries Associated With Severe Coronary Spasm.

Coronary vasospasm is a recognized but underappreciated cause of out-of-hospital cardiac arrest. However, the intracoronary imaging features associated with life-threatening vasospastic events have not been fully characterized. We present a patient who experienced recurrent cardiac arrest due to severe coronary vasospasm. Optical coherence tomography (OCT) revealed multiple focal intimal injuries, microthrombus formation, and vessel shrinkage, despite the absence of fixed coronary obstruction. The patient developed repeated ischemic events and ultimately died from nonocclusive mesenteric ische

Identifying cardiac safety signals of disproportionate reporting for CGRP antagonists: evidence from the FDA Adverse Event Reporting System.

The objective of this study was to investigate the potential association between the use of calcitonin gene-related peptide (CGRP) antagonists and the reporting of cardiac adverse events (cAEs) by analyzing data from the US Food and Drug Administration Adverse Event Reporting System (FAERS). CGRP antagonists are a novel class of effective treatments for migraine. However, given CGRP's crucial role as a potent vasodilator, concerns about the cardiac safety of its long-term blockade persist. This study aimed to assess real-world post-marketing safety signals for this drug class. FAERS data from
