Prevalence and factors associated with uncontrolled systolic blood pressure among treated older adults in Sri Lanka: a nationwide multicenter cross-sectional study.

BACKGROUND: Uncontrolled systolic blood pressure (SBP) is a major modifiable determinant of cardiovascular morbidity and mortality in older adults. This study aimed to evaluate the prevalence, associated factors, and cardiovascular implications of uncontrolled SBP among treated older adults in Sri Lanka. METHODS: A nationwide multicenter cross-sectional study was conducted in medical outpatient clinics of ten tertiary care hospitals representing all nine provinces of Sri Lanka. A total of 1,339 adults aged ≥ 65 years receiving antihypertensive therapy for at least six months were recruited us

Effects of polypills on cardiovascular outcomes: Meta-analysis of clinical trials and observational studies.

BACKGROUND AND OBJECTIVE: Cardiovascular polypills, that combine aspirin, antihypertensive and statin, are controversial with respect to cardiovascular risk factors (CVRFs) and cardiovascular events (CVE). We conducted a meta-analysis to examine their effects. METHODOLOGY: A systematic review was conducted, searching clinical studies, both randomised and observational, that compared polypills versus administering the individual drugs separately. Four bibliographic databases (PubMed, Cochrane Library, Science Direct, and Google Scholar) were searched till December 2024. Risk of bias was assess

Dynamic diversification of lignan metabolism in sesame via coordinated oxygenation and glucosylation across germination.

Sesame (Sesamum indicum) seeds accumulate specialized lignans, including (+)-sesamin, (+)-sesamolin, and (+)-sesaminol triglucoside (SL-TG). Although lignan biosynthesis during seed development is well characterized-with SiCYP92B14 recognized as a (+)-sesamin-specific oxygenase-the molecular basis of the metabolic transition during germination, where lipophilic lignans are fully converted into glucosides, remained unclear. Herein, we identify a set of (+)-sesamin oxygenases, SiCYP706V12-V14, cytochrome P450 enzymes (CYPs) that exhibit a broader substrate range than SiCYP92B14. These enzymes ox

Testing community-based strategies to increase hypertension screening in Nigeria: a protocol for an effectiveness-implementation hybrid type III cluster-randomized trial.

BACKGROUND: Hypertension is a major risk factor for cardiovascular disease and stroke, with a growing burden in Nigeria. Limited access to blood pressure (BP) screenings, low awareness, and barriers to healthcare drive low diagnosis rates. Community-based screenings for other conditions have been successful and could improve diagnosis and care linkage. This study evaluates the effectiveness of a community-based screening and referral intervention to increase hypertension detection and facilitate connections to healthcare. METHODS: This cluster-randomized effectiveness-implementation hybrid ty

Visualizing the Spatiotemporal Redistribution of Drugs in Destabilizing Nanoemulsions: A Quantitative Solution 1H NMR Approach.

While self-nanoemulsifying drug delivery systems (SNEDDS) spontaneously generate nanosized emulsions, they are susceptible to thermodynamic destabilization. Characterizing the spatiotemporal fate of drug molecules during this transition remains challenging due to the limited chemical resolution of conventional particle analysis. Herein, we employed solution-state 1H NMR to elucidate the redistribution of three model drugs, fenofibrate (FFB), felodipine (FEL), and lopinavir (LPV), during the destabilization of a Labrafac PG-based nanoemulsion. While dynamic light scattering and cryo-transmissio

From concept to clinical practice: the evolution of low- and ultra-low-dose triple combination therapy for hypertension.

Hypertension remains a leading contributor to global cardiovascular (CV) morbidity and mortality, yet blood pressure (BP) control rates remain suboptimal worldwide. Therapeutic inertia, delayed treatment intensification, dose-dependent adverse effects, and limited healthcare access continue to hinder effective management. Low-dose combination therapy has emerged as a strategy to enhance efficacy while improving tolerability by targeting complementary pathophysiological pathways at reduced drug doses. This review describes the evolution of low- and ultra-low-dose combination therapy from its ph

Crafting probiotics a self-polymerized smart outfit for effective ulcerative colitis treatment.

Probiotic therapy has recently emerged as a promising approach for ulcerative colitis (UC) treatment. However, oral probiotic therapy is often hindered by challenges such as inferior colonization at lesion sites, where elevated intestinal hydrogen sulfide (H2S) level exacerbates epithelial barrier disruption. Moreover, without adjunct drugs, probiotics alone struggle to mitigate oxidative stress and persistent inflammation effectively. To address these issues, an alkali-labile polymerized ferulic acid (pFA) outfit was developed through a self-polymerization strategy, designed to both shield pr

Optimising hyperacute intracerebral haemorrhage management: factors influencing timely intervention.

BACKGROUND: Bundled-care protocols, which include rapid lowering of blood pressure, have improved functional outcomes in patients with intracranial haemorrhage (ICH). Specifically, the intensive blood pressure reduction in acute cerebral hemmorhage trail (INTERACT) 3 trial aimed to achieve rapid lowering of systolic blood pressure (SBP) to 140 mmHg within an hour of arrival. We analyse factors that influenced the timely delivery of hyperacute ICH management. AIM: We aimed to identify the factors that delay the timely delivery of antihypertensive therapies for hyperacute ICH. METHODS: A retro

Single Interventions (Pharmacological or Renal Denervation) Are Not Sufficient to Achieve Blood Pressure Control in Resistant Hypertension: A Systematic Review and Meta-Analysis.

BACKGROUND: Resistant hypertension remains a major clinical challenge. This systematic review/meta-analysis evaluated whether the addition of a single pharmacological or device-based intervention (renal denervation [RDN]) can achieve effective blood pressure (BP) control in patients with true resistant hypertension. METHODS: A systematic search of MEDLINE/PubMed was performed to identify studies assessing the antihypertensive effects of adding a single pharmacological or device-based intervention in patients with true resistant hypertension. Primary analyses were conducted using a single-arm

Acute physical exercise enhances memory persistence in female rats via hippocampal catecholaminergic receptors.

Acute physical exercise (PE) is a promising non-pharmacological strategy for improving cognitive function; however, the neurochemical mechanisms underlying its effects on memory persistence remain unclear, especially in females, who are underrepresented in preclinical research. Here, we investigated whether a single session of moderate-intensity treadmill exercise enhances memory persistence in female rats and whether this effect depends on hippocampal catecholaminergic signaling. Adult female Wistar rats were trained in the novel object recognition (NOR) task and, immediately afterward, were

Associations between fall risk increasing drugs (FRID) and fall & fall injuries in older Black and White men and women: the Health, Aging and Body Composition Study.

OBJECTIVES: Use of medications defined as fall risk increasing drugs (FRIDs) have increased in recent years mainly due to antihypertensives, antidepressants, and anticonvulsants. FRIDs and fall injury risk is largely unknown. This study aims to determine if FRIDs use/count, separately considering antihypertensive and other FRIDs use, is associated with non-injurious falls/fall injuries. STUDY DESIGN: We analyzed data from 1273 community-dwelling older Black and White adults in the Health, Aging, and Body Composition Study, a longitudinal cohort (2007-08 visit; 2 years of follow-up; median age

Elucidating the role of TPGS in felodipine/PVPVA amorphous solid dispersions during water uptake: dissipative particle dynamics simulations using quantum chemistry-based interaction parameters.

Amorphous solid dispersions (ASDs) are an effective formulation approach for improving the solubility of poorly water-soluble drugs. Nevertheless, their drug-release performance depends on the phase separation morphology during the dissolution process. Although the addition of surfactants has been experimentally reported to improve the dissolution behavior of ASDs, experimental methods alone are insufficient to resolve the complex and heterogeneous intermolecular interactions between drugs and excipients in multicomponent systems at the molecular level. In this study, we used dissipative parti

Comparative bleeding risk between angiotensin II receptor blockers with high versus low P-glycoprotein inhibitory activity in patients with atrial fibrillation receiving direct oral anticoagulants.

Direct oral anticoagulants (DOACs) are frequently co-prescribed with angiotensin II receptor blockers (ARBs) in atrial fibrillation (AF). Because ARBs differ in P-glycoprotein (P-gp) inhibition, bleeding risk may vary. We compared major bleeding when DOACs were combined with telmisartan (high P-gp inhibition) versus olmesartan (low P-gp inhibition). We conducted a nationwide cohort study from 2016-2023 in Korea. We identified AF patients who started a DOAC with telmisartan or olmesartan. The primary outcome was a composite of intracranial hemorrhage and gastrointestinal bleeding. Secondary out

Benefits Beyond Hypertension Control: Current and Evolving Treatment Strategies for Prevention of Cardiovascular Diseases.

INTRODUCTION: Cardiovascular Disease (CVD) continuum often begins with uncontrolled hypertension, progressing through structural/functional impairment of the cardiovascular system and atherosclerosis toward myocardial infarction, stroke, and cardiovascular death. Hypertension affects over one billion people globally, yet less than half have been diagnosed and received treatment. Suboptimal management of elevated Blood Pressure (BP) remains a challenge across low- and middle-income countries, including India. This review outlined the CVD continuum associated with hypertension, along with guidel

External validation of the BAN-ADHF diuretic-resistance score in the DAPA-RESIST clinical trial.

Diuretic resistance in worsening heart failure (HF) is associated with poorer outcomes and, although common, can be challenging to identify. The recently developed BAN-ADHF score can potentially identify patients with worsening HF at risk of diuretic resistance. We externally validated the BAN-ADHF diuretic-resistance score in a multicentre randomized trial comparing adjunctive dapagliflozin to metolazone in patients with HF and diuretic resistance. We calculated a seven-variable modified BAN-ADHF score at baseline and dichotomised patients by the median BAN-ADHF score (8). We assessed loop di

Clinical Remission of an Unresectable Presumptive Haemangiosarcoma in a Dog Treated With Toceranib, Piroxicam, and Propranolol: A Case Report.

Haemangiosarcoma (HSA) is a highly aggressive vascular tumour in dogs, characterised by rapid growth, early metastasis, and poor prognosis despite conventional treatment with surgery and doxorubicin-based chemotherapy. A 14-year-old spayed female Maltese (4.14 kg) was presented with progressive abdominal distension and anorexia. Imaging revealed a large retroperitoneal mass, and computed tomography identified a 12.1 × 8.0 × 8.5 cm heterogeneous tumour. Exploratory laparotomy confirmed the lesion was unresectable, and biopsy findings were most consistent with presumptive haemangiosarcoma (HSA)
