Application of machine learning approaches to develop predictive models for diabetes and hypertension among Bangladeshi Adults.

With rapid urbanization, lifestyle changes, and an aging population, non-communicable diseases (NCDs), including hypertension and diabetes, pose significant public health challenges in Bangladesh and many other low- and middle-income countries. This study used machine learning (ML) approaches to develop predictive models for hypertension and diabetes among Bangladeshi adults. Bangladesh Demographic and Health Survey 2022, a nationally representative cross-sectional survey, data were analyzed. Hypertension was defined as systolic/diastolic blood pressure 140/90 mmHg (or more) or taking any anti

Carvedilol attenuates doxorubicin-evoked renal toxicity in rats: The role of PI3K/AKT/mTOR, Nrf2/HO-1 and Bax/Bcl2 signals.

Doxorubicin (DOX) is considered a highly effective antineoplastic agent for different types of cancers. Regretfully, its clinical application is restricted by its renal intoxication. Carvedilol (CAR), a non-selective β-adrenoceptor blocker, possesses unique antioxidant, anti-inflammatory and antifibrotic features. The current investigation was established to investigate the protective impacts and the implicated molecular mechanisms of CAR against DOX-induced renal toxicity in rats. Rats were randomly assigned into three groups: control, DOX and CAR+DOX. DOX was given as a single intraperitonea

Comparative Effectiveness of Chlorthalidone Versus Hydrochlorothiazide and Dose-Response of Amiloride on Blood Pressure and Biochemical Parameters.

BACKGROUND: Diuretics are widely used alone or in combination with other medications to lower blood pressure (BP). The dosing ratio between hydrochlorothiazide and chlorthalidone for lowering BP, as well as the effects of different amiloride doses on BP and their ability to counteract thiazide's adverse metabolic effects, remain unclear. METHODS: In a factorial superiority randomized clinical trial with blinded participants and investigators, we compared the effects of chlorthalidone 25 mg with hydrochlorothiazide 50 mg and amiloride 5 mg with 10 mg on BP measured by office and ambulatory BP

Investigating Endogenous Hypercortisolism Prevalence in a U.S. Population With Resistant Hypertension: MOMENTUM Rationale and Design.

BACKGROUND: Hypertension is a leading modifiable risk factor for cardiovascular disease and premature death among adults. Up to 18% of individuals with hypertension have resistant hypertension (rHTN), which substantially increases the risk of adverse clinical outcomes. Endogenous hypercortisolism can result in rHTN through multiple mechanisms. OBJECTIVES: MOMENTUM (NCT06829537) is the first large, observational, multicenter study examining the prevalence of endogenous hypercortisolism among adults with rHTN in the United States. METHODS: Target enrollment is approximately 1,000 participants.

Perindopril: A comprehensive profile.

Perindopril (PER) is classified as angiotensin-converting enzyme (ACE) inhibitors and is used primarily for the management of hypertension and cardiovascular conditions, including heart failure and stable coronary artery disease. Two primary salts of PER have been developed, perindopril arginine (PER-Arg) and perindopril erbumine (PER-E). This chapter presents a detailed overview on the physicochemical attributes of PER-Arg, molecular structure, synthetic pathway, crystallographic behavior, thermal stability, and UV absorbance profile of PER-Arg. Moreover, the chromatographic, spectrophotometr

Patient Preferences for Hypertension Treatment Outcomes: A Quantitative Field Study Using the Analytic Hierarchy Process.

BACKGROUND: Hypertension is a major risk factor for fatal and nonfatal cerebrovascular and cardiovascular events. Poor treatment adherence, a key measure impeding successful treatment, can be improved by incorporating patient preferences. We aimed to identify patient preferences for hypertension treatment outcomes by performing a quantitative exploratory field study using the analytic hierarchy process (AHP). We also explored the associations between patient preferences with prior cardiovascular and cerebrovascular events, sociodemographic factors, and health-related quality of life (HRQoL).

ADHD and adherence to antihypertensive medication treatment: a multinational cohort study.

BACKGROUND: Adherence to antihypertensive medication, alongside lifestyle modifications, is fundamental to managing hypertension and reducing the risk of cardiovascular disease. Attention-deficit/hyperactivity disorder (ADHD) is a common neurodevelopmental disorder associated with a range of cardiovascular diseases, including hypertension. ADHD medication has also been associated with hypertension. However, the influence of ADHD and ADHD medication on discontinuation and adherence to antihypertensive treatments is unknown. METHODS: We conducted a multinational cohort study using electronic he

Study of risk assessment scores as surrogate endpoints in pulmonary arterial hypertension trials.

RATIONALE: Pulmonary arterial hypertension (PAH) clinical trials largely rely on clinically relevant intermediate outcomes, as mortality and lung transplant are infrequent and remote outcomes. The change in 6-minute walk distance (Δ6MWD) is utilized as an intermediate endpoint and is used as a surrogate for clinical worsening events. OBJECTIVES: We investigate whether the change in 4 composite risk assessment scores-REVEAL Lite 2 (ΔRL2), European Four Strata (ΔE4), European Three Strata (ΔE3), and French Noninvasive score (ΔFPHN)-are potentially stronger surrogate endpoints than the single-co

Electrophysiological evidence about sexual dimorphism in the clonidine-induced inhibition of trigeminal wide dynamic range cell activity.

BACKGROUND: The trigeminal system plays a key role in headaches pathophysiology. In this regard, nociceptive experiments in male rodents have shown that at the trigeminal level, clonidine induces antinociception through α2A- but not α2B/2C-adrenoceptors. Interestingly, although behavioural experiments suggest that males are more sensitive to clonidine-induced antinociception than female rats (an effect linked to hormonal factors), little is known about how neuronal nociceptive processing is affected in females (including the role of the α2A/2B/2C-adrenoceptors subtypes involved). Since trigemi

Poorer Outcomes With Blood Pressure Lowering in Severe White Matter Disease After Thrombectomy.

BACKGROUND: Intensive blood pressure (BP) management after successful endovascular thrombectomy (EVT) worsens outcomes in acute ischemic stroke. White matter hyperintensity (WMH), associated with impaired cerebral autoregulation, is associated with poor outcomes after EVT; however, its role in modifying BP management effects remains unclear. We investigated whether WMH severity modifies the effect of post-EVT BP management on outcomes. METHODS: This secondary analysis of the OPTIMAL-BP trial (Outcome in Patients Treated With Intra-Arterial Thrombectomy-Optimal Blood Pressure Control; conducte

Pair-Feeding Differentiates Feed-Dependent and Pharmacodynamic Effects of Telmisartan in Wistar Rats.

Telmisartan, an angiotensin II receptor blocker with partial PPAR-γ agonist activity, exhibits pleiotropic effects beyond its antihypertensive action, including modulation of lipid metabolism, erythropoiesis, and renal function. However, its anorexigenic properties complicate interpretation of systemic outcomes in toxicology studies. This study employed a pair-feeding design to differentiate feed-dependent effects from direct pharmacodynamic actions in male Wistar rats treated with telmisartan (100 mg/kg/day) for 28 days. Both telmisartan and pair-fed groups showed reduced body weight and hepa

Association Between 24-Hour Systolic Blood Pressure Time in Target Range and Mortality.

BACKGROUND: Time in target range (TTR) reflects the proportion of time blood pressure (BP) remains within a defined range, integrating BP variability and control. We examined associations of systolic BP (SBP) TTR during ambulatory BP monitoring with cardiovascular and all-cause mortalities. METHODS: Patients from the Spanish Ambulatory BP Monitoring Registry who were receiving antihypertensive medications or who had sustained or masked hypertension without treatment, defined by office BP ≥140/90 mmHg and 24-hour BP ≥130/80 mmHg, were included. TTR was estimated by linear interpolation between

Losartan primes pancreatic tumors for effective exosome-biomimetic chemo-immunotherapy.

Pancreatic ductal adenocarcinoma (PDAC) is protected by a dense fibrotic stroma that restricts drug delivery and enforces immune exclusion. We describe a clinically actionable, two-stage strategy in which losartan preconditioning transiently converts a high-stiffness tumor microenvironment into a more compliant state with reduced mechanical resistance and thereby enables efficient delivery of an exosome-liposome hybrid nanoparticle (IGEL) co-encapsulating gemcitabine and IL-12 circular RNA. In orthotopic, stroma-rich PDAC models, losartan pretreatment increased vascular perfusion and nanoparti

Modification of tuna (Katsuwonus pelamis) hydrolysates with galangal active ingredients: Enhancing flavor and functional properties.

Marine protein hydrolysates hold great promise for functional foods, but undesirable flavors often limit their application. This study aimed to enhance both the sensory and functional properties of tuna (Katsuwonus pelamis) hydrolysates by introducing galangal (Alpinia officinarum) active ingredients. Eight preparation routes were designed to generate galangal-flavored peptide-rich bases (GPBs) with different addition strategies. GPBs contained flavonoids and polysaccharides derived from galangal, and exhibited stronger xanthine oxidase inhibitory (XOI) activity compared with tuna hydrolysates
