Sexual function among controlled and uncontrolled hypertensive females receiving beta-blockers or ACEI/ARB and thiazides: a prospective randomized controlled study.

BACKGROUND: Female sexual dysfunction (FSD) among females with hypertension (HTN) is frequently overlooked, with a reported prevalence of 42.1%. OBJECTIVES: We aimed to determine the impact of beta-blockers (BBs), angiotensin-converting enzyme inhibitors/angiotensin receptor blockers (ACEIs/ARBs), and thiazides on sexual function in hypertensive females. METHODS: A prospective randomized controlled trial enrolled 125 female participants. Group (1) included 25 normotensive females serving as the controls. Groups (2) and (3) consisted of 50 controlled and uncontrolled hypertensive patients who

Medication non-adherence and apparent treatment-resistant hypertension.

Medication adherence refers to the extent to which a person's medication-taking behaviour corresponds with agreed recommendations from their clinician. Research shows that more than one-third of patients are non-adherent to their prescribed medications. Non-adherence is particularly significant in hypertension, as it complicates chronic disease management and may result in apparent treatment-resistant hypertension (aTRH), in which blood pressure remains uncontrolled despite appropriate pharmacological therapy and lifestyle measures. It is therefore important to assess adherence routinely, as n

Scalable treatment algorithm focused on hypertension management for the University of California.

Uncontrolled hypertension remains a leading contributor to morbidity and mortality, particularly among underserved populations. To address care variability and disparities in treatment, the University of California Health system developed and implemented the UC Way Hypertension Medication Algorithm across six academic health centres. This standardised, evidence-based protocol was co-developed by a team of multidisciplinary experts in pharmacy, cardiology, primary care and data science, with an emphasis on medication affordability and the use of two-drug fixed dose combinations as first-line th

Systolic Blood Pressure Trajectory and Outcomes in Acute Intracerebral Hemorrhage: Pooled Analysis of the 4 INTERACT and ATACH-II Clinical Trials.

BACKGROUND AND OBJECTIVES: While moderate and rapid systolic blood pressure (SBP) lowering was associated with better functional outcomes after intracerebral hemorrhage (ICH), large reductions in SBP within 1 hour, for example, reductions from >200 to <140 mm Hg, diminished or even reversed these beneficial effects. We aimed to define the optimal trajectory of SBP control in relation to favorable functional outcomes after acute ICH. METHODS: We conducted a pooled analysis of individual patient-level data from all 4 Intensive Blood Pressure Reduction in Acute Cerebral Hemorrhage Trial (INTERAC

Patient-profiled treatment responses in a large hypertension trial: a posthoc analysis of the INSIGHT study.

OBJECTIVE: Guidelines on the treatment of hypertensive patients usually refer to 'average' patients. However, in clinical practice, individual patient characteristics may differ substantially from the average. Thus, it seems worthwhile to examine the relationship between comprehensive patient profiles and blood pressure responses. METHODS: We divided the patient population from the INSIGHT trial into exploration and validation cohorts and constructed composite patient profiles based on predictors of blood pressure control (age, severity of hypertension, comorbidities, and previous treatment s

Impact of post approval quantitative changes in excipient composition on nitrosamine formation: De-risking strategy.

Between 2018 and 2021, losartan was the most often recalled product (24% of total recalls) followed by metformin tablets (19% of total recalls) due to the presence of nitrosamine impurities. The objective of this work is to analyse the de-risking method in terms of reducing nitrosamine generation and establishing nitrite specifications in excipients. In light of these recalls, we simulated the influence of post-approval level 1 and level 2 changes in excipient compositions on nitrosamine formation in metformin SR (sustained release) and losartan film coated tablets. Furthermore, we investigate

Effects of adrenergic receptor modulators on nuclear LCC‑channels in Purkinje neurons.

Large conductance cation channels (LCC‑channels), located in the nuclear membrane, are potential mediators of potassium (K+) countercurrent during calcium ion (Ca2+) release from intracellular stores. This study examined the effects of epinephrine, norepinephrine, isoprenaline, and propranolol on the electrophysiological properties of these channels to evaluate their potential as blocking agents in future studies of LCC‑channel involvement in Ca2+ release. The patch‑clamp method, using voltage‑clamp mode and a nucleus‑attached configuration, was employed to record currents passing through LCC‑

Chronic pinacidil attenuates the pentylenetetrazole‑induced kindling in rats.

Seizures in epilepsy result from excessive neuronal electrical discharges. Imbalances in potassium homeostasis can increase neuronal excitability and trigger epileptic seizures. Drugs that enhance potassium currents are expected to suppress seizures. Pinacidil, an ATP‑sensitive potassium channel opener, has shown anticonvulsant effects in acute seizure models in rodents. However, its effects following chronic systemic administration in pentylenetetrazole (PTZ)-kindled rats remain unknown. The aim of this study was to investigate the impact of long‑term pinacidil treatment in the PTZ‑kindling m

Prediction of worsening of renal function by the combination of an initial decline in renal function and blood pressure-lowering effect after renin-angiotensin system blockade in patients with type 2 diabetes mellitus (JDDM 83).

AIMS: This study investigated the effect of an initial decline in the estimated glomerular filtration rate (eGFR) and blood pressure lowering after renin-angiotensin system (RAS) blockade on subsequent renal function in patients with type 2 diabetes mellitus. METHODS: A retrospective follow-up study was conducted on 5345 patients with an eGFR >60 mL/min/1.73 m2. The patients were grouped by initial eGFR decline: group 1 (<0%), group 2 (0%-10%), group 3 (10%-20%), and group 4 (≥20%), and subgrouped by the reduction in systolic blood pressure (SBP) (≥ or <10 mmHg). The risk of progression to an
