Gut-Brain Axis in Comorbid Constipation and Mood Disorders: Navigating the Efficacy-Tolerability Dilemma Through Emerging Evidence and Future Directions.

BACKGROUND: The comorbidity of constipation and mood disorders (anxiety/depression) creates a vicious cycle of symptom superposition and treatment resistance. The persistent "efficacy-tolerability" dilemma-whereby effective treatment for one condition may exacerbate the other-remains unresolved. METHODS: This narrative review is based on a systematic search of PubMed, Embase, and Cochrane Library (2015-2026), focusing on epidemiology, gut-brain axis mechanisms, pharmacotherapy limitations, and emerging strategies. The search formula was: ("constipation" OR "functional constipation") AND ("moo

Using supermarket loyalty card data to investigate seasonal variation in laxative purchases in the UK.

While laxatives are designed to manage the symptoms of constipation, they are also known to be misused for weight management, particularly by individuals with eating disorders. This study investigates the relationship between laxative purchases and weight management by examining seasonal trends. Using real-world loyalty card data from a major UK pharmacy retailer spanning December 2013 to December 2014, we analyse self-medication purchasing patterns from 748,375 buyers to explore potential links with weight management behaviours. In pre-registered analysis, we use regression models to investig

Pharmacological management of childhood constipation: Bridging today's gaps with tomorrow's therapies.

Childhood constipation is a prevalent yet frequently underestimated global health concern, affecting approximately 10% of children across diverse populations. Its impact extends well beyond bowel-related symptoms, with affected children often experiencing extraintestinal manifestations such as pain in multiple sites, behavioral disturbances, and a markedly diminished quality of life. The burden is further compounded in low- and middle-income countries, where financial and logistical constraints limit access to effective treatment. The impact is intensified in developing countries due to limite

Association of Initial Laxative Strategy With Clinical Outcomes in Critically Ill Patients With Acute Myocardial Infarction: A Retrospective Cohort Study.

BACKGROUND: Although laxatives are commonly used for constipation in critically ill patients with acute myocardial infarction (AMI), the optimal initial choice is unclear. We are aimed at comparing the clinical outcomes of initiating therapy with stimulant, stool softener, and osmotic laxatives. METHODS: This retrospective study analyzed ICU patients with AMI from the MIMIC-IV database, comparing three initial laxative strategies using propensity score matching (PSM). The primary outcome was 28-day mortality. Secondary outcomes included ICU, in-hospital, and 365-day mortality; cardiogenic sho

Senna Laxative-Associated Severe Ischemic Bowel Disease in an 87-Year-Old Patient: A Clinical Warning.

Senna laxative, one of the major stimulant laxatives, has received less attention regarding its potential risk of intestinal ischemia in the elderly population. An 87-year-old female with a past medical history of chronic constipation developed abdominal pain and fever after oral ingestion of senna laxative for constipation and was subsequently diagnosed with ischemic bowel disease upon hospitalization. Symptomatic and supportive treatment, such as antibiotics, acid inhibition and stomach protection, fluid replenishment, and intravenous nutrition, was given. After treatment, the patient's cond

Dependence-related screening positivity among regular stimulant-laxative users with chronic constipation: a multicenter prospective observational study.

BACKGROUND: Chronic constipation is common, and the appropriate use of stimulant laxatives remains challenging. Although dependence due to habitual stimulant-laxative use is frequently observed, the long-term effects of regular use on dependence and tolerance have not been fully elucidated. This study aimed to characterize the clinical features and substance use-related symptoms associated with stimulant-laxative use in patients with chronic constipation. METHODS: This multicenter prospective observational study was conducted at three centers and enrolled patients with chronic constipation, i

Targeted Ileal Bile Acid Transporter Inhibition as Rescue Therapy for Tirzepatide-Induced Severe Constipation: A Case Report.

INTRODUCTION: Constipation is a common, dose-dependent adverse effect of incretin-based anti-obesity therapies that affects patient adherence and may necessitate discontinuation, resulting in loss of metabolic benefits and weight regain. Elobixibat, an ileal bile acid transporter (IBAT) inhibitor, may offer a targeted alternative when conventional laxatives fail. CASE PRESENTATION: A 56-year-old woman with obesity (95 kg; BMI 38.5 kg/m2) developed severe constipation after increasing tirzepatide to 10 mg weekly. Her bowel movements decreased from 6 to 8 times weekly (Bristol Stool Chart types

Metastatic hypercalcemia from laxative misuse: a rare and serious complication of anorexia nervosa.

A 33-year-old woman with anorexia nervosa (AN) presented with lethargy, diarrhea, and abdominal pain and developed progressive hypercalcemia during hospitalization. Biochemistry demonstrated an inappropriately normal parathyroid hormone (PTH) level in the setting of chronic kidney disease, hyperphosphatemia, and severe vitamin D deficiency. Despite initial denial, chronic misuse of calcium-containing stimulant laxatives was identified, with ingestion of up to 200 tablets daily, including covert use during admission. Imaging revealed extensive metastatic calcification involving the kidneys, lun

Acute Functional Colonic Dysmotility Mimicking Mechanical Bowel Obstruction Following Self-Limited Diarrheal Illness: A Case Report.

Acute constipation following diarrheal illness is commonly encountered and is often benign. However, when absolute constipation persists despite adequate hydration, preserved mobility, and failure of multiple laxative classes, it can closely mimic mechanical bowel obstruction and create significant diagnostic uncertainty. We report a case of a 56-year-old female with a previously normal daily bowel habit who developed acute obstipation following a self-limited diarrheal episode, unresponsive to bulk-forming, osmotic, and stimulant laxatives. Despite adequate oral intake and activity, symptoms

Slow Transit Constipation: Pathophysiological Perspectives and Management Updates.

Slow transit constipation (STC) is a chronic colonic motility disorder characterized by markedly delayed transit, leading to reduced bowel movements, abdominal discomfort, and significant quality-of-life impairment. It predominantly affects women and is associated with abnormalities in enteric neuronal signaling, smooth muscle contractility, interstitial cells of Cajal, gut peptides, bile acid homeostasis, and autonomic regulation. Secondary causes of constipation and structural lesions must be excluded before the diagnosis of STC, with colonic transit studies serving as the gold standard. Com

Structural basis for Mg²⁺-glycine synergy in the tandem glycine riboswitch revealed by cryo-EM.

Part I of this article discussed the potential functional importance of genetic mutations and alleles of the human cytochrome P450 2D6 (CYP2D6) gene. The impact of CYP2D6 polymorphisms on the clearance of and response to a series of cardiovascular drugs was addressed. Since CYP2D6 plays a major role in the metabolism of a large number of other drugs, Part II of the article highlights the impact of CYP2D6 polymorphisms on the response to other groups of clinically used drugs. Although clinical studies have observed a gene-dose effect for some tricyclic antidepressants, it is difficult to establ

Clinical factors affecting colonic iodine-131 distribution after radioactive iodine therapy for thyroid cancer.

PURPOSE: Radioactive iodine therapy (RAIT) is used to treat patients with thyroid cancer at high risk of recurrence or those with distant metastases. Small amounts of iodine-131 (I-131) are excreted in the stool after RAIT. Thyroid hormone withdrawal (THW) before RAIT can cause constipation, increasing radiation exposure to the colon. Although measuring colonic radiation using I-131 dosimetry would be challenging, colonic radiation dose can be estimated using I-131 whole-body scintigraphy post-RAIT. Therefore, we aimed to determine the clinical risk factors, including THW, associated with colo

End-of-Life Palliative Care: Role of the Family Physician.

To care for patients at the end of life, family physicians should be able to evaluate the causes of symptoms, differentiate between distressing symptoms and common end-of-life changes, and balance treatment effectiveness with potential adverse effects, while ensuring alignment with the patient's values and wishes. For severe pain and dyspnea, opioids are the mainstay of treatment. Palliation of pain with adjuvant medications and nonpharmacologic measures may delay or decrease the need for opioids. Nausea can be treated by reducing exacerbating factors and choosing agents that target the specif

Colonic Manometry in Pediatric Patients with Spina Bifida: Results from a Retrospective Cohort Study.

BACKGROUND/OBJECTIVES: Patients with spina bifida (SB) commonly experience neurogenic bowel dysfunction, characterized by defecation-related symptoms. While anorectal dysfunction and slow transit constipation (STC) have been implicated, the role of colonic motility in SB remains unclear. This study aimed to evaluate colonic motility in SB patients with refractory bowel dysfunction. METHODS: This retrospective cohort study included SB patients who failed the repeated optimization of a bowel regimen including stimulant laxatives and subsequently underwent anorectal manometry (ARM), colonic tran
