Pharmacotherapy for Chronic Idiopathic Constipation and Constipation-Predominant Irritable Bowel Syndrome Beyond Conventional Laxatives: A Narrative Review.

Chronic idiopathic constipation (CIC) and constipation-predominant irritable bowel syndrome (IBS-C) are two highly prevalent functional gastrointestinal disorders that share overlapping features but differ in their pathophysiology and symptom profiles. Both conditions impose a significant socioeconomic burden due to their high prevalence, chronic nature, and impact on daily life. Treatment for chronic constipation begins with lifestyle modifications, including fiber supplementation, and progresses to osmotic and/or stimulant laxatives if the response is inadequate. About half of the patients t

Impact of antipsychotics, antiparkinsonian drugs, and anthraquinone stimulant laxatives on severe constipation in patients with schizophrenia.

AIM: Constipation in schizophrenia patients receiving antipsychotics can lead to severe intestinal complications, such as paralytic ileus or megacolon. Despite the clinical importance of the topic, few studies have thoroughly investigated the risk factors of constipation-related complications. Therefore, the present study aimed to identify clinical and pharmacological factors associated with severe, constipation-related conditions in patients with schizophrenia. METHODS: The present, retrospective study analyzed inpatients with schizophrenia and constipation at Tokyo Metropolitan Matsuzawa Ho

Effect of glycerin on colonic motility in children.

OBJECTIVE: Colonic manometry (CM) involves measurement of colonic neuromuscular activity and administration of a stimulant laxative, most commonly bisacodyl, which is part of the study protocol. Glycerin is another laxative used in the treatment of constipation. Our objective was to evaluate the effect of glycerin on colonic motility and compare the response to glycerin with each patient's response to bisacodyl. METHODS: We performed a retrospective review of all CM studies performed between May 2015 and May 2022. All studies with glycerin administration were included. Patient demographics, m

Evaluating the neuroprotective potential of Mucuna pruriens and Boswellia serrata in Huntington's disease: an integrative bioinformatics approach.

Hepatitis E causes large-scale epidemics in endemic areas. The disease, during epidemics, has increased incidence and severity in pregnant women. Sporadic acute viral hepatitis (AVH) is common in endemic areas. The relationship of sporadic AVH and pregnancy has not been well studied. Over a 3-year period we prospectively studied 76 pregnant women and 337 non-pregnant women of childbearing age with sporadic acute viral hepatitis for aetiology, clinical course and outcome of disease. The aetiology in sporadic AVH was hepatitis A virus (HAV) in six (1.5%), hepatitis B virus (HBV) in 62 (15%), hep

Simultaneous identification of three emergent stimulant laxative adulterants in slimming foods using only one antibody.

Stimulant laxatives were recently found to be abused in slimming foods, resulting in harmful effects on consumers. To ensure the safety of relative products, sensitive yet multiplex immunoassays are crucial in rapid screening of stimulant laxatives. However, there are few immunoassays for these substances, and even less for broad-specific recognition. Thus, in this work, four theoretically promising haptens of emerging stimulant laxative bisacodyl were rationally designed using molecular modeling and synthesized to immune animals, whose feasibility was confirmed by the obtained broad-specific

Opioid-induced Constipation: Old and New Concepts in Diagnosis and Treatment.

Daily use of opioid analgesics has significantly increased in recent years due to an increasing prevalence of conditions associated with chronic pain. Opioid-induced constipation (OIC) is one of the most common, under-recognized, and under-treated side effects of opioid analgesics. OIC significantly reduces the quality of life by causing psychological distress, lowering work productivity, and increasing access to healthcare facilities. The economic and social burden of OIC led to the development of precise strategies for daily clinical practice. Key aspects are the prevention of constipation t

[Pharmacologic Treatment of Chronic Constipation].

Chronic constipation is a common disease that can impair the quality of life, with a prevalence of 14% globally and 16.5% in South Korea. Straining, hard stools, the sensation of incomplete evacuation, the sensation of anorectal blockage, and manual maneuvers to facilitate defecation are the related symptoms of chronic constipation. On the other hand, medications commonly referred to as laxatives are the essentials of treatment for constipation compared to non-pharmacological treatment, such as lifestyle modifications, biofeedback, or surgery. Unfortunately, there is still an unmet need to det

Review article: do stimulant laxatives damage the gut? A critical analysis of current knowledge.

Stimulant laxatives are well established as first- or second-line treatments for constipation and although they have a reliable therapeutic effect, alleged safety concerns still exist, particularly with long-term use. The potential harmful effects on the gastrointestinal system (including carcinogenicity) of the long-term use of diphenylmethane [bisacodyl, sodium picosulfate (SPS)] and senna stimulant laxatives were assessed in a comprehensive review of the publications identified in literature searches performed in PubMed and Embase up to and including June 2023. We identified and reviewed 43

Current practice in the care of children with functional constipation: What is the hold up?

OBJECTIVES: For children with intractable functional constipation (FC), there are no evidence-based guidelines for subsequent evaluation and treatment. Our objective was to assess the practice patterns of a large, international cohort of pediatric gastroenterologists. METHODS: We administered a survey to physicians who attended the 2nd World Congress of Pediatric Neurogastroenterology and Motility held in Columbus, Ohio (USA) in September 2023. The survey included 29 questions on diagnostic testing, nonpharmacological and pharmacological treatment, and surgical options for children with intra

Activation of chloride channels and promotion of bowel movements by heat-killed Bifidobacterium longum CLA8013.

Constipation is strongly associated with the deterioration of quality of life (QOL), and patients with constipation desire clear spontaneous defecation without the feeling of incomplete evacuation, rather than improved defecation frequency. The use of common osmotic or stimulant laxatives has not been shown to lead to a satisfactory improvement of bowel movements. In addition, softening of stools by increasing their water content has been reported to increase the frequency of spontaneous defecation and improve hard stools, straining during defecation, and abdominal symptoms, such as abdominal

Colonoscopic Resolution of Melanosis Coli After Cessation of Senna Laxative Use.

Melanosis coli occurs with the administration of stimulant laxatives for the relief of constipation. However, the duration of macroscopic improvement of melanosis coli after discontinuation of anthracene laxatives is not well understood. We describe the case of an 81-year-old female diagnosed with melanosis coli via colonoscopy who had been taking senna laxatives for 5 years. Seven months after cessation of senna laxatives, colonoscopy showed no melanosis coli in the colon. This impressive report describes the observation of melanosis coli with colonoscopy; 7 months after the withdrawal of sen

Prospective association between food protein-induced allergic proctocolitis in infancy and constipation after age 3.

We sought to prospectively evaluate whether food protein-induced allergic proctocolitis (FPIAP) during infancy is associated with increased constipation later in childhood. Using the Gastrointestinal Microbiome and Allergic Proctocolitis (GMAP) cohort, we reviewed charts of children with prospective parent-reported constipation after age 3 to confirm the diagnosis of constipation. A diagnosis of FPIAP was based on pediatrician diagnosis and required guaiac-positive or grossly bloody stools, as previously published. Three hundred and seventy-five subjects had sufficient data for these analyses.

Improving Diagnosis and Management of Opioid-Induced Constipation (OIC) in Clinical Practice: An Italian Expert Opinion.

Opioid-induced constipation (OIC) is a very common and troublesome gastrointestinal side effect following the use of opioids. Despite existing international guidelines, OIC is largely underdiagnosed and undertreated. ECHO OIC is a European project designed to improve the diagnosis and management of OIC at the primary care level. The next phase of the ECHO OIC project is to review and adapt the proposed European pathway at national level, considering the local patient journey and clinical practice. A multidisciplinary group of 12 Italian experts reviewed and discussed the European path and form

A smartphone-based dual-modal lateral flow immunochromatographic assay for multiplex detection of illegal stimulant laxatives in slimming foods.

Stimulant laxatives (especially bisacodyl and sodium picosulfate) are frequently found to be adulterated into slimming foods, causing health-threatening effects to consumers. Sensitive, accurate, easy-to-operate and portable multiplex analytical techniques are still desired for the rapid screening of stimulant laxatives in slimming foods. In this work, a highly sensitive dual-modal colorimetric/photothermal lateral flow immunochromatographic assay (LFIA) was established based on facilely prepared concentrated gold nanoparticles (cAuNPs). Due to the enhanced local surface plasmon resonance and

Pharmacological prevention and treatment of opioid-induced constipation in cancer patients: A systematic review and meta-analysis.

BACKGROUND: Cancer-related pain often requires opioid treatment with opioid-induced constipation (OIC) as its most frequent gastrointestinal side-effect. Both for prevention and treatment of OIC osmotic (e.g. polyethylene glycol) and stimulant (e.g. bisacodyl) laxatives are widely used. Newer drugs such as the peripherally acting µ-opioid receptor antagonists (PAMORAs) and naloxone in a fixed combination with oxycodone have become available for the management of OIC. This systematic review and meta-analysis aims to give an overview of the scientific evidence on pharmacological strategies for t

Effects of a Revision to the Standard Admission Order Set on Laxative Prescribing for Opioid-Induced Constipation.

PURPOSE: Opioid-induced constipation (OIC) is highly prevalent in patients with cancer-related pain on opioid analgesics and has negative consequences on physical and psychological well-being and quality of life. Oncology clinical practice guidelines recommend the use of osmotic and stimulant laxatives for the prevention and management of opioid-induced constipation, not stool softeners such as docusate sodium. Prescribing practices continue to fall behind these recommendations. METHODS: This quality improvement project revised the laxative options available in the standard admission order se
