A survey of paediatric prescribing and dispensing in Karachi.

In an effort to understand the paediatric prescribing practices of family physicians and dispensing of medicines from pharmacies, a survey was conducted in Karachi. Another objective was to understand the factors influencing such practices. Hundred family physicians and 55 pharmacists were surveyed. Four groups of drugs namely antidiarrhoeals, appetite stimulants, multivitamins and brain tonics were identified for assessment, these being drugs widely promoted by the pharmaceutical industry. Some of these drugs are known to have deleterious effects and therapeutic benefit of most of them is dub

Dispensing habits of Johannesburg pharmacists in treating acute infantile diarrhoea.

Sixty pharmacists in Johannesburg were randomly selected and questioned on their suggested treatment of acute infantile diarrhoea as well as their knowledge of oral rehydration therapy (ORT); 53 (88%) responded. Of 51 pharmacists who had treated diarrhoea in children, 8 (16%) had never heard of ORT, while 20 (39%) treated this condition with antidiarrhoeal drugs. These findings are perturbing, because acute diarrhoeal disease is the biggest cause of mortality of children aged 1-4 years in South Africa and ORT is recognised as an effective means of prevention of death.

[Diarrhea and functional digestive pathology].

Functional bowel disorders are the most frequent cause of chronic diarrhoea. Functional disorders may present as typical "diarrhée motrice" (colonic hypermotility) or as watery diarrhoea. The diagnosis cannot be made unless an organic cause has been ruled out. Although colonic motor abnormalities have been described in some patients, its physiopathology remains obscure. Treatment is the same as for functional bowel disorders, i.e. fibres and antidiarrhoeal drugs.

Current management of acute diarrhoeas. Use and abuse of drug therapy.

Acute diarrhoeas in infants and children should be regarded as a protective physiological response of the gastrointestinal tract to flush out harmful organisms and toxins, thus limiting the morbidity from this illness. The approach to therapy of acute diarrhoeas must be physiological rather than pharmacological. Prevention and early correction of acute dehydration with oral rehydration is the most physiological approach to the successful management of acute diarrhoeas, which generally have a self-limited course. The use of antidiarrhoeal drugs is limited to a few isolated cases. These may incl

Oral rehydration in infants in developing countries.

Diarrhoeal diseases are the major cause of infant mortality in developing countries. Dehydration is the most common complication of diarrhoea, and severe dehydration causes up to 80% of diarrhoeal fatalities. For more than 100 years, physicians focused the treatment of diarrhoeal diseases on the symptom diarrhoea, and there were many 'antidiarrhoeal' drugs, such as water adsorbents (kaolin and pectin) and antiperistaltics (opium, paregoric elixir, diphenoxylate hydrochloride with atropine sulphate and loperamide). This approach focused on a non-dangerous symptom and diverted attention from the

Effect of codeine and loperamide on upper intestinal transit and absorption in normal subjects and patients with postvagotomy diarrhoea.

Patients with chronic severe diarrhoea after truncal vagotomy and pyloroplasty are often difficult to treat using conventional antidiarrhoeal drugs and remain severely disabled. We examined the effect of two drugs, codeine phosphate and loperamide, on upper intestinal transit and carbohydrate absorption, measured non-invasively by serial exhaled breath hydrogen monitoring, in patients with postvagotomy diarrhoea who had previously failed to gain relief from drug therapy. Orocaecal transit was consistently faster in these patients than a group of controls and was associated with malabsorption o

Self-prescribing by way of pharmacies in three Asian developing countries.

The pattern of advice given and drugs dispensed at 75 Asian pharmacies in response to the presentation of a fictitious infant with diarrhoea were studied. Only 16 of the 75 pharmacies gave the appropriate advice--oral rehydration or consultation with a health worker. 19 of 25 pharmacies in Bangladesh, 16 of 25 in Sri Lanka, and 24 of 25 in Yemen Arab Republic dispensed drugs, with or without oral rehydration solution. Fixed-dose combinations of antibiotics and antidiarrhoeal drugs were common. The results are discussed in relation to national drug and diarrhoeal control policies. After further

The pharmacological modification of secretory responses.

Electrolyte transport across the intestinal mucosa can be modulated by several neurotransmitters, hormones and drugs. Opiate agonists and endogenous opioid peptides inhibit electrolyte secretion both in vitro and in vivo. These drugs appear to act at several levels. Thus, opioid effects can be elicited at the local mucosal level. Secondly, antisecretory effects can be demonstrated when opioids are administered into the brain. These central effects appear to involve activation of the sympathetic innervation of the intestine. Thirdly, some antidiarrhoeal drugs such as loperamide may have ancilla

[Neuro-hormonal regulation of the function of the enterocyte].

The intestinal epithelial cell, or enterocyte, performs a dual function: absorption and secretion. Absorption takes place in the villi and secretion in the crypts (and perhaps also in the villi). Both functions are controlled by complex extra- and intracellular systems. The extracellular system is mediated by neuromediators released in the subcellular space. Some mediators, like the vasoactive intestinal peptide (VIP) or the prostaglandins, stimulate secretion while others, like enkephalins, stimulate absorption. All these mediators act on intracellular metabolic processes by activating "secon
