The Economic Impact of Medetomidine on Critical Care Utilization: A Longitudinal Analysis of Intensive Care Unit Charges for Opioid Withdrawal Across the Adulterated Era.

OBJECTIVES: To quantify the impact of the adulterated fentanyl supply, through the eras of xylazine and medetomidine predominance, on critical care charges associated with treatment of opioid withdrawal. METHODS: A longitudinal retrospective analysis was conducted at 2 intensive care units (ICUs) in Philadelphia, PA, USA, from April 1, 2017, to September 30, 2025. Primary outcomes included median per quarter and per patient charges and median quarterly ICU admissions with a primary or secondary ICD-10 diagnosis of opioid withdrawal (F11.23) from Q2 2017 to Q3 2025. These dates were correlated

Case Report: Oral-Nasal Damage in a Person Using Fentanyl and Xylazine.

BACKGROUND: Xylazine, an alpha-2-adrenergic agonist, was added to illicitly manufactured fentanyl within the past decade. Intranasal drug use is common and associated with damage to nasal structures and the palate. There have been no case reports detailing intranasal damage secondary to fentanyl and xylazine use. CASE SUMMARY: We present a case of a 39-year-old woman with a long history of intranasal opioid use and, more recently, xylazine use who presented for treatment due to nasal septum perforation and oral-nasal fistulas. She started treatment for opioid use disorder and was referred to

Comparison of full-dose succinylcholine and rocuronium for rapid sequence intubation in an academic emergency department: a retrospective cohort study.

BACKGROUND Succinylcholine and rocuronium are the principal neuromuscular blocking agents used for rapid sequence intubation (RSI). Prior studies comparing these agents have shown conflicting results, although a Cochrane review suggested that succinylcholine may provide superior intubating conditions. This study aimed to compare the first-pass success rate, intubating conditions, and peri-intubation safety between full-dose succinylcholine (1.5 mg/kg) and rocuronium (1.2 mg/kg) during RSI. METHODS This retrospective cohort study was conducted in a single academic emergency department between 2

Passive sampling for grazer cues incorporates top-down effects in harmful algal bloom monitoring and toxin prediction.

Domoic acid (DA) produced by toxigenic species of the diatom genus Pseudo-nitzschia during harmful algal blooms (HABs) bioaccumulates in filter feeders, bait fish, and commercial seafood products, posing a threat to marine organisms and public health. Copepodamides, a group of polar lipids exuded by copepods, are known to induce DA production in multiple species of Pseudo-nitzschia. However, copepodamides are not currently considered in HAB monitoring and prediction, in part because of challenges associated with measuring ambient concentrations of dilute and labile compounds. Here we employ so

Current approaches and challenges in pediatric neuromuscular blockade monitoring: A scoping review.

BACKGROUND: Neuromuscular blocking agents (NMBAs) are a fundamental component of balanced anesthesia and are widely used in pediatric perioperative and intensive care settings. Despite their benefits, NMBA administration carries the risk of postoperative residual curarization (PORC), which may lead to respiratory complications, particularly in small infants. Developmental differences in physiology, pharmacokinetics, and pharmacodynamics further complicate NMBA management in this population, making accurate neuromuscular blockade monitoring (NMBM) essential to ensure efficacy and safety. Association of Preoperative Midazolam Administration With Patient Anxiety During the Transition From the Preoperative Holding Bay to the Operating Room: A Prospective Observational Study.

Background Among patients awaiting surgical procedures, up to 80% experience anxiety. Midazolam is commonly administered in the preoperative holding bay to reduce anxiety. However, the effect of benzodiazepines can last longer than surgery and may lead to an increased risk of neurocognitive impairment. Thus, we looked into the effects of midazolam administration beyond the preoperative holding bay. We hypothesized that omitting midazolam in the holding bay increases anxiety in the operating room and the risk of unpleasant explicit recall. Methods This study included adult patients undergoing s

Enhancing the Outgrowth of Oral Mucosal Cell Sheets for Cultivated Rabbit Oral Mucosal Epithelial Transplantation with ROCK Inhibitor.

PURPOSE: Cultivated oral mucosal epithelial transplantation (COMET) is an effective way to treat limbal stem cell deficiency (LSCD), but its clinical usage is limited due to prolonged cultivation time and complexity. Rho-associated protein kinase (ROCK) inhibitors have been found to enhance cell proliferation, migration, and cytoskeleton maintenance. We investigated whether ROCK inhibitor (Y-27632) can be used to enhance the growth of oral mucosal epithelial cell sheet while remaining their characteristics, optimizing COMET usage for LSCD treatment. MATERIALS AND METHODS: Oral mucosal epithel

In vivo and computational investigation of the antidepressant-like activity of boldine from Peumus boldus in mice subjected to chronic unpredictable mild stress.

Boldine, an aporphine alkaloid mainly isolated from the bark and leaves of Peumus boldus, is reported to possess neuroprotective, antioxidant, and nootropic activities. Due to its antioxidant and neuroprotective potential, this study explored the antidepressant-like effects of boldine. Boldine (1.5, 3.0, and 6.0 mg/kg, p.o.) was evaluated for antidepressant-like activity in unstressed and stressed Swiss young male albino mice, while molecular docking (Autodock Vina) and pharmacokinetic predictions (SwissADME) provided mechanistic insights. In stressed mice, boldine significantly decreased the

Sugammadex in Perioperative Neuromuscular Management: Current Advances and Best Practices.

Neuromuscular blocking agents (NMBAs) are crucial for anesthesia, enabling intubation and optimal surgical conditions. Timely reversal of blockade is critical for safe extubation and recovery. While neostigmine, a traditional reversal agent, is effective for moderate blockade, it has limitations in reversing deep blockade and requires anticholinergics to mitigate side effects. Sugammadex, a novel agent, addresses these limitations by selectively encapsulating aminosteroid NMBAs like rocuronium, providing rapid and reliable reversal. It demonstrates significant advantages, including faster reco

Xylazine: Evaluation of antemortem and postmortem specimen storage conditions.

Xylazine, a popular compound found independently and in polydrug mixtures throughout the United States, is intended for use in veterinary medicine and not for human consumption. When xylazine is ingested by humans, life-threatening central nervous system depression responses could potentially occur. With its increase in prevalence, several methods of detection were developed within forensic toxicology. Since xylazine is not approved for human clinical use, little is known about its behavior, such as its stability in stored biological matrices. Knowledge of drug stability in biological matrices

Xylazine and major urinary metabolites detected in patients positive for fentanyl and xylazine.

Xylazine in an anesthetic drug used for the sedation of animals and increasingly appearing as an adulterant in uncontrolled drug supplies, primarily illicit fentanyl. The ability to detect xylazine exposure by urine drug testing may improve monitoring of this drug trend and our understanding of the effects and risks associated with xylazine exposure. Currently, limited information is available regarding the elimination of xylazine or its metabolites in humans. In this study, we report quantification of xylazine and 4-hydroxy-xylazine (4-OH-x) in hydrolyzed urine specimens collected from 109 pa

Postoperative benefits of sugammadex: A narrative review.

PURPOSE: To describe the systemic benefits of sugammadex as a reversal agent for neuromuscular blockade. SUMMARY: Sugammadex is the preferred reversal agent for neuromuscular blockade as recommended in guidelines from the American Society of Anesthesiology. This recommendation is primarily based on a faster time to recovery from neuromuscular blockade as compared to that seen with neostigmine. Systemic adverse effects of neostigmine are often mitigated by the administration of glycopyrrolate. However, glycopyrrolate is an antimuscarinic agent that may be associated with its own adverse effect

Thiopentone in Combination with Xylazine: An Experimental Investigation for Induction of Surgical Anesthesia and Safe Recovery in Experimental Rats.

INTRODUCTION: Achieving reliable anesthesia induction and rapid postoperative recovery are crucial aspects of animal research, particularly in small experimental animals. Thiopentone is still a widely used anesthetic drug, but its safety is a primary concern in delivering a safe dose to experimental animals. However, the use of thiopentone in combination with xylazine for surgical procedures in small experimental animals remains underexplored. Therefore, this study assesses the efficacy and safety of combining thiopentone and xylazine for inducing anesthesia and facilitating recovery in Spragu
