Mucus plugs in COPD and cardiovascular mortality: An emerging risk pathway and the therapeutic potential of mucolytic strategies.

Airway mucus plugging is an underrecognized feature of COPD that is increasingly associated with adverse outcomes, including higher mortality rates. Computed tomography studies demonstrate that mucus plugs are prevalent, variable, and frequently persistent or dynamic over time. The presence of mucus plugs is independently linked to a worse prognosis, even after adjusting for cardiovascular and respiratory risk factors. Patients with mild COPD appear to be at particularly high risk, suggesting that mucus plugging may represent an early and potentially modifiable disease trait. Mucus plugging co

The combination of osimertinib and chemotherapy successfully enabled surgical resection of locally advanced lung adenocarcinoma: a Case Report.

We present the case of a 33-year-old female with a 4.3 × 3.2 cm high-density mass in the apical segment of the right upper lobe, detected on non-contrast chest CT, which showed suspected invasion of the chest wall and other tissues. CT-guided lung biopsy confirmed lung adenocarcinoma, and genetic testing revealed an EGFR exon 19 deletion. The patient received neoadjuvant therapy with oral Osimertinib combined with chemotherapy. After 4 months of treatment, she underwent thoracoscopic right upper lobectomy, mediastinal lymph node dissection, and pleural adhesion lysis. Postoperative management

Heated nebulization of ambroxol hydrochloride: engineering submicron aerosols for pulmonary drug delivery.

Chronic respiratory diseases (CRDs) remain challenging to treat, largely due to the difficulty in delivering therapeutic agents to the deep lung. Ambroxol hydrochloride (AH), a widely used mucolytic agent that promotes mucus clearance, is indicated for conditions such as asthma, emphysema, and chronic bronchitis. This study reports the in vitro characterization of a heated medical nebulizer (MD6-HA20-2) for the generation of AH inhalable aerosols. The device vaporizes a liquid AH formulation via a battery-operated heating element; upon inhalation, the vapor condenses into aerosols exhibiting a

Photodegradation of expectorant drug ambroxol in seawater environment: Seasonal impact on a global scale.

The ocean is a globally important zone for the photochemical transformation of various organic contaminants. Its unique chemical composition, especially the high concentration of halide ions, can significantly alter the photochemical reactivity and fate of these contaminants. Herein, we systematically investigated the effects of marine constituents on the sunlight-driven photolysis of ambroxol (AMB), an expectorant drug extensively used following COVID-19 pandemic, and assessed its spatiotemporal persistence on a global scale. The results revealed that marine matrix significantly promoted AMB

Disintegration Fingerprinting: A Low-Cost and User-Friendly Tool for Identifying Substandard and Falsified Solid-Dosage Medicines.

Substandard and falsified medicines cause thousands of deaths and waste billions of dollars worldwide every year. There is an urgent need for low-cost and simple-to-use tools for identifying these "fake drugs." In this work we demonstrate "Disintegration Fingerprinting" (DF), a technique that identifies pills, tablets, caplets, and other solid-dosage drugs based on how the drug disintegrates and dissolves in liquid. The DF hardware consists of a water-filled transparent plastic cup atop a conventional magnetic stirrer. An inexpensive sensor mounted on the outside of the cup shines infrared lig

From Traditional Remedy to Evidence-Based Phytotherapeutic Agent: Hedera helix L. in Respiratory Medicine.

Hedera helix L. leaf extracts are regulated expectorants for productive cough, yet their evidence is dispersed across pharmaceutical quality, nonclinical pharmacology, disposition, toxicology, clinical pharmacology, efficacy, safety, and regulatory assessment. This review integrates those domains in a single study-level account and explicitly distinguishes isolated constituents, incompletely specified extracts, defined monograph preparations, proprietary extracts, and combination products. Hederacoside C is the pharmacopoeial marker, while α-hederin is the constituent most extensively examined

Erdosteine Provides Effective Analgesia in Inflammatory Pain Without Impairing Pain Resolution: A Preclinical Comparison With Non-Steroidal Anti-Inflammatory Drugs.

BACKGROUND: Nonsteroidal anti-inflammatory drugs (NSAIDs) provide effective analgesia but may paradoxically delay tissue recovery, pain resolution and contribute to chronic pain. Erdosteine, a thiol-based prodrug with antioxidant and anti-inflammatory properties, is a mucolytic used for treatment of respiratory diseases but has recently been suggested to possess analgesic potential. METHODS: We examined the analgesic efficacy of erdosteine and its metabolite, Met-1, in experimental models of inflammatory and neuropathic pain and compared its effects with diclofenac and gabapentin. Protocol of ASPro-PD: a phase 3 trial of ambroxol to slow progression in genetically stratified Parkinson's disease.

INTRODUCTION: Genetic studies have identified the GBA1 gene as a significant genetic risk factor for Parkinson's disease (PD), with 10-15% of PD patients carrying GBA1 variants. GBA1 variants affect the glucocerebrosidase (GCase) enzyme, often leading to reduced GCase activity and associated altered lysosomal function, implicated in PD pathogenesis. Ambroxol, a small molecule widely used for respiratory diseases, has emerged as a potential therapeutic agent for PD, acting by increasing GCase activity. A phase 2 trial demonstrated ambroxol's safety and efficacy in penetrating cerebrospinal flui

Treatment of Lung Disease in Primary Ciliary Dyskinesia: A Review of Current and Emerging Interventions.

Primary ciliary dyskinesia (PCD) is a rare genetic disease caused by impaired function of motile cilia. There have been few clinical trials conducted in people with PCD, and current management strategies are extrapolated from expert consensus and evidence-based approaches for cystic fibrosis and non-cystic fibrosis bronchiectasis. Clinical guidelines for PCD have focused on symptom management, primarily a combination of antibiotics and airway clearance techniques, but many people are treated with bronchodilators, inhaled corticosteroids, and inhaled mucolytic agents, despite the lack of eviden

A functional lyoprotectant platform enables storage-stable, mucus-penetrating siRNA delivery to the lung.

Respiratory diseases remain a major global health concern, where pathological mucus accumulation and chronic inflammation severely compromise lung function. RNA therapeutics have emerged as a transformative modality to address underlying molecular pathologies beyond the capabilities of small-molecule drugs. However, effective delivery of RNA therapeutics to the lungs remains hindered by significant challenges. The instability of lipid nanoparticles (LNPs) in liquid formulations compromises their storage and cold-chain transport, while the pathological mucus hypersecretion characteristic of chr
