Comparative Efficacy of Current Topical Treatments for Dry Eye Disease: A Review of Pivotal Clinical Trials Evaluating Corneal Staining Outcomes.

Dry eye disease (DED) is a multifactorial disorder characterized by disruption of tear film homeostasis, resulting in ocular surface inflammation and damage. Although several Food and Drug Administration-approved topical treatments are available, direct comparisons of their efficacy and safety are complicated by variability in study designs and corneal staining grading scales. This review systematically evaluates and compares the efficacy and safety of topical therapies approved in the United States, focusing on anti-inflammatory and semi-fluorinated alkane (SFA)-based therapies. A systematic

Atypical cutaneous leishmaniasis in a patient under tumour necrosis factor alpha inhibitor for psoriasis.

Tumor necrosis factor (TNF) inhibitors are associated with a higher risk of infection. Cases of cutaneous leishmaniasis have been rarely reported in this population, mostly with those taking adalimumab and infliximab.A man in his 60s with chronic plaque psoriasis developed a cutaneous lesion on his leg after 2 years of adalimumab therapy. It was initially misdiagnosed and unsuccessfully treated as a bacterial ulcer, but a skin biopsy later confirmed Leishmania donovani complex infection. Adalimumab was discontinued, and the patient was treated with intravenous liposomal amphotericin B, leading

A Difficult Case of Ectopic ACTH Syndrome: Is Treatment Possible Even Without Accurate Localization?

Occult ectopic adrenocorticotropic hormone (ACTH) syndrome (EAS) is a relatively rare form of ACTH-dependent Cushing's Syndrome (CS). We describe a middle-aged female with symptomatic CS, where despite thorough investigations including hormone suppression testing, multiple imaging modalities, and inferior petrosal sinus sampling (IPSS), an ectopic source could not be demonstrated. Our case demonstrates that ketoconazole can be an effective therapeutic to suppress cortisol production even when the source of ectopic ACTH production is unknown. However, long-term evaluation is required.

Native Bio-Lenticules Mediated Rapamycin Delivery for Corneal Immunomodulation and Tissue Repair Promotion.

Corneal damage-related disorders, characterized by stromal opacity due to trauma, infection, or other etiologies, remain a major cause of visual impairment. Despite therapeutic advances, the scarcity of donor corneal grafts and the complexity of immune regulation continue to pose critical challenges. To address these limitations, we developed a novel immunomodulatory biomaterial composed of decellularized human stromal lenticules and rapamycin (RAPA) solubilized with deep eutectic solvents. Designed for interlamellar corneal implantation, this drug-loaded construct enables localized, sustained

Subcutaneous Anti-inflammatory Therapies to Prevent Burn Progression in a Swine Model of Contact Burn Injury.

INTRODUCTION: If left untreated, burn injuries can deepen or progress in depth within the first 72 hours after injury as a result of increased wound inflammation, subsequently worsening healing outcomes. This can be especially detrimental to warfighters who are constrained to resource-limited environments with delayed evacuation times to higher roles of care and more effective treatment. Preventing this burn progression at the point of injury has the potential to improve healing outcomes but requires a field-deployable therapy and delivery system. Subcutaneous therapies known to treat inflamma

Chitosan hydrogels enriched with bioactive phloroglucinol for controlled drug diffusion and potential wound healing.

We report a facile strategy to prepare chitosan (CS) hydrogels that eliminates the need for chemical crosslinking for advanced biomedical therapies. This approach gives controlled properties to the hydrogels by incorporating a natural bioactive phenolic compound, phloroglucinol (PG), into their microstructure. The adsorption of PG onto CS chains enhanced the hydrogels' antioxidant activity by up to 25 % and resulted in a denser, more entangled structure, reducing the pore size by 59 μm while maintaining porosity above 94 %. This allowed us to finely adjust pore size and swelling capacity. Thes

Formulation study of PLGA in situ films for topical delivery of salicylates.

A film-forming system (FFS) represents a convenient topical dosage form for drug delivery. In this study, a non-commercial poly(lactic-co-glycolic acid) (PLGA) was chosen to formulate an FFS containing salicylic acid (SA) and methyl salicylate (MS). This unique combination is advantageous from a therapeutic point of view, as it enabled modified salicylate release. It is beneficial from a technological perspective too, because it improved thermal, rheological, and adhesive properties of the in situ film. DSC revealed complete dissolution of SA and good miscibility of MS with the polymer. MS als

Insights into the antioxidant, anti-inflammatory and anti-microbial potential of Nigella sativa essential oil against oral pathogens.

Oral disorders can exert systemic ramifications beyond their localized effects on dental tissues, implicating a wide array of physiological conditions. The utilization of essential oils (EOs) for protection of oral health represents a longstanding practice. Consequently, in this investigation, essential oil derived from Nigella sativa seeds (NSEO) underwent isolation via the hydro-distillation process, followed by a comprehensive evaluation of its antioxidant, anti-inflammatory, anti-fungal, antibacterial activities, and cytocompatibility. The isolated NSEO manifested as a pale-yellow substanc

Overexpression of PtNRPS1 enhances diatom-mediated bioremediation of salicylate pollution.

The accumulation of the emerging pollutant salicylic acid (SA) in the environment has gained much attention. In this study, overexpression of the non-ribosomal peptide synthase (NRPS) gene, PtNRPS1 in the marine diatom Phaeodactylum tricornutum (PtNRPS1-OE) increased resistance to SA pollutants. It was assumed that the enhanced tolerance was due to the high binding affinity between recombinant PtNRPS1 (rNRPS1) and SA pollutants. Moreover, tandem mass spectrometry analysis determined the amino acids that participated in the covalently binding of SA. The removal efficiency of SA pollutants by Pt

Regulatory T cells engineered with polyphenol-functionalized immunosuppressant nanocomplexes for rebuilding periodontal hard tissue under inflammation-challenged microenvironment.

Global aging heightens the risk of oral disorders, among which periodontitis is the major cause of tooth loss in the aging population. The regeneration of damaged periodontal hard tissue is highly challenging due to the existence of the refractory local inflammation. Owing to the potent anti-inflammatory capabilities, regulatory T cells hold great promise in immunotherapies for tissue regeneration. However, the transferred regulatory T cells can alter their phenotypes and functions in local inflammatory milieu, significantly impairing their therapeutic efficacy. Herein, we introduce a novel re

[Expert consensus on diagnosis and treatment of severe COVID-19 associated pulmonary aspergillosis and mucormycosis].

The incidence and mortality of COVID-19 associated pulmonary aspergillosis (CAPA) are high in critically ill patients. Although COVID-19 associated mucormycosis (CAPM) is relatively rare, its severity and often a delayed diagnosis or misdiagnosis lead to its high mortality. The diagnosis and treatment of CAPA and CAPM in critically ill patients are challenging. Early diagnosis and a standardized therapy are the two most important factors for a good outcome. Therefore, a working group of experts from Chinese Thoracic Society and Chinese Association of Chest Physicians Critical Care Group was or

pH- and Matrix Metalloproteinase-Responsive Multifunctional Bilayer Microneedles Platform for Treatment of Tinea Pedis.

Persistent foot odor and itchiness are common symptoms of tinea pedis, significantly disrupting the daily life of those affected. The cuticular barrier at the site of the tinea pedis is thickened, which impedes the effective penetration of antifungal agents. Additionally, fungi can migrate from the skin surface to deeper tissues, posing challenges in the current clinical treatment for tinea pedis. To effectively treat tinea pedis, we developed a platform of bilayer gelatin methacrylate (GelMA) microneedles (MNs) loaded with salicylic acid (SA) and FK13-a1 (SA/FK13-a1@GelMA MNs). SA/FK13-a1@Gel

Hormesis of glyphosate on ferulic acid metabolism and antifungal volatile production in rice root biocontrol endophyte Burkholderia cepacia LS-044.

Glyphosate (GP, N-phosphonomethyl glycine) is one of the most popular organophosphate herbicides widely used in agricultural practices worldwide. There have been extensive reports on the biohazard attributes and hormetic impacts of GP on plant and animal systems. However, the effects of GP on plant growth-promoting microbes and its ecological relevance remain unknown. Here, we show that GP does exert a hormetic impact on Burkholderia cepacia LS-044, a rice (Oryza sativa ssp. japonica cv. Tainung 71) root endophytic isolate. We used increasing doses of ferulic acid (FA, 1-25 mM) and GP (0.5-5 m
